Large internal carotid - ophthalmic artery aneurysm with anterior optic pathway compression

Internal carotid-ophthalmic artery aneurysms (IC-OA) are rare lesions that become symptomatic only when they are quite advanced or when complicated by subarachnoid hemorrhage (SAH). We report a case of IC-OA aneurysm in a middle-aged lady who presented with relative afferent pupillary defect, optic disc pallor, significant color vision impairment, and rather preserved visual acuity in the right eye. Magnetic resonance imaging revealed a fuso-saccular aneurysm of the ophthalmic segment causing a pressure effect on the optic nerve. This case emphasizes the importance of careful ocular examination supplemented by neuro-imaging in cases of unexplained visual loss. Early detection can prevent a potentially fatal subarachnoid hemorrhage.

Hemodynamic Effect of the Last Finishing Coils in Packing the Aneurysm Neck.

Background: Using the finishing coils to densely pack the aneurysm neck is necessary. However, the exact hemodynamic effect of finishing coils in packing the aneurysm neck is unknown.Objective: To evaluate the hemodynamic characteristics of finishing coils to densely pack the aneurysm neck, using finite element method simulation.Methods: A computational study was performed based on a 44-year-old female patient with an unruptured wide-necked carotid-ophthalmic artery aneurysm treated with low-profile visualized intraluminal support stent-assisted coil embolization. Four computational fluid dynamics models including pre-treatment, post-stenting, common stent-assisted coil embolization (SACE), and common SACE with finishing coils were evaluated qualitatively and quantitatively.Results: Compared with the baseline of pretreatment model (100%), sac-averaged velocity in post-stenting, common SACE, and common SACE with finishing coil models decreased to 95.68%, 24.38%, and 13.20%, respectively; high flow volume (>0.1 m/s) around the aneurysm neck decreased to 92.19%, 9.59%, and 5.57%, respectively; and mean wall shear stress increased or decreased to 107%, 25.94%, and 23.89%, respectively.Conclusion: Finishing coils to densely pack the aneurysm neck can generate favorable hemodynamic modifications, which may decrease the recurrence.

Are hemodynamics of irregular small carotid-ophthalmic aneurysms different from those of regular ones and large aneurysms based on numerical simulation?

The purpose of this study is to investigate whether hemodynamics of unruptured, irregular small intracranial aneurysms (SIAs) are different from those of regular ones and large intracranial aneurysms (LIAs) in ophthalmic artery segment of internal carotid artery (ICA). Between April 2015 and June 2018, 106 carotid-ophthalmic artery aneurysms were retrospectively analyzed using 3D angiographic images and computational fluid dynamics (CFD). Three categories were stratified: regular SIAs, irregular SIAs and LIAs. Statistical comparisons of the differences in clinical, morphological and hemodynamic parameters among regular SIA, irregular SIA and LIA groups were performed by one-way analysis of variance (ANOVA) or Kruskal-Wallis Test. The median maximal height of regular SIA, irregular SIA and LIA were 3.03 (interquartile range: 2.49-4.22) mm, 4.59 (interquartile range: 3.86-5.32) mm and 11.06 (interquartile range: 9.28-13.69) mm, all P < 0.05). Low shear-stress area percentage (LSA%) and oscillatory shear index (OSI) of irregular SIAs and LIAs were not significantly different (P = 0.72, P = 0.27 respectively), and were significantly higher than those of regular SIAs (all P < 0.01). Wall shear stress (WSS) and normalized wall shear stress (NWSS) of irregular SIAs were significantly higher than those of LIAs (P < 0.01, P < 0.01 respectively), but lower than those of regular SIAs (P < 0.01, P < 0.01 respectively). Special unruptured irregular SIAs share a similarity of hemodynamic characteristics with LIAs in high LSA% and high OSI, and are different from regular SIAs in hemodynamics.

Application of micro-mirror in microsurgical clipping to the intracranial aneurysm

