Analysis of Pulmonary Vein Antrums Motion with Cardiac Contraction Using Dual-Source Computed Tomography.

Purpose: The purpose of the study was to determine the extent of displacement of the pulmonary vein antrums resulting from the intrinsic motion of the heart using 4D cardiac dual-source computed tomography (DSCT).Methods: Ten consecutive female patients were enrolled in this prospective planning study. In breath-hold, a contrast-injected cardiac 4-dimensional (4D) computed tomography (CT) synchronized to the electrocardiogram was obtained using a prospective sequential acquisition method including the extreme phases of systole and diastole. Right and left atrial fibrillation target volumes (CTVR and CTVL) were defined, with each target volume containing the antral regions of the superior and inferior pulmonary veins. Four points of interest were used as surrogates for the right superior and inferior pulmonary vein antrum (RSPVA and RIPVA) and the left superior and inferior pulmonary vein antrum (LSPVA and LIPVA). On our 4D post-processing workstation (MIM Maestro™, MIM Software Inc.), maximum displacement of each point of interest from diastole to systole was measured in the mediolateral (ML), anteroposterior (AP), and superoinferior (SI) directions.Results: Median age of the enrolled patients was 60 years (range, 56-71 years). Within the CTVR, the mean displacements of the superior and inferior surrogates were 3 mm vs. 1 mm (p=0.002), 2 mm vs. 0 mm (p= 0.001), and 3 mm vs. 0 mm (p=0.00001), in the ML, AP, and SI directions, respectively. On the left, mean absolute displacements of the LSPVA vs. LIPVA were similar at 4 mm vs. 1 mm (p=0.0008), 2 mm vs. 0 mm (p= 0.001), and 3 mm vs. 1 mm (p=0.00001) in the ML, AP, and SI directions.Conclusion: When isolated from breathing, cardiac contraction is associated with minimal inferior pulmonary veins motion and modest (1-6 mm) motion of the superior veins. Target deformation was thus of a magnitude similar or greater than target motion, limiting the potential gains of cardiac tracking. Optimal strategies for cardiac radiosurgery should thus either incorporate the generation of an internal target or cardiac gating. In either case, cardiac 4D DSCT would allow for personalized margin definition.

Value of dual-source computed tomography in evaluating left ventricular function in patients with coronary heart disease.

We aimed to assess parameters reflecting left ventricular function by dual-source computed tomography (DSCT) with echocardiography (ECG) as control. Fifty-eight patients with coronary heart disease (CHD) were recruited from January to June 2011; 29 CHD patients had type II diabetes. All patients were assessed by cardiac DSCT and ECG examination. DSCT and ECG correlated well for ejection fraction (EF) (r = 0.70), end-systolic volume (ESV) (r = 0.87), stroke volume (SV) (r = 0.83), and end-diastolic volume (EDV) (r = 0.90). The mean ESV and EDV values measured by the two methods in CHD patients with type II diabetes were higher than those in non-diabetic patients, whereas the mean EF was lower. DSCT is an accurate and practical method for assessing left ventricular function.

Reconstruction Method for Nonconcyclic Dual-Source Circular Cone-Beam CT with a Large Field of View

In industrial computed tomography (CT), it is often required to inspect large objects whose size is beyond a reconstructed field of view (FOV). Some multiscan modes have been developed to acquire the complete CT projection data for a larger object using small panel detectors. In this paper, we give a non-concyclic dual-source circular cone-beam scanning geometry based on the idea of multiscan modes and propose a backprojection-filtration-based (BPF) reconstruction algorithm without data rebinning. Since the FOV calculated according to this nonconcyclic dual-source circular CT scanning geometry is larger than cardiac dual-source CT scanning geometry, our method can reconstruct larger horizontal slices (i.e., the slices perpendicular to rotation axis) than cardiac dual-source CT. The quality of CT images is expected to be superior to those obtained using larger panel detectors. The simulation results have indicated that CT images obtained by the proposed method are satisfying.

Accuracy and variability of right ventricular volumes and mass assessed by dual-source computed tomography: influence of slice orientation in comparison to magnetic resonance imaging

To evaluate the accuracy and variability of right ventricular (RV) volumes and mass using dual-source computed tomography (DSCT) and the influence of slice orientation in comparison to cardiac magnetic resonance imaging (CMR). In 33 patients undergoing cardiac DSCT and CMR, RV parameters were calculated using the short-axis (DSCT, CMR) and axial orientation (DSCT). Intra- and interobserver variability were assessed by Bland-Altman analysis. Short-axis orientation: RV parameters of the two techniques were not statistically different. Axial orientation: RV volumes and mass were significantly overestimated compared with short-axis parameters whereas EF was similar. The short-axis approach resulted in low variability, although the axial orientation had the least amount of intra- and interobserver variability. RV parameters can be more accurately assessed by DSCT compared with CMR using short-axis slice orientation. RV volumes and mass are significantly higher using axial compared with short-axis slices, whereas EF is unaffected. RV parameters derived from both approaches yield high reproducibility.

