Coronary artery aneurysm and thrombosis following chronic ephedra use

Ephedra, also known as Ma Huang, was commonly used to enhance athletic performance, “fat burning", and weight loss before its removal from the United States in April 2004 due to acute adverse health reactions including lethal arrhythmias, stroke, vasoconstriction, and myocardial infarction. We report the case of a 29-year-old patient with acute myocardial infarction, secondary to coronary artery aneurysms and thrombosis who reported use of Ma Huang, Xenadrine(r)RFA, and Hydroxycut at recommended dosages for a combined total of approximately 2 years. Other causes of coronary artery aneurysm and hypercoagulability were ruled out. Our case exemplifies the long-term (as opposed to acute) danger of ephedrine products and the first case of coronary artery aneurysm associated with its use.

Coronary artery aneurysm related to percutaneous coronary intervention

We report a case of coronary artery aneurysm possibly related to percutaneous coronary intervention. The various clinical features, epidemiology and management of the condition are also discussed in detail.

Resection of Giant Coronary Artery Aneurysms in a Takayasu's Arteritis Patient

Takayasu's arteritis is a chronic vasculitis of unknown cause. Coronary arteries are affected in approximately 10% of cases with aneurysm formation being extremely rare. Coronary aneurysms (not related to Takayasu's arteritis) have been surgically treated with aneurysm resection and coronary bypass. We describe the case of a young woman found to have giant coronary artery aneurysms. She underwent resection of the aneurysms and short segment saphenous vein bypass to the right and left coronary arteries. In reviewing the literature, this seems to be the first case of Takayasu's arteritis related to coronary artery aneurysm treated surgically with a good result.

Progressive coronary artery aneurysm presenting as acute myocardial infarction

Coronary artery aneurysms (CAA) are commonly of atherosclerotic origin and asymptomatic. The natural history is uncertain given its uncommon occurrence and lack of longitudinal data. We report a case of CAA, presenting with myocardial infarction. Follow-up coronary angiography revealed enlargement of the CAA, suggesting a progressive nature.

Treatment of a coronary artery aneurysm by use of a covered stent graft - a case report

Coronary artery aneurysm is a rare finding at coronary angiography. Most coronary aneurysms remain asymptomatic. There is no consensus on its management; some advocate aggressive approach while others advocate conservative management. A case of coronary artery aneurysm successfully treated by implantation of a polytetrafluoroethylene-covered coronary stent is presented.

Economic evaluation of intravenous immune globulin therapy for Kawasaki syndrome

To determine the costs and clinical outcomes of three alternative treatments of the acute phase of Kawasaki syndrome: aspirin alone; low doses of intravenously administered immune globulin (IVIG-LD), 400 mg/kg per dose for 4 days; and high doses of intravenously administered immune globulin (IVIG-HD), 2.0 gm/kg for one dose. A model was developed that assumed the inclusion of 100 patients with acute Kawasaki syndrome in each treatment option. Costs were valued by using the Chedoke-McMaster Corporate Cost Model in 1992 Canadian dollars. Clinical outcome, based on the published literature, was measured by the prevalence of coronary artery dilation at 7 weeks from the diagnosis of Kawasaki syndrome. For every 100 patients with Kawasaki syndrome, the cost was reduced by $323,400 when aspirin therapy alone was changed to IVIG-HD therapy and 14 cases of coronary artery dilation were thereby prevented. When IVIG-HD therapy was compared with IVIG-LD therapy, the cost was reduced by $118,200 because two cases of coronary artery aneurysm were prevented. This latter result was sensitive to the duration of hospitalization, with IVIG-HD costing $8500 more for every 100 patients than IVIG-LD when it was assumed that both groups were hospitalized for 5 days, an unlikely occurrence. Treatment with IVIG-HD for Kawasaki syndrome is preferred because it results in both lower costs and lower rates of coronary artery dilation.

Multiple coronary artery aneurysms resulting in myocardial infarction in a young man: Treatment by double aorta-coronary saphenous vein bypass grafting

A 26-year-old Japanese man was treated for a transmural myocardial infarct caused by multiple aneurysms of the left main (LMC), left anterior descending (LAD), and the right coronary arteries (RCA). He underwent successful double aorta-coronary saphenous vein bypass grafting. The etiology of the aneurysm remains uncertain but an inflammatory origin is most probable. Review of the literature has indicated that this is the seventh case of coronary artery aneurysms without arteriovenous fistulas to be managed by grafting techniques with the saphenous vein. This experience has suggested that young patients presenting with anginal pain or myocardial infarction whould be carefully examined for coronary artery aneurysms. Since most of the patients developed myocardial infarction probably from thrombotic occlusion or embolism of the distal vessel, this lesion should be considered for surgery whenever anatomically feasible. Coronary artery reconstruction by grafting techniques, with or without resection of the aneurysm, is the treatment of choice. Although surgical treatment has provided good clinical amelioration to our patient as well as the patients previoulsy reported, a careful long-term follow-up should be continued for patients with multiple coronary artery aneurysms of doubtful origin.

Congenital localized coronary artery aneurysm without fistula: Report of a preoperatively diagnosed case

The case of a 55-year-old man with an apical coronary artery aneurysm is presented. The aneurysm was in communication with the dilated descending branch of the left coronary artery. There was no shunt. The physical findings suggested aneurysm with a fistula to the left ventricle. Coronary angiography demonstrated a localized aneurysm in the wall of the left ventricle, but no fistula. The aneurysm was surgically obliterated and the patient made a good recovery. This seems to be the first described case of coronary artery aneurysm without fistula to be fully diagnosed prior to surgical exploration.

Arteriosclerotic aneurysm of the cardiac coronary arteries: Report of a case

DESPITE the frequency and severity of involvement of the coronary arteries by atherosclerosis, true aneurysm formation resulting from this type of purely degenerative process is rare. The most recent comprehensive collection of cases of coronary artery aneurysms due to all causes is that of Packard and Wechsler. 1 Of thirty aneurysms discussed by them, seven were classified as “mycotic-embolic,” while twelve were considered to be definitely arteriosclerotic. In the remaining eleven cases insufficient data were presented to permit of their accurate classification. Since the report of Packard and Wechsler, additional single cases of atherosclerotic aneurysms have been presented by Eliasoph, 10 Cox and Christie, 11 Chiari, 12 Nagayo and Takahashi, 14 and Domenichini. 15 It is the purpose of this presentation to analyze the anatomic details of the aneurysms previously reported and to add an additional case.