Objective To explore the value and disadvantage of micro-mirror in the intracranial aneurysm surgery. Methods Micro-mirror was used to assist microsurgical clipping to 36 intracranial aneurysms in 31 cases, of which 3 were carotid-ophthalmic artery aneurysms, 3 anterior choroidal artery aneurysm, 11 were posterior communicating artery aneurysms, 7 were middle cerebral artery aneurysms, 10 anterior communicating artery or anterior cerebral artery aneurysms, and the others were a posterior cerebral artery aneurysm and a posterior inferior cerebellar artery aneurysm. The micro-mirror was used before and after clipping to observe the anatomic features of necks hidden behind and medial to aneurysms, to visualize surrounding neurovascular structures, and to verify the optimal clipping position. Intraoperative indocyanine green fluorescein angiography confirmed the success of sufficient clipping. Results All aneurysms were clipped successfully. The parent arteries were occluded temporarily in 26 cases, and 9 aneurysms ruptured during the operation. Postoperative follow-up lasted from 2 weeks to 1 year. After operations digital subtraction angiography (DSA) were repeated in 10 cases, computed tomography angiography(CTA) in 21 cases, and no insufficient clipping or occlusion of parent arteries were revealed. Conclusions Micro-mirror assisting microsurgical clipping to the intracranial aneurysm is conducive to accurate clipping, and is a convenient and practical operation. Surgical management of ophthalmic artery aneurysms via minimally invasive frontolateral approach

Objective: To explore the minimally invasive techniques and outcome of carotid ophthalmic artery aneurysms clipping via a frontolateral approach. Methods: The clinical data of 95 patients with carotid ophthalmic artery aneurysms treated via frontolateral approach in the last 1.5 years in Beijing Tiantan Hospital and Beijing Anzhen Hospital were analyzed retrospectively.Before the operation, digital subtraction angiogram (DSA) was performed among all patients.The patients were divided into two groups by the lateral approach.According to preoperative classification, surgical characteristics and prognosis were summarized. Results: Ninety-five cases of ophthalmic aneurysms were divided into type Ⅰ of 44 cases (46.3%), type Ⅱ of 34 cases (35.7%) and type Ⅲ of 17cases (17.9%), according to the results of DSA.The diameter of aneurysm was <10 mm (35 cases), 10-25 mm (34 cases), and >25 mm (26 cases). In the 17 cases of subarachnoid hemorrhage (SAH), 8 cases were ruptured carotid-ophthalmic artery aneurysms.Among those 95 patients, 93 were clipped successfully, 2 was trapped.Multiple aneurysms in 5 cases were treated in one surgical session through the same approach.No aneurysm residual was found after postoperative CTA review.Ipsilateral vision of 3 cases were decline.Cerebral infarction was appeared in 9 cases.All the others had a good recovery. Conclusions: The carotid-ophthalmic artery aneurysms could be well exposed. Microsurgery through frontolateral approach has the advantages such as minimal invasion, less effect on the patients' look and simple procedure.The frontolateral approach is safe and effective in surgery for ophthalmic segment of the internal carotid artery aneurysms.

Unexpected angiographic and visual findings after clipping of a carotid-ophthalmic aneurysm

A 56-year-old woman underwent surgery for a ruptured carotid-ophthalmic artery aneurysm. Intraoperative visual inspection confirmed that the ophthalmic artery was left intact. She had no light perception on the operated side right after surgery. Angiography one week after surgery confirmed exclusion of the aneurysm, no filling of the proximal portion of the ophthalmic artery, and a very faint filling of its distal orbital part. She gradually recovered from this deficit and 9 months after surgery she is capable of counting fingers. At this time angiography displayed filling of all the portions of the ophthalmic artery, absence of recruitment of collateral blood supply, and exclusion of the aneurysm as before. Surgical manipulation seems to either have induced vasospasm or thrombosis of the ophthalmic artery. Regression of vasospasm or secondary recanalization of the thrombus without development of collateral blood supply may account for the gradual improvement of vision. A risk of monocular blindness is associated with the surgical treatment of para-clinoid aneurysms. Nevertheless, when it occurs, a perspective of recovery may exist if certain etiologies are involved.

Early Carotid Endarterectomy after Ischemic Stroke Improves Diffusion/Perfusion Mismatch on Magnetic Resonance Imaging: Report of Two Cases

