Analysis of the work of nursing staff in the burn unit

The development of new, scientifically based approaches, improvement of existing ones for the organization of treatment and nursing of patients with extensive and deep burns is one of the most important tasks of medicine. The recovery process of burn patients ultimately depends on how coordinated and professional the joint actions of doctors and nurses will be. Target. To analyze the work of the nursing staff of the burn department in order to identify problems in the organization of nursing practice and their prevention in the future. Results. Weaknesses in the organization of nursing practice in the burn department were studied, analyzed and identified, which made it possible to introduce the Nursing Observation Sheet for a Burn Patient and bring it into line with the standard Protocol for the Management of a Burn Patient. Conclusion. The analysis of the assessment of the provision of nursing care to patients with burn injuries revealed the absence of the necessary standard operating procedures (SOP). Currently, only the SOP developed in accordance with the document «Protocol for the Management of a Patient. Bedsores» is in effect.

The correlation of BMI and insulin resistance in moderately burned patients.

<b>Introduction:</b> Obesity is strongly related to serious comorbidities that might affect the healing process. Elevated Body Mass Index (BMI) and insulin resistance have a significant impact on the development of the metabolic syndrome often leading to lethal cardiovascular complications.<b>Aim:</b> The aim of the study was to verify the correlation of BMI and insulin resistance with clinical parameters of moderately burned patients.<b>Materials and methods:</b> There were 149 patients enrolled in the study and their clinical data was retrospectively analyzed. The laboratory tests, insulin demand, BMI, and surgical procedures were evaluated on admission and discharge.<b>Results:</b> Burned patients who required insulin were characterized by worse laboratory results on admission to the burn unit, they had lower hemoglobin (HGB) levels (P = 0.0001), higher creatinine levels by 0.323 units (P = 0.009), higher C-reactive protein (CRP) by approximately 94 units (P = 0.0001), as higher procalcitonin (PCT) by approximately 0.5 units (P = 0.001) as compared to non-insulin-treated patients. Moreover, burned patients who required insulin stayed in the hospital for an average of 10 days longer. All patients from the insulin-demand subgroup had elevated triglycerides (Tg) levels on admission with increased indexes of insulin resistance.<b>Discussion:</b> Our study suggests that the protective effect of a higher BMI in burned patients, known as the 'obesity paradox' may be compromised by insulin resistance.<b>Conclusions and significance of the study:</b> The results show that elevated Tg on admission to the burn unit coexisting with a BMI over 25 kg/m<sup>2</sup> may be used as an important prognostic factor and may help with prediction of insulin demand and worse outcome in moderately burned patients.

The Individual-Level and Community-Level Social Determinants of Burn Injuries: A Single-Institution Study From the Southwestern United States.

Burn injuries are a significant public health concern, causing life-threatening complications and substantial hospitalization costs for patients. It has been shown that burn injuries may affect individuals differently based on demographic factors and socioeconomic status, among other variables. In the Southwestern United States with high ambient temperatures, specific burn etiologies, such as pavement burns, may pose a disproportionately high risk for disadvantaged communities and homeless individuals. This study uniquely explores burn injuries in relation to patients' socioeconomic status in Las Vegas, Nevada by using the Distressed Community Index to quantify socioeconomic status utilizing individual-level and community-level indicators. This single-institution and retrospective study collected data from all patients admitted to a burn center located in Las Vegas. Data were analyzed through Chi-square, one-way ANOVA, and post-hoc analysis with Tukey's test. Patients residing in distressed communities contributed to the greatest number of burn injuries, however there was a lack of significant association between socioeconomic status and burn injury (p = 202). Additionally, specific burn etiologies and demographic characteristics were associated with variations in burn patient hospital course, complications, resources utilized and outcomes. Distressed patients were significantly associated with public insurance (p < 0.001), and public insurance users were associated with pavement burns-one of the most severe burn injuries (p < 0.001). This study emphasizes the importance of developing comprehensive burn prevention resources tailored to vulnerable populations, especially in regions with increased incidence of severe burn injuries, in order to reduce burn burden and mortality.

The Unhoused Burn Population: An Alarming Increase of Leaving Against Medical Advice.

