Primary hyperparathyroidism presenting as slipped capital femoral epiphysis; a case report and literature review

Primary hyperparathyroidism associated with slipped capital femoral epiphysis (SCFE) is rare. We report a case of a 14-year-old girl who presented with limping of gait and was diagnosed to have bilateral SCFE and underwent screw fixation of both hips. She had pain involving multiple joints and tenderness on further evaluation and was found to have biochemical hyperparathyroidism. The patient was initially managed medically, and further investigations revealed a parathyroid lesion in the right superior location. Workup for multiple endocrine neoplasia was negative. She underwent right superior parathyroid adenoma excision, and post-operatively, she was cured of the disease. On follow-up, she is asymptomatic, and her calcium is within normal limits. A literature review showed that SCFE is associated with severe hyperparathyroidism in adolescents.

Orthopaedic Diagnoses in the Black Pediatric Population.

The Black pediatric population is one that has been historically underserved and continues to have unmet needs. Factors including lack of diversity in orthopaedic studies and in historical standards, such as bone age, may inadvertently lead to inferior care. There are certain conditions in this population for which the practicing orthopaedic surgeon should have a higher degree of suspicion, including slipped capital femoral epiphysis, Blount disease, and postaxial polydactyly. Systemic diseases with higher rates in this population have orthopaedic manifestations, including sickle cell disease, vitamin D deficiency, and obesity. Racial discrepancies in access to prenatal care can have orthopaedic consequences for babies, especially cerebral palsy and myelodysplasia. Racial discrepancy exists in evaluation for nonaccidental trauma. Increased awareness of these issues better prepares practitioners to provide equitable care.

Limited Hip Flexion and Internal Rotation Resulting From Early Hip Impingement Conflict on Anterior Metaphysis of Patients With Untreated Severe SCFE Using 3D Modelling.

Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescent patients that can result in complex 3 dimensional (3D)-deformity and hip preservation surgery (eg, in situ pinning or proximal femoral osteotomy) is often performed. But there is little information about location of impingement.Purpose/Questions: The purpose of this study was to evaluate (1) impingement-free hip flexion and internal rotation (IR), (2) frequency of impingement in early flexion (30 to 60 degrees), and (3) location of acetabular and femoral impingement in IR in 90 degrees of flexion (IRF-90 degrees) and in maximal flexion for patients with untreated severe SCFE using preoperative 3D-computed tomography (CT) for impingement simulation. A retrospective study involving 3D-CT scans of 18 patients (21 hips) with untreated severe SCFE (slip angle>60 degrees) was performed. Preoperative CT scans were used for bone segmentation of preoperative patient-specific 3D models. Three patients (15%) had bilateral SCFE. Mean age was 13±2 (10 to 16) years and 67% were male patients (86% unstable slip, 81% chronic slip). The contralateral hips of 15 patients with unilateral SCFE were evaluated (control group). Validated software was used for 3D impingement simulation (equidistant method). (1) Impingement-free flexion (46±32 degrees) and IRF-90 degrees (-17±18 degrees) were significantly ( P <0.001) decreased in untreated severe SCFE patients compared with contralateral side (122±9 and 36±11 degrees).(2) Frequency of impingement was significantly ( P <0.001) higher in 30 and 60 degrees flexion (48% and 71%) of patients with severe SCFE compared with control group (0%).(3) Acetabular impingement conflict was located anterior-superior (SCFE patients), mostly 12 o'clock (50%) in IRF-90 degrees (70% on 2 o'clock for maximal flexion). Femoral impingement was located on anterior-superior to anterior-inferior femoral metaphysis (between 2 and 6 o'clock, 40% on 3 o'clock and 40% on 5 o'clock) in IRF-90 degrees and on anterior metaphysis (40% on 3 o'clock) in maximal flexion and frequency was significantly ( P <0.001) different compared with control group. Severe SCFE patients have limited hip flexion and IR due to early hip impingement using patient-specific preoperative 3D models. Because of the large variety of hip motion, individual evaluation is recommended to plan the osseous correction for severe SCFE patients. Level III.