OBJECTIVE AND IMPORTANCE The functional magnetic resonance imaging techniques of diffusion-weighted imaging and perfusion-weighted imaging allow for ultra-early detection of brain infarction and concomitant identification of blood flow abnormalities in surrounding regions, which may represent brain “at risk.” CLINICAL PRESENTATION We report two patients with acute ischemic stroke associated with ipsilateral high-grade carotid stenosis. The first patient, a 64-year-old woman with a remote history of ischemic stroke and a vertebral artery aneurysm, presented with worsening of her preexisting right hemiparesis. The second patient, another 64-year-old woman with known multiple intracranial aneurysms and bilateral high-grade internal carotid artery stenosis, was admitted for the elective microsurgical clipping of an enlarging giant left carotid-ophthalmic artery aneurysm. Postoperatively, she developed right hemiparesis and mild aphasia. Both patients showed progressive worsening of their neurological deficits in the setting of small or undetected diffusion-weighted imaging abnormalities and large perfusion-weighted imaging defects. INTERVENTION After prompt carotid endarterectomy, symptoms in both patients resolved or improved. Follow-up magnetic resonance imaging scans demonstrated resolution or significant improvement in the perfusion abnormalities in both patients. CONCLUSION Carotid endarterectomy in the setting of diffusion-weighted/perfusion-weighted imaging mismatch can lead to improvement in cerebral perfusion as evidenced by resolution of the perfusion-weighted imaging lesion. Diffusion/perfusion magnetic resonance imaging may be useful in identifying patients with severe neurological deficits but without large territories of infarction who may safely undergo early surgical revascularization.

Two surgical cases of internal carotid–ophthalmic artery aneurysms: Special reference to the usefulness of three-dimensional CT angiography

Three-dimensional computerized tomography angiography (3D-CTA) is a noninvasive tool for the diagnosis of cerebral aneurysms. 3D-CTA is helpful in the evaluation of the configuration of aneurysms and their surrounding vessels and anatomical structures. The aim of this study is to assess the usefulness of 3D-CTA for patients with unruptured internal carotid-ophthalmic artery aneurysms. We pre-operatively obtained surgical simulation images using 3D-CTA and 3D reconstruction and then compared them with magnetic resonance angiography (MRA), conventional cerebral angiography and operative findings in the patients. Two patients with unruptured internal carotid-ophthalmic artery aneurysm were selected. These patients underwent direct neck clipping after the optic canal was unroofed through a combined epidural-subdural approach. The cerebral aneurysm was detected by 3D-CTA, MRA and conventional cerebral angiography in each case. Only by 3D-CTA, however, could we easily detect the relationships among the aneurysm neck, ophthalmic artery and optic canal. Based on this information, direct clipping operations were performed safely without any complications. 3D-CTA is an excellent noninvasive diagnostic method not only for detecting cerebral aneurysms, but also for evaluating the relationships between the aneurysms and surrounding structures.

Preliminary Results on the Management of Unruptured Intracranial Aneurysms with Magnetic Resonance Angiography and Computed Tomographic Angiography

Objective The goal was to assess the capability of magnetic resonance angiography (MRA) and computed tomographic angiography (CTA) to replace catheter angiography in the evaluation of unruptured intracranial aneurysms. Methods A prospective evaluation of a 1-year period included all patients suspected of harboring unruptured intracranial aneurysms at a single institution. All patients underwent magnetic resonance imaging, MRA, and CTA, for comparison with intraoperative findings or results from catheter angiography. Both MRA and CTA now provide submillimeter resolution of vascular structures, with accurate detection of intracranial aneurysms of a diameter greater than or equal to 3 mm. This resolution calls into question the universal need for catheter angiography in the care of patients with suspected intracranial aneurysms. When the catheter angiography can be avoided, radiological costs can be reduced by as much as two-thirds while eliminating the risk of arterial injury and stroke. Results Excellent visualization of the intracranial vasculature was provided by both MRA and CTA. No vascular lesion was detected at surgery or by formal angiography that was not visualized by noninvasive angiographic techniques. The three-dimensional anatomy of the aneurysm complex (unavailable with catheter angiography) was well depicted by both MRA and CTA. CTA was unique in its capacity to display the relationship of vascular structures to bone, information that is invaluable for planning operative strategies for lesions such as carotidophthalmic artery aneurysms. Additionally, acquisition of CTA images was very rapid, with a scanning time of less than 1 minute. Both MRA and CTA allowed for retrospective manipulation of data into an infinite number of views, including views that paralleled those encountered through the operative microscope. Additionally, both MRA and CTA can depict the internal anatomy of aneurysms, an ability not possessed by intra-arterial angiography. This ability alerts the surgeon to possible intraoperative risks, such as plaque in the lumen of an aneurysm or calcium within the walls of the arteries. Conclusion Both MRA and CTA provide several advantages over digital subtraction angiography, in addition to reduced costs and avoidance of arterial injury and stroke. These include retrospective manipulation of data in a 360-degree format, visualization of the internal anatomy of arteries and aneurysms, three-dimensional depiction of anatomy, and rapid data acquisition. Preliminary data and a review of the literature suggest that MRA, when used in concert with CTA, can replace catheter angiography in the assessment of select patients harboring unruptured intracranial aneurysms. Although no firm conclusions or generalizations can be drawn from this small cohort of patients, it is hoped that this report will stimulate interest and further study at other institutions.

