A thirty-nine-year survival with the Starr-Edwards mitral valve prosthesis

We report a case of a 57 year-old woman with Starr-Edwards model 6120 mitral valve replacement and Kay-Shiley bioprosthetic tricuspid valve replacement in 1968 at Niguarda Hospital in Milan. The mitral caged-ball has proved its excellent durability and its good hemodynamic performance in many patients, even if subject to high tendency to thrombosis. In literature there is no evidence of durability of this prosthesis longer than 35 years. Our patient after 39 years from mitral valve replacement lives a happy and fulfilling life (NYHA II), with no evidence of hemolysis, ball variance, symptomatic embolization or major bleeding.

Fluctuations in warfarin dose response after heart valve surgery: implications for cardiac rehabilitation

In patients undergoing heart valve surgery (HVS) who require warfarin therapy, the maintenance of low variability in the level of anticoagulation early after operation is generally difficult. Aim of this study was to evaluate the time in therapeutic range (TTR) in HVS patients receiving oral anticoagulation therapy (OAT) during phase I-II of cardiac rehabilitation (CR), and, secondly, to identify clinical variables associated with inadequate anticoagulation. Observational study of consecutive in-hospital patients directly tracked from a cardiac surgery unit to a CR facility. OAT was monitored both in terms of administered warfarin doses and resulted INR values, from day 1 to day 15 after operation. Clinical variables were tested in a logistic regression model for the prediction of inadequate anticoagulation, defined as the presence of nontherapeutic INRs for > or = 5 days between day 8 and 15. Eighty-one patients (males 56%, age 62 +/- 19 yrs.), following valvuloplasty (37%), mechanical (17%), and bioprosthetic (45%) valve replacement were considered. The prescribed warfarin dosages were significantly higher from day 1 to day 7 than from day 8 to day 15 (4.6 +/- 3.6 and 3.0 +/- 1.1 mg respectively, p< 0.001). Overall, TTR was 6 +/- 3 days, while time with elevated and lower INRs accounted for 1.3 +/- 1.6 and 8.0 +/- 3.5 days respectively. At day 7, only 25% of patients (n= 20) showed a therapeutic INR value. Inadequate anticoagulation between postoperative day 8 and 15 was displayed in 41 (51%) patients, with hypertension as the only independent predictor (p< 0.001) at multivariate analysis. Despite intensive monitoring, half of patients have nontherapeutic INR values (mainly subtherapeutic) in the first two weeks after HVS while on warfarin. Giving the high risk of completing the hospitalization phase without a stable OAT in many patients, both cardiac surgeons and cardiologists should not miss the opportunity to improve patients education, and consider a direct track to anticoagulation management services after discharge.

Surgical Outcome in Hemodialysis Patients with Active-Phase Infective Endocarditis.

The aim of this study was to elucidate the characteristics of chronic hemodialysis (HD) patients requiring surgery during the active phase of infective endocarditis (IE). From December 2004 to July 2015, 58 patients underwent surgery in our institute for active IE. Seven patients had been on HD for 1-15 years. Their preoperative profiles and surgical outcomes were compared to those of the other 51 patients (non-HD group). The predominant causative microorganisms in the HD group were Staphylococcus spp, particularly methicillin-resistant Staphylococcus aureus (MRSA), whereas Streptococcus spp were predominant in the non-HD group. Prosthetic dysfunction (stuck valve after mechanical and structural valve dysfunction following bioprosthetic valve replacement), complete atrioventricular (AV) block, and annular abscess formation were more frequent in the HD group. In-hospital mortality was higher in the HD group (29% vs. 6%, p = 0.044). Actuarial survival in the HD and non-HD groups was 43% vs. 87% at 5 years and 43% vs. 76% at 10 years (p = 0.007). Early and long term outcomes in patients with chronic HD were poor. Compared to other patients, chronic HD patients undergoing valve surgery during active IE had higher incidences of MRSA infection, annular abscess formation, postoperative valve dysfunction, and postoperative complete AV block.

Severe Bioprosthetic Mitral Valve Stenosis and Heart Failure in a Young Woman with Systemic Lupus Erythematosus.

