Comprehensive Analysis of CT,MRI and PET for Solitary Pulmonary Nodules

Objective To study the comprehensive diagnostic value of solitary pulmonary nodules with CT, MRI and PET.Methods A retrospective study was undertaken in 65 patients with solitary pulmonary nodules using CT, MRI and PET. Results The sensibility and specificity between CT,MRI and PET were not significant discrepancy (P value were 0.377 and 0.7 respectively) by contrast analysis.With applying CT,MRI and PET, there was only 1 case of misdiagnosis in 44 cases of lung cancer and there were 19 cases of correct diagnosis in 21 cases of benign lesion, the sensibility, specificity, positive predictive value, negative predictive value, and accuracy were 97.7% , 90.5% , 95.6% , 95%, and 95.4% respectively.Conclusion Comprehensive methods could raise the accuracy of diagnosis in solitary pulmonary nodules.

Video-assisted thoracic surgery (VATS) lobectomy: the Japanese experience.

Between September 1992 and October 1997, we performed 128 video-assisted thoracic surgery (VATS) lobectomies. The indications for surgery were 103 cases of lung cancer, 11 cases of bronchiectasis, 8 cases of granuloma, 4 cases of benign lesions, and 2 cases of metastatic tumors. Of the 103 cases of lung cancer, 62 were treated by VATS lobectomy with extended lymph node dissection for clinical stage I lung cancer, and the 4-year survival rate of final stage I lung cancer was 94.4%. VATS lobectomy is far less invasive than open thoracotomy, and survival rates after VATS lobectomy with extended lymph node dissection are comparable with those after open thoracotomy. Thus, VATS lobectomy with extended lymph node dissection should be considered as a standard surgical alternative to open thoracotomy for stage I lung cancer.

Lobectomy with extended lymph node dissection by video-assisted thoracic surgery for lung cancer.

Between September 1992 and September 1996, we performed 88 VATS (video-assisted thoracic surgery) lobectomies and two VATS pneumonectomies. The indications for surgery were 68 cases of lung cancer, nine cases of bronchiectasis, six cases of tuberculosis, and seven cases of benign lesions. Of the 68 cases of lung cancer, 36 were treated by VATS lobectomy with extended lymph node dissection for clinical stage I lung cancer, making full use of recently developed devices for thoracoscopic surgery, such as roticulating endoscissors, miniretractors, endoclips, and harmonic scalpels. Twenty-four lymph nodes were resected on average (range, 10 to 51) by VATS. This number was comparable to lymph nodes resected in open thoracotomy during the same period. Among the 36 patients who underwent extended lymph node dissection, 20 showed no lymph node metastasis postoperatively (stage I), while 16 had N1 or N2 cancer. All patients with stage I cancer have survived 4 to 36 months (median: 17 months) with no signs of recurrence. This survival of stage I lung cancer after VATS is comparable to that of open thoracotomy. We thus believe that VATS lobectomy with extended lymph node dissection can be an alternative to standard posterolateral thoracotomy for stage I lung cancer.

Studies on morphological typing of argyrophilic nucleolar organizer regions.

Morphological typing of argyrophilic nucleolar organizer regions (AgNOR) in 300 cases of malignant tumors and 140 cases of benign lesions was analyzed and five morphological types of AgNOR were described in detail. In malignant tumors, the diffuse type (78%) was the most frequently seen, and in benign lesions, the nucleolar type (92.85%); the difference was thus highly significant (P < 0.001). The intranucleolar and aggregate types were not observed in benign lesions. There was no obvious difference in the proportion of the mixed type in benign and malignant lesions (P > 0.05). The relationship between grade of malignancy and morphological typing of AgNOR and its clinical significance are discussed.

The value of nuclear parameters were determined using interactive computer image analysis system on 100 preoperative FNAC smears of breast lesion were radical mastectomy was performed subsequently. Twenty cases of benign lesion were taken as control. The lymph node status was correlated with the nuclear and nucleolar variables. It is found that few nuclear parameters have good discriminatory power. Computerised interactive morphometry proved to be far superior to eye-piece measurements with respect to accuracy, reproducibility and time taken for parameter estimation which leads to the determination of better prognostic indicator.

Duct endoscopy and endoscopic biopsy in the evaluation of nipple discharge

Microdochectomy is usually performed on patients with nipple discharge caused by intraductal proliferative lesions, such as intraductal papilloma and carcinoma. But this operation often sacrifices large amounts of normal mammary gland even when the lesion is a benign intraductal papilloma a few millimeters in diameter. We have developed duct endoscopy for the mammary duct system, and have reliably performed biopsies for intraductal proliferative lesions intraductally. From June 1989 to April 1990, we examined 22 cases by duct endoscopy, and performed endoscopic biopsy in 16 cases. The method of endoscopic biopsy is as follows. First, a bougie is inserted, without anesthesia other than Xylocaine jelly, into the orifice of the duct to enlarge it. Second, the outer cylinder and the inner needle are inserted; then the inner needle is removed, and the endoscope is inserted. After examination, the outer cylinder is moved up to the lesion to be biopsied and the endoscope is taken out. Then a sample is taken into the outer cylinder by aspiration. We diagnosed 10 cases of benign lesion and 5 cases of malignant lesion by cytological and/or histological examination. In conclusion, endoscopic biopsy, aided by duct endoscopy, is a useful and harmless diagnostic procedure in the evaluation of nipple discharge.

Benign lesions mimicking malignant tumors of the esophagus

Three cases of benign lesions which mimicked malignant tumors of the esophagus are described. In all three cases, two inflammatory pseudotumors and one case of diffuse leiomyomatosis, the clinical presentations, radiologic features, and gross pathologic findings led to the mistaken diagnosis of carcinoma at thoracotomy. The benign nature of the processes was recognizable only on microscopic examination. Although most benign tumors of the esophagus are localized solitary lesions that are easily distinguished from carcinoma, occasionally benign conditions may present as infiltrative, ulcerated mass lesions. Inflammatory pseudotumor and diffuse leiomyomatosis should be included in the differential diagnosis of esophageal malignancies.

Histological sampling with a 23 gauge modified Menghini needle.

A 23 gauge modified Menghini (Surecut) needle biopsy technique for obtaining tissue core biopsies was compared with the conventional fine needle aspiration biopsy technique in the diagnosis of ultrasonically detected abdominal mass lesions. In 30 consecutive cases (19 malignant and 11 benign), adequate material for histological examination was obtained in 87% and for cytological examination in 97%. The diagnostic accuracy with respect to malignancy was 84% for histology and 89% for cytology. The predictive value of malignancy was 100% in both. The histological material provided additional information in four cases of malignancy, concerning the type and origin of the tumours, and in nine cases of benign lesions, indicating the type of lesion that appeared as a tumourlike mass in the ultrasound study. The Surecut needle biopsy has been found valuable in obtaining histological material from abdominal mass lesions and may supplement or even replace the fine needle aspiration biopsy in such lesions.