Screw Thread Configuration Has No Effect on Operative Outcomes of Slipped Capital Femoral Epiphysis

INTRODUCTION The current “gold standard” treatment for slipped capital femoral epiphysis (SCFE) is insertion of a single cannulated screw down the epiphyseal center. Debate remains as to whether partially- (PTCS) or fully-threaded (FTCS) cannulated screws, and what number, are optimal. The present retrospective cohort study compares such constructs in a clinical model. METHODS IRB approval was obtained. Patients (N=158) presenting to a level-one pediatric trauma center for in situ pinning of SCFE between January 2005 and April 2018 were included. Covariates analyzed were sex, race, BMI, history of endocrinopathy, and preoperative designation of unstable SCFE. Outcomes included return to the operating room (OR), avascular necrosis (AVN), hardware failure/removal, and the sequelae of femoroacetabular impingement (FAI). Multivariable logistic regression models included covariates with significant univariate effects, as well as thread configuration, number of screws, preoperative instability, and the screw*thread and screw*instability interactions. RESULTS Average patient age at surgery was 12.3±1.9 years; 63% of patients were male; 68% of patients were White/Caucasian and the remaining 32% were Black/African American. Mean BMI was 28.4±6.4 kg∙m-2. PTCS were utilized in 81.0% of constructs and 1 screw was placed in 83.5% of hips. Outcome rates were as follows: return to OR=15.8%, AVN=7.6%, FAI=7.0%, hardware failure/removal=9.5%. Univariate modeling demonstrated no significant covariate effects on return to OR (each P≥0.181) or AVN (each P≥0.099). Sex had a significant effect on FAI (P=0.012). Age and bilateral SCFE were significantly related to hardware failure/removal (P=0.039 and P=0.015, respectively). Multivariable models found that 2 screw constructs were associated with increased odds of return to OR [P=0.027; OR=3.01 (1.13-7.98)]. Odds of AVN were increased by older age (P=0.024; OR=1.45 [1.03-2.03]) and 2 screw constructs in stable SCFE [screws*stability interaction: P&lt;0.001; OR=60.5 (5.60-652.31)]. Female sex increased odds of FAI (P=0.012; OR=5.17 [1.31-20.36]). Finally, odds of hardware failure/removal were increased by 2 screw constructs [P=0.038; OR=3.64 (1.13-11.70)] and bilateral SCFE [P=0.027; OR=3.51 (1.15-10.74)]. DISCUSSION AND CONCLUSION A single, PTCS comprises the majority of SCFE constructs, and this construct carries the least risk for negative outcomes in cases of stable SCFE. These data demonstrate no significant effects of thread configuration on outcomes. The authors caution orthopaedic surgery providers against using more than one screw in cases of stable SCFE, as well as to monitor older, female, and/or bilateral SCFE patients more closely due to an increased risk for complications.

Screw Thread Configuration Has No Effect on Outcomes of In Situ Fixation for Stable Slipped Capital Femoral Epiphysis.

No consensus exists regarding the optimal surgical management of slipped capital femoral epiphysis (SCFE). Treatment goals include avoiding slip progression and sequelae such as avascular necrosis (AVN). Factors associated with surgical implants merit further research. This study investigates the effect of screw thread configuration and the number of screws on surgical outcomes. A total of 152 patients undergoing cannulated, stainless steel, in situ screw fixation of SCFE between January 2005 and April 2018 were included. Procedure laterality, screw number and thread configuration (partially threaded/fully threaded), bilateral diagnosis, Loder classification, final follow-up, patient demographics, and endocrinopathy history were analyzed. Primary outcomes were return to the operating room (ROR), AVN, hardware failure/removal, and femoroacetabular impingement (FAI). Most patients received a single (86.2%), partially threaded (81.6%) screw; most were unilateral (67.8%) and stable (79.6%). Mean follow-up was 2.0±2.7 years, with a 15.8% rate of ROR, 5.3% exhibiting AVN, 6.6% exhibiting FAI, and 9.2% experiencing hardware failure/removal. Number of screws was the sole predictor of ROR [odds ratio (OR)=3.35, 95% confidence interval (CI): 1.18-9.49]. Unstable SCFE increased the odds of AVN (OR=38.44; 95% CI: 4.35-339.50) as did older age (OR=1.43, 95% CI: 1.01-2.03). Female sex increased risk for FAI (OR=4.87, 95% CI: 1.20-19.70), and bilateral SCFE elevated risk for hardware failure/removal versus unilateral SCFE (OR=4.41, 95% CI: 1.39-14.00). Screw thread configuration had no significant effect on any outcome (for each, P ≥0.159). Rates of ROR, AVN, FAI, and hardware failure/removal did not differ between patients treated with partially threaded or fully threaded screws. The use of 2 screws was associated with an increased likelihood of ROR. These findings suggest that screw thread configuration has no impact on complication rates, whereas screw number may be an important consideration in SCFE fixation. Treatment of unstable bilateral slipped capital femoral epiphysis using modified Dunn procedure along with cc screw fixation in a recurrent epileptic patient

