Coexistence of Cesarean Scar Endometriosis and Intramuscular Endometriosis

Endometriosis is defined as presence of functional endometrial tissue outside the uterus. With decreasing order of frequency, ovaries, peritoneum, subperitoneal deep pelvic cavity, intestinal and urinary system are the most common areas where the disease occurs. Endometriotic foci may develop on the scar tissue after previous surgery or abdominal wall in addition to pelvic region. This is often observed after obstetric and gynecological operations, and it needs to be distinguished with many different pathologies. Therefore, radiological modalities like ultrasound (US) imaging and m agnetic r esonance i maging (MRI) provide important clues for diagnosis. As far as we know, the coexistence of two separate foci, which are in subcutaneous fatty issue and in rectus muscles, has not been specified in any case report related to the abdominal scar endometriosis. Surgical Treatment of Scar Endometriosis Following Cesarean Section, a Series of 12 Cases.

It is difficult to conduct studies with larger series in rarely observed diseases. We report our experience in managing cesarean scar endometriosis (CSE) and emphasize the diagnosis and treatment options. The objective of our study is to review the clinical characteristics of CSE and to evaluate our surgical outcomes. We have collected and documented a case series of 12 patients who underwent surgical wide en bloc excision with surrounding clear margins for CSE. Patients' demographic features, symptoms, and clinical and operative findings were evaluated. The mean age was 34.6years. Cyclical pain was documented in seven patients, while three patients presented with noncyclical pain. Menstrually-related enlargement of the nodule was observed in four patients, and only one patient had a complaint of dark brown leakage. The mean operation time was 26min. The endometriotic lesions ranged from a diameter of 2 to 8cm in size. Patients recovered completely, and no recurrence was observed. To prevent iatrogenic transplantation, additional attention is needed during surgery that exposes endometrial tissue. Complete wide excision of CSE is both diagnostic and therapeutic. To avoid unnecessary referrals, awareness of its typical clinical manifestations remains the mainstay for intervention. The most important issues to be considered during surgery is nonspreading endometriosis while manipulation.

Application of Mini-abdominoplasty after Conservative Excision of Extensive Cesarean Scar Endometriosis

Endometriosis is defined as the presence of functioning endometrial tissue outside the uterine cavity, and the standard treatment is extensive surgical excision. Cesarean scar endometriosis is a type of cutaneous endometriosis arising on or near a Cesarean section scar. A 44-year-old woman presented with a 9×6 cm sized dark-brown, stony-hard, irregular, lower abdominal mass of four years duration. The patient had a history of two Cesarean deliveries, 14 and 16 years ago. Suspecting endometriosis, we excised the tumor conservatively rather than extensively to prevent incisional hernia considering the benign nature of the tumor and the low possibility of recurrence because the patient's age was near menopause, along with simultaneous bilateral salpingo-oophorectomy that was performed in this case. On reconstruction, mini-abdominoplasty was adopted to avoid possible wound complications and cosmetic deformities. The patient was satisfied with the cosmetic results, and neither recurrence nor functional problems occurred during the 1-year follow-up period. Plastic surgeons should keep in mind the possibility of cutaneous endometriosis in an abdominal mass of a female of reproductive age with a previous history of pelvic or intra-abdominal surgery. An optimal result from oncological, functional, and cosmetic standpoints can be achieved with conservative excision followed by mini-abdominoplasty of extensive Cesarean scar endometriosis.

Clear cell carcinoma arising in the abdominal wall: two case reports and literature review

Clear cell carcinoma originating in the abdominal wall is rare and usually develops within endometriotic implants in the scar. We describe 2 patients: a 42 year old with a 15 cm mass on the abdominal wall treated with neoadjuvant chemotherapy and excision and a 51 year old with a 6 cm abdominal mass treated with excision and adjuvant radiotherapy.

Topical Egyptian Bee Honey and Royal Jelly following Post Surgical Removal of Recurrent Cesarean Scar Endometriosis

Aims: Proper healing and avoidance of recurrence are considerable issues following surgical removal of cutaneous endometriosis. The work aimed to show the effect of topical Egyptian bee honey and royal jelly on wound healing and recurrence rate after removal of recurrent cesarean section scar endometriosis.

Scar endometriosis is rare and difficult to diagnose. This condition is often confused with other surgical patologies and preoperative diagnosis is rarely established. Medical treatment is not helpful. The patients required wide surgical excision of the lesion. In this study we are reporting eleven cases of abdominal wall endometriosis which were developed following cesarean section. The mean age of the patients was 28.3 and there were no any operation other than cesarean section. All masses were totally resected with one cm surgical margin. Whenever a female patient is presented with abdominal wall mass previous gynecological operations should be evaluated and endometriosis must be regarded between differential diagnosis.

Caesarean scar endometriosis

Caesarean section incidence is rising and caesarean scar endometriosis could be encountered more in the day to day clinical practice. We present three cases of caesarean scar endometriosis which were managed in our unit with excision biopsy. Complete excision is both diagnostic and therapeutic. FNAC could be a tool to exclude malignancy and allows a quick diagnosis.

Caesarean scar endometriosis presenting as an acute abdomen: a case report and review of literature

To report a case of Caesarean scar endometriosis presenting as acute abdomen and a review of literature. Case report and literature review. A 27-year-old woman presented in Accident and Emergency Department with pain and lump near left edge of pfannenstiel incision scar. After initial investigations the patient underwent examination under anesthesia. Excision of a tumour-like mass adherent to the skin and the surrounding subcutaneous tissue. The mass was dissected free from the surrounding fat tissue and excised with clear margins. Histology of the mass confirmed endometriosis in tumour and showed a 2 cm fibrotic nodule within. In light of increasing rate of caesarean section, it is important to emphasize the early diagnosis as well as optimum management of scar endometrioma. Many recommendations have been given to modify practices at caesarean section to prevent transplantation of decidual endometrial tissue in the abdominal scar but without any published randomised trials.

Leuprolide acetate in the management of cesarean scar endometriosis

Progestin or estrogen-progestin combination therapy has not proven useful in the treatment of endometriosis of the abdominal scar after cesarean delivery. We report our experience in managing this condition with a gonadotropin agonist. A 22-year-old black woman with a history of two previous cesareans developed endometriosis of the abdominal scar. The extent of the lesion was estimated by computed tomographic (CT) scan, and a 6-month preoperative course of leuprolide acetate was administered. The patient exhibited prompt symptomatic response to the gonadotropin agonist, but the physical examination and CT scan findings were unchanged. Pathologic examination after surgical removal of the lesion confirmed the clinical diagnosis. Leuprolide acetate administered to a patient with cesarean scar endometriosis was associated with an improvement in symptoms, but there was no change in lesion size.
