A Descriptive Study To Assess The Knowledge Of Undergraduate Students Regarding The Needle Stick Injury And Its Prevention In Smt. Lakshmi Bai Batra College Of Nursing, New Delhi

Background: A needle stick injury (NSI) is an accidental skin-penetrating stab wound from a hollow-bore needle containing another person’s blood or body fluid. Healthcare workers are at an occupational risk of exposure to blood-borne pathogens following needle stick injuries and sharp injuries. Objective: To assess the knowledge of undergraduate students regarding needle stick injuries and its prevention in Smt. Lakshmi Bai Batra College of Nursing. Method and material: The research was conducted from December 22nd, 2022 on 62 students of Smt. Lakshmi Batra College of Nursing, Delhi. A purposive sampling technique was used to select the sample and data was collected through questionnaire for accessing the knowledge regarding needle stick injuries and its prevention. Result: 62 students of Smt. Lakshmi Bai Batra College of Nursing were taken as sample using purposive sampling technique and the finding of the study indicates that knowledge regarding needle stick injury is average among 61 students and poor among 1 student and good among 0 student. Conclusion: The study indicates that poor to average knowledge regarding needle stick injury & its prevention among undergraduate students of Smt. Lakshmi Bai Batra College of Nursing

Inhibition of Mac-1 allows human macrophages to migrate against the direction of shear flow on ICAM-1.

All immune cells must transit from the blood to distal sites such as the lymph nodes, bone marrow, or sites of infection. Blood borne monocytes traffic to the site of inflammation by adhering to the endothelial surface and migrating along endothelial intracellular adhesion molecule 1 (ICAM-1) by their ligand's macrophage 1 antigen (Mac-1) and lymphocyte functional antigen 1 (LFA-1) to transmigrate through the endothelium. Poor patient prognoses in chronic inflammation and tumors have been attributed to the hyper recruitment of certain types of macrophages. Therefore, targeting the binding of ICAM-1 to its respective ligands provides a novel approach to targeting the recruitment of macrophages. To that end, we determined whether the loss of Mac-1 expression could induce this upstream migration behavior by using blocking antibodies against Mac-1 to examine the effects of hydrodynamic flow on the migration of the human macrophage cell line U-937 on ICAM-1 surfaces. Blocking Mac-1 on U-937 cells led to upstream migration against the direction of shear flow on ICAM-1 surfaces. In sum, the ability of macrophages to migrate upstream when Mac-1 is blocked represents a new avenue to precisely control the differentiation, migration, and trafficking of macrophages.

Assessing the Impact of Educational Strategies on Reducing Needle Stick Injuries for Nurses in Jordanian Hospitals

Background: Needle stick injuries (NSIs) are frequent occupational health hazards among nurses with several consequences including blood-borne infections. Literature indicates inadequate knowledge among nurses as an important associated risk factor. Notwithstanding, little attention has been given to the intervention programs to reduce the occurrences of NSIs in Jordan. The main objective of this research was to implement and assessing the impact of educational modules and strategies to minimize NSIs for nurses in Jordanian hospitals. Methods: a randomized control trial design with four arms including three intervention groups and one control group was applied. A total of 400 nurses were selected based on stratified random sampling from the four randomly sampled private hospitals. The educational intervention was then provided through three different strategies (Social Media (SM), Audio-Visual (AV), and combined method). Data were collected in three phases, at baseline, after three months, and after six months of the intervention. Results: There were statistically significant differences in the number of NSIs between the control and combined strategy groups (P= 0.002). After 6 months, significant differences were found between control and SM groups (P=0.032), control and AV groups (P= 0.007), and control and combined groups (P<0.001). The leading risk factors of NSIs included fatigue (P<0.001), lack of assistance (P= 0.001), emotional distress (P= .021), being rushed (P= .002), and Lack of skills (P= .001). The hierarchical regression for the prediction of changes in NSIs occurrence produced a model with four predictors after three months (P< .001), and six predictors after six months (P< .001). Conclusion: The educational intervention significantly decreased the occurrences of NSIs. Hospital administrators must consider significant risk factors for NSIs.