A 23-year-old African American woman with a past medical history of systemic lupus erythematous (SLE), secondary hypertension, and end stage renal disease (ESRD) on hemodialysis for eight years was stable until she developed symptomatic severe mitral regurgitation with preserved ejection fraction. She underwent a bioprosthetic mitral valve replacement (MVR) at outside hospital. However, within a year of her surgery, she presented to our hospital with NYHA class IV symptoms. She was treated for heart failure but in view of her persistent symptoms and low EF was considered for heart and kidney transplant. This was a challenge in view of her history of lupus. We presumed that her stenosis of bioprosthetic valve was secondary to lupus and renal disease. We hypothesized that her low ejection fraction was secondary to mitral stenosis and potentially reversible. We performed a dobutamine stress echocardiogram, which revealed an improved ejection fraction to more than 50% and confirmed preserved inotropic contractile reserve of her myocardium. Based on this finding, she underwent a metallic mitral valve and tricuspid valve replacement. Following surgery, her symptoms completely resolved. This case highlights the pathophysiology of lupus causing stenosis of prosthetic valves and low ejection cardiomyopathy.

Infected Thrombus of the Bioprosthetic Aortic Valve with Distal Arterial Embolization after 3 Years of Valve Replacement Surgery

Current guidelines recommend the use of warfarin in the first 3 months after bioprosthetic valve replacement and suggest longterm anticoagulation only in patients with obvious risk factors for thrombosis. Despite at low risk of thromboembolism, we present a case of a 72-year-old male patient with infected thrombus at bioprosthetic aortic valve with distal arterial embolization after 3 years of replacement surgery. The incidence of asymptomatic bioprosthetic valve thrombosis (BPVT) may be higher than anticipated and active surveillance echocardiography is needed to early detect BPVT before leading to fatal complication.

Dysfunction of mechanical heart valve prosthesis: experience with surgical management in 48 patients.

Dysfunction of mechanical heart valve prostheses is an unusual but potentially lethal complication after mechanical prosthetic valve replacement. We seek to report our experience with mechanical valve dysfunction regarding etiology, surgical techniques and early outcomes. Clinical data of 48 patients with mechanical valve dysfunction surgically treated between October 1996 and June 2011 were analyzed. Mean age was 43.7±10.9 years and 34 were female (70.8%). The median interval from primary valve implantation to dysfunction was 44.5 months (range, 1 hour to 20 years). There were 21 emergent and 27 elective reoperations. The etiology was thrombosis in 19 cases (39.6%), pannus in 12 (25%), thrombosis and pannus in 11 (22.9%), improper disc orientation in 2 (4.1%), missing leaflet in 1 (2.1%), excessively long knot end in 1 (2.1%), endogenous factor in 1 (2.1%) and unidentified in 1 (2.1%). Surgical procedure was mechanical valve replacement in 37 cases (77.1%), bioprosthetic valve replacement in 7 (14.9%), disc rotation in 2 (4.2%) and excision of excessive knot end in 1 (2.1%). Early deaths occurred in 7 patients (14.6%), due to low cardiac output in 3 (6.3%), multi-organ failure in 2 (4.2%) and refractory ventricular fibrillation in 2 (4.2%). Complications occurred in 10 patients (20.8%). Surgical management of mechanical valve dysfunction is associated with significant mortality and morbidity. Earlier identification and prompt reoperation are vital to achieving better clinical outcomes. The high incidence of thrombosis in this series highlights the need for adequate anticoagulation and regular follow-up after mechanical valve replacement.

Mid- to long-term outcome comparison of the Medtronic Hancock II and bi-leaflet mechanical aortic valve replacement in patients younger than 60 years of age: a propensity-matched analysis.

