Comparison of Canal Transportation and Centering Ability of ProGlider and WaveOne Gold Glider in Curved Canals

Objectives The aim of this study was to compare the volume variation and maintenance of the root canal position when using the ProGlider 16.02 (PG) and the WaveOne Gold Glider 15.02 (WOGG) file systems for glide path preparation. Materials and Methods Twenty-four moderately curved mesiobuccal canals of maxil-lary first molars were selected and randomly divided into two groups: PG and WOGG. The selected teeth were scanned using microtomography before and after root canal preparation to assess centralization and linear transport at 1, 3, 5, and 7 mm from the apical foramen. Statistical Analysis The data were analyzed with the Mann–Whitney U test. The level of significance was 5%. Results There were no significant differences in volume variation or root canal transport ( p > 0.05). There was a significant difference in the centralization of the root canal at 3 mm from the foramen ( p < 0.05). Conclusions WOGG and PG instruments presented similar results regarding the root canal volume increase and transport. WOGG caused higher decentralization at 3 mm from the apical foramen.

The frequency of the second mesiobuccal canal in maxillary first molars among a sample of the Kurdistan Region-Iraq population - A retrospective cone-beam computed tomography evaluation

Background/purposeOne of the most complexes in root and canal anatomy is the maxillary first permanent molar. This study aimed to analyze the frequency of mesiobuccal (MB2) canal and its apical portals of exit, in patients belonging to different gender-age groups and determine the possible relation with sex and age. Materials and methodsA CBCT scan images were accessed from a documented database of 343 patients (178 male, 165 female), who had an oral and maxillofacial examination. The age of the patients ranging from 12 to 63 years. The frequency of the MB2 canal and its portals of exit apically were observed. Age and gender correlation were calculated using the χ2 test. P < 0.05 was considered significant. ResultsA total of 634 maxillary first molars (335 male, 299 female) were included. The MB2 canal was found in 53.78% of the cases. A significantly higher incidence of the MB2 canal was detected in males than in females. No correlation in the presence and/or absence of the MB2 canal in terms of age. When MB2 canal present, a single apical foramen was observed in 66.28% of the cases, two apical foramina were present in 33.72% of the cases. ConclusionMB2 canals may be present in approximately half of the cases, from which one third of the cases have their own apical portal of exit.

Influence of the Intracanal Material and Metal Artifact Reduction Tool in the Detection of the Second Mesiobuccal Canal in Cone-beam Computed Tomographic Examinations

IntroductionThe aim of this study was to evaluate the influence of different intracanal materials and the metallic artifact reduction (MAR) tool of cone-beam computed tomographic (CBCT) imaging on the detection of the second mesiobuccal (MB2) canal in maxillary first molars. MethodsForty maxillary first molars were selected; half of them had the MB2 canal in the mesiobuccal root confirmed by micro–computed tomographic imaging. All teeth were instrumented, except for the MB2 canal, and filled with gutta-percha, which was passively inserted up to the working length. Each tooth was individually inserted into a dry human skull, and CBCT scans were performed using the OP300 device (Instrumentarium Dental, Tuusula, Finland) with and without the use of the MAR tool. For each scanned tooth, the following intracanal materials were used in the palatal root: gutta-percha, silver palladium, nickel chromium, and cobalt chromium. Five examiners assessed the images for the detection of the MB2 canal using a 5-point scale. The weighted kappa test was applied to evaluate intra- and interexaminer reproducibility. The diagnostic values (sensitivity, specificity, and area under the receiver operating characteristic curve) were calculated in the different groups and compared using 2-way analysis of variance and the Tukey post hoc test. ResultsThe intra- and interobserver agreement ranges for the different conditions were studied. The diagnostic values were not significantly different (P > .05) regardless of the intracanal material and the use of the MAR tool. ConclusionsThe presence of different intracanal materials and MAR activation does not influence CBCT-based detection of MB2 canals.

Morphological measurements of two separate mesiobuccal canals in maxillary first molars using micro-computed tomography.

