CT-guided Percutaneous Microwave Ablation Combined with Local Radiotherapy or Chemotherapy of Malignant Pulmonary Tumors.

The study aimed to investigate the clinical efficacy of CT-guided microwave ablation (MWA) combined with 125I seed implantation or bronchial arterial infusion (BAI) chemotherapy in the treatment of malignant pulmonary tumors. A total of 56 patients who underwent MWA, MWA combined with 125I particle implantation, or MWA combined with BAI chemotherapy for advanced lung cancer or metastatic lung cancer from January 2015 to June 2021 in Guangdong Provincial People's Hospital were enrolled. Among them, 21 patients were treated with MWA (MWA), 18 with MWA combined with 125I seed implantation (MWA+125I), and 17 with MWA combined with BAI chemotherapy (MWA+BAI). The short-term outcomes, complications, Eastern Cooperative Oncology Group (ECOG) performance score (Zubrod-ECOG-WHO, ZPS), survival, and factors related to survival were compared between the three groups. The response rate of the MWA group (9.52%) was significantly lower than that of the MWA+125I group (50.00%) and MWA+BAI chemotherapy group (47.06%), and the differences were statistically significant (p < 0.05). The incidence of complications in the MWA, MWA+125I, and MWA+BAI chemotherapy groups was 47.62%, 55.56%, and 52.94%, respectively, with no significant difference (p > 0.05). Three months after the treatment, the ZPS of the MWA+125I and MWA+BAI chemotherapy groups was significantly lower than before treatment and significantly lower than that of the MWA group in the same period; the differences were statistically significant (p < 0.05). The median survival time of the MWA+125I group was 18 (9.983, 26.017) months and that of the MWA+BAI chemotherapy group was 21 (0.465, 41.535) months, both of which were higher than that of the MWA group [11 (6.686, 15.314) months]; the differences were statistically significant (p < 0.05). Cox regression analysis was performed on the factors related to survival and revealed treatment mode as a protective factor [HR = 0.433, 95% CI = (0.191, 0.984), p = 0.046]. Other factors, such as gender, age, and tumor size, did not independently affect survival. CT-guided MWA combined with 125I seed implantation and MWA combined with BAI chemotherapy are safe and effective for the treatment of advanced lung cancer and metastatic lung cancer, and can control tumor progression and prolong survival time.

Drug-eluting beads bronchial arterial chemoembolization/bronchial arterial infusion chemotherapy with and without PD-1 blockade for advanced non-small cell lung cancer: a comparative single-center cohort study.

Drug-eluting beads bronchial arterial chemoembolization (DEB-BACE)/bronchial artery infusion chemotherapy (BAI) have been investigated as treatment options for advanced non-small cell lung cancer (NSCLC), especially for those patients who develop refractoriness to or are intolerant to systemic chemotherapy. This retrospective study aimed to compare the outcomes of DEB-BACE/BAI with and without programmed cell death protein 1 (PD-1) blockade for advanced NSCLC, and to investigate the effectiveness and safety of combination regimens. This retrospective cohort study included advanced NSCLC patients who were intolerant to or were resistant to systemic chemotherapy, radiotherapy, or molecular targeted therapy and underwent DEB-BACE/BAI between October 2016 and October 2021 in Beijing Hospital, National Center of Gerontology. A total of 84 advanced NSCLC patients (DEB-BACE/BAI + PD-1 blockade group: group A, n=27; DEB-BACE/BAI: group B, n=57) were enrolled finally. The embolic agent CalliSpheres (100-300, 300-500, or 500-700 µm) loaded with gemcitabine (800 mg) was administered during the DEB-BACE procedure. The adverse events (AEs) and outcomes were compared. Of these, the median progression-free survival (PFS) and overall survival (OS) were compared via Kaplan-Meier (KM) methods. Univariate and multivariate Cox regression analyses were used to investigate the predictors of PFS and OS. KM methods showed that group A had longer median PFS (12.0 vs. 3.0 months, P<0.001) and OS (27.0 vs. 8.0 months, P<0.001) than group B. The predictors of PFS for DEB-BACE/BAI included tumor diameter (P=0.013), immunotherapy (P<0.001), and DEB-BACE/BAI cycles (P=0.012), whereas the predictors of OS included tumor diameter (P=0.021), extrapulmonary metastases (P=0.041), immunotherapy (P<0.001), and DEB-BACE/BAI cycles (P=0.020). The incidence rates of overall AEs in groups A and B were 40.7% (11/27) and 36.8% (21/57), respectively, and no significant difference was found (P=0.731). Group A had an incidence rate of 11.1% for grade 3 immunotherapy-related AEs (irAEs). There were no incidences of ectopic embolization or spinal artery injury. Compared with DEB-BACE/BAI, PD-1 blockade plus DEB-BACE/BAI could improve the prognosis for advanced NSCLC despite the associated risk of grade 3 irAEs. The combination regimens are promising and safe approaches for advanced NSCLC.

