Cytochrome P450-mediated cardiovascular drug interactions

Introduction: There are numerous drug–drug interactions (DDIs) related to cardiovascular medications and many of these are mediated via the cytochrome P450 (CYP) system. Some of these may lead to serious adverse events and it is, therefore, essential that clinicians are aware of the important interactions that occur.Areas covered: An extensive literature search was performed to analyze the CYP-mediated cardiovascular DDIs that lead to a loss of efficacy or potential toxicity. Cardiovascular drugs may be victims or act as perpetrators of DDIs. The paper analyzes CYP-mediated drug interactions concerning anticoagulants, antiplatelet agents, antiarrhythmics, β-blockers, calcium antagonists, antihypertensive medications, lipid-lowering drugs and oral antidiabetic agents.Expert opinion: Cardiovascular DDIs involving the CYP system are numerous. Additionally, the spectrum of drugs prescribed is constantly changing, particularly with cardiovascular diseases and it is not necessarily the case that drugs that had shown safety earlier will always show safety. Clinicians are encouraged to develop their knowledge of CYP-mediated DDIs so that they can choose safe drug combination regimens, adjust drug dosages appropriately and conduct therapeutic drug monitoring for drugs with narrow therapeutic indices.

Cardiovascular drug interactions with tyrosine kinase inhibitors

Summary Imatinib mesylate, a selective inhibitor of tyrosine kinases, has excellent efficacy in the treatment of chronic myeloid leukaemia (CML) and gastrointestinal stromal tumour (GIST). Inducing durable responses and achieving prolonged survival, it has become the standard of care for the treatment of these diseases. It has opened the way to the development of additional tyrosine kinase inhibitors (TKIs), including sunitinib, nilotinib, dasatinib and sorafenib, all indicated for the treatment of various haematological malignancies and solid tumours. TKIs are prescribed for prolonged periods and are often taken by patients with – notably cardiovascular – comorbidities. Hence TKIs are regularly co-administered with cardiovascular drugs, with a considerable risk of potentially harmful drug-drug interactions due to the large number of agents used in combination. However, this aspect has received limited attention so far, and a comprehensive review of the published data on this important topic has been lacking. We review here the available data and pharmacological mechanisms of interactions between commonly prescribed cardiovascular drugs and the TKIs marketed at present. Regular updating of the literature on this topic will be mandatory, as will the prospective reporting of unexpected clinical observations, given the fact that these drugs have been only recently marketed.

Cytochrome P (CYP) 450 is a superfamily of hemoproteins that play an important role in the metabolism of steroid hormones, fatty acids, and many medications. Many agents used for management of cardiovascular diseases are substrates, inhibitors, or inducers of CYP450 enzymes.When two agents that are substrates, inhibitors, or inducers of CYP450 are administered together, drug interactions with significant clinical consequences may occur. This review discusses CYP450-mediated cardiovascular drug interactions as well as noncardiovascular drug interactions that produced significant cardiovascular side effects. The principles in predicting drug interactions are also discussed.

Uninformed Complementary and Alternative Supplement Use: A Risky Behavior for Cardiovascular Patients

Growing use of complementary and alternative supplements (CAS) is of concern because of the potential for herb—drug interaction among cardiovascular patients. Literary searches were conducted on PubMed to identify reports of extent and purpose of CAS use, disclosure of use by patients, physician knowledge, and possible drug—CAS interactions for cardiovascular patients. Additional published studies were located through the Web sites of various organizations. Further searches of case reports, case series, controlled trials, and laboratory evidence were performed for each of the top 10 CAS and their possible cardiovascular drug interactions. More research is needed to understand supplement—drug interactions, particularly in terms of how this potentially affects patients taking cardiovascular drugs. With this lack of research and clarity on supplement—dug interactions and the underreporting of CAS use by many patients, physician education is also in need of improvement.

An increasing number of clinical studies address interactions of cardiovascular drugs in terms of both their importance in the treatment of disease and their potential to cause drug-induced toxicity. Several books and reviews are devoted to extensive listings of both observed and theoretical drug interactions [1–4]. Such listings often include extrapolation of animal data, anecdotal case reports, and interactions of questionable importance. Consequently, their literal use could result in withholding therapy or overcomplicating therapeutic decisions. Some drug interactions are critical for optimum patient care, such as combined use of drugs for their additive or synergistic effects, while others, although academically interesting, have little clinical relevance. Though the sequelae of cardiovascular drug interactions are often serious, they often are not recognized (1-5). Many patients with cardiovascular diseases are desperately ill, and the manifestations of drug toxicity may be difficult to distinguish from those ofthe primary disease. For example, arrhythmias due to digitalis toxicity may be impossible to differentiate from those that are disease induced (6-9). Simi­ larly, arrhythmias may be a toxic manifestation of antiarrhythmic drugs, and may be misinterpreted as lack of efficacy of the drug (10-14). Avoidance of and appropri­ ate response to drug interactions requires understanding of the pharmacology of the drugs and the pathophysiology of the diseases in which they are used.