Comparison of lecture-based learning with presentation-assimilation-discussion method in occupational bloodborne exposure education of nursing students, a randomised trial

BackgroundOccupational Bloodborne Exposures (OBEs) are incidents where healthcare workers come into contact with blood or other potentially infectious materials, leading to risks of transmitting bloodborne pathogens. Nursing students, often in direct contact with patients, face heightened risks due to their duties.MethodsFirst, we conducted a cross-sectional survey using a OBEs questionnaire to explore the knowledge, attitudes, practices, and needs regarding OBEs among nursing students. Subsequently, we used a randomized controlled trial (RCT) to compare the impact of the Presentation-Assimilation-Discussion (PAD) method with the traditional lecture-based learning (LBL) method on OBEs education for nursing students. Pre-test, post-test, and retention test were used to observe the teaching effectiveness, and the students’ feedback on the teaching method was also observed.ResultsIn the cross-sectional survey, we found that nursing students lacked sufficient knowledge and management skills regarding OBEs but recognized the importance of standard precautions and expressed a desire for systematic OBEs training during their education and internships. In the RCT, the total, theoretical, and practical scores of the PAD and LBL groups were comparable in the pre-test (56.70 ± 3.47 vs. 56.40 ± 3.95, 33.09 ± 3.39 vs. 33.33 ± 2.44, 23.61 ± 4.66 vs. 23.07 ± 4.84, p > 0.05). After training, the PAD model demonstrated an advantage over the LBL model in immediate total (84.25 ± 4.06 vs. 78.95 ± 4.23, p < 0.001), theoretical (54.32 ± 2.43 vs. 51.44 ± 2.58, p < 0.001), and practical scores (29.93 ± 3.90 vs. 27.51 ± 4.33, p < 0.01). It also showed superior retention of total (69.05 ± 3.87 vs. 65.77 ± 2.94, p < 0.001) and theoretical scores (39.05 ± 3.05 vs. 36.23 ± 3.18, p < 0.001). However, there was no significant difference in the retention of practical scores between the two groups (30.00 ± 4.76 vs. 29.53 ± 3.73, p > 0.05). The PAD group benefited more across various learning dimensions but reported a higher study load.ConclusionsOur study reveals that the PAD model could be a valuable approach for teaching OBEs to nursing students.

Identifying the hidden parasitic intruders: Molecular insights into the health of common Myna

Birds are known to act as the parasite reservoir and can transmit them to other organisms through food chains. This study aims to report the molecular prevalence and phylogenetic evaluation of various blood borne pathogens (Toxoplasma gondii, Isospora spp., Plasmodium spp., Haemoproteus spp., Leucocytozoan spp. and Neospora caninum) in blood samples of common Myna (Acridotheres tristis: N = 80) collected from four region (Jhang, Khanewal, Multan and Muzaffargarh) in Punjab Pakistan. Effect of pathogens on the complete blood count of the host was also determined. Results revealed by 2/80 Myna (2.5 %) amplified ITS-1 gene of Toxoplasma (T.) gondii (confirmed by DNA sequencing) while 2/80 (2.5 %) birds amplified 18S rDNA gene and Isospora spp. Phylogenetic analysis of both pathogens showed that Pakistani isolates were clustered together and were closely related to isolates that were reported from worldwide countries.Risk factor analysis revealed that prevalence of both pathogens was not restricted to a particular sampling site or a particular bird sex (P > 0.05). T. gondii infected birds had elevated red cell distribution width while Isospora sp. infected birds had elevated % monocytes and platelet distribution width while decreased mean cell hemoglobin, mean corpuscular hemoglobin concentration and platelets hematocrit than their respective uninfected birds. In conclusion, we are reporting the presence of T. gondii and Isospora sp. among Pakistani common Myna that had disturbed the complete blood count parameters that may have affected their normal physiology.

Experience with HCV Detection and Molecular Genetic Characterization among Otherwise Healthy Pregnant Women and Their Partners in the Republic of Guinea.