Unhoused burn patients (UBP) have historically been more likely to leave against medical advice (AMA) and suffer worse health outcomes than the general population. The coronavirus disease 2019 (COVID-19) pandemic created a major strain on the healthcare system, resulting in worse overall health outcomes for burn patients. We sought to investigate how COVID-19 impacted treatment for UBP, specifically the rate of leaving AMA. We conducted a retrospective chart analysis of patients admitted to a regional burn center between June 2015 and January 2023. March 1, 2020, was used as a cut point to separate the cohorts into patients seen pre-COVID-19 (p-CV) and during COVID-19 (CV). Outcomes included leaving treatment AMA and readmission within 30 days. 385 patients met criteria for being unhoused and were included in our analytic sample, of which 199 were in the p-CV cohort and 186 in the CV cohort. UBP were significantly more likely to leave AMA during CV compared to p-CV (22.6% vs. 7.5%, p<0.001). Housed burn patients did not experience an increase in discharges AMA during this time period. The COVID-19 pandemic resulted in an increase in discharges AMA among unhoused patients only. While the etiology is unclear, our findings suggest that this vulnerable patient population is receiving inadequate care post-COVID. Future research should determine the driving force behind these increases and identify early interventions to mitigate them.

Effect of Hydroxyethyl Starch (HES) versus 5% Albumin Solution on Intra-Abdominal Pressure in Severe Burn Patients: A Randomized Clinical Study

Abstract Introduction Massive burn patients are at high risk of developing intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) as a complication of aggressive resuscitation. This study aimed to evaluate the effect of Hydroxyethyl starch (HES) versus 5% albumin solution on intra-abdominal pressure (IAP) in massive burn patients. Methods This was a single-center, prospective randomized clinical trial carried on at Ain Shams University (ASU) burn unit for 2 years. Where adult patients with burns 20 to 60 % of TBSA were equally randomized into HES group or albumin group. Results Fifty-two patients were equally randomized into 2 groups. There was no difference in age, sex, weight, type of burn, and TBSA between the two groups. The mean total resuscitation fluid volume in the first 48 hours was 213 ml/kg and 206.2 ml/kg for the HES group and the albumin group respectively (p = 0.674). IAP statistically was non-significantly higher in the HES group. There was no statistical difference between the two groups as regards the renal function tests. Conclusion Both HES and 5% albumin solution are effective and safe colloids for burn resuscitation. They partially have an equal effect on intra-abdominal pressure, renal involvement, and vital data stability in massive burn patients.

Ineffectiveness of carbapenems in real-world clinical practice according to therapeutic drug monitoring data and Roszdravnadzor AIS reports

Relevance. The use of carbapenems is associated with significant variability in pharmacokinetics/pharmacodynamics (FC/PD) parameters, particularly in critically ill patients. The combination of variability in these parameters and standardized dosing regimens can lead to irrational dosing, excessively high or low doses, and consequently less effective treatment and resistance. Optimal management of these factors is essential for combating the development of resistance, particularly for reserve antibiotics.The aim of this study was to evaluate the achievement of the target levels of carbapenems (imipenem/cilastatin) in plasma concentrations in patients with burn injury based on therapeutic drug monitoring and to analyze spontaneous reports registered in the AIS Roszdravnadzor database regarding the effectiveness of therapy.Methods. The analysis included patients receiving antibiotic therapy with imipenem/cilastatin in the burn unit for adults at the University Hospital of the Volga Region Research Medical University for burn trauma who were hospitalized from 01.03.2023 to 30.06.2023. The study was conducted without correcting the trade name of imipenem/cilastatin. Therapeutic drug monitoring was performed after the 4th administration of imipenem/cilastatin. Blood was drawn at 3 h (1st sample), 5 h (2nd sample), and 8 h (3rd sample) after infusion in a clotting activator tube. The analysis was performed using a liquid chromatograph "LC-20 Prominance" (Shimadzu, Japan) in reversed-phase mode with a matrix photodiode detector for UV and visible spectra. Data processing was performed using the LCsolution program. Spontaneous reports regarding the use of carbapenems recorded in the AIS of Roszdravnadzor from January 2020 to November 2023 were also analyzed as the object of the study.Results. The results of the study of carbapenems (imipenem/cilastatin) concentration level achievement in the plasma of patients with burn injury showed that the effective imipenem concentration exceeding the MPC value in relation to the isolated Gram-negative pathogen was found only in 1 out of 5 patients included in the study. Two patients showed dynamically changing IPC values during treatment, indicating the necessity of therapeutic drug monitoring, as well as the probability of failure to achieve target concentrations and optimal FC/FD values. In another two patients, imipenem concentrations were insufficient to maintain optimal FC/PD values, indicating that the antimicrobial regimen was ineffective. An analysis of spontaneous reports registered in the AIS of Roszdravnadzor regarding identified cases of the ineffectiveness of carbapenems (imipenem/cilastatin, meropenem) revealed 5,2% of reports regarding meropenem and 1.4% regarding imipenem/cilastatin in the total structure of reports.Conclusion. The implementation of therapeutic drug monitoring procedures can reduce therapy ineffectiveness and antibiotic resistance through personalized antimicrobial dosing.

