An Update on Physical Activity Research among Children in Hong Kong: A Scoping Review.

Similar to their Western counterparts, children in Hong Kong generally fail to reach the recommended levels of physical activity (PA). As an ultra-dense metropolis, Hong Kong is different from most Western cities. It is therefore important to update and appraise previous PA research in order to inform future PA promotion for Hong Kong children. Using a scoping review, the current study aimed to evaluate PA research among preschool and school-aged children in Hong Kong aged 3–12 years old who are at a critical development stage. Literature was searched from four English databases: Medline via EBSCOhost, SPORTDiscus, ERIC and PsycINFO via ProQuest; and three Chinese databases: CNKI, CQVIP and WAN-FANG. PA research among Hong Kong children published from 1 January 1997 to the searching date, 31 March 2020 was included. A total of 63 studies were identified, with the majority of studies focused on school-aged children as compared to preschoolers, adopted a cross-sectional design, using self-reported PA measures, and with small to medium sample sizes. We classified eligible studies into five main categories: (a) Health benefits of PA (k = 12). Consistent evidence on the health benefits of skeletal and cardiovascular capacity, quality of life, cognitive function, and sleep quality was revealed. However, inconsistent evidence was found on the benefits of weight-related indicators and academic performance. (b) Patterns of PA (k = 12). There is a general pattern of low levels of PA among Hong Kong children, in particular girls and children with special educational needs. (c) Measures of PA and related constructs (k = 11). The Chinese versions of self-reported measures of PA, PA-related social environment, and PA-related psychological constructs showed acceptable reliabilities and validities. (d) Correlates of PA (k = 18). The correlates of PA include physical environment, social environment, physical factors, psychological factors, and multiple correlates, which is in line with the social-ecological model. (e) Interventions for promoting PA (k = 10). PA interventions among Hong Kong children were conducted for healthy children, children with special educational needs, and children with cancer. Overall, there is a growing volume of PA research among children in Hong Kong in the recent decade. Yet, there is a lack of high-quality research for measuring, understanding, and promoting PA among Hong Kong children. It is highly recommended that future PA research among children should pay more attention on the preschoolers, adopting robust research design (e.g., randomized controlled trials), recruiting large and representative sample, and collecting device-assessed data.

Personalised health education against health damage of COVID-19 epidemic in the elderly Hungarian population (PROACTIVE-19): protocol of an adaptive randomised controlled clinical trial

BackgroundEarly reports indicate that COVID-19 may require intensive care unit (ICU) admission in 5–26% and overall mortality can rise to 11% of the recognised cases, particularly affecting the elderly. There is a lack of evidence-based targeted pharmacological therapy for its prevention and treatment.We aim to compare the effects of a World Health Organization recommendation-based education and a personalised complex preventive lifestyle intervention package (based on the same WHO recommendation) on the outcomes of the COVID-19.MethodsPROACTIVE-19 is a pragmatic, randomised controlled clinical trial with adaptive “sample size re-estimation” design. Hungarian population over the age of 60 years without confirmed COVID-19 will be approached to participate in a telephone health assessment and lifestyle counselling voluntarily. Volunteers will be randomised into two groups: (A) general health education and (B) personalised health education. Participants will go through questioning and recommendation in 5 fields: (1) mental health, (2) smoking habits, (3) physical activity, (4) dietary habits, and (5) alcohol consumption. Both groups A and B will receive the same line of questioning to assess habits concerning these topics. Assessment will be done weekly during the first month, every second week in the second month, then monthly. The composite primary endpoint will include the rate of ICU admission, hospital admission (longer than 48 h), and mortality in COVID-19-positive cases. The estimated sample size is 3788 subjects per study arm. The planned duration of the follow-up is a minimum of 1 year.DiscussionThese interventions may boost the body’s cardiovascular and pulmonary reserve capacities, leading to improved resistance against the damage caused by COVID-19. Consequently, lifestyle changes can reduce the incidence of life-threatening conditions and attenuate the detrimental effects of the pandemic seriously affecting the older population.Trial registrationThe study has been approved by the Scientific and Research Ethics Committee of the Hungarian Medical Research Council (IV/2428- 2 /2020/EKU) and has been registered at clinicaltrials.gov (NCT04321928) on 25 March 2020.