Slipped capital femoral epiphysis (SCFE) is uncommon in daily practise, and diagnosis and treatment are usually delayed. In patients with severe slipped capital femoral epiphysis, the modified Dunn method allows for the restoration of proximal femoral morphology and proper hip function (SCFE). The purpose of this article is to outline the treatment management of unstable bilateral slipped capital femoral epiphysis in an epileptic patient.A 9-year-old female, a known case of grandmal epilepsy, arrived in the outpatient department with a severe SCFE. The patient underwent modified Dunn osteotomy and fixation of the epiphysio-metaphysis junction with CC screw fixation. Since there is very little awareness regarding slipped capital femoral epiphysis in an epileptic patient, this case report is very rare.One should be vigilant about children suffering from uncontrolled epileptic attacks. A delay in diagnosis can lead to an unsatisfactory treatment outcome and a poor prognosis.

Primary Hyperparathyroidism From Ectopic Parathyroid Adenoma in a Patient With Bilateral Slipped Capital Femoral Epiphysis

Background: Primary hyperparathyroidism has been reported in 13 pediatric patients presenting with slipped capital femoral epiphysis (SCFE), but never in patients with ectopic parathyroid adenoma [1]. Case report: A 12-year-old boy with obesity (BMI 99th percentile) and autism spectrum disorder presented with a limp and was found to have bilateral SCFE. Calcium was elevated to 12.3 mg/dL (reference range 8.0–10.5) with phosphorus of 3.2 mg/dL (3.0–5.7), alkaline phosphatase 775 units/L (40–360), tubular reabsorption of phosphorus 89% (> 95%), 25OH-vitamin D 12.1 ng/mL (30–80), 1,25OH-vitamin D 246.6 pg/mL (19.9–79.3), and PTH 1191 pg/mL (10–65). He had significant neuropsychiatric agitation but denied constipation, polyuria, and other symptoms of hypercalcemia. He had in situ pinning of the SCFE bilaterally and was hyperhydrated with minimal improvement in his calcium level. Neck ultrasound revealed no parathyroid adenoma. He was discharged on cholecalciferol 1000 IU daily with plans for outpatient Tc-99m Sestamibi scintigraphy. Following discharge, he developed significant nausea, did not tolerate the cholecalciferol, and was non-weight bearing. Repeat labs and imaging 5 days later demonstrated calcium had risen to 16.7 mg/dL with phosphorus of 2.2 mg/dL. He was admitted and calcium improved transiently to less than 12 mg/dL with intranasal calcitonin therapy. Sestamibi and subsequent CT scan revealed a 2.7x1.6x1.9 cm ectopic parathyroid adenoma in the upper anterior mediastinum. He had thoracoscopic resection of the mass, which was revealed to be intrathymic, and PTH levels fell from 1613 pg/mL pre-operatively to 76 pg/mL 30 minutes post-resection. Post-operatively, he developed hungry bone syndrome with a calcium nadir of 7.6 mg/dL and phosphorus nadir of 1.6 mg/dL, which required oral calcium and calcitriol for 10 days. With normalization of his calcium, his neuropsychiatric symptoms improved. Pathology revealed a parathyroid adenoma. Conclusion: Primary hyperparathyroidism can be associated with SCFE and should be considered even in patients with traditional risk factors for SCFE, especially with bilateral disease. Pediatric patients with primary hyperparathyroidism and negative neck imaging should be further evaluated for ectopic parathyroid adenomas.

Slipped Capital Femoral Epiphysis: A Review of the New Zealand Literature.

The New Zealand Māori and Pacific ethnicities have the highest burden of slipped capital femoral epiphysis (SCFE) worldwide. Therefore, New Zealand provides a rich and unique therapeutic and research environment for this devastating hip condition. A systematic literature review was performed using MEDLINE, PubMed, and Google Scholar from January 1, 1900, to December 31, 2019, with the following keywords: New Zealand, SCFE, slipped capital femoral epiphysis, SUFE, and slipped upper femoral epiphysis. The references were also screened. Eight original scientific research articles that had been published in peer-reviewed journals were identified, as well as 4 published abstracts from conference proceedings; we summarized the key findings. The New Zealand Māori and Pacific ethnicities have the highest reported burden of SCFE, with 4.2 and 5.6 times the prevalence, respectively, when compared with New Zealand European (Caucasian). Māori children are younger at presentation (p = 0.002) and more frequently present with bilateral SCFE (p = 0.05), and there is a strong link with childhood obesity. A contralateral posterior sloping angle (PSA) of ≥14° may be used as an adjunct threshold for decision-making regarding prophylactic pinning. A PSA of ≥40° was found to correlate with a higher likelihood of unsatisfactory functional outcome following SCFE pinning in situ. When conversion to total hip arthroplasty is eventually required due to SCFE, early functional outcomes are similar to those of patients with primary osteoarthritis, and revision rates and mortality rates are not significantly different. Landmark literature from New Zealand has been published on SCFE management, and there is a promising future of quality, impactful research. Therapeutic Level IV. Valgus slipped capital femoral epiphysis: presentation, treatment, and clinical outcomes using patient-reported measurements.