According to recent data, there are currently 170 to 200 million people infected with HCV worldwide, and the number of new cases annually is approximately 40,000. Thus, the overall prevalence of the pathogen in the world is about 1.8-3%. The dynamic monitoring of circulating viral variants in specific groups that reflect the situation in the wider population, including potential pathogen spread, is of high importance for predicting the epidemiologic situation. Pregnant women are such a group. The Republic of Guinea is one of the poorest countries in the world, in which medicine receives little finance from the state. Among other conditions, HCV infection is not monitored in the country. This work used blood plasma from pregnant women living in the Republic of Guinea and their partners (1810 and 481). ELISA diagnostic kits were used to detect serologic markers, and PCR diagnostic kits were used to detect molecular biologic markers. Sanger sequencing, followed by phylogenetic analysis, was used for genotyping. The present study shows that HCV antibodies were detected in 3.2% of the pregnant women examined and in 3.33% of their male partners. HCV RNA was detected in 0.5% of cases in women and in all anti-HCV-positive male partners (3.33%). HCV RNA was more common in the men than in the pregnant women (χ2 = 25.6, df 1, p < 0.0001, RR = 6.69 with 95% CI: 2.97-15.04). The HCV viral load was determined for all the RNA-HCV-positive samples. The HCV viral load exceeded 1000 IU/mL in all nine women and only in two cases in men. The HCV genes NS5A and NS5B and the NS3 gene fragment were sequenced for 11 samples. Subtype 2q was determined for three isolates and 2j for another three isolates. Another five isolates could not be confidently assigned a subtype because different results were obtained with different methods of analyzing the three viral regions. It can be assumed that these isolates belong to new viral subtypes or to recombinant forms between genotype 2 subtypes. No drug resistance mutations were identified, but a large number of natural polymorphisms in the analyzed genomic regions of the HCV isolates were shown. These results may serve as baseline data for the future planning of a nationwide estimate of the prevalence of bloodborne infections among pregnant women.

Occupational biological risk in healthcare workers

Biohazardous occupational accidents represent a significant challenge in the healthcare field due to the constant exposure of healthcare workers to pathogens. These accidents, which include needle sticks, cuts, splashes of body fluids, and contact with contaminated materials, can lead to serious infections, such as hepatitis B (HBV), hepatitis C (HCV), and HIV. World Health Organization (WHO) estimates that approximately two million healthcare workers experience sharps injuries annually, resulting in a significant risk of exposure to blood-borne pathogens [1]. A study conducted in hospitals in Europe revealed that the prevalence rate of needlestick injuries ranges from 10% to 30% per year, with the highest rates in areas such as surgery and intensive care [2]. The high frequency of these incidents underscores the importance of implementing effective preventive strategies to protect these workers.

Congenital transmission of Chagas disease: The role of newborn therapy on the disease’s dynamics

Chagas disease, also known as American trypanosomiasis, is caused by a protozoan blood-borne pathogen called Trypanosoma cruzi. The World Health Organization (WHO) has classified Chagas as one of 21 neglected tropical diseases present in the world and estimates that 6-7 million people are currently infected with Chagas. Congenital transmission of Chagas disease contributes to a significant amount of new infections, especially in endemic areas where 22.5% of new infections are due to congenital transmission. In this paper, we investigate congenital transmission’s impact on Chagas disease dynamics through a mathematical model. Specifically, we examine how treating a proportion of infants born to infected individuals impacts the progression and spread of Chagas disease. The influence of newborn therapy on the dynamics of the model is thoroughly investigated, both theoretically and numerically. The results illustrate the importance of treating a high proportion of newborns to reduce the number of infected cases of the disease. The findings show that the therapy given to newborns is necessary but not sufficient to curb the transmission of Chagas disease, and a comprehensive approach that includes vector and vertical transmission control strategies is essential for eradicating Chagas disease. We also observed that if vector transmission can be controlled, then at least 55% of the newborns need to be treated to eliminate the disease.

Frequency of exposure incidents in hospital workers before and during the COVID-19 pandemic based on the hospital status and the use of personal protective equipment: a descriptive study with a historical comparison group