This study aims to compare mid-long-term clinical outcomes between patients younger than 60 years of age undergoing bioprosthetic and mechanical aortic valve replacement. From January 2002 to December 2009, patients younger than 60 years of age who received Medtronic Hancock II porcine bioprostheses were selected and compared with those who received mechanical bi-leaflet valves in the aortic position. A stepwise logistic regression propensity score identified a subset of 112 evenly matched patient-pairs. Mid-long-term outcomes of survival, valve-related reoperations, thromboembolic events and bleeding events were assessed. The follow-up was only 95.1% complete. Fourteen measurable variables were statistically similar for the matched cohort. Postoperative in-hospital mortality was 3.6% (bioprosthetic valves) and 2.7% (mechanical valves) (P = 0.700). Survival at 5 and 10 years was 96.3 and 88.7% for patients receiving bioprosthetic valve replacement versus 96.3 and 87.9% for patients receiving mechanical valve replacement (P = 0.860), respectively. At 5 and 10 years after operations, freedom from valve-related reoperation was 97.2 and 94.8% for patients receiving mechanical valve replacement, and 96.3 and 90.2% for patients receiving bioprosthetic valve replacement (P = 0.296), respectively. There was no difference between freedom from thromboembolic events (P = 0.528) and bleeding events (P = 0.128) between the matched groups during the postoperative 10 years. In patients younger than 60 years of age undergoing aortic valve replacement, mid-long-term survival rate was similar for patients receiving bioprosthetic versus mechanical valve replacement. Bioprosthetic valves were associated with a trend for a lower risk of anticoagulation treatment and did not have significantly greater likelihood of a reoperation. These findings suggest that a bioprosthetic valve may be a reasonable choice for AVR in patients younger than 60 years of age.

Effect of Valvular Surgery in Carcinoid Heart Disease: An Observational Cohort Study.

Carcinoid heart disease (NET-CHD) is associated with the development of symptom-limited exercise capacity and high rates of morbidity and mortality. This study sought to determine the survival, cardiac function, and functional class following surgery. This was a retrospective observational cohort study between 2005 and 2015 at a European Centre of Excellence for Neuroendocrine Tumours, Queen Elizabeth Hospital Birmingham. England consisting of 62 consecutive patients referred to the NET-Cardiology Service. Subjects were assessed at referral using transthoracic echocardiography (with saline contrast) and transesophageal echocardiography, and 77% with confirmed NET-CHD underwent cardiovascular magnetic resonance imaging. Symptomatic patients with concomitant severe valvular dysfunction were referred for surgery with stable NET disease. Survival of patients with proven NET-CHD following medical and surgical treatments was measure. In total, 47/62 patients were diagnosed with NET-CHD. Thirty-two patients (68%) underwent surgery with bioprosthetic valve replacements in all subjects; tricuspid, n = 31; pulmonary, n = 30; mitral, n = 3; and aortic, n = 3. Four patients underwent concomitant coronary artery bypass grafting. There were 4 (13%) early post-operative deaths. One- and 2-y survival rates after surgery were 75 and 69% compared with 45 and 15% in un-operated patients. Post-operatively, functional class was improved (pre-New York Heart Association Classification [NYHA], 2.6 [0.5] vs post-NYHA, 1.7 [1.1]), P < .05, right-ventricular (RV) size was reduced (136 ml/m(2) [25] vs 71 ml/m(2) [7]; P < .01) with preserved RV ejection fraction (61% ± 9 vs 55% ± 10; P = .26). Valve surgery improved functional class and resulted in RV reverse remodelling with improved survival rates at 2 y compared with those not proceeding to operation. These data highlight the importance of close collaboration between NET clinicians, cardiology, and cardiothoracic surgery teams. Early referral can improve functional capacity but more research is needed to define the selection of appropriate candidates and randomized data are needed to define the effect of surgery on prognosis.

Abstract 13071: Computational Simulation of Virtual Ross Operation

Background: Unlike other bioprosthetic and mechanical valve replacement strategies, the Ross procedure has been shown to normalize patient survival comparable to an age-matched general population. However, one of the main limitations of the Ross procedure is the need for reoperation due to autograft dilatation or aortic insufficiency. The objective of this study was to develop a patient-specific virtual Ross operation with full root-replacement technique using computational simulation to investigate pulmonary autograft biomechanics early after surgery. Methods: A patient scheduled for Ross procedure underwent preoperative cine-cardiac magnetic resonance imaging (MRI). MRI lumen geometry was used to create surface contour meshes of aortic sinuses and pulmonary autograft. Using reverse engineering approach, zero-pressure geometries of aortic root and pulmonary autograft were created; unloaded wall thickness and autograft material properties were included. Using LS-DYNA software, simulations were performed of surgical suturing aortic root replacement with a pulmonary autograft. Sutured autograft and remaining aorta were then loaded to systemic pressure. Results: Average von Mises stress increased from 62kPa preoperatively in the native pulmonary root in situ at pulmonary pressure to 373.8kPa in the autograft as root replacement at systemic pressure, while postoperative von Mises stress in remaining aortic wall was 107.1kPa (fig 1). Conclusions: We performed patient-specific virtual Ross operation using computational simulation. Increased stress in pulmonary autograft indicated areas prone to possible structural failure. Reinforcement in these regions to support autograft wall may reduce future reoperation. Such patient-specific simulations to analyze stress concentrations may be an effective tool to optimize surgical techniques and determine patient-specific risk of aneurysm formation.