Efficacy and Safety of Super-Selective Bronchial Arterial Infusion Chemotherapy in the Treatment of Advanced Non-Small Cell Lung Cancer

Objective: The objective was to study the effectiveness and safety of super-selective bronchial arterial infusion (SBAI) chemotherapy in the treatment of advanced stage non-small cell lung cancer (NSCLC). Methods: The clinical data of 120 advanced NSCLC patients were retrospectively analyzed. Among them, 60 NSCLC patients were treated with SBAI method, another 60 NSCLC patients received systemic intravenous chemotherapy as the control group. The efficacy and safety between two groups of patients were compared. Results: The objective response rate and disease control rate of NSCLC patients treated with SBAI were significantly higher than those of the control group (p < 0.05). The 3-month progression-free survival (PFS) rate (96.67%) and 6-month PFS rate (86.67%) of the SBAI group were significantly higher than those of the control group (73.33% and 56.67%) (p < 0.01). After treatment, the FACT-L scores of patients in the SBAI group were significantly higher than those of the control group (p < 0.05). The scores of all the 13 core symptom items and six symptom interference items of NSCLC patients in the SBAI group were lower than those of the control group (p < 0.05). The adverse reactions rate in the SBAI group were significantly lower than those in the control group (p < 0.05). Conclusion: The short-term efficacy of SBAI chemotherapy for advanced NSCLC is significantly higher than that of traditional peripheral intravenous chemotherapy, and it can significantly improve patients’ quality of life and reduce the incidence of adverse reactions.

Combination of Bronchial Arterial Infusion Chemotherapy plus Drug-Eluting Embolic Transarterial Chemoembolization for Treatment of Advanced Lung Cancer—A Retrospective Analysis of 23 Patients

PurposeTo determine the efficacy and safety of the combination of bronchial arterial infusion (BAI) chemotherapy and transarterial chemoembolization with the use of drug-eluting embolic (DEE) particles in the treatment of unresectable advanced lung cancer. Materials and MethodsA retrospective review was performed of 23 patients with unresectable lung cancer (stage III/IV) who received BAI chemotherapy and DEE chemoembolization. Treatment response was assessed by enhanced CT and evaluated on the basis of Response Evaluation Criteria In Solid Tumors at 30 d after the last combination treatment. Patients were followed up until death or March 15, 2020, whichever was first. Overall survival (OS) was estimated by Kaplan–Meier analysis, and factors associated with OS were evaluated by Cox proportional-hazards test. ResultsComplete response, partial response, stable disease, and progressive disease were seen in 2, 16, 5, and 0 patients at 30 d after the last combination treatment, respectively; therefore, the overall response rate was 78.3% and the disease control rate was 100%. Preprocedure symptoms (hemoptysis in 7 patients and dyspnea in 10) resolved in all cases after combination therapy. Nineteen patients died during follow-up, and 4 survived. Median OS was 15.6 mo (95% confidence interval, 10.1–21.1 mo). On univariate analysis and multivariate analysis, tumor/node/metastasis staging was an independent risk factor for prognosis. There were no serious adverse events during the procedures. ConclusionsThe combination of BAI chemotherapy plus DEE chemoembolization appears to be a promising method for treatment of advanced lung cancer.