BackgroundAn occupational exposure, i.e. exposure incident (EI), is contact with potentially contaminated material that may contain bloodborne pathogens and that occurs during occupational activities inside or outside a health care facility, either during direct work with a patient or during contact with a patient’s body fluids and tissues. This study aimed to compare the frequency of EIs in a university hospital before and during the Coronavirus Disease 2019 (COVID-19) pandemic.MethodsThis was a descriptive study with a historical comparison group conducted at the Dubrava University Hospital (DUH) in Zagreb, Croatia. We compared the frequency of EIs among healthcare and non-healthcare workers before (from March 11, 2018, to March 10, 2020) and during (from March 11, 2020, to March 11, 2022) the COVID-19 pandemic, expressed as the number of EIs per number of hospitalized patients and the total number of hospital activities. We analyzed data based on the status of the hospital (a COVID-19 hospital or not) and the use of personal protective equipment (PPE) as recommended by the World Health Organization.ResultsDuring the total analyzed period, 241 EIs were reported in DUH. Before the pandemic, 128 EIs were reported, compared to 113 during the pandemic. Before the pandemic, 91% of EIs were recorded in healthcare workers, while during the pandemic, 96% of EIs were recorded in healthcare workers. Slightly more EIs were recorded during the period of mixed work form and de-escalation of PPE. The rate of EIs relative to the total number of hospital patients was significantly higher during the pandemic (3.9/1000) than in the pre-pandemic period (2.5/1000). The rate of EIs relative to the total number of hospital activities was significantly higher during the pandemic (0.4/1000) than in the pre-pandemic period (0.2/1000).ConclusionThe rate of EIs relative to the total number of hospitalized patients and the total number of hospital activities in DUH was significantly higher during the pandemic, and the rate of total EIs increased among healthcare workers during the COVID-19 pandemic. The results of this study show that it is necessary to constantly and effectively work on the prevention of EI.

Seroprevalence of Hepatic Viruses and Associated Risk Factors Among Rehabilitated Patients During the Post-Addictive Phase

Background: Drug addiction poses a major public health risk, particularly in terms of the transmission of blood-borne infections like Hepatitis B (HBV), Hepatitis C (HCV), and Human Immunodeficiency Virus (HIV) among users.Objective: To assess the seroprevalence of HBV, HCV, and HIV and identify associated risk factors among drug users in rehabilitation centers in Islamabad, Pakistan.Methods: A cross-sectional study was conducted on 203 drug users from July to October 2022. Participants were screened for HBV, HCV, and HIV using immunochromatographic tests (ICT) and confirmed by Enzyme-Linked Immunosorbent Assay (ELISA). Demographic and risk factor data were collected through structured forms. Statistical analysis was performed using SPSS version 20.0, including descriptive statistics and chi-square tests for associations.Results: The prevalence rates for HCV, HBV, and HIV were 15.8%, 9.3%, and 3.4%, respectively. Intravenous drug users showed a higher prevalence of HCV (8.4%) compared to non-intravenous users (7.4%). Needle sharing (55.7%) and low socio-economic status (83.2%) were significant risk factors.Conclusion: HCV is highly prevalent among drug users, especially intravenous users, necessitating targeted harm reduction strategies and enhanced rehabilitation facilities.

Deceased Organ Donor HTLV Screening Practices Postelimination of Universal Screening in the United States.

In the United States, universal screening for human T-lymphotropic virus (HTLV) in deceased organ donors was discontinued in 2009. Since then, the transplant guideline suggests considering targeted screening. However, the outcomes of this change in HTLV screening have not been evaluated. Using the Organ Procurement and Transplantation Network database between 2010 and 2022, we analyzed the HTLV antibody screening frequency and seroprevalence in potential deceased organ donors and their correlations with HTLV infection risks, including race and high-risk behaviors for blood-borne pathogen infection. Although targeted screening has not been established for HTLV, we hypothesized that screening rates should correlate with the proportions of donors with infection risk if screening is targeted. We also evaluated the organ utilization of HTLV-seropositive donors. Of 130 284 potential organ donors, 22 032 (16.9%) were tested for HTLV antibody. The proportion of donors tested for HTLV varied between Organ Procurement Organizations (median [interquartile range], 3.8% [1.0%-23.2%]; range, 0.2%-99.4%) and was not correlated to HTLV infection risks. There were 48 seropositive donors (0.22%), and at least 1 organ from 42 of these donors (87.5%) was transplanted. The number of organs recovered and transplanted per donor was significantly lower in HTLV-seropositive than in HTLV-negative donors (recovered, 2 [2-3] versus 3 [3-5], P < 0.001; transplanted, 2 [1-3] versus 3 [2-4], P < 0.001). However, HTLV-1 infection was not attributed as the cause of nonrecovery except for only 1 HTLV-seropositive donor. HTLV screening practices varied across the United States. Our findings suggest that targeted screening was not performed after the elimination of universal screening.

Recent Advancement in Drug Development for Treating Malaria using Herbal Medicine and Nanotechnological Approach.