To obtain radicular measurements of two separate mesiobuccal (MB) root canals in maxillary first molars using micro-computed tomography (μCT) with customized software. Human maxillary first molar MB roots (N = 36) with two separate canals (MB1, MB2) and apical foramina were scanned by μCT and analyzed with Kappa2 software to reconstruct three-dimensional (3D) surface models of roots and canals. These models were sectioned at 0.1mm intervals perpendicular to the central axis of each canal. Canal widths, 3D curvatures, and surrounding dentine thicknesses were measured concurrently on each section. Dentine thicknesses were analyzed statistically for differences between each direction and the different levels of both canals. Dentine walls around MB1 were thicker than MB2 (p < 0.05). Thinnest dentine was most often located at disto-inside direction in both canals. Canal widths were significantly smaller in MB2 than MB1 (p < 0.01). Apical constrictions were smaller (p < 0.05) and further (p < 0.05) from the apex in MB2 than MB1. Canal curvatures were greatest in the apical third of both canals (p < 0.001), and they were greater in MB2 than MB1 (p < 0.05). MB2 canals had shorter lengths, smaller widths, and more severe curvatures and were surrounded by thinner dentine walls. In MB2, apical constrictions were between 1 and 2mm from the apex, compared to about 1mm for MB1. These detailed measurements and in-depth 3D analyses of maxillary first molar MB roots with two separate canals and apical foramina provide morphologic references for root canal therapy.

Endodontic management of rare aberrant root canal morphology with two palatal canals in maxillary first molar using cone beam computed tomography: A case report

A detailed acquaintance of root canal anatomy is essential for the endodontic root canal therapy. Aberrations or unusual variations in the root canal morphology, mainly in multirooted teeth, can cause a substantial challenge to the endodontist during endodontic management. Knowledge of these variations, mostly concerning the position and treatment of all canals, is very essential for the successful outcomes of endodontic therapy; the failure to find and properly treat the root canals may cause treatment failure because it is important to assess individual case for variations. Endodontist should be suspicious while treating maxillary first molars because of its root curvatures, additional canals and variations in internal morphology. Bilateral Presence of Two Distobuccal Canals in Maxillary First Molars: A Case Report.

This article describes the diagnosis and treatment of bilaterally maxillary first molars with two canals in their distobuccal roots. A 13-year-old male came to the clinic with a severe pain of tooth #14. In the first session, after access and finding the main canals and the second mesio-buccal (MB2) canal, another canal was noticed in the distobuccal root. All teeth were examined and a severe decay was observed in the left maxillary first molar. After preparing the access cavity, another orifice was found between the palatal and distobuccal canals. The presence of the second distobuccal canal was confirmed using an apex locator and radiography.

Quality of Root Filling after Obturation with Gutta-percha and 3 Different Sealers of Minimally Instrumented Root canals of the Maxillary First Molar

IntroductionThe aim of this study was to compare the quality of root fillings completed by a modified single-cone (MSC) technique with 3 different sealers after minimal instrumentation and multisonic cleaning of root canals of maxillary first molars. MethodsRoot canals of 18 maxillary first molars were instrumented to size 15/.04 taper using rotary files. Sodium hypochlorite 5.25% was used during instrumentation; the final cleaning was performed by the GentleWave System (Sonendo Inc, Laguna Hills, CA). The specimens were allocated into 3 groups and root filled by the MSC technique using a size fitted gutta-percha master cone and GuttaFlow Bioseal (Coltene Whaledent GmBH + Co KG, Langenau, Switzerland), GuttaFlow 2 (Coltene Whaledent GmBH + Co KG), and MTA Fillapex (Angelus Industria de Produtos Odontológicos S/A, Londrina, PR, Brazil) sealers. Micro–computed tomographic scans were obtained before and after instrumentation, post-GentleWave, and after obturation. Reconstructed images were analyzed for the volumetric percentage of filling materials. Mesiobuccal roots of the selected teeth were sectioned at 0.5-mm increments starting at the apex of the root. The cross sections were further examined using a light microscope. ResultsThe 3 groups had 90%–99% of the canal space filled with the root filling material. The mean volume of the filling material was higher in the GuttaFlow Bioseal and GuttaFlow 2 groups than in the MTA Fillapex group (P < .05). There was no significant difference among the apical, middle, and coronal thirds. The cross-sectional images showed no obvious gaps or voids in the GuttaFlow groups. After instrumentation, 49 of the 189 canal thirds (25.9%) had hard tissue debris in the root canal system. After GentleWave cleaning, only 4 of 63 canals (6.3%) and 4 of the 189 canal thirds (2.1%) still had debris. ConclusionsThe MSC method with GuttaFlow 2 and GuttaFlow Bioseal sealers after multisonic cleaning of minimally instrumented molar canals resulted in high-quality root fillings. Multisonic cleaning of minimally instrumented molars seems to be effective in debris removal.