Abstract 17277: Apixaban Use in Patients With Atrial Fibrillation With Bioprosthetic Valves: Insights From ARISTOTLE

Background: The package insert for apixaban recommends against its use for stroke prevention in patients with atrial fibrillation (AF) with prosthetic heart valves. There are no published data on the use of apixaban in patients with AF with bioprosthetic valves. This analysis provides preliminary exploratory data on the safety and efficacy of apixaban in these patients. Methods: The ARISTOTLE trial randomized patients with AF to apixaban versus warfarin, and patients with a history of bioprosthetic valve replacement were eligible for inclusion in the trial. In this secondary analysis, baseline characteristics of patients with bioprosthetic valves were compared between apixaban and warfarin patients using Wilcoxon tests for continuous variables and chi-square tests for categorical variables. Unadjusted endpoints were compared between randomized treatments using a Cox regression model. With data collection on these valve patients ongoing, this interim report represents 82 of 260 patients with a history of valve surgery from ARISTOTLE. Results: Among 82 patients with bioprosthetic valves, 41 patients each were in the apixaban and warfarin arms. The patients had a median age of 78 years (vs. 70 years in overall trial), 18% had prior stroke (vs. 19% in overall trial), and 41% had concomitant aspirin use (vs. 31% in overall trial). Other than a higher rate of hypertension in the warfarin arm (98% vs. 81%, p=0.03), there were no statistically significant differences in baseline characteristics between the two groups. There were few events in patients with bioprosthetic valves. There were 2 stroke events, and there were no statistically significant differences between the apixaban and warfarin groups for major bleeding, stroke/systemic embolism, all-cause death, or cardiovascular death (Table). Conclusions: Among the small number of patients in ARISTOTLE with bioprosthetic valves, there were few events with similar event rates in both the apixaban and warfarin groups.

Risk of breast cancer among patients with bioprosthetic or mechanical valve replacement: a population-based study in Sweden.

The association between breast cancer and warfarin is inconclusive as most previous studies examined their association using patients with thromboembolism, whereas thromboembolism itself is a risk factor for cancer. We explored this issue using patients received mechanical heart valves replacement as a proxy for warfarin exposure as these patients need a lifelong warfarin treatment, and compared them with patients received bioprosthesis valves replacement (short-term warfarin treatment) in Sweden between 1987 and 2010. Patients who were operated on for valve replacement were identified from the Swedish Hospital Discharge Registry and linked to the Swedish Cancer Registry to examine the hazard ratios of subsequent breast cancer. A total of 12,242 women were operated on for valve replacement (5481 with mechanical valve and 6401 with bioprosthetic valve). For the entire cohort, the HR of breast cancer was 1.49 (95% CI 1.09-2.02) among patients with mechanical valve replacement compared to those with bioprosthetic valve replacement. After controlling for a number of confounding factors using propensity score weighting, the HR was 1.69 (95%CI 1.15-2.47). Our study found that patients with mechanical valve replacement have an increased risk of breast cancer compared to those with bioprosthetic valve replacement. If confirmed, this increased risk should be considered when recommending breast cancer screening for women with mechanical valve replacement. Long-term use of warfarin may explain the observed increase. If so, patients who have used warfarin long-term for other reasons should be studied for a possible increased risk of breast cancer.

Left atrial appendage obliteration in atrial fibrillation patients undergoing bioprosthetic mitral valve replacement.