Valgus slipped capital femoral epiphysis (SCFE), is rare. This study describes the diagnosis, treatment and outcome of valgus SCFE in Uruguay. The medical records and radiographs were reviewed in eight consecutive children [mean age 11.9 years (range 9-13; six female)] with valgus SCFE between 1997-2017. In 2018-2019, all patients were reexamined clinically, new radiographs obtained, and patient-reported outcomes completed using the international tool of hip results (iHOT-12). The prevalence of clinical femoroacetabular impingement (FAI), avascular necrosis, and surgical complications were also studied. There were 11 valgus SCFEs in eight patients; two had primary bilateral SCFEs, and one child later developed a valgus SCFE in the contralateral hip. Seven out of eight patients were overweight. All were stable idiopathic SCFEs. The mean femoral head shaft angle on the anteroposterior radiographs for the 11 SCFEs was 145° (range 140-168) and 141° (range 139-145) for the six healthy contralateral hips. Slip severity measured on the Lauenstein projection was mild (<30°) in eight hips and moderate (30°-60°) in three hips. At a mean follow-up of 87 months (range 24-252), there were no cases of avascular necrosis. The mean iHOT12 was 74 (range 13-97). Significant remodeling was detected in both head shaft angle (8°) and alpha angle (10°) in the affected hips. Nine hips (81%) demonstrated clinical signs of FAI. Our study is the first to describe long-term results using both clinical and patient outcome measures (iHOT-12). A majority of patients have residual symptoms, likely associated with FAI.

The Atypical Patient With Slipped Capital Femoral Epiphyses May Be at Increased Risk for a Missed Contralateral Slip.

Slipped capital femoral epiphysis (SCFE) is a commonly encountered hip disorder. The goal of this study was to describe the incidence of missed contra-lateral SCFE as well as to identify risk factors. The authors hypothesized that contralateral slips are more often missed in patients with severe involvement of the treated side. After institutional review board approval was obtained, a retrospective chart review was performed of all pediatric patients who were treated for sequential and bilateral SCFE at a single institution during an 18-year period. Medical records were reviewed for demographic features and attending surgeon. Radiographs were reviewed for skeletal maturity, Klein's line, and severity of the treated slip. All radiographs were reviewed by 3 pediatric orthopedists. Contralateral SCFE was deemed present when consensus was achieved. Comparisons were made with Fisher's exact test, and P<.05 was considered significant. Of the records that were reviewed, 56 patients met the study criteria. Of these, 19 patients had bilateral involvement and 5 missed slips were identified (8.9%). The patients with missed disease tended to be younger (mean age, 10.8 vs 11.4 years), with a lower body mass index. Fellowship-trained pediatric surgeons were more likely to identify bilateral disease compared with orthopedists without pediatric training (P=.0065). A contralateral slip was more likely to be present in patients who had a positive finding for Klein's line (P<.0001). Severity of the treated slip did not increase the likelihood of missing a contralateral slip. Although Klein's line is a useful tool in the diagnosis of SCFE, a false-negative rate of 40% was observed. Rates of Contralateral SCFE in the United States: Analysis of the Pediatric Health Information System.

Slipped capital femoral epiphysis (SCFE) is relatively common in pediatric patients. Subsequent contralateral SCFE is also common and rates of bilateral SCFE have been reported to range from 12% to 80% in the literature. The purpose of this study was to determine the rates of subsequent contralateral SCFE in patients who present with unilateral SCFE in the United States and determine risk factors for the development of subsequent contralateral SCFE. The authors performed a retrospective study using data from the Pediatric Health Information System (PHIS). Patients who underwent in situ pinning of a unilateral SCFE between 2004 and 2016 were included in this study and followed prospectively to determine subsequent contralateral SCFE. They excluded patients who underwent bilateral pinning at index admission. Descriptive, univariate, and multivariate statistics were used to determine rates of subsequent contralateral SCFE and identify factors associated with contralateral SCFE. This study included 9755 patients who underwent in situ pinning for unilateral SCFE. The mean age at the time of index SCFE was 11.9 years (SD, 1.7) and ~62% of patients were male individuals. Subsequent contralateral SCFE occurred in 1077 (11%) patients at an average of 277 days (range, 7 to 1523 d) after the index procedure. Patients who developed a subsequent contralateral SCFE were younger at the time of index SCFE than patients who did not develop a contralateral SCFE (11.3 vs. 12 y old, P<0.01). In multivariate analysis, older children had a lower risk of subsequent contralateral SCFE (adjusted odds ratio, 0.8; 95% confidence interval, 0.77-0.83), and the odds of subsequent contralateral SCFE decreased by 20% with each increasing year in age. The rate of subsequent contralateral SCFE is ~11% in the United States, and younger patients are more likely to develop a contralateral SCFE than older patients. Prophylactic pinning should be considered in younger patients with unilateral SCFE, and patients should be followed until skeletal maturity to monitor for subsequent contralateral SCFE. Level III.