Determinants of the Cardiovascular Capacity of Amateur Long-Distance Skiers during the Transition Period.

The aim of this study was to identify determinants of the cardiovascular capacity of 16 male amateur long-distance skiers during the transition period. These factors can vary from amateur marathon skiers, who represent a sort of midpoint between inactive people and professional athletes. Cardiovascular capacity depends mainly on the volume and intensity of the training, which are different between these groups. Finding the factors affecting heart condition of amateur athletes can be an important element in their health care and can help the athletes to achieve their full performance potential. Therefore, ergospirometric and hematological tests were performed. As a result, predictors for volume oxygen uptake were determined using a regression model, which included the following variables: the percentage of monocytes (p = 0.031), the concentration of sodium (p = 0.004), and total calcium (p = 0.03). All these parameters negatively affected VO2 max. Biochemical and physiological monitoring of amateur athletes can help to protect their health and prepare them properly for their training. The growing popularity of long-distance competitions among middle-aged amateur athletes and the lack of guidance on how to assess their health indicate the need for further research.

Cardiac rehabilitation meta-analysis of trials in patients with coronary heart disease using individual participant data (CaReMATCH): Project protocol.

BackgroundExercise-based cardiac rehabilitation (CR) has long been a cornerstone in the secondary prevention of coronary heart disease (CHD). Despite meta-analyses of randomised trials demonstrating a positive impact of CR on cardiovascular mortality, hospitalisation, exercise capacity and health related quality of life, the impact of CR on all-cause mortality remains uncertain, especially in the context of contemporary clinical practice. This CR meta-analysis of trials in patients with coronary heart disease using individual participant data (IPD) (CaReMATCH) seeks to (1) provide definitive estimates of the effectiveness of CR in terms of all-cause mortality, cardiovascular mortality, hospitalisation and health-related quality of life, and (2) determine the influence of individual patient characteristics (e.g. age, sex, risk factors) on the effectiveness of CR to inform a personalised CR-approach.MethodsRandomised controlled trials will be identified that were performed in the last decade, to ensure that CR was performed in combination with contemporary medical care (2010–2020). For our first aim, outcomes of interest include all cause- and CVD-related mortality and hospitalisations. To answer our second research question, we will collect data on exercise capacity, health-related quality of life, and patient baseline demographic and clinical data. Original IPD will be requested from the authors of all eligible trials; we will check original data and compile a master dataset. IPD meta-analyses will be conducted using a one-step meta-analysis approach where the IPD from all studies are modelled simultaneously whilst accounting for the clustering of participants within studies.DiscussionFindings from CaReMATCH will inform future (inter)national clinical and policy decision-making on the (personalised) application of exercise-based CR for patients with CHD.

Significance of Neutrophil/Lymphocyte Ratio with Coronary Artery Calcium Score Detected by Multislice Computed Tomography as A Predictive Marker of Ischemia in Type 2 Diabetes Mellitus Patient

Neutrophil-to-lymphocyte ratio (NLR) is an inexpensive, easy to obtain, widely available marker of inflammation. T2DM and inflammatory markers including NLR were shown to have impact on coronary artery disease (CAD) or carotid artery atherosclerosis. to examine the affiliation and essentialness of Neutrophil/Lymphocyte Ratio (NLR) with Coronary Artery Calcium Score (CACS) and Obstructive Coronary Artery Disease (CAD) identified by Multislice Computed Tomography (MSCT) Angiography in Type 2 Diabetes Mellitus (T2DM) patients. The aim of this study was to assess changes in the cardiovascular capacity and body mass composition of male cross-country skiers in the preparation period of their macrocycle. Variables were analyzed using basic descriptive statistics: mean and standard deviation (SD). To compare the results from both measurements (initial and final) the paired Wilcoxon test was used. A statistically significant increase was noted in maximum oxygen uptake and maximum minute ventilation, and a decrease in body fat content, maximum lactate concentration and lactate threshold, and heart rate on anaerobic threshold. Research indicated that in the amateur group increases similar to those in top competitors were achieved in the parameters tested, but the initial level was often significantly lower.