Hyperbaric oxygen (HBO2) therapy in thermal burn injury revisited. Pressure does matter. Review.

For over five decades, many experimental and clinical studies have shown predominantly positive but controversial results on the efficacy of hyperbaric oxygen (HBO2) therapy in burns. The study aimed to define a common denominator or constellations, respectively, linked to the effects of HBO2 in burns with a special focus on dosage parameters. Based on original work since 1965, species, number of individuals, type of study, percentage of total body surface area (TBSA), region, depth of burn, causative agent, interval between burn and first HBO2 session, pressure, duration of individual session, number of HBO2 sessions per day, cumulative number of HBO2 sessions and type of chamber were assessed. Out of 47 publications included, 32 were animal trials, four were trials in human volunteers, and 11 were clinical studies. They contained 94 experiments whose features were processed for statistical evaluation. 64 (67.4%) showed a positive outcome, 16 (17.9%) an ambiguous one, and 14 (14.7%) a negative outcome. The only factor independently influencing the results was pressure with ATA (atmospheres absolute) lower than 3 ATA being significantly associated with better outcomes (p=0.0005). There is a dire need for well-designed clinical studies in burn centers equipped with hyperbaric facilities to establish dedicated treatment protocols.

Study of the Influencing Factors and their Predictive Value of Skin Graft Take in Early Excision and Grafting in Severe Burns: A Prospective Study

Abstract Background Burn injury is one of the most life-threatening traumas worldwide especially in developing countries. After severe burns, the patients suffer from loss of multiple functions of skin, such as fluid loss, and high infection rate with burn sepsis and septic shock. Objective To investigate the various factors that affect the survival rate of skin grafts in early excision and grafting procedures (from 3rd to 7th day after burn) in patients with severe burns. Patients and Methods This Prospective cohort study was done in Ain Shams University Burn Center attached to Plastic, burn and maxillofacial department during the period from May 2021 to May 2022. Results Analysis was performed on 75 individuals who had early burnt skin excised and early skin graft coverage. Two patient groups were analyzed, in the form of good skin graft take &amp;gt;70% in 45 patients, and poor skin graft take &amp;lt; 70% in 30 cases, based on the survival rate of the skin graft post operatively. Conclusion The univariate analysis and repeated measurement showed that burn intensity, the post-operative sepsis, the neutrophil lymphocytic ratio on day 3 (NLR3) in the poor skin graft take group were significantly higher than those in the good skin graft take group, while the estimated albumin levels in perioperative period and the estimated pre-albumin levels in perioperative period were higher in the good skin graft take group. Receiver operating characteristic curve showed that NLR3, the estimated albumin levels and pre-albumin levels in perioperative period had a good predictive value for the survival of skin grafts. The estimated albumin levels ≤3.18 gm/dl, estimated pre-albumin ≤23.9 mg/dl, and NLR3 ≥ 12 were independent risk factors for graft take in perioperative period.

Resource Requirements in a Burn Mass Casualty Event.

Burn mass casualty event occurrences are rare but will place significant burdens on any burn unit or healthcare system. Effective disaster preparedness plays a significant role in mitigating the aftermath of a burn mass casualty. The aim of this study was to assess the resource requirements during the initial two weeks of a burn mass casualty event. Eight patients in a burn mass casualty event were simulated using the Emergo Train System®. These simulated patients were matched with real historical patients treated in our burn centre, and their resource requirements were analysed. An average of eight staff is required to care for a patient per day along with almost 75 h of operating time (excluding anaesthesia and turnover time). A substantial quantity of consumables was used in the first two weeks. This study has demonstrated the substantial material consumption and staff requirements in the first two weeks of management in a burn mass casualty event. Such findings will offer valuable insight for disaster preparedness planning and resource management strategies.