Clinical effect of bronchial arterial infusion chemotherapy and CalliSpheres drug-eluting beads in patients with stage II-IV lung cancer: A prospective cohort study.

BackgroundCalliSpheres are drug‐eluting beads used for tumor artery embolization, with clinical benefits in a number of cancer types. The aim of the study was to examine the clinical benefits and complications of patients with stage II–IV lung cancer treated with CalliSpheres drug‐eluting beads for embolization versus conventional vascular interventional treatment.MethodsThis was a prospective cohort study conducted from August 2018 to May 2019. The patients were grouped according to traditional bronchial arterial infusion chemotherapy (infusion group) or bronchial arterial chemoembolization with CalliSpheres drug‐eluting beads loaded with adriamycin (CallisSphere group). Short‐term effects, serum tumor markers, and adverse reactions during follow‐up were compared between the two groups.ResultsThere were 60 participants enrolled into the study with 30 in each group including 54 men and six women, 42–78 years of age. In the CalliSphere group, compared with the infusion group, the disease control rate was 93.3% versus 73.3% (P = 0.080) and the objective remission rate (ORR) was 86.7% versus 60.0% (P = 0.039); the three‐ and six‐month progression‐free survival (PFS) and six‐month overall survival (OS) were better in the CalliSphere group (three‐month PFS: 96.7% vs. 73.3%, P = 0.026; six‐month PFS: 87.5% vs. 57.1%, P = 0.045; six‐month OS: 87.5% vs. 52.7%, P = 0.024); after treatment, the tumor markers in the CalliSphere group were lower (CEA: P < 0.001; CYFRA21‐1: P = 0.014). Clinical effect of selective bronchial artery infusion chemotherapy for central squamous cell carcinoma of the lung

Objective To investigate the clinical effect of selective bronchial artery infusion chemotherapy for central squamous cell carcinoma of the lung. Methods 93 cases of central lung squamous cell carcinoma patients who were treated in our hospital from February 2015 to February 2017 were enrolled in the study. They were divided into the observation group (n=45) and the control group (n=48) according to the patients' final treatment. The observation group was treated with bronchial artery perfusion chemotherapy, and the control group was treated with systemic venous chemotherapy. The short-term effect and the improvement of lung function in the two groups were observed, and the prognosis of the patients was followed up. Results The short-term effect of the observation group was better than that of the control group (P<0.05), and the total effective rate of the observation group was 95.56%; The forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1) of the observation group were (75.39±6.88)% and (78.19±7.79)%, respectively, which were significantly higher than those in the control group (P<0.05). The reoperation ratio of the observation group was 44.44% after chemotherapy, which was significantly higher than that of the control group, with statistically significant difference (P<0.05). The median total survival time of the observation group was 17 months, which was significantly higher than that of the control group, with statistically significant difference (P<0.05). The incidence of adverse reaction in the observation group was 6.67%, which was significantly lower than that in the control group, with statistically significant difference (P<0.05). Conclusions Selective bronchial artery infusion chemotherapy for central lung cancer is effective and worthy of clinical application. Bronchial arterial infusion chemotherapy combined with icotinib hydrochloride to treat advanced non-small-cell-lung cancer: A randomised controlled trial

Background: Both interventional therapy and molecular targeted therapy are important methods of treatment for lung cancer, but the combined use of the two methods has been less commonly reported. We aimed to observe and evaluate the safety and clinical effectiveness of bronchial arterial infusion chemotherapy (BAI) combined with icotinib hydrochloride as first-line treatment for advanced lung cancer. Methods: 24 adult patients with advanced non-small-cell lung cancer (NSCLC) received BAI only (control group), while 22 adult patients received BAI plus icotinib (combined group). The study was done in the department of oncology, West China Hospital, Sichuan University, from January 2014 to January 2017. Icotinib was given to patients of the combination group until they had severe side effects or could not tolerate. The clinical effect and safety was compared and data analysed using statistical software SAS version 9.4. The study registration number is ChiCTR-OPN-15006795. Findings: In the combined group, the response rate (RR) was 86.36%, while the RR was 29.00% in the control group (p<0.0001). In the combined group, after 1 months of the treatment, the incidence of quality of life (QOL) improvement was 86.36%, which was higher than the 58.33% of the control group (p=0.0349). Both of the two groups had no severe complication. The number of BAI regimens for the combined group and control group were 1.5±0.74 and 3.71±1.46, respectively (p<0.0001). There was no significant difference in 1-year cumulative survival rate between the two groups (p=0.7524). Interpretation: Combined BAI plus icotinib treatment can lead to better short-term clinical efficacy and improved QOL, reduced BAI number, and does not increase the incidence of complication. However, the limitation of this trial was that it did not compare the clinical effect and safety of icotinib alone versus other regimens in the treatment advanced lung cancer. Funding: None. Selected arterial infusion chemotherapy combined with target drugs for non-small cell lung cancer with multiple brain metastase

