Nurses’ attitudes, beliefs and confidence levels regarding care for those who abuse alcohol: Impact of educational intervention

Alcohol abuse is a worldwide public health concern. Nurses, representing the largest body of health care providers, are a potential resource to provide screening and brief intervention for patients with alcohol problems. This study evaluates the effect of an educational intervention on the attitudes, beliefs, and confidence levels of nurses regarding screening and brief intervention for alcohol problems. One hundred eighty-one students at Vanderbilt University School of Nursing participated in a four-hour educational intervention to train providers in brief negotiated intervention (BNI) for screening, early detection and brief treatment of alcohol problems. Participants completed questionnaires before and after this training. Analysis of the data using paired t-test and one-way analysis of variance showed statistically significant positive change in the nurses' attitudes, beliefs, and confidence levels regarding alcohol abuse and its treatment after the educational intervention. For example, the percentage of nurses who reported always having confidence in assessing patients' readiness to change their behavior increased from 8.3% to 23.5% after training. In conclusion, the BNI educational intervention can be effective in promoting positive changes among nurses in attitudes, beliefs, and confidence levels regarding alcohol abuse and its treatment.

Effectiveness of skills-based training using the Drink-less package to increase family practitioner confidence in intervening for alcohol use disorders

BackgroundMisuse of alcohol is second only to tobacco as a leading cause of preventable death in Australia. There is an opportunity in family practice to detect problems and intervene with people at risk of alcohol-related harm before complications occur. However, family practitioners (FPs) report low levels of confidence in managing patients with drinking problems. The aim of this study was to determine whether the interactive training session using the 'Drink-less' package led to improvement in FPs' self-reported level of confidence in detecting and providing interventions for risky alcohol consumption.MethodFPs in urban and rural New South Wales were invited to training sessions in their local area. An introductory overview preceded a practical skills- based session, using the Drink-less package. Participants completed before and after evaluation forms.ResultsWhile 49% (CI 43 – 55) of the attending FPs indicated at baseline that they felt confident in identifying at-risk drinkers, this proportion rose to 90% (95% CI: 87 – 93) post-session, and they also reported increases in confidence from 36% (95% CI: 31 – 41) to 90% in their ability to advise patients. Urban FPs reported lower levels of confidence than rural FPs, both pre- and post-session.ConclusionTraining sessions in the Drink-less intervention resulted in increased self-reported confidence in detection and brief intervention for alcohol problems. Further research is needed to determine the duration of this effect and its influence on practice behaviour.

Alcohol Interventions for Trauma Patients Treated in Emergency Departments and Hospitals

To determine if brief alcohol interventions in trauma centers reduce health care costs. Alcohol-use disorders are the leading cause of injury. Brief interventions in trauma patients reduce subsequent alcohol intake and injury recidivism but have not yet been widely implemented. This was a cost-benefit analysis. The study population consisted of injured patients treated in an emergency department or admitted to a hospital. The analysis was restricted to direct injury-related medical costs only so that it would be most meaningful to hospitals, insurers, and government agencies responsible for health care costs. Underlying assumptions used to arrive at future benefits, including costs, injury rates, and intervention effectiveness, were derived from published nationwide databases, epidemiologic, and clinical trial data. Model parameters were examined with 1-way sensitivity analyses, and the cost-benefit ratio was calculated. Monte Carlo analysis was used to determine the strategy-selection confidence intervals. An estimated 27% of all injured adult patients are candidates for a brief alcohol intervention. The net cost savings of the intervention was 89 US dollars per patient screened, or 330 US dollars for each patient offered an intervention. The benefit in reduced health expenditures resulted in savings of 3.81 US dollars for every 1.00 US dollar spent on screening and intervention. This finding was robust to various assumptions regarding probability of accepting an intervention, cost of screening and intervention, and risk of injury recidivism. Monte Carlo simulations found that offering a brief intervention would save health care costs in 91.5% of simulated runs. If interventions were routinely offered to eligible injured adult patients nationwide, the potential net savings could approach 1.82 billion US dollars annually. Screening and brief intervention for alcohol problems in trauma patients is cost-effective and should be routinely implemented.

Teaching brief intervention for alcohol problems to emergency practitioners: Development, implementation, and testing of a model

Study objectives: As part of a National Institute on Alcohol Abuse and Alcoholism (NIAAA)–funded project to evaluate the efficacy of a brief intervention, the brief negotiation interview (BNI) was developed for harmful and hazardous drinkers presenting to the emergency department (ED). Hazardous drinkers are defined as those drinking over the NIAAA guidelines for low-risk drinking who are not dependent. Harmful drinking was defined as an injury with blood alcohol content 0.2 g/dL or greater. We determine the feasibility and acceptability of teaching emergency practitioners to perform these interventions. Methods: All emergency medicine faculty, third- and fourth-year emergency medicine residents, and physician associates practicing in a Level I urban teaching hospital were eligible for training. Two-hour teaching sessions were conducted, including a 30-minute didactic presentation, a 10-minute role-playing demonstration, and a 50-minute skills-based workshop. All emergency practitioners were later tested using a standardized patient scenario that was audiotaped to ensure adherence to and competence with the BNI protocol. A rater identified that critical actions were completed. If emergency practitioners failed the testing station, they were given additional instruction and retested at a later time. Results: All 57 emergency practitioners were trained during 7 teaching sessions. One attending physician left the institution before testing. Fifty-one (91%) of 56 passed the first test, including 19 faculty, 28 emergency medicine residents, and 9 physician associates. A total of 5 (9%) emergency practitioners needed remediation: 1 faculty, 1 emergency medicine resident, and 3 physician associates. The 1 faculty and 1 emergency medicine resident left the institution before remediation. The remaining 3 physician associates successfully completed the test. Once tested, the emergency practitioners were responsible for providing BNIs for 250 harmful and hazardous drinkers identified on their ED shifts as part of the NIAAA project. These interventions were audiotaped and rated for adherence to protocol. Conclusion: We were able to develop and successfully teach a targeted intervention for harmful and hazardous drinkers to practicing emergency practitioners. They were successful in performing the intervention in a real-world ED setting.

Screening and Brief Intervention for Alcohol Problems Among College Students Treated in a University Hospital Emergency Department

The authors evaluated a protocol to screen and provide brief interventions for alcohol problems to college students treated at a university hospital emergency department (ED). Of 2,372 drinkers they approached, 87% gave informed consent. Of those, 54% screened positive for alcohol problems (Alcohol Use Disorders Identification Test score < 6). One half to two thirds of the students who screened positive drank 2 to 3 times a week, drank 7 or more drinks per typical drinking day, or had experienced alcohol dependence symptoms within the past year. Ninety-six percent of screen-positive students accepted counseling during their ED visit. Three quarters of those questioned at 3-month follow-up reported that counseling had been helpful and that they had decreased their alcohol consumption. The prevalence of alcohol problems, high rates of informed consent and acceptance of counseling, and improved outcomes suggest that the ED is an appropriate venue for engaging students at high risk for alcohol problems.