Second Mesiobuccal Canal Evaluation Features with Cone-Beam Computed Tomography

Aim The aim of the study is to evaluate the difference in MB2 prevalence with different slice thicknesses in maxillary first molars. Materials and Methods Two hundred nonfilled MB2 canals in maxillary first molars of 156 people (75 females and 81 males) aged from 20 to 73 years old were evaluated with CBCT with different slice thicknesses: 0.5 mm, 1 mm, 3 mm, and 10 mm. A general analysis was performed out, as well as in the age groups and on gender groups. Results Visualization with 0.5 mm and 1 mm slice thicknesses was 100% and generally equal, in both the male and the female group. General MB2 visualization with 3 mm slice thickness was 42% and 29% for the male group and 27% for the female group. No canals were visualized with 10 mm slice thickness. The study did not demonstrate a statistical difference in the MB2 prevalence between gender and age groups with the 3 mm slice thickness. Conclusion The most valuable way to evaluate the root canal system in first maxillary molars with CBCT is using 1 mm slice thickness for both genders and every age group.

The Frequency of Mesio Buccal Second Canals in Permanent Maxillary First Molar Using Different Methods: A Cross-Sectional Study

Objective: This study aimed to find out the fr equency of the MB2 canal in maxillary first molars, using different diagnostic methods. Materials &amp; Methods: 106 patients, clinically diagnosed with irreversible pulpitis and exposed pulp, requiring endodontic treatment were selected via random non-probability purposive sampling technique for the study . This cross-sectional study was conducted for 12 weeks in the dentistry department of Ayub Medical College from August 2018 to October 2018. A detailed history was taken and documented in the pro-forma. After administration of Local anaesthesia and rubber dam application, the access cavity was prepared and floor of the pulp chamber visualized. MB2 Canal location was done with a naked eye and under magnification (x2.0 to x6.0) Magnification and confirmed by inserting size 08 K file. The data obtained were analyzed using SPSS version 20. Chi-square test was run to determine the association of the presence of the MB2 canal with other variables. P-value of 0.05 was considered to be significant.

Micro-CT analysis of danger zone thickness in the mesiobuccal roots of maxillary first molars.

To evaluate dentine thickness from both distal ('danger zone') and mesial ('safety zone') aspects of the MB1 and MB2 canals of maxillary first molars at the furcation level using micro-CT imaging. One hundred maxillary first molars with two canals in the MB root were selected from a Brazilian subpopulation and scanned at a resolution of 19.6μm in a micro-CT device. From each specimen, two axial cross-sectioned images were selected at 2 and 3mm apical to the furcation level of the MB root and evaluated for the smallest dentinal thickness. Measurements were made from the outer perimeter of the MB1 and MB2 canals towards the external root surface from both distal (danger zone) and mesial (safety zone) aspects of the root. The influence of an isthmus was also considered in the analysis, and samples were classified according to the presence or absence of an isthmus in the axial sections. Non-parametric Wilcoxon signed-rank tests were used to compare dentinal thickness between MB1 and MB2 canals with a significant level set at 5%. Colour-coded 3D models of dentine thickness throughout the MB root were created and evaluated qualitatively. At both levels, median dentine thickness of the MB2 canal was significantly lower in both distal and mesial aspects of the root in comparison with the MB1 canal (P<0.01). The presence of an isthmus was not a variable that influenced dentinal thickness. Colour-coded models indicated that dentine thickness was dependent on the cross-sectional shape of the MB root. At least 50% of the sample had <1mm of dentine thickness at the danger zone of the MB2 canal. Danger zones in the MB roots of maxillary first molars were not symmetrical. MB2 canals had less dentine thickness on both aspects of the root compared to the MB1 canal.