背景与目的本研究旨在回顾性分析选择性动脉灌注化疗联合赖氨酸激酶抑制剂（tyrosine kinase inhibitor, TKI）药物治疗非小细胞肺癌（non-small cell lung cancer, NSCLC）多发脑转移的临床疗效及预后相关因素。方法自2008年9月-2011年10月共入组31例诊断明确的非小细胞肺癌经CT或MRI证实多发脑转移瘤（> 3个）的患者，行选择性颅内动脉、支气管动脉及相关靶动脉化疗药物灌注化疗2个-6个周期，每周期间隔4周，同步或后续联合厄洛替尼、吉非替尼或埃克替尼治疗。介入治疗2个周期或联合靶向治疗后每4周应用CT和MRI对肿瘤进行疗效评价，直至肿瘤进展或发生不可耐受性化疗药物不良反应。结果31例患者平均行3个周期介入治疗，随访时间4个-40.9个月，完全缓解5例（16.1%），部分缓解7例（22.6%），疾病稳定11例（35.5%），疾病进展8例（25.8%）。客观缓解率（objective response rate, ORR）为38.7%，疾病控制率（disease control rate, DCR）为74.2%。中位无进展生存期（progression free survival, PFS）为13.1个月，中位总生存期（overall survival, OS）为15.1个月。6个月的生存率为79%，1年生存率为61.1%，2年生存率为31.1%。分层分析显示PFS、OS与吸烟状态、病理类型、颅外转移情况、靶向药物应用时间、PS评分具有相关性；与性别、年龄、既往治疗情况、脑转移数目无明显相关。结论选择性动脉灌注化疗联合靶向药物是治疗非小细胞肺癌多发脑转移安全有效的方法之一，吸烟状态、PS评分、肿瘤病理类型、颅外转移状况、靶向药物应用时间均可影响患者预后。

Successful closure of intractable tracheoesophageal fistula using a combination of a modified silicon stent and metallic stents

Although surgery is the usual management strategy for acquired benign tracheoesophageal fistula, sometimes this approach is contraindicated or the patient declines surgical management. In this report, we describe a case of a patient with tracheoesophageal fistula at the level of the carina due to bronchial arterial infusion chemotherapy. Closure could not be achieved in response to multiple treatment strategies, including airway stenting, esophageal stenting, occlusion with microcoils, or cyanoacrylate glue. We subsequently achieved closure of this fistula through the combination of a modified silicon stent and metallic stents.

Combination chemotherapy of gemcitabine and cisplatin by double way plus implantation of radioactive seed 125I in treating stage III non-small cell lung cancer