An Epigenetic Signature for Within-Generational Plasticity of a Reef Fish to Ocean Warming

Elevated temperature can have detrimental effects on the physiological performance of many marine organisms. However, phenotypic plasticity may enable some populations to maintain their performance under thermal stress. Two longitudinally separated populations of the coral reef fish, Acanthochromis polyacanthus from the Great Barrier Reef have shown differing capacities for thermal plasticity – the southernmost Heron Island population restored aerobic scope within one generation at a higher temperature, whereas the northernmost Palm Island population restored aerobic scope only when two generations were exposed to warmer conditions. We recently discovered an epigenetic signature associated with transgenerational plasticity in the Palm Island population. Here, we aimed to determine if epigenetic changes are also associated with the within-generational plasticity observed in the Heron Island population and, if so, how this epigenetic signature compares to the Palm Island transgenerational epigenome. By sequencing and analyzing the genome-wide DNA methylome of fish reared at control (+0°C) or elevated temperatures (+1.5 and +3°C) since early life, we identified 480 differentially methylated genomic regions and 372 adjacent protein-coding genes associated with within-generational plasticity in the Heron Island population. Functions related to insulin, cardiovascular capacity, development, and heat response were significantly enriched in differentially methylated genes, suggesting that these functions are the core mechanisms for within-generational restoration of aerobic scope. Comparison to the differentially methylated genes identified from F2 Palm Island population revealed little overlap of genes and enriched functions, indicating that distinct genetic toolkits may be used for within- and between-generational plasticity to ocean warming in the same species from different latitudes.

Effects of incentive spirometry on cardiopulmonary parameters, functional capacity and glycemic control in patients with Type 2 diabetes.

Background:Patients with Type 2 diabetes mellitus (T2DM) suffer cardiopulmonary impairment and may present with weakness of the inspiratory muscles.Objective:This study was designed to determine the effects of incentive spirometry (IS) on selected cardiopulmonary parameters, functional capacity and glycemic control in patients with T2DM.Methods:Fifty-nine participants (25 males and 34 females) recruited from the out-patient clinic of the Department of Medicine of two hospitals in Lagos State, Nigeria, who were randomly assigned into two groups, completed the study. In addition to the medical management of T2DM, IS group received incentive spirometry while control group continued with the medical management of T2DM alone. Selected cardiovascular parameters, pulmonary parameters, functional capacity (using 6-min walk test) and fasting blood glucose level were assessed at baseline and at the end of eight weeks intervention period. Data were analyzed using the Statistical Package for Social Sciences (SPSS Version 21). Level of significance was set at .Results:There were statistically significant improvements in all the cardiovascular parameters () of IS group except systolic blood pressure. There were significant changes in all the pulmonary parameters, functional capacity and glycemic control () of IS group while there was none in control group. There were significant differences between the mean changes of various selected outcome measures of the two groups () except for diastolic blood pressure and blood glucose level.Conclusion:IS had positive effects in improving cardiopulmonary function, functional capacity and glycemic control in patients with T2DM.

Physical Fitness and Health-Related Physical Activity Programs in People With Down Syndrome: A Systematic Review

Objective: Down Syndrome (DS) is the most common genetic disease in people with intellectual disability, with a prevalence of 1 in 800-1000 live births. Youth and adolescents with DS are a unique population in terms of health-related physical fitness factors. Methods: In this review study, to investigate the physical fitness factors and physical activity programs for youth and adolescents with DS, a search was conducted in international and national databases such as EBSCO, Medline, PubMed, Elsevier, Google Scholar and SID among studies conducted during 1996-2016 using following keywords in Persian and English: Physical fitness, physical activity, Down syndrome, rehabilitation, intellectual disability, muscular strength, muscular endurance, aerobic fitness, balance, agility and flexibility. Results: Initial search yielded 130 articles. After removing duplicates, 35 were remained for the review. The studies indicated lower cardiorespiratory capacity and physical fitness in people with DS than in their normal peers in strength, endurance, balance and agility as well as increased subcutaneous fat and body fat distribution. Conclusion: Adolescents and youth with DS have low cardiovascular and muscular capacity and physical fitness, overweight/obesity, and reduces health-related physical activities. Future studies should focus on strength testing and training protocols, methods for determining physical activity levels, and practical interventions to increase physical activities in DS patients.

Circadian modulation of the cardiac proteome underpins differential adaptation to morning and evening exercise training: an LC-MS/MS analysis.