Asymptomatic, Unruptured Carotid-Ophthalmic Artery Aneurysms: Angiographical Differentiation of Each Type, Operative Results, and Indications

Background Some types of carotid-ophthalmic artery aneurysms are still difficult to clip successfully because their exposure requires opening the cavernous sinus and/or retracting the optic nerve. It is useful to know the complications and to determine the type of aneurysm preoperatively for the management of carotid-ophthalmic artery aneurysms. Methods The operative results in 15 patients with asymptomatic unruptured carotid-ophthalmic artery aneurysms were surveyed. The aneurysms were small in all the patients, and they underwent direct operation. Four patients presented with other ruptured aneurysms, four with other diseases (infarction, trauma, or pituitary adenoma), and seven were evaluated with magnetic resonance angiography for symptoms such as vertigo or headache. Among them, five had carotid cave aneurysms and one had paraclinoid aneurysm. Results Neck clipping was performed in 13 patients. Postoperatively, ipsilateral visual loss was encountered in one patient, and ipsilateral visual field defect was encountered in three patients. The visual field defect was lower nasal quadrant hemianopsia in two patients and lower hemianopsia in one patient. The cause of this complication was suspected to be retraction and/or the heat of the drill near the optic nerve. It seemed to be possible to distinguish the carotid cave or the paraclinoid aneurysm from the other carotid-ophthalmic aneurysms using carotid angiography preoperatively. Conclusion When direct operation is performed for a carotid-ophthalmic artery aneurysm, care must be taken to avoid optic nerve injury caused by the retraction and/or the heat of the drill.

Surgery for paraclinoidal carotid artery aneurysms.

Aneurysms arising from the proximal carotid artery between the roof of the cavernous sinus and the origin of the posterior communicating artery pose conceptual and technical surgical problems with regard to acquisition of proximal control and safe intracranial exposure. Over the past 3 1/2 years, 89 patients with paraclinoidal aneurysms have been treated at the University of Texas Southwestern Medical Center. Thirty-nine (44%) of these patients presented with subarachnoid hemorrhage. A total of 149 aneurysms and six arteriovenous malformations have been identified in this patient group such that 38 (43%) of the patients suffered multiple vascular anomalies. Temporary artery occlusion has been employed during operation in 48 cases (54%), permanent carotid artery occlusion in four (4%), and hypothermic circulatory arrest in two (2%). Twenty-two patients harbored giant aneurysms, seven of which had ruptured. Outcome was considered good in 77 patients (86.5%), fair in eight (9%), and poor in three (3%); one patient died. This concentrated experience permitted a practical anatomical grouping of aneurysms into three types: carotid-ophthalmic artery aneurysms with a superior or superomedial projection (44 cases); superior hypophyseal aneurysms with a medial or inferomedial projection (26 cases); and proximal posterior carotid artery wall aneurysms projecting posteriorly or posterolaterally (19 cases). Despite the fact that paraclinoidal aneurysms often disobey the traditional teachings of aneurysm development, having no vessel of origin or clear hemodynamic cause, this practical grouping has allowed individualized and focused operative approaches unique to each aneurysm projection with good visual function and outcome in most patients.