Prevalence and characteristics of paediatric burns at the National Reconstructive Plastic Surgery and Burns Centre, Korle Bu Teaching Hospital, Ghana

IntroductionBurn injuries among children have emerged as a significant global public health concern, accounting for substantial morbidity and mortality worldwide. Low and Middle-Income Countries bear a significant burden of paediatric burn injuries. In Ghana, limited data exists on the prevalence, characteristics and outcomes of paediatric burns. This study provides vital data on the local burden and epidemiology of paediatric burns at Ghana’s main burns treatment center. The findings have significant value in informing prevention strategies, allocating resources, and improving quality of paediatric burn care. Study objectiveTo establish the prevalence and characteristics of paediatric burns injuries in Ghana’s leading burn center. MethodologyA retrospective cohort study analyzed data from 662 adult and paediatric patients who presented with burns from all over Ghana between 2020 and 2022. The study encompassed medical records of paediatric burn cases (aged 0–14 years) admitted to the center from January 2020 to December 2022. A purposive sampling technique was used to select cases based on the inclusion criteria and elimination of cases with 20 % missing data. A total of 335 paediatric records were reviewed. Data analytical methodsDescriptive statistics, such as frequencies, mean, percentages were used to establish period prevalence and demographic characteristics of paediatric burns. Chi-square analysis and univariate logistic regression was utilized to explore associations between categorical variables, and predictors of burn outcome. ResultsThe period prevalence of paediatric burns was found to be 50.6 %. Paediatric burn cases predominantly affected males (57.6 %). Majority of cases fell within the age range of 1 to 5 years (70.1 %). Hot water emerged as the leading cause (58.4 %), whiles superficial partial thickness burns (51.9 %) was the predominant type of burn. Most cases involved burns affecting less than 20 % Total Body Surface Area (59.0 %). Multi-part body injuries were common (87.2 %), with the lower limb being the most affected (25.36 %). The mortality rate was calculated to be 21.0 %. Higher TBSA was a significant predictor of mortality (p < 0.001). ConclusionThis study found a high prevalence of paediatric burns with hot water and foods being the predominant causes. Higher TBSA predicted lower chances of survival. The findings have scientific value in informing prevention strategies, allocating resources, and improving the quality of care.

The relationship between job satisfaction and motivation levels of nurses working in burn units

BackgroundFor nurses working in specialized units such as burn units, having adequate levels of job satisfaction and motivation is crucial for enhanced productivity in order to maintain quality nursing care. MethodsA cross-sectional, descriptive, correlational design was used in the study. The study aimed to investigate the job satisfaction and motivation levels of nurses working in burn units, the relationship between job satisfaction and motivation levels, and the influencing factors. The study sample consisted of 73 nurse managers/clinicians who worked in burn services or intensive care units in September and December 2023. The data were collected with “Demographic and work characteristics form,” “Job Satisfaction Scale for Nurses (JSSN)”, “Nurse Job Motivation Scale (NJMS)”. Data were analyzed using Independent Samples T-test, One-Way ANOVA, and Mann-Whitney U tests to compare the descriptive characteristics of nurses and the scale scores. Correlation and regression analyses were conducted to reveal the relationship between the descriptive characteristics and scale scores of the nurses. ResultsThe mean age of the nurses was 30.26 ± 7.99 years, and 60.3 % were bachelor's degree graduates. The nurses had an average of 8.64 ± 8.25 years of experience in the profession and an average of 4.77 ± 5.39 years in the burn unit. The total score average for the NJMS was found to be 66.49 ± 5.56, and the JSSN score average was 3.46 ± 0.58. A weak negative relationship was found between the JSSN subscale "Suitable Support from Seniors" and age (p < 0.05), "Perceived importance at the job" and the working years in the burn unit (p < 0.05). ConclusionsA positive correlation was observed between job satisfaction and job motivation of the nurses. If older nurses cannot get support from their supervisors, their satisfaction will be decreased. In addition, if more experienced nurses in the burn unit do not feel importance on the job, their satisfaction will be decreased.

Modifying burn care in a low- and middle-income countries (LMIC) to meet global best practice: A burn unit or burn isolation ward?