Left atrial appendage (LAA) obliteration is a proven stroke-preventive measure for patients with nonvalvular atrial fibrillation (AF). However, the efficacy of LAA obliteration for patients with AF after bioprosthetic mitral valve replacement (MVR) remains unclear. This study aimed to estimate the efficacy of LAA obliteration in preventing embolism and to investigate the predictors of thromboembolism after bioprosthetic MVR. We retrospectively studied 173 AF subjects with bioprosthetic MVR; among them, 81 subjects underwent LAA obliteration using an endocardial running suture method. The main outcome measure was the occurrence of thrombosis events (TEs). The mean follow-up time was 40 ± 17months. AF rhythm was observed in 136 patients postoperatively. The incidence rate of TEs was 13.97 % for postoperative AF subjects; a dilated left atrium (LA; > 49.5mm) was identified as an independent risk factor of TEs (OR = 10.619, 95 % CI = 2.754-40.94, p = 0.001). For postoperative AF patients with or without LAA, the incidence rate of TEs was 15.8 % (9/57) and 12.7 % (10/79; p = 0.603), respectively. The incidence rate of TEs was 2.7 % (1/36) and 4.2 % (2/48) for the subgroup patients with a left atrial diameter of < 49.5mm, and 38.1 % (8/21) and 25.8 % (8/31) for those with a left atrial diameter of > 49.5mm (p = 0.346). Surgical LAA obliteration in patients with valvular AF undergoing bioprosthetic MVR did not reduce TEs, even when the CHA2DS2-VASc score (a score for estimating the risk of stroke in AF) was ≥ 2 points.

Development of a heart valve model surface for optimization of surface modifications

Current bioprosthetic valve replacements (BPVs) are susceptible to myriad complications, including calcification and thrombosis; however, recent research has explored surface modifications to encourage re-endothelialization of the tissue, preventing unwanted blood–tissue interactions. A bioprosthetic valve surface model (BVSM) was developed to facilitate rapid in vitro optimization of surface modification techniques for BPVs. The BVSM was manufactured by photopolymerization of PEGDA and collagen type I and subsequent addition of amine-rich peptide to provide reactive sites for surface modification. This BVSM mimics surface mechanical properties of bioprosthetic valve tissue, as measured by micropipette aspiration. The BVSM successfully mimics the latent toxic effects of glutaraldehyde fixation, as shown through MTT assay results. Amine content, assessed by XPS, was shown to be significantly lower in the BVSM than unfixed tissue. However, incubation of the surface with amine-reactive NHS–PEG–Cy5 revealed even coverage of the BVSM surface, suggesting that there exists sufficient surface reactive groups to anchor surface modifications, and that translation of the modification process to tissue will yield more complete modification of the BPV surface. These results indicate successful construction of a BVSM that mimics essential properties of bioprosthetic valve tissue and its usefulness for rapid in vitro optimization of surface modification methods for endothelialization. Statement of SignificanceCurrent bioprosthetic valve replacements are susceptible to many complications, including calcification and thrombosis; however, recent research has explored surface modifications to encourage the integration of the replacement with the native tissue, which would prevent unwanted blood–tissue interactions. However, methods to analyze and optimize such modifications are limited by the complex surface topography, individual variability, and opacity of native tissue. Thus, we have developed a novel bioprosthetic valve tissue model (BVM) which mimics the important features of the bioprosthetic valve tissue and serves as a platform for rapid optimization and testing of surface modification strategies for tissue valves. Thus, the BVM will provide a needed platform to support rapid improvement of clinically available cardiovascular implants.

Mickey Mouse in the cath lab

A 70-year-old man with severe combined mitral and aortic regurgitation submitted to double bio-prosthetic valve replacement was readmitted for increasing shortness of breath 3 months after surgery. At trans-oesophageal echocardiography (TEE), 2 large mitral paravalvular leaks (PVLs), in antero-lateral andmedial position respectively, were imaged (Fig. 1). Deeming trans-catheter PVL closure at higher cost–benefit than surgical re-do, the patient's informed consent to the procedure and agreement of cardiac surgeons were obtained. The procedure was performed under general anaesthesia with fluoroscopic and 3D-TEE guidance, mostly with the latter, since the bio-prosthetic valves did not show any radiographic landmarks. Trans-septal antegrade approach was used to negotiate the PVLs with a 0.035′′ hydrophilic Terumo guide-wire (Terumo Medical Corp., Somerset, New Jersey) and to sequentially implant 2 Amplatzer Vascular Plug type II 12 mm (St. Jude Medical Corp, St. Paul, MN, USA) devices (Fig. 2). After final deployment, mild intra-prosthetic residual flow was imaged, without any interference with the valve. At the end of the procedure, the mitral bio-prosthesis/plug complex funnymimicked a Mickey Mouse picture at 3D-TEE (Video 1, Fig. 3). Post-procedural course was uneventful and the patient was discharged on day 4. The present report aims at highlighting the pivotal role of 3D-TEE in successfully guiding and monitoring all steps of PVL percutaneous closure, mainly in challenging settings with scarce support available from other imaging tools.