More than two hundred million people around the world are infected with malaria, a blood-borne disease that poses a significant risk to human life. Single medications, such as lumefantrine, primaquine, and chloroquine, as well as combinations of these medications with artemisinin or its derivatives, are currently being used as therapies. In addition, due to rising antimalarial drug resistance, other therapeutic options are needed immediately. Furthermore, due to anti-malarial medication failures, a new drug is required. Medication discovery and development are costly and time-consuming. Many malaria treatments have been developed however, most treatments have low water solubility and bioavailability. They may also cause drug-resistant parasites, which would increase malaria cases and fatalities. Nanotechnology may offer a safer, more effective malaria therapy and control option. Nanoparticles' high loading capacity, concentrated drug delivery, biocompatibility, and low toxicity make them an attractive alternative to traditional therapy. Nanotechnology-based anti-malarial chemotherapeutic medications outperform conventional therapies in therapeutic benefits, safety, and cost. This improves patient treatment compliance. The limitations of malaria treatments and the importance of nanotechnological approaches to the treatment of malaria were also topics that were covered in this review. The most recent advancements in nanomaterials and the advantages they offer in terms of medication delivery are discussed in this article. The prospective therapy for malaria is also discussed. Additionally, the limitations of malaria therapies and the importance of nanotechnology-based approaches to the treatment of malaria were explored.

Self-reported exposure to blood and body fluids and serological evidence of lifetime exposure to hepatitis B virus among health care workers in Ghana: a cross-sectional study

IntroductionIn Sub-Saharan Africa alone, about 40–65% of Hepatitis B Virus infections among HCWs were a result of percutaneous occupational exposures to contaminated blood and body fluids of patients. Occupational exposure to blood and body fluids among healthcare workers is on the rise in Ghana. However, the relationship between self-reported exposures to blood and body fluids suspected to be contaminated with the hepatitis B virus and actual serological evidence of exposure remains unknown. The aim of the study however was to assess the self-reported exposure to HBV as against the serological evidence of lifetime exposure to HBV and associated factors among Ghanaian HCWs.MethodsThe study was a cross-sectional analytical survey that involved 340 HCWs who were recruited using a simple random sampling procedure from six cadres of staff from five districts in Greater Accra. The participants were surveyed using a validated instrument and 5mls of venous blood was aseptically withdrawn for qualitative detection of Anti-HBc. SPSS version 23.0 was used to analyze the data to obtain proportions, odds ratios and their corresponding confidence intervals with the level of significance set at 0.05.ResultsThe response rate was 94% with Nurses and Doctors in the majority with a mean age of 35.6 ± 7.2. Self-reported exposure to HBV was 63% whereas lifetime exposure to HBV (Anti-HBc) prevalence was 8.2% (95% CI = 5.0-11.0%). Females were 60% less likely to be exposed to HBV (aOR = 0.4; 95% CI = 0.1–0.9) than their male counterparts. HCWs without training in the prevention of blood-borne infections had almost three times higher odds of being exposed to HBV in their lifetime (aOR = 2.6; 95% CI = 1.0-6.4).ConclusionsThe findings of this study suggest that self-reported exposure to HBV-contaminated biological materials was high with a corresponding high lifetime exposure to HBV. The female gender was protective of anti-HBc acquisition. Apart from direct interventions for preventing occupational exposures to HBV in the healthcare setting, periodic training of all categories of healthcare workers in infection prevention techniques could significantly reduce exposure to the Hepatitis B virus.

Sexually transmitted and blood-borne infections by sex, methamphetamine use, and houselessness before, at, and after HIV diagnosis in Manitoba, Canada

ObjectivesDescribe the proportion of people newly living with HIV with sexually transmitted and blood-borne infections (STBBIs) before, at, and after HIV diagnosis in Manitoba, Canada. MethodsA retrospective cohort study reviewed clinical charts of all 404 people ≥18 years old newly diagnosed with HIV in Manitoba, Canada between 2018 and 2021. Syphilis, hepatitis C and B, gonorrhea, and chlamydia infections before, at, and after HIV diagnosis were recorded and analyzed by sex at birth, injection drug use status, use of methamphetamines, and housing status. ResultsA total of 53% of people were diagnosed with syphilis, 44.1% with gonorrhea, 42.8% with chlamydia, and 40.6% with hepatitis C at least once. Among females, 64.1% had at least one or more STBBIs diagnoses before HIV diagnosis compared with 44.8% of males. Over 70% of people experiencing houselessness had at least one STBBI diagnosis before their HIV diagnosis compared with 43.9% of people not houseless. Among people who used methamphetamines, 68.3% had one or more STBBIs before HIV diagnosis compared with 28.9% of people who do not use methamphetamines. In a multivariable analysis houselessness, methamphetamine use, and younger age were associated with increased risk of any STBBIs. ConclusionsIn our Manitoba cohort of people living with HIV, disproportionately more females, people experiencing houselessness, and those who use methamphetamine were diagnosed with STBBIs. The proportion of new infections before HIV diagnoses highlights a missed opportunity to provide prevention modalities, including pre-exposure prophylaxis, and the proportion after HIV diagnosis emphasizes the importance of enhancing engagement, repeated testing, and educational strategies to ameliorate ongoing exposures.