Computed microtomography study of untreated, shaped and filled mesiobuccal canals of maxillary first molars.

This study assessed the effectiveness of modern endodontic shaping and filling procedures on mesiobuccal roots of maxillary molars with two canals. The canals of 20 mesiobuccal roots were treated with Mtwo rotary files, passive ultrasonic irrigation and Guttafusion obturators. X-ray computed microtomography analysis was carried out prior to treatment, after canal shaping and after canal filling to determine the alterations of the canal volume before and after the instrumentation, the volume of the hard tissue debris, and percentage of the volume occupied by filling materials. The shaping instruments and filling materials reached only partially the endodontic space of the second mesiobuccal canal and the accessory endodontic structures. Canal irregularities, ramifications, and interconnections were accumulation sites of hard tissue debris. This study demonstrated that rotary files, passive ultrasonic irrigation and carrier-based filling systems could be partially effective for the treatment of the mesiobuccal canals and their accessory endodontic structures.

Second Mesiobuccal Root Canal of Maxillary First Molars in a Brazilian Population in High-Resolution Cone-Beam Computed Tomography.

Introduction:The second canal of the mesiobuccal root (MB2) of the maxillary first molars (MFM) is difficult to detect in conventional radiographs and can be a major cause of failure in endodontic treatments. The aim of this study was to investigate the prevalence and anatomy of the MB2 by using high-resolution cone-beam computed tomography (CBCT). Methods and Materials:Three radiologists examined 414 high-resolution CBCTs. Of these, the CBCTs of 287 patients (mean age 49.43±16.76) who had at least one MFM were selected, making a total of 362 teeth. Prevalence and its relation with gender and age of the patients, side of the tooth, and Vertucci’s classification were analyzed. Data were statistically analyzed (P<0.05). Results:A total of 68.23% of the teeth exhibited the MB2. The presence of the MB2 was equivalent in both genders and significantly higher in younger patients. There was no correlation between the presence of the MB2 in relation to both the sides of the MFM and the FOV size. Smaller FOV recognized higher Vertucci’s grades.Conclusions:It was concluded that the prevalence of the MB2 canal in maxillary first molars in this Brazilian population examined with high-resolution CBTCs is 68.23%, being more prevalent in young patients. Gender and the side examined are no factors for determining the presence of MB2. Although the both FOVs of the high-resolution CBTCs (FOV 8 and 5) detect the MB2 canal, smaller FOV (FOV 5) is more accurate in the analysis of the internal anatomy of such root canals, according to the Vertucci´s classification.

Prevalence of Second Mesiobuccal Canals in Maxillary First Molars Detected Using Cone-beam Computed Tomography, Direct Occlusal Access, and Coronal Plane Grinding

IntroductionThe purpose of this study was to determine the prevalence of the second mesiobuccal canal (MB2) in 100 maxillary first molars using 3 independent methods and a combination method. MethodsOne hundred extracted human maxillary first molars were collected. The teeth were mounted in the maxillary first molar extraction sockets of a human cadaver head. A cone-beam computed tomographic (CBCT) scan was taken of each tooth. Two radiology faculty independently evaluated the CBCT volume for the presence of an MB2 canal. Additionally, teeth were accessed. If a canal was not found, a preoperative CBCT scan was viewed followed by a second attempt to locate an MB2 canal. Lastly, the mesiobuccal root was dissected by grinding in a coronal plane. ResultsA review of CBCT volumes found the presence of an MB2 canal 69% of the time. Accessing the tooth led to an MB2 detection of 78%. When a CBCT scan was viewed, this brought the access detection rate up to 87%. Coronal plane root grinding had an MB2 canal detection rate of 92%. Differences between each method were statistically significant. ConclusionsThe results of this study show that an MB2 canal is present up to 92% of the time. Direct access of teeth found statistically significant more MB2 canals than viewing CBCT volumes alone (P = .032). Therefore, exposing every patient to a preoperative CBCT scan may not be appropriate. However, taking a CBCT scan when an MB2 canal is not found clinically can significantly increase the chances of finding an MB2 canal (P < .001).