Semi-automatic measurement of left ventricular function on dual source computed tomography using five different software tools in comparison with magnetic resonance imaging

To compare left ventricular (LV) function assessment using five different software tools on the same dual source computed tomography (DSCT) datasets with the results of MRI. Twenty-six patients, undergoing cardiac contrast-enhanced DSCT were included (20 men, mean age 59±12 years). Reconstructions were made at every 10% of the RR-interval. Function analysis was performed with five different, commercially available workstations. In all software tools, semi-automatic LV function measurements were performed, with manual corrections if necessary. Within 0-22 days, all 26 patients were scanned on a 1.5 T MRI-system. Bland-Altman analysis was performed to calculate limits of agreement between DSCT and MRI. Pearson's correlation coefficient was calculated to assess the correlation between the different DSCT software tools and MRI. Repeated measurements were performed to determine intraobserver and interobserver variability. For all five DSCT workstations, mean LV functional parameters correlated well with measurements on MRI. Bland-Altman analysis of the comparison of DSCT and MRI showed acceptable limits of agreement. Best correlation and limits of agreement were obtained by DSCT software tools with software algorithms comparable to MRI software. The five different DSCT software tools we examined have interchangeable results of LV functional parameters compared to regularly analysed results by MRI. The best correlation and the narrowest limits of agreement were found when the same software algorithm was used for both DSCT and MRI examinations, therefore our advice for clinical practice is to always evaluate images with the same type of post-processing tools in follow-up.

Comparison of (semi-)automatic and manually adjusted measurements of left ventricular function in dual source computed tomography using three different software tools

To assess the accuracy of (semi-)automatic measurements of left ventricular (LV) functional parameters in cardiac dual-source computed tomography (DSCT) compared to manually adjusted measurements in three different workstations. Forty patients, who underwent cardiac DSCT, were included (31 men, mean age 58 ± 14 years). Multiphase reconstructions were made with ten series at every 10% of the RR-interval. LV function analysis was performed on three different, commercially available workstations. On all three workstations, end-systolic volume (ESV), end-diastolic volume (EDV), LV ejection fraction (LVEF) and myocardial mass (MM) were calculated as automatically as possible. With the same DSCT datasets, LV functional parameters were also calculated with as many manual adjustments as needed for accurate assessment for all three software tools. For both semi-automatic as well as manual methods, time needed for evaluation was recorded. Paired t-tests were employed to calculate differences in LV functional parameters. Repeated measurements were performed to determine intra-observer and inter-observer variability. (Semi-)automatic measurements revealed a good correlation with manually adjusted measurements for Vitrea (LVEF r = 0.93, EDV r = 0.94, ESV r = 0.98 and MM r = 0.94) and Aquarius (LVEF r = 0.96, EDV r = 0.94, ESV r = 0.98 and MM r = 0.96). Also, good correlation was obtained for Circulation, except for LVEF (LVEF r = 0.45, EDV r = 0.93, ESV r = 0.92 and MM r = 0.86). However, statistically significant differences were found between (semi-)automatically and manually adjusted measurements for LVEF (P < 0.05) and ESV (P < 0.001) in Vitrea, all LV functional parameters in Circulation (P < 0.001) and EDV, ESV and MM (<0.001) in Aquarius Workstation. (Semi-)automatic measurement of LV functional parameters is feasible, but significant differences were found for at least two different functional parameters in all three workstations. Therefore, expert manual correction is recommended at all times.

The reliability of automatic measurement of left ventricular function with dual-source computed tomography datasets

The purpose of the study was to assess the reliability of (semi-) automatic left ventricular (LV) function measurements using three different software packages on the same dual-source computed tomography (DSCT) datasets and to compare agreement among the software packages. Forty consecutive patients, undergoing cardiac DSCT were included (31 men, mean age 58±14 years). LV function analysis was performed with all three software packages. ANOVA testing was used to determine the difference among the repeated measurements and the difference among the software packages. Bland-Altman plots were computed to describe the agreement among the software packages. No significant difference was found among the repeated measurements. In the comparison of the three software packages, a significant difference was observed when measurements were used with minimal user interaction. When end-diastolic and end-systolic phases were manually set, there was no overall significant difference, but in 12.5% of patients a large (>10%) difference in LVEF was found. All three software packages have good intraobserver variability, but the results of the three packages were significantly different. For clinical use, one should be aware of the clinical impact of possible segmentation flaws when (semi-)automatic LV function assessment is used.