The overwhelming burden of burns in low-income (LIC) and low-middle-income (LMIC) countries has been well-documented. Though best global practice is for major burns to be treated in burn units, the prohibitive cost makes it difficult. In this article we attempt to present the improvement in outcome recorded over a period of 3 years as we transitioned from nursing our burns patients in the general surgical wards (Group A), then a separate cubicle within the wards (Group B) and then an isolated burn ward (Group C). Other practices such as physiotherapy, traditional wound dressings, and limb splinting remained the same.Data of patients who met criteria for burn unit admission during these periods were analysed and compared. The groups were matched across age, depth of burn, total body surface area burned (TBSA) and length of hospital stay (LOHS). There was an improvement in the lethal area 50% (LA50) at 31.1 %, and in mean LOHS of 18 ± 3.8 days in group C.Most LICs and LMICs do not have functional health insurance schemes for burns patients and overall resources allocated for healthcare cannot support a state-of-the-art burn unit. Our report attempts to encourage such countries to adapt global practice to their economic reality. Minimal changes like an isolated burn ward, separate shower room for wound dressings, strict hygiene practices, and limiting visitor traffic may go a long way to improve burn patient outcome.

Epidemiology and first aid measures in pediatric burn patients in northern China during 2016-2020: A single-center retrospective study.

Burn and scald injuries are the fourth most common type of trauma. Pediatric burns account for a high proportion of the total number of burn patients and impose a high burden on public health. Understanding the epidemiology of pediatric burns can help improve science education and reduce the incidence of burn injuries. This study is a single-center retrospective study. One thousand five hundred and twenty-seven pediatric burn patients admitted to our burn center from January 2016 to December 2020 were included. Demographic and epidemiological data of included patients were extracted and analyzed. The correlations of categorical data were tested by the Chi-square tests, and differences of continuous data were tested by the Kruskal-Wallis tests. A p-value of less than 0.05 was considered to be statistically significant. The results showed that children under 3 years of age were most susceptible to burn and scald injuries. Burn injuries were most likely to occur in the season of winter and at the place of home. 56.6% of included patients did receive first aid measures, while 1.8% received gold-standard first aid. Clinical variables related to the severity of injuries were statistically different between patients with and without cooling measures in first aid. Linear regression models showed that emergency treatment of burns in children and adolescents was associated with outcome indicators, including number of operations, total operation duration per total burn surface area (TBSA), cost per TBSA, and length of stay per TBSA. This study summarized the epidemiology and outcomes of pediatric burn patients admitted to a burn center in northern China. Adopting cooling measures in first aid can reduce the severity of injuries and reduce the burden on the medical system. Education on burn prevention and first aid measures to caregivers of children, especially preschool children, should be strengthened.

Frequency of fistula in patients operated for primary cleft palate repair in a tertiary care center.

Objective: To determine the frequency of fistula in patients undergoing primary cleft palate repair. Study Design: Descriptive Case Series study. Setting: Burns and Plastic Surgery Center, HMC, Peshawar. Period: February 2023 to December 2023. Methods: Patient of both genders males and females undergoing primary cleft palate repair were included in the study with age ranging from 9 to 18 months age excluding patients with comorbids and syndromic patients. Non-probability consecutive sampling was adopted. The data was acquired from patients and hospital record and analyzed with statistical package for social sciences (SPSS). Results: Our study shows that among 169 patients, mean age was 10 months with SD ± 5.11. 100(59%) patients were male and 69(41%) patients were female. More over 3(2%) patients had fistula while 166(98%) patients didn’t had fistula. Conclusion: In this study we observed an internationally acceptable frequency of post-palatal repair fistula for primary cleft palate in our study population.

Severity of Inhalation Injury and Risk of Nosocomial Pneumonia: A Retrospective Cohort Study