All living beings on earth are influenced by the circadian rhythm, the rising and the setting of the sun. The ubiquitous effect of exercise is widely believed to maximize health benefits but has not been formally investigated for cardiac responses in the exercise-induced circadian rhythms. We hypothesized that the exercise-related proteome is differentially influenced by circadian rhythm and analyzed the differences between the effects of morning and evening exercise. Twenty-four Sprague-Dawley rats were randomly divided into four groups (n = 6 per group): morning control, morning exercise, evening control, and evening exercise groups. The exercise groups were subjected to 12-week treadmill exercise (5days/week) performed either during daytime or nighttime. After 12weeks, the physiological characteristics (e.g., body weight, heart weight, visceral fat, and blood metabolites), cardiovascular capacity (ejection fraction (%) and fractional shortening (%)), circadian gene expression levels (clock, ball1, per1, per2, cry1, and cry2), and the proteomic data were obtained and subjected to univariate and multivariate analysis. The mRNA levels of per1 and cry2 increased in the evening group compared with those in the morning group. We also found that per2 decreased and cry2 increased in the evening exercise groups. The evening exercise groups showed more decreased triacylglycerides and increased blood insulin levels than the morning exercise group. The principal component analysis, partial least squares discriminant analysis, and orthogonal partial least squares discriminant analysis indicated that the circadian rhythm differently influenced the protein networks of the exercise groups. In the morning exercise group, the transcription-translation feedback loop (TTFL) (clock, per1, per2, cry1, and cry2) formed a protein-protein interaction network with Nme2, Hint1, Ddt, Ndufb8, Ldha, and Eef1a2. In contrast, the TTFL group appeared close to Maoa, Hist2h4, and Macrod1 in the evening exercise group. Interestingly, the evening exercise group decreased the mRNA level of per2 but not per1. Per1 and Per2 are known to transport Cry1 and Cry2 into the nucleus. Taken together, we summarized the characteristics of enriched proteins in the aspect of their molecular function, cellular component, and biological process. Our results might provide a better understanding of the circadian effect on exercise-related proteins.

Compromised thermal tolerance of cardiovascular capacity in upstream migrating Arctic char and brown trout-are hot summers threatening migrating salmonids?

Heat waves are threatening fish around the world, leading sometimes to mass mortality events. One crucial function of fish failing in high temperatures is oxygen delivery capacity, i.e. cardiovascular function. For anadromous salmonids, increased temperature could be especially detrimental during upstream migration since they need efficiently working oxygen delivery system in order to cross the river rapids to reach upstream areas. The migration also occurs during summer and early autumn exposing salmonids to peak water temperatures, and in shallow rivers there is little availability for thermal refuges as compared to thermally stratified coastal and lake habitats. In order to shed light on the mechanisms underpinning the capacity of migrating fish to face high environmental temperatures, we applied a physiological and molecular approach measuring cardiovascular capacities of migrating and resident Arctic char (Salvelinus alpinus) and brown trout (Salmo trutta) in Northern Norway. The maximum cardiovascular capacity of migrating fish was significantly lower compared to the resident conspecifics. The onset of cardiac impairment started only 2°C higher than river temperature, meaning that even a small increase in water temperature may already compromise cardiac function. The migrating fish were also under significant cellular stress, expressing increased level of cardiac heat shock proteins. We consider these findings highly valuable when addressing climate change effect on migrating fish and encourage taking action in riverine habitat conservation policies. The significant differences in upper thermal tolerance of resident and migrating fish could also lead changes in population dynamics, which should be taken into account in future conservation plans.