Global left ventricular function: assessment with dual-source CT versus conventional ventriculography in a porcine model

Objective — Cardiac computed tomography (CT) and conventional biplane ventriculography are established methods for the determination of ventricular function. Recently dual source CT was introduced for cardiac CT scanning providing a temporal resolution of 83ms independent of the patient’s heart rate.The purpose of this study was to assess the global left ventricular function using retrospectively ECG-gated dual source computed tomography (DSCT) in comparison to conventional ventriculography.Methods and results — Contrast-enhanced cardiac DSCT and conventional ventriculography were performed in 10 domestic pigs with an approximate weight of 60kg using standardized examination protocols under general anaesthesia. From manually drawn endocardial contours, LV end-systolic (ESV) and end-diastolic volume (EDV), stroke volume (SV), ejection fraction (EF) were calculated by means of dedicated analysis software separately for both examination modalities. LV functional parameters were analysed using Bland-Altman plots, Student t-test, and Pearson correlation coefficient. Left ESV and EDV determined with DSCT correlated well with conventional ventriculography results (ESV: r=0.86; EDV: r=0.72) with a good correlation also for SV (r=0.67).An only moderate correlation was found for EF (r=0.52). Student t-test showed a significant underestimation of SV and EF derived from DSCT in comparison to ventriculography.Conclusion — Retrospectively ECG-gated DSCT can accurately determine LV volumes in comparison to conventional ventriculography but provides lower SV and EF values; however, the correlation was only moderate.

Assessment of left ventricular function and mass in dual-source computed tomography coronary angiography: Influence of beta-blockers on left ventricular function: Comparison to magnetic resonance imaging

Purpose To quantify left ventricular (LV) function and mass (LVM) derived from dual-source computed tomography (DSCT) and the influence of beta-blocker administration compared to cardiac magnetic resonance imaging (CMR). Methods Thirty-two patients undergoing cardiac DSCT and CMR were included, where of fifteen received metoprolol intravenously before DSCT. LV parameters were calculated by the disc-summation method (DSM) and by a segmented region-growing algorithm (RGA). All data sets were analyzed by two blinded observers. Interobserver agreement was tested by the intraclass correlation coefficient. Results. 1. Using DSM LV parameters were not statistically different between DSCT and CMR in all patients (DSCT vs. CMR: EF 63 ± 8% vs. 64 ± 8%, p = 0.47; EDV 136 ± 36 ml vs. 138 ± 35 ml, p = 0.66; ESV 52 ± 21 ml vs. 52 ± 22 ml, p = 0.61; SV 83 ± 22 ml vs. 87 ± 19 ml, p = 0.22; CO 5.4 ± 0.9 l/min vs. 5.7 ± 1.2 l/min, p = 0.09, LVM 132 ± 33 g vs. 132 ± 33 g, p = 0.99). 2. In a subgroup of 15 patients beta-blockade prior to DSCT resulted in a lower ejection fraction (EF), stroke volume (SV), cardiac output (CO) and increase in end systolic volume (ESV) in DSCT (EF 59 ± 8% vs. 62 ± 9%; SV 73 ± 17 ml vs. 81 ± 15 ml; CO 5.7 ± 1.2 l/min vs. 5.0 ± 0.8 l/min; ESV 52 ± 27 ml vs. 57 ± 24 ml, all p < 0.05). 3. Analyzing the RGA parameters LV volumes were not significantly different compared to DSM, whereas LVM was higher using RGA (177 ± 31 g vs. 132 ± 33 g, p < 0.05). Interobserver agreement was excellent comparing DSM values with best agreement between RGA calculations. Conclusion Left ventricular volumes and mass can reliably be assessed by DSCT compared to CMR. However, beta-blocker administration leads to statistically significant reduced EF, SV and CO, whereas ESV significantly increases. DSCT RGA reliably analyzes LV function, whereas LVM is overestimated compared to DSM.