The removal of the anterior clinoid process is required for clipping of cavernous sinus aneurysms internal carotid-ophthalmic artery aneurysms (IC-OP AN) and lower positioned internal carotid-posterior communicating artery aneurysms (IC-PC AN). The anterior clinoid process has tradionally been removed introdurally after exposure of the optic nerve and ICA. Recently the extradural removal of the anterior clinoid process and roof of the optic nerve has been reported. It is a matter for discussion whether to remove the anterior clinoid process and the roof of the optic nerve intradurally or extradurally.In this paper, we have identified the indication and method of removing the ala minor when operating on cavernous sinus aneurysms, IC-OP aneurysms and IC-PC aneurysms.The proximal neck of lower positioned IC-PC aneurysms; was lower than the anterior clinoid process. For clipping of lower positioned IC-PC aneurysms, removal of the anterior clinoid process was required. It is very difficult to tell the condition of the anterior clinoidal bone from pre-operative angiography. When operating for IC-PC aneurysms, we usually don't open the neck for the spare of the carotid artery. But carotid artery preparation in the neck is recommended when preoperative angiograms show an extremely lower positioned IC-PC aneurysm. As the neck of the aneurysm and anterior clinoid process is near, intradural removal of the anterior clinoid process is recommended. Extradural removal of the anterior clinoid process might cause the rupture of lower positioned IC-PC aneurysms, and it is difficult to control bleeding from the extradural space.When operating the IC-OP aneurysms, intradural removal of the anterior clinoid process and unroofing of the optic canal was performed after the preparation of the carotid artery in the neck. Intradural removal of the anterior clinoid process is advisalble when operating for IC-OP aneurysms because it is very important to determine the course of the ophthalmic artery. The ophthalmic artery usually runs beneath the optic nerve.When operating for cavernous sinus aneurysms, extensive removal of the ala minormbone is required. Intradural procedure restricts the space, so it is usually difficult to remove the ala minor bone extensively. It is advisable to remove the anterior clinoid process and the roof of the optic canal extradullary.With basilar tip aneurysms, extradural removal of the ala minor bone is useful because this procedure allows wide retraction of the optic nerve and the internal carotid artery.In conclusion, removal of the ala minor bone was required when operating for cavernous sinus aneurysms, IC-OP aneurysms and lower positioned IC-PC aneurysms. Intradural removal of the ala minor bone is preferable for clipping of IC-OP aneurysms and lower positioned IC-PC aneurysms. Extradural removal of the ala minor bone is better when operating for cavernous sinus aneurysms.

Unilateral proptosis secondary to partially thrombosed giant carotid ophthalmic artery aneurysm.

<h3>Abstract</h3> The COVID-19 pandemic has created an urgent need for models that can project epidemic trends, explore intervention scenarios, and estimate resource needs. Here we describe the methodology of Covasim (COVID-19 Agent-based Simulator), an open-source model developed to help address these questions. Covasim includes country-specific demographic information on age structure and population size; realistic transmission networks in different social layers, including households, schools, workplaces, long-term care facilities, and communities; age-specific disease outcomes; and intrahost viral dynamics, including viral-load-based transmissibility. Covasim also supports an extensive set of interventions, including non-pharmaceutical interventions, such as physical distancing and protective equipment; pharmaceutical interventions, including vaccination; and testing interventions, such as symptomatic and asymptomatic testing, isolation, contact tracing, and quarantine. These interventions can incorporate the effects of delays, loss-to-follow-up, micro-targeting, and other factors. Implemented in pure Python, Covasim has been designed with equal emphasis on performance, ease of use, and flexibility: realistic and highly customized scenarios can be run on a standard laptop in under a minute. In collaboration with local health agencies and policymakers, Covasim has already been applied to examine epidemic dynamics and inform policy decisions in more than a dozen countries in Africa, Asia-Pacific, Europe, and North America.

Management of giant intracranial ICA aneurysms with combined extracranial-intracranial anastomosis and endovascular occlusion

Nine patients with giant internal carotid artery (ICA) aneurysms (greater than 2.5 cm in diameter) were subjected to a combined extracranial-intracranial (EC-IC) bypass procedure and endovascular ICA occlusion during 1987 and 1988. The procedures were performed under one anesthetic. In all cases the collateral circulation had been judged insufficient on the basis of a strict preoperative testing protocol including: cerebral panangiography, electroencephalography, somatosensory potential recording, and cerebral blood flow monitoring during manual compression of the ICA in the neck. There were four intracavernous ICA aneurysms, four carotid-ophthalmic artery aneurysms, and one supraclinoid ICA aneurysm. All patients showed symptoms and signs of compression of the surrounding nervous structures. In the five cases of intradural lesions, the artery was occluded at the level of the aneurysm neck, so the ophthalmic artery had to be occluded. There was, nevertheless, no case of worsening of vision following surgery, and all nine patients showed significant improvement following the combined procedure. A combined EC-IC bypass procedure and endovascular ICA occlusion allows for immediate verification of the surgical results and appears to be a worthwhile method for treating giant intracranial aneurysms.

Treatment of a giant carotid ophthalmic artery aneurysm by intravascular balloon embolization therapy

This report describes the successful treatment by detachable balloon embolization therapy of a giant aneurysm arising at the left carotid-ophthalmic artery junction. Two previous surgical attempts to clip the aneurysm were unsuccessful and the aneurysm continued to enlarge leading to complete loss of light perception. After the placement of two detachable balloons within the aneurysm, there was thrombosis of the aneurysm with diminished mass effect. After 2 months the patient regained light perception and was able to count fingers in the nasal hemifield of her right eye. Detachable balloon embolization therapy may offer a form of alternative treatment for surgically difficult and inaccessible intracranial aneurysms.