Early Mobilization in an Oncology Intensive Care Unit

Introduction: Quality of life and functionality of critically ill cancer patients are significantly affected after ICU admission, with changes due to immobility, hemodynamic instability, muscular atrophy, cognitive deficits, psychological difficulties, decreased cardiovascular functional capacity, neurological deleterious effects on the respiratory system. The physiotherapeutic intervention is effective in preventing and reducing the deleterious dysfunctions of immobility, as well as improving the quality of life of the patient during his / her stay in the ICU. Objective: To carry out a systematic review of literature seeking the benefits of early mobilization in intensive care patients. Methodology: Refers to a systematic literature review, approaching the PRISMA methodology, with two independent examiners who analyzed the quality of the study. We selected as inclusion criteria articles in Portuguese, English and Spanish that refer to the topic of early mobilization in intensive care oncology. Documents that do not associate with the proposed theme were excluded from the search. The articles were searched in the databases: VHL, PUBMED, COCHRANE, PEDRO and EBSCO. Results: For the elaboration of the study, 66 articles were found in the total, where only 8 articles were used because they refer to the use of early mobilization in intensive oncologic therapy. Final considerations: Early mobilization in the ICU is essential in the intervention of the multidisciplinary team on the cognitive, physical and functional aspects along with the kinesiotherapeutic progressivity adequate to the individual needs of each patient, providing significant benefits in the prevention of dysfunctions caused by the immobilism syndrome, resulting in the best quality of life for the patient.

Spinal Cord Injury Impairs Cardiovascular Capacity in Elite Wheelchair Rugby Athletes.

To examine differences in heart rate (HR) responses during international wheelchair rugby competition between athletes with and without a cervical spinal cord injury (SCI) and across standardized sport classifications. Observational study. The 2015 Parapan American Games wheelchair rugby competition. Forty-three male athletes (31 ± 8 years) with a cervical SCI (n = 32) or tetraequivalent impairment (non-SCI, n = 11). Average and peak HR (HRavg and HRpeak, respectively). To characterize HR responses in accordance with an athletes' International Wheelchair Rugby Federation (IWRF) classification, we separated athletes into 3 groups: group I (IWRF classification 0.5-1.5, n = 15); group II (IWRF classification 2.0, n = 15); and group III (IWRF classification 2.5-3.5, n = 13). Athletes with SCI had lower HRavg (111 ± 14 bpm vs 155 ± 13 bpm) and HRpeak (133 ± 12 bpm vs 178 ± 13 bpm) compared with non-SCI (both P < 0.001). Average HR was higher in group III than in I (136 ± 25 bpm vs 115 ± 20 bpm, P = 0.045); however, SCI athletes showed no difference in HRavg or HRpeak between groups. Within group III, SCI athletes had lower HRavg (115 ± 6 bpm vs 160 ± 8 bpm) and HRpeak (135 ± 11 bpm vs 183 ± 11 bpm) than non-SCI athletes (both P < 0.001). This study is the first to demonstrate attenuated HR responses during competition in SCI compared with non-SCI athletes, likely due to injury to spinal autonomic pathways. Among athletes with SCI, IWRF classification was not related to differences in HR. Specific assessment of autonomic function after SCI may be able to predict HR during competition and consideration of autonomic impairments may improve the classification process.

This document reflects the key points of a consensus meeting of the Heart Failure Association of European Society of Cardiology (ESC) held to provide an overview the role of physiological monitoring in the complex multimorbid heart failure (HF) patient. This article reviews assessments of the functional ability of patients with HF. The gold standard measurement of cardiovascular functional capacity is peak oxygen consumption obtained from a cardiopulmonary exercise test. The 6-min walk test provides an indirect measure of cardiovascular functional capacity. Muscular functional capacity is assessed using either a 1−repetition maximum test of the upper and lower body or other methods, such as handgrip measurement. The short physical performance battery may provide a helpful, indirect indication of muscular functional capacity.

Meta-analysis of short- and long-term efficacy of mononuclear cell transplantation in patients with myocardial infarction

Mononuclear cells (MNCs) have been tested in clinical trials across multiple cardiovascular pathologies with mixed results. Major adverse cardiac events (MACE) and markers of cardiovascular capacity have been particularly challenging to interpret because of small size. This meta-analysis is aimed to assess the efficacy of MNC therapy in randomized clinical trials to identify the markers of efficiency that could influence future trial design. PubMed, Embase, Cochrane library, and ClinicalTrials.gov were searched from inception through November 8, 2018. Changes in left ventricular ejection fraction (LVEF) and infarct size from baseline to follow-up were selected as primary outcomes. Changes in the left ventricular end-systolic volume, left ventricular end-diastolic volume, brain natriuretic peptide/N-terminal pro-B-type natriuretic peptide, 6-minute walk test, New York Heart Association class, and MACE incidences were considered secondary outcomes. In short-term follow-up, patients treated with MNCs demonstrated a significant increase in absolute LVEF of 2.21% (95% CI 1.59-2.83; P < .001; I2 = 32%) and 6.01% (95% CI 4.45-7.57; P < .001; I2 = 0%) in acute myocardial infarction (AMI) and ischemic cardiomyopathy studies, respectively. This effect was sustained in long-term follow-up. MNC therapy significantly reduced left ventricular end-systolic volume; however, infarct size, 6-minute walk test, New York Heart Association class, and MACE rates were comparable. MNC therapy may convey a modest but sustained increase in LVEF in ischemic cardiomyopathy patients, supporting further investigation. AMI trials, however, demonstrated minimal improvement in LVEF of unclear clinical significance, suggesting a limited role for MNC therapy in AMI.