Remodelling of the aortic root in severe tricuspid aortic stenosis: implications for transcatheter aortic valve implantation

Detailed knowledge of aortic root geometry is a prerequisite to anticipate complications of transcatheter aortic valve (TAV) implantation. We determined coronary ostial locations and aortic root dimensions in patients with aortic stenosis (AS) and compared these values with normal subjects using computed tomography (CT). One hundred consecutive patients with severe tricuspid AS and 100 consecutive patients without valvular pathology (referred to as the controls) undergoing cardiac dual-source CT were included. Distances from the aortic annulus (AA) to the left coronary ostium (LCO), right coronary ostium (RCO), the height of the left coronary sinus (HLS), right coronary sinus (HRS), and aortic root dimensions [diameters of AA, sinus of Valsalva (SV), and sino-tubular junction (STJ)] were measured. LCO and RCO were 14.9 +/- 3.2 mm (8.2-25.9) and 16.8 +/- 3.6 mm (12.0-25.7) in the controls, 15.5 +/- 2.9 mm (8.8-24.3) and 17.3 +/- 3.6 mm (7.3-26.0) in patients with AS. Controls and patients with AS had similar values for LCO (P = 0.18), RCO (P = 0.33) and HLS (P = 0.88), whereas HRS (P < 0.05) was significantly larger in patients with AS. AA (r = 0.55,P < 0.001), SV (r = 0.54,P < 0.001), and STJ (r = 0.52,P < 0.001) significantly correlated with the body surface area in the controls; whereas no correlation was found in patients with AS. Patients with AS had significantly larger AA (P < 0.01) and STJ (P < 0.01) diameters when compared with the controls. In patients with severe tricuspid AS, coronary ostial locations were similar to the controls, but a transverse remodelling of the aortic root was recognized. Owing to the large distribution of ostial locations and the dilatation of the aortic root, CT is recommended before TAV implantation in each patient.

Dual-source computed tomography: Estimation of radiation exposure of ECG-gated and ECG-triggered coronary angiography

Purpose The aim of the study was to estimate radiation exposure of coronary calcium scoring and angiography using ECG-gated and ECG-triggered dual-source computed tomography. Materials and methods An Alderson Rando phantom equipped with thermoluminescent dosimeters was used for all dose measurements. Effective dose was calculated according to ICRP 103. Radiation exposure was performed on a dual-source computed tomography (DSCT) scanner with standard protocols for calcium scoring (DSCT-Ca) and coronary angiography (DSCTA) at different heart rates (40–100 beats/min). Furthermore, a scanning protocol with ECG-triggering as well as a standard chest CT scan were evaluated. Results Depending on gender, heart rate and ECG-pulsing, the effective dose of a complete cardiac DSCT (DSCT-Ca and DSCTA) scan varies from 10.2 to 32.6 mSv. The effective radiation dose increased significantly with lower heart rates ( p < 0.035). ECG-pulsing reduced the radiation exposure significantly in DSCTA ( p < 0.001). Due to breast tissue in the primary scan range, females’ doses showed an increase up to 69.9% compared to males in scan protocols without ECG-pulsing. Prospective ECG-triggered DSCTA resulted in estimated effective doses from 2.8 mSv (males) to 4.1 mSv (females). Conclusion The ECG-pulsing technique has proven its effectiveness to reduce effective dose in coronary CT angiography and is recommended for all patients with regular heart rates. The patient's heart rate influences the radiation exposure with a significant decrease at higher heart rates. Due to its lower dose, ECG-triggered DSCTA should be implemented for special indications, i.e. for diagnosis of pathologies of the aortic root and the ascending aorta.

Assessment of Global Left and Right Ventricular Function Using Dual-Source Computed Tomography (DSCT) in Comparison to MRI

We sought to evaluate the ability of retrospectively ECG-gated dual-source computed tomography (DSCT) to assess left (LV) and right ventricular (RV) functional parameters in comparison to 1.5 T magnetic resonance imaging (MRI). Ten domestic pigs (60 kg) underwent both contrast-enhanced cardiac DSCT and cardiac MRI using standardized examination protocols under general anesthesia. From manually drawn endocardial and epicardial contours, LV and RV end-systolic (ESV) and end-diastolic volume (EDV), stroke volume (SV), ejection fraction (EF), myocardial mass (MM), peak filling rate (PFR), peak ejection rate (PER), time to peak ejection (TPE), and time to peak filling (TPF) were calculated by means of dedicated analysis software. LV and RV functional parameters were analyzed using Bland-Altman plots, Student t test, and Pearson's correlation coefficient. Both left and right ESV and EDV, SV and EF determined with DSCT correlated well with MR imaging results (left, r = 0.98/0.92/0.82/0.98; right, r = 0.90/0.94/0.96/0.94). PER, PFR, TPE, TPF, and MM showed only a moderate to low correlation (left, r = 0.67/0.37/0.23/0.35/0.57; right, r = 0.78/0.69/0.12/0.11/0.44). PER and PFR were significantly underestimated by DSCT when compared with MRI. Retrospectively ECG-gated DSCT correctly depicts end-systole and can accurately determine LV and RV volumes, SV, and EF in comparison to MRI. DSCT showed a significant underestimation of PER and PFR in comparison to MRI.