BackgroundThe impact of inhalation injury on risk of nosocomial pneumonia, an important complication in burn patients, is not well established. Research QuestionIs more severe inhalation injury associated with increased risk of nosocomial pneumonia? Study Design and MethodsWe performed a retrospective cohort study of patients with suspected inhalation injury admitted to a regional burn center from 2011 to 2022 who underwent diagnostic bronchoscopy within 48 hours of admission. We estimated the association of high-grade inhalation injury (abbreviated injury score [AIS] 3-4) versus low-grade inhalation injury (AIS 1-2) with nosocomial pneumonia (NP) adjusted for age, burn size, and comorbid obstructive lung disease. Death and hospital discharge were considered competing risks. ResultsOf the 245 patients analyzed, 51 (21%) had high-grade injury, 180 (73%) had low-grade injury, and 14 (6%) had no inhalation injury. Among the 236 patients hospitalized for >48 hours, NP occurred in 24/50 (48%) patients in the high-grade group, 54/172 (31%) in the low-grade group, and 2/14 (14%) in the no inhalation injury group. High-grade (vs low-grade) inhalation injury was associated with higher hazard of NP in both the proportional cause-specific hazard model (CSHR 2.04; 95% CI, 1.26-3.30; p=0.004) and Fine-Gray subdistribution hazards model (SHR for NP, 2.24; 95% CI, 1.38-3.64; p=0.001). InterpretationAmong patients with inhalation injury, more severe injury was associated with higher hazard of NP in competing risk analysis. Additional research is needed to investigate mechanisms that may explain the relationship between inhalation injury and NP and to identify more effective prevention strategies.

Aminoglycoside Susceptibility among Extensive Drug Resistant Pseudomonas aeruginosa from Hospitalized Patients in Al-Diwaniyah, Iraq

Abstract Background: Pseudomonas aeruginosa is one of the most prevalent Gram-negative microbes associated with serious and fatal nosocomial infections. It is routine to use aminoglycosides as a treatment for P. aeruginosa infections. Objectives: The dissemination of aminoglycoside resistance offers a major challenge to the treatment of life-threatening infections caused by these bacteria. Aim of Study: Determination of aminoglycoside-modifying enzymes and 16S ribosomal RNA methylases in extensive-drug resistant (XDR) P. aeruginosa isolates. Materials and Methods: From November 2021 to August 2022, a total of 200 samples were collected in this cross-sectional study including burns (n = 80, 40%), wounds (n = 66, 33%), and diabetic foot ulcers (n = 54, 27%) from admitted patients to Al-Diwaniyah Burn Center and Al-Diwaniyah Teaching Hospital. The identification and antibiotic susceptibility profile of P. aeruginosa were done by vitek2 compact system. The isolates were subjected to polymerase chain reaction assays with specific primers for ant(4’)-IIa, ant(4’)-IIb, acc(6’)-Ia, aph(3’)-IIb, rmtA, and rmtD. Results: The recovery rate of P. aeruginosa isolates was (n = 50, 25.0%) from the clinical samples. Antibiotic-susceptibility patterns demonstrated that 18% of the isolates were multi-drug resistant and 22 (44%) were XDR. The XDR isolates were resistant to all 14 antibiotics related to the seven antibiotic classes tested in this study. The prevalence of aminoglycoside resistance genes among XDR isolates is ant(4’)-IIa (22.7%), (4’)-IIb (27.3%), acc(6’)-Ia (18.18%), aph(3’)-IIb (100%), rmtA (36.36%), rmtD (36.36%), mexZ and parR (100%). Conclusion: Increased resistance to aminoglycosides in Al-Diwaniyah Hospitals serves to highlight how critical this issue is when treating multidrug-resistant P. aeruginosa infections that are life-threatening. All the resistance isolates harbored aph(3’)-IIb gene.

Microcirculatory depth of focus measurement shows reduction of tissue edema by albumin resuscitation in burn patients

BackgroundSevere burns induce volume shifts via capillary leaks, eventually requiring massive fluid resuscitation and promoting tissue edema. Albumin may help to mitigate the edema, thereby improving perfusion. This study shows that sublingual microcirculation measurements can quantify both tissue perfusion and edema. MethodsThis prospective observational study was conducted between November 2018 and December 2019 in the intensive care unit of Maasstad Hospital Burn Center, Rotterdam, The Netherlands. Patients with severe burns affecting >15% of the total body surface area were included. Fluid management was conducted in accordance with the Parkland formula. Albumin (20%) was administered at a rate of 0.5 mL/(kg·h), starting 12 h after the burn incident. Alterations in the sublingual microcirculation, including capillary perfusion and density, were measured at admission (T0) and at 4 h (T4) and 12 h (T12) after admission. Sublingual depth of focus (DOF) of the microcirculation was used to quantify the tissue edema. ResultsNine patients were recruited with a mean total body surface area of 36% ± 23%. By T12, a median of 4085 mL (interquartile range [IQR]: 3714–6756 mL) of crystalloids and 446 mL (IQR: 176–700 mL) of 20% albumin were administered. The DOF increased significantly after crystalloid administration (T4 vs. T0, mean difference [MD]=27.4 µm, 95% confidence interval [CI]: 3.4 to 50.9, P=0.040). Following albumin administration, DOF significantly decreased (T12 vs. T4, MD=-76.4 µm, 95% CI: -116.6 to -36.1, P=0.002). Total vessel density decreased significantly with crystalloid administration (T4 vs. T0, MD=-3.5 mm/mm2, 95% CI: -5.7 to -1.4, P=0.004) but increased after albumin administration (T12 vs. T4, MD=6.2 mm/mm2, 95% CI: 3.2 to 9.3, P=0.001). ConclusionSublingual microcirculation measurement of DOF and other parameters provide a valuable tool for the assessment of tissue perfusion and edema in patients with severe burns. Further investigation is required to evaluate the role of albumin in increasing microcirculatory convection and reducing tissue edema.