A Survey of Nursing Students at a selected Saudi University to evaluate their familiarity with Blood-Borne infection Prevention

Transmission of infectious diseases via blood is known as blood borne pathogens. An infectious illness that may transmit from person to person by contact with contaminated blood or other bodily fluids. When healthcare personnel come into contact with infected patients, they are at high risk of contracting blood-borne pathogens via percutaneous exposure. The researchers in this study set out to determine how well nursing students at a few different schools in Riyadh, Saudi Arabia, understood how to avoid contracting blood-borne infections.

Exploring the Perspectives of Unhoused Adults and Providers Across the HCV Care Continuum.

Hepatitis C virus (HCV), the most common blood-borne infection, disproportionately affects people experiencing homelessness (PEH); however, HCV interventions tailored for PEH are scarce. This study utilized a community-based participatory approach to assess perceptions of HCV treatment experiences among HCV-positive PEH, and homeless service providers (HSP) to develop and tailor the "I am HCV Free" intervention which integrates primary, secondary, and tertiary care to attain and maintain HCV cure. Four focus groups were conducted with PEH (N = 30, Mage = 51.76, standard deviation 11.49, range 22-69) and HSPs (n = 10) in Central City East (Skid Row) in Los Angeles, California. An iterative, thematic approach was used to ensure the trustworthiness of the data. Barriers and facilitators emerged from the data which have the potential to impact initiating HCV treatment and completion across the HCV care continuum. Understanding and addressing barriers and strengthening facilitators to HCV treatment will aid in HCV treatment completion and cure for PEH.

EVALUATION OF NEEDLE STICK INJURY AND ASSOCIATED FACTORS AMONG HEALTHCARE WORKERS IN AL NAJAF CITY/ IRAQ

Introduction:In healthcare-related situations, needle sticks represent a substantial work-related health risk. Healthcare professionals face the danger of contracting blood-borne infections as well as the psychological impact of these wounds. The study aimedtoevaluate the prevalence of needle stick injuries and the associated socio-demographic determinants among healthcare workers.Methods: The research employed a cross-sectional design, with data collection occurring from February 1st, 2023 to February 1st, 2024. 380participants were conveniently selected from different Al-Najaf City/Iraq healthcare institutions. The study sample included a variety of specialists, including anesthetics, surgeons, dentists, surgeon assistants, anesthetics assistants, nurses, physicians, and laboratory workers. A self-administered questionnaire from previously published articles was used for data collection. It covers socio-demographic information, a history of previous exposure to, and the reasons for needle stick injuries.Quantitative variables were summarized with means and standard deviations; descriptive statistics for categorical variables included frequencies and percentages. P ≤ 0.05 indicated statistical significance.Results: Most healthcare workers (85%) were previously exposed to needle sticks. Job category was significantly associated with a history of needle stick exposure (p &lt; 0.001). The anesthetics represent the highest job class exposed to needle sticks (100%). More than one-fifth of participants (21.85%) reported recapping needles followed by bending needles in 16.84% of them as a cause of needle stick injuries. Conclusion: Most healthcare workers are exposed to needle stick injuries; job category is an important risk factor

Structural Barriers to Primary Care Among Sex Workers: Findings from a Community- Based Cohort in Vancouver, Canada (2014-2021).