Objective To assess the therapeutic effect of combination chemotherapy of gemcitabine and cisplatin by double way plus implantation of radioactive seed 125I implantation in treating stage Ⅲ non-small cell lung cancer. Methods Sixty cases with stage Ⅲ non-small cell lung cancer were randomly divided into two groups with random number table. In group A (in interventional treatment group, n = 30),the gemcitabine 1000 mg/m2 and one third of the cisplatin 100 mg/m2 was given using seldinger technique for transcatheter bronchial arterial infusion chemotherapy on day 1. Two-thirds of the cisplatin 100 mg/m2 was infused in veins on day 2 and 3. The gemcitabine 1000 mg/m2 was infused in veins on day 8, 21 days for a period. In group B (interventional - 125I groups), the method of combination chemotherapy of gemcitabine and cisplatin was the same as in Group A. After ten days of arterial perfusion, 125I seeds were implantated, 21 days for a period. All patients received at least 2 cycles. The imaging evaluation of patients after treatment standards included complete remission (CR), partial remission (PR), stable (SD),progressive disease (PD), effective rate (CR + PR)/30 and clinical benefit rate (CR + PR + SD)/30.Non-parametric rank sum test was used to compare short-term effect of the two groups treatment of two cycles.x2 test was used to compare year survival, Kaplan-Meier method was used to calculate median survival,log-rank test method was used to difference between the groups. Results In group A, there were 17 PR,9SD and 4 PD. The overall response rate was 56. 7% (17/30) and clinical beneficial rate was 86. 7% (26/30). In Group B, there were 2 CR, 21 PR, 7 SD. The overall response rate was 76.7% (23/30) and clinical beneficial rate was 100% (30/30). There was significant difference between the two groups (P =0. 036). In group A, the 1 year survival rate was 46. 7% (14/30) and the 2 year survival rate was 36. 7%(11/30), median survival time (MST) was 10 months . In group B, the 1 year survival rate was 76. 7%(23/30) and the 2 year survival rate was 63. 3% (19/30) , median survival time (MST) was 27 months.There was a significant difference between two group in 1 year survival rate (P = 0. 017), 2 year survival rate (P = 0. 039) and median survival time (P = 0. 006). Conclusion The treatment effects of Ⅲ stage non-small cell lung cancer by gemcitabine and cisplatin combination chemotherapy with double way plus radioactive seed 125I implantation was better than gemcitabine and cisplatin combination chemotherapy with double way. Evaluation of the efficacy of bronchial arterial infusion chemotherapy for the treatment of central non-small cell lung cancer

To evaluate the long-term efficacy of bronchial arterial infusion (BAI) chemotherapy in the treatment of centeral non-small cell lung cancer. Fifty-eight patients with central non-small-cell lung cancer, who were assessed as difficult operable or non-operable by imaging examination, received BAI of cisplatin, epirubicin and mitomycin alone or in combination. It includes 51 cases of squamous cell carcinoma, 6 cases of adenocarcinoma and 1 case of adenosquamous carcinoma. The cinical stage before BAI wasIIb in 3 cases, IIIa in 26 cases and IIIb in 29 cases. Long term follow-up was conducted and the results were statistically analyzed. The total effective rate of BAI was 43.1%. The mediam survival (MS) of all 58 patients was 29.1 months. 31 patients after BAI became operable and were resected, had a median survival of 65.2 months. 27 patients after BAI were not resected and had a MS of 15.9 months. There was a significant difference between the patients who had been resected and not. The MS of IIIa stage patients was 39.0 months, and IIIb stage 20.4 months. Bronchial arterial infusion chemotherapy is a better choice with a definite efficacy for treatment of center-based NSCLC patients, estimated as difficult operable but without distant metastasis.

How to improve the postoperative 5-year survival rate for lung cancer and to give more patients a chance of surgery have become research hot spots. The aim of this research is to evaluate the clinical and pathohistological responses and effects of preoperative bronchial artery infusion (BAI) chemotherapy in patients with locally advanced (stage III) non-small cell lung cancer (NSCLC). A total of 92 patients with locally advanced NSCLC were randomly divided into two groups. BAI group received BAI chemotherapy for 2 cycles before surgical resection. Surgery group received operation only. The complete resection rate and clinical response were compared between the two groups. In the BAI group, the clinical response rate and the pathohistological response rate were 68.3% and 51.3% respectively. The complete resection rate in the BAI group was 89.7%, which was significantly higher than that in the surgery group (72.5%) (P < 0.05). The 1- and 2-year survival rate was 100.0% and 80.6% in the BAI group, and 94.1% and 60.0% in the surgery group. BAI neoadjuvant chemotherapy is safe and effective, which has a good clinical and pathohistological response. It might increase the complete resection rate of the tumor and improve the long-term survival rate of stage III NSCLC patients.