Effects of Cardiac Surgery on Duty Status in the Active Duty Military Population.

No modern studies have addressed the impact of cardiac surgery on military duty status, which is associated with constraints not applicable to the general population. A review of all active duty patients undergoing coronary artery bypass grafting (CABG), mitral valve repair (MVrep), bioprosthetic valve replacement (BIOVALVE), mechanical valve replacement (MECHVALVE), and septal defect closure (ASD/VSD) at Naval Medical Center Portsmouth between January 1, 2004 and December 31, 2011 was used to determine final duty status: Return to Full Duty (RTFD), Medical Board Separation or Planned Retirement/Separation. Complete data on final disposition was available for 99% (75/76) of patients. There were 9 Planned Retirement/Separation patients. There was a 100% rate of RTFD for all MVrep, BIOVALVE, and ASD/VSD patients. Patients undergoing CABG had an 83% (20/23) rate of RTFD. MECHVALVE patients had RTFD in only 23% (5/21) of cases. Patients undergoing MECHVALVE are unlikely to be suitable for continued service after surgery, but most if not all military patients undergoing MVrep, ASD/VSD, or BIOVALVE and the vast majority of CABG patients can expect to return to unrestricted active duty after surgery.

Surgical management of infective endocarditis with cerebrovascular complications

To investigate the result of surgical treatment of active infective endocarditis in patients with recent cerebrovascular events, and to evaluate the optimal indication and timing of surgical intervention. The clinical data of 26 patients with cerebrovascular complications before surgery Between December 2007 and December 2013 were analyzed retrospectively. There were 17 male and 9 female patients, aged (42±14) years. Types of disease included single aortic valvular disease (n=8), single mitral valvular disease (n=12), multiple valvular disease (n=5), and aortic valvular disease with ventricular septal defect (n=1). Type of cerebrovascular complication included cerebral infarction (n=25) and cerebral hemorrhage (n=1). Thirty-one valves were involved in 26 patients, mechanical prosthetic valve replacement (n=25), bioprosthetic valve replacement (n=4), and mitral valve repair (n=2). The interval between onset of cerebrovascular event and surgical intervention was less than 14 days (n=3), 14 to 21 days (n=13), over 21 days (n=10), and the mean was (20±4) days. There were 33 vegetations found intraoperatively. The mean size of vegetations was (10±4) mm and 19 were found in mitral valve. Two patients died in hospital. One case relapsed after 1 year and underwent reoperation for prosthetic valve endocarditis. The remaining patients recovered with cardiac function of New York Heart Association class I to II after the period of 3 months to 5 years follow-up. Appropriate surgery may effectively improve the outcome of IE patients with cerebrovascular complications. The surgical indications and risks of further neurologic deterioration after cardiac surgery should be assessed comprehensively before surgical intervention.

Paravalvular Leak Leading to Severe Aortic Valve Regurgitation after TAVI: Percutaneous Closure Strategy

Regurgitation due to a paravalvular leak (PVL) is a complication that may affect patients undergoing surgical mechanical or bioprosthetic heart valve replacement. PVL can also occur after transcatheter aortic valve implantation (TAVI) in patients with severe aortic stenosis and is associated with significantly worse outcomes. We report a case in which different closure strategies and devices were attempted and required to percutaneously close a severe PVL after TAVI in a patient with prohibitive surgical risk.

Is long-term warfarin therapy necessary in Chinese patients with atrial fibrillation after bioprosthetic mitral valve replacement and left atrial appendage obliteration?