Evaluation of Distolingual Canal/Roots in Mandibular Molars and Mesiobuccal Canals in Maxillary Molars by CBCT.

Background/Purpose:The objectives of this study were to identify the bilateral distolingual (DL) canals / roots of the mandibular first molars and second mesiobuccal (MB2) canals of the maxillary first molars in the same Turkish individuals using cone-beam computed tomography (CBCT).Materials and Methods:A total of 150 CBCT images including all mandibular and maxillary first molars were retrospectively investigated in a Turkish subpopulation. The patient age, sex and presence of roots and root canals were assessed. The frequency, of bilateral DL canals, DL roots, and MB2 canals were reviewed. Data were analyzed using Fisher’sexact test and Chi-square test.Results:The prevalences of right DL canals, DL roots and MB2 canals were 31.3, 14.0 and 34.7%, respectively. The prevalences of left DL canals, DL roots and MB2 canals were 31.3, 4 and 27.3%, respectively. There was no statistically difference in the frequency of right and left DL canals, DL roots of mandibular first molars and MB2 canals of maxillary first molars according to gender.Conclusion:CBCT is a competent tool for the detection of additional distolingual canals/roots and second mesio buccal canals, and it is a valuable aid for dentists providing root canal treatment.

Efficacy of Different Cone-beam Computed Tomographic Protocols in the Identification of Mesiobuccal Canals of Maxillary First Molars: A Tomographic and Ex Vivo Study

IntroductionComplementary methods of anatomic diagnosis, including cone-beam computed tomographic (CBCT) imaging, may be useful in the diagnosis of mesiobuccal (MB) canals of maxillary molars. MethodsThirty-five maxillary first molars were subjected to ex vivo analysis and 4 CBCT protocols: i-CAT Classic (Imaging Sciences International, Hatfield, PA) (ICC group), i-CAT Next Generation (Imaging Sciences International) (ICN group), PreXion 3D (PreXion, San Mateo, CA) (16.8-second acquisition time; PX1 group), and PreXion 3D (33.5-second acquisition time; PX2 group). The images were evaluated by 2 calibrated endodontists who kept records following protocols previously published in the literature. Micro–computed tomographic imaging was used as the control. ResultsThe correct identification of anatomic variations of MB roots was 54.3% (n = 19) in the ICC group, 65.7% (n = 23) in the ICN group, and 80.0% (n = 28) in the PX1 and PX2 groups. The probability of agreement between the ICC and the control groups was significantly different from the probability of agreement with the PX1 (P < .05) and PX2 (P < .05) groups. The other groups showed no significant differences compared with the control group. There was no difference in the probability of agreement between the tomographic protocols and the ex vivo results. ConclusionsThe PX1 and PX2 groups were more effective for the diagnosis of MB canals. PX1 is the most suitable because it results in a shorter radiation time and diagnostic imaging similar to PX2. The single-canal anatomy was the most prevalent in the study.

An in vitro study to find the incidence of mesiobuccal 2 canal in permanent maxillary first molars using three different methods.

Aim:In-vitro study was done to evaluate the incidence of MB2 canals using three different methods (CBCT, CLINICAL ANALYSIS AND DENTAL LOUPES) and to compare the efficacy of the three methods in identifying the incidence of MB2 canals in maxillary permanent first molars.Materials and Methods:The study sample consisted of 120 extracted intact permanent maxillary molars. These extracted teeth were subjected to CBCT. Later the teeth were access opened with naked eye to find the incidence of MB2 canal, and then the teeth were visualised under dental loupe to locate MB2 canal if they were missed under naked eye. Results was statistically analysed by Mc Nemar's tests with Bonferroni correction, Chi square test and Cochran's Q test.Result:CBCT showed high incidence (68.3%) of MB2 canal in maxillary first molars and it showed to be a reliable method in detecting MB2 canal. When compared to dental loupe (52.5%) and naked eye (25%), the dental loupe improved the detection of MB2 canal.Conclusion:Within the parameter of this study in detecting the incidence of MB2 canal, using CBCT dental loupes and naked eye, detection of MB2 canal was significantly higher with CBCT followed by dental loupe and least with naked eye.