Here, we compared the postprandial hemodynamics, cutaneous and psychomotor performance responses after ingestion of a classical Swiss festive meal [cheese fondue (CF)] versus a light ready-meal [Nasi Goreng (NG)], both accompanied with white wine.Methods: In a randomized cross over design, we examined in 12 healthy young men, the continuous cardiovascular, cutaneous, and reaction time responses to ingestion of cheese fondue versus a standard meal at rest (sitting position) and hemodynamic changes in response to orthostatic challenge (active standing) in pre- and postprandial phases.Results: Breath alcohol concentration after wine ingestion was similar after both meal types. Compared to the standard meal, consumption of CF induced higher increases in heart rate (HR), cardiac output (CO), double product (DP) and cardiac power output (CPO), greater vasodilation, and rises in skin blood flow and skin temperature. Greater increases in HR, DP, and mean blood pressure (MBP) were observed during orthostatic challenges with CF compared to NG. A two-choice reaction time task revealed similar reaction times with both meals, suggesting no influence of meal composition on psychomotor performance.Conclusion: In sitting position, CF ingestion induced a more important cardiovascular load compared to NG. Although the dose of alcohol and the festive meal used here did not lead to orthostatic hypotension, eating CF induced a greater cardiometabolic load suggesting that hemodynamic reserves have been encroached during active standing. This may impede the cardiovascular capacity during physical exercise or stress situations, particularly in elderly subjects who are at greater risk for postprandial hypotension and cardiovascular diseases.

Are acceleration and cardiovascular capacities related to perceived load in professional soccer players?

ABSTRACTThis study aims at assessing physical fitness performance and its relationship with the differential ratings of perceived exertion of training load (dRPE TL) and match load (dRPE ML) in a Spanish professional soccer team at the beginning of several in-season periods: 1–4 weeks, 5–8 weeks and 1–8 weeks. Performance and mechanical variables over the acceleration phase, as well as cardiovascular performance variables were evaluated in 20 male professional soccer players of a team competing in the Spanish Second Division League. Moreover, dRPE TL and dRPE ML were quantified. The dRPE TL showed negative and large associations between both maximal aerobic speed (MAS) and maximal oxygen consumption (VO2max) (from r = −0.53; ± 0.06 to r = −0.53; ± 0.05 95% CL, p = 0.035 to 0.036) and RPEres TL values throughout the 5–8 and 1–8 week periods. Furthermore, dRPE ML positive and large associations were found between players initial MAS or VO2max (from r = 0.50; ± 0.17 to r = 0.56; ± 0.11 95% CL, p = 0.026 to 0.049) and RPEmus ML in 1–4 and 1–8 week periods. The current study suggests that a better cardiovascular capacity could be connected with a lower RPEres TL and higher RPEmus ML.

El ejercicio físico para la salud del hombre

Se darán respuesta a varias interrogantes como: ¿Qué tipo de ejercicio debo realizar?,¿Cuál es la intensidad adecuada? entre otras. El deporte tiene una gran influencia en la prevención de muchas enfermedades como la obesidad, la hipertensión y la diabetes. Prolongan el tiempo socialmente útil de la persona; al mejorar su capacidad física, cardiovascular, ósea y muscular eleva sus niveles productivos, por lo que retarda los cambios propios de la vejez. Asegura una mayor capacidad de trabajo y asegura la longevidad al favorecer la eliminación de toxinas y oxidantes. Mejora el aspecto físico de la persona.