The prevalence and predictors of reconstructive surgery in pediatric burn care

ObjectiveThis study aimed to examine the prevalence and predictors of reconstructive surgery among pediatric burn patients in the Netherlands. MethodsPediatric burn patients were identified through the Dutch Burn Repository R3. Eligibility criteria included a burn requiring hospital admission or surgical treatment at one of the Dutch burn centers in 2009–2019. First, patient, burn, and treatment characteristics were summarized using descriptive statistics. Second, time to the first reconstructive surgery was modelled using Kaplan Meier curves. Third, a prediction model was developed using univariate and multivariate logistic regression. The model’s performance was assessed using calibration, discrimination, and explained variance. Fourth, internal validation was performed using bootstrapping. ResultsApproximately three percent (n = 84) of pediatric patients (n = 3072) required reconstructive surgery between the initial burn-related hospital admission and September 2021. Median time to the first reconstructive surgery was 1.2 (0.7–1.6) years. Most surgeries were performed on the face, arm, neck, hand, or anterior trunk, owing to contractures or hypertrophic scarring. Predictors of reconstruction included the etiology, anatomical site, extent of full-thickness burn, surgical treatment in the acute phase, and length of hospital stay. ConclusionOur study provided an overview of the prevalence and independent predictors of reconstructive surgery in the pediatric burn population.

The predictive role of Interleukin 6 and Procalcitonin associated with bacterial infection causing sepsis in patients with severe burn

Background. Sepsis is a major contributor to both death and illness on a worldwide scale. Bacteremia characterized by the presence of gram-negative, gram-positive, or mixed growth bacteria has been shown to be associated with a significantly increased risk of septic shock and a bad prognosis. Biomarkers are used to aid in the diagnosis of sepsis and to differentiate between bacterial causes, namely Gram-negative, Gram-positive, and mix growth bacteria. Objectives. This study aimed to evaluate the role of PCT and IL-6 in predicting bacterial infection in burn patients with sepsis due to gram-negative, gram positive and mixed growth bacteria. Methods. Forty-four patients with age (1.5–75 years), and nineteen healthy controls with the same age range, were involved in this study during their attendance to Specialized Burns Hospital in the Medical City, Al-Kindi Teaching Hospital in Baghdad, and Al-Nasiriyah General Hospital in Thi-Qar province. The study was conducted from 1st December 2023 to 1st April 2024 and approved by ethical committees of Institute of Genetic Engineering and Biotechnology for Postgraduate Studies/ Baghdad University. Blood samples and data were collected to evaluate the level of PCT and IL-6 associated with bacterial infection for burn patients by enzyme linked immunosorbent assay (ELISA). Results. 25 (50%) showed growth for Gram-negative, 13 (26%) for Gram-positive and 12 (24%) for mix growth bacteria. Results appeared a significant increase at (P≤0.01) in each of PCT and IL-6 in burn patients to control group according to gender. Patients exhibiting a combination of bacterial growth have notably elevated levels of procalcitonin (PCT). Levels of PCT were significantly elevated in burn patients with mixed growth sepsis compared to patients with Gram-negative and Gram-positive sepsis. However, there were no significant differences in IL-6 levels between burn patients with mix growth sepsis and patients with gram-negative and gram-positive sepsis (P&gt; 0.05). Measuring PCT levels may assist in selecting the appropriate empirical antibiotic treatment. Conclusions. The PCT biomarker demonstrated superior performance in distinguishing between microbial sepsis caused by various kinds of pathogens. This finding has great promise for its use in clinical practice, particularly in burn units.
