HETEROTOPIC OSSIFICATION AND PSAMMOMATOUS CALCIFICATION IN GASTRIC CARCINOMA

Two cases of heterotopic ossification occurring in a carcinoma of the stomach are reported. In one of these cases, bone formation is seen only in the regional metastasis, while in the other case, it is found in the primary tumor alone. In addition, there is seen in the former case a deposit of laminated psammoma bodies in the primary tumor and its regional metastasis. These two conditions are both so unusual for gastric carcinoma that only nine pertinent cases with heterotopic ossiflcation and two authentic cases with psammomatous calcification have been reported in the literature. The feature of these conditions la described and the pathogenesis discussed on the basis of personal Findings and a review of literature.

The surgical treatment of heterotopic ossification at the elbow following long-term coma.

Nine cases of heterotopic ossification at the elbow in seven patients who had a cerebral injury and prolonged coma were treated by excision of the extra-articular bone bridge from the humerus to the ulna. The bridge was either anterior or posterior, and did not involve the radius. The articular cartilage in all elbows showed no significant evidence of degeneration in spite of the prolonged immobility of the joints during the period of coma. Two of the patients also had resection of heterotopic bone about the hip joint which resulted in rapid reformation of bone. In all patients there was restoration of satisfactory function in the elbow operated on without reformation of bone. Follow-up was from ten months to 8.7 years.

Heterotopic ossification in abdominal operation scars.

Five cases of heterotopic ossification in surgical abdominal scars are described. No common etiologic factors were observed. Possible causative factors are discussed. Combined surgery and irradiation are proposed as treatment of choice in symptomatic cases.

Our Experience with Irrigation Therapy for Heterotopic Ossification

There have been established no final treatments for heterotopic ossification which has been found not infrequently.We have tested continuous irrigation therapy with EDTA (ethylen-diamine tetraacetic acid) and predonisolon solution for one months following surgical removal of ossified lesion on two cases of heterotopic ossification. Four to six months follow-up study revealed satisfactory result on both radiological and clinical findings.Case 1, was a 23-year-old male who developed heterotopic ossification at the right femoral joint and both knee joints 5 months after spinal injury due to dislocation and fracture of the first lumbar vertebra. Five months later the lesions were removed and irrigation was initiated.Case 2, was a 20-year-old female who noticed pain and motor disturbance at the right femoral joint followed by heterotopic ossification. Operation was performed 10 months later followed by irrigation therapy.