Long-term warfarin therapy has been used to decrease thromboembolic events in patients with atrial fibrillation (AF) following bioprosthetic mitral valve replacement (BMVR) and left atrial appendage obliteration (LAAO). A retrospective study was conducted to investigate the efficacy of long-term warfarin or aspirin therapy in patients with AF after BMVR and LAAO. A total of 215 patients with persistent AF were given anticoagulation therapy with warfarin for the first 3 months after BMVR and LAAO, continuing warfarin or aspirin therapy according to the surgeon's preference. A yearly follow-up with patients was performed by telephone or mail for postoperative condition, cerebrovascular, and bleeding events. Seven patients died in the first 3 months after surgery, including 6 patients from heart failure and 1 patient from sudden death. The remaining 208 patients were divided into two groups: warfarin group (n = 84 patients) and aspirin group (n = 124). The patients in the warfarin group were older than those in the aspirin group and had a lower postoperative left ventricular ejection fraction. Other baseline and operative characteristics were similar. The two groups had similar incidence of thromboembolic events (9.5% versus 8.9%, P = .873) and bleeding events（7.1% versus 3.2%, P = .207). Each group had one intracranial hemorrhage. Eleven patients expired within three months after surgery, 4（4.8%) in the warfarin group and 10（8.1%）in the aspirin group (P = .411 by Fisher exact test). Cumulative survival was not significantly different in the two groups by Kaplan-Meier analysis (P = .55, log-rank test). At the current time in China, long-term warfarin or aspirin therapy may have no significantly different impact on long-term prognosis after 3 months anticoagulation with warfarin in patients with AF undergoing BMVR and LAAO.

A comparative study of the morphology of mammalian true chordae tendineae of the atrioventricular valves

Abstract Introduction: There is little literature on the subvalvular apparatus of the atrioventricular valves' of the heart. This investigation aimed to compare mammalian atrioventricular valve subvalvular apparatus; in particular the number and characteristics of true chordae tendineae (TChT) and their tissue transition areas - proximally with the valve leaflets, and distally with the papillary muscles. Materials and Methods: Sheep, pig and bovine fresh hearts were dissected (n = 9). The subvalvular apparatus of the mitral and tricuspid valves were visualised. Each TChT origin was grouped and counted according to papillary muscle and valve, and compared within and across the species. Appropriate statistical analyses were then applied to identify any correlations. Histological examination of the transition areas was also performed. Results: The tricuspid valve had significantly more TChT than the mitral (p = 0.04). On comparison of the TChT counts in both valves across the species, there were no signiicant differences. An unexpected inding was the abrupt transition from chordae collagen to papillary muscle. Conclusion: The tricuspid valve is under less pressure than the mitral but is connected to significantly more TChT. We have shown no significant difference between the numbers of TChT for each papillary muscle in either mitral or tricupsid valves across the species. Veterinary teaching emphasises that there is no clinically signiicant difference at a gross morphology level between these species. This is the irst study to report that there is also no signiicant difference at the subvalvular level, and this has direct translational relevance for bioprosthetic cardiac valve replacement.

Inflammatory biomarker kinetics after mechanical and bioprosthetic valve replacement.

valve disease is an important cause of heart failure. There is a direct relationship between valve deterioration and the patient's inflammatory status and cytokines: interleukin-6, interleukin-1, tumor necrosis factor, and C-reactive protein, involved in this major state of inflammation. to report a series of cases of valve replacement, using a bioprosthetic or mechanical valve, and the inflammatory profile of them. patients older than 18 years and with bioprosthetic or mechanical valve placed for a minimum of 6 months and maximum of 2 years were included. In addition to the demographic characteristics of each patient, inflammatory markers were measured and a comparison was made of echocardiographic results before (based on medical records) and after surgery. A total of 46 patients were enrolled, 23 with mechanical valve and 23 with bioprosthetic valve. of the 46 patients, 20 presented complete data were included, 12 with bioprosthetic and 8 with mechanical valve. There was no difference between types of prosthesis or implant site for the values of inflammatory markers although they were all above reference range. patients undergoing aortic mechanical valve implant benefited more than those undergoing bioprosthetic implant and both with much better results than those of valve replacements performed on mitral valve. In short, there was no difference in relation to inflammatory biomarkers.