Due to structural marginalization, sex workers experience health inequities including a high prevalence of sexually transmitted and blood-borne infections, mental health disorders, trauma, and substance use, alongside a multitude of barriers to HIV and substance use services. Given limited evidence on sex workers' broader primary healthcare access, we aimed to examine structural factors associated with primary care access among sex workers over a 7-year period. Data were derived from An Evaluation of Sex Workers Health Access (AESHA), a community-based open prospective cohort of women (cis and trans) sex workers in Metro Vancouver, from 2014 to 2021. Descriptive statistics were used to summarize the proportion of primary care use in the past six months and to assess primary care trends over time from 2014-2021. We used multivariate logistic regression with generalized estimating equations (GEE) to identify structural factors associated with primary care access (seeing a family doctor in the last six months), after adjusting for confounders. Amongst 646 participants, most (87.4%) accessed primary care at some point during the study period, and primary care use in the in the last 6 months was relatively stable (ranging from 60-78%) across each follow-up period. At first available observation, participants faced a high burden of sexually transmitted and blood-borne infections (STBBIs) (48.0%, 11.5%, and 10.4% were HCV, HIV, or STI seropositive, respectively), 56.8% were diagnosed with a mental health disorder, 8.1% had recently overdosed, and 14.7% were recently hospitalized. In multivariable GEE analysis, exposure to intimate partner violence was associated with reduced access to primary care (Adjusted odds ratios (AOR) 0.63, 95% Confidence interval (CI): 0.49 - 0.82), and limited English fluency was marginally associated (AOR 0.76 CI: 0.51 - 1.14). This study characterized primary care access and its structural determinants among sex workers over 7-years. Participants faced a high burden of STBBIs and other health disparities, and a proportion faced gaps in primary care access. Scale-up of trauma-informed, culturally and linguistically tailored, sex worker-friendly primary care models are needed, alongside structural interventions to decriminalize and destigmatize sex work and substance use.

Situation and associated factors of needle stick and sharps injuries among health-care workers in a tertiary hospital: a cross-sectional survey

PurposeThis study aimed to assess the prevalence of and factors associated with needle stick and sharps injuries (NSSIs) among health-care workers (HCWs) in a tertiary hospital in China.Materials and methodsThis retrospective survey was conducted with 562 HCWs at a tertiary hospital in China in July 2023. Information was collected using a self-designed questionnaire, and all enrolled members were required to fill in the demographic characteristics, occurrence of NSSIs and other associated factors in the past year. Logistic analysis was used to identify variables associated with NSSIs.ResultsThe proportion of participants with at least one injury within the year preceding the investigation was 21.2%. Male (AOR = 2.116 [1.265, 3.538]), working hours per week > 40 (AOR = 1.718 [1.056,2.796]), rarely checking blood-borne infections before invasive operations (AOR = 2.219 [1.303,3.782]) were significantly associated with NSSIs.ConclusionThe prevalence of NSSIs was not low in the survey area, especially in male, individuals with longer working hours, and rarely checking blood-borne infections before invasive operations. Therefore, it is necessary to promote educational programs to enhance awareness of standard prevention measures, especially for key populations, and reduce heavy workloads to decrease the occurrence of such injuries.

Intrafamilial Occurrence of Hepatitis B Virus Infection in Family Contacts of HBsAg Positive Subject

Background: Hepatitis B virus (HBV) infection is the global public health challenge. Family members of the HBV infected patients have a high risk of exposure to many blood-borne diseases including Hepatitis B and Hepatitis C viral infections. Of these Hepatitis B is not only the most transmissible infection, but also the only one (HBV) that is preventable by vaccination. Objectives: To assess Occurrence of Hepatitis B Virus Infection in family contacts of HBsAg positive subject. Methods: A six-month cross sectional study was carried out at the department of Medicine, Comilla Medical College. Ethical approval from the institutional review board was obtained to ensure patient privacy and confidentiality. Results: Among 120 selected patients analysis of relatives with the infected patients revealed that most were son and daughters. In the study, out of 379 family members 316(83%) attended the study and 63 (17%) family members were unattended. Among attended family members 37 (11.7%) were known to be positive for HBV, 279 (88.3%) were found negative. Among the study subjects 68 (56.3%) were female and rest 52 (43.8%) were male. Female to male ratio was 1.28:1. Occupation of the study subjects where most were housewives (22.2%) and students (55%). Total 120 index cases, 15 (12.5%) cases were found positive family history for HBV, 90 (75%) were found negative and 15 (12.5%) were unaware of their all family members HBsAg status. Conclusion: Family members staying same house of the HBV infected patients are at highest risk. Total 15(12.5%) index cases were found positive family for HBV infection.