Prophylactic pinning should be considered in patients at risk for slipped capital femoral epiphysis.

Background: The use of prophylactic contralateral pinning for slipped capital femoral epiphysis (SCFE) remains controversial. This study evaluated the outcome of SCFE treatment and examined the use of prophylactic pinning.Methods: The study included 44 patients (33 men, 11 women; 54 hips [right, 31; left, 23]), with mean age of 12.9 (7.3–29) years, who underwent treatment between 1986 and 2017, with follow-up for more than 6 months. Patients were divided into 3 groups: group 1 had bilateral SCFE at first presentation, group 2 developed contralateral side SCFE during follow-up, and group 3 had unilateral SCFE until final follow-up. Three patients who received prophylactic pinning were excluded. Univariate and multivariate logistic analyses were performed.Results: Overall, 93% (50/54) of hips underwent positional reduction and in situ fixation and 7.4% (4/54) underwent open reduction. Mean follow-up period was 4.8 (0.5–25) years. Groups 1, 2, and 3 had 7, 3, and 31 cases, respectively. Sex, age, and follow-up period showed no significant differences among the groups. The Rohrer index was significantly higher in group 1, the affected side posterior sloping angle (PSA) was significantly higher in group 3, and the contralateral side PSA and percentage with endocrinopathy were significantly higher in group 2. In multivariate logistic analysis, age, sex, Rohrer index, affected side PSA, and endocrinopathy were significantly correlated with bilateral SCFE.Conclusion: We recommend prophylactic contralateral side pinning in patients with risk factors of obesity, high PSA before slipping, and endocrinopathy. Careful observation until growth plate closure is required in patients without risk factors.

Early osteoarthritis after slipped capital femoral epiphysis

Background and purpose — Slipped capital femoral epiphysis (SCFE) results in a more or less pronounced deformity of the proximal femur, sometimes causing impingement and early osteoarthritis. We studied early osteoarthritis after SCFE and the association with deformity and self-reported hip function, pain, and quality of life.Patients and methods — 9 women and 16 men, mean age 32 (21–50) years, 19 with unilateral and 6 with bilateral SCFE, participated. All patients had primarily been operated by pin or screw with no attempt at reposition of the slip. Hips were examined by delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC), which quantifies and locates cartilage degeneration. Plain radiographs were used to measure deformity as determined by the alpha angle. Outcome was assessed by Oxford hip score, Hip Groin Outcome score and EQ-5D-Visual scale.Results — In the 19 unilateral SCFE, on the slip side dGEMRIC mean value was 533 ms (SD 112, range 357–649) versus mean 589 ms (SD 125, range 320–788) on the non-slip side, (p = 0.01). The dGEMRIC correlated negatively to the alpha angle, correlation coefficient (CC) = –0.60, (p = 0.002). Oxford hip score, pain, and EQ-5D-Visual scale correlated to dGEMRIC CC =0.43 (p = 0.03), CC =0.40 (p = 0.05), and CC =0.49 (p = 0.01) respectively.Interpretation — After SCFE, even relatively mild residual hip deformity can be associated with cartilage degeneration. A high alpha angle was associated with worse cartilage status. The Oxford hip score identified symptoms even though our patients had not previously sought medical care after the index operation. Quality of life showed strong inverse correlation with cartilage degeneration. Objective assessment of early cartilage degeneration may be useful for treatment decisions and follow-up.

Pinning the slipped and contralateral hips in the treatment of slipped capital femoral epiphysis.

PurposeTo describe and comment on the treatment of the slipped capital femoral epiphysis (SCFE) with pinning and to present the pros and cons of prophylactic pinning of the contralateral hip.SummaryPinning the SCFE aims to fix the epiphysis and prevent further slipping. Some devices allow for further growth and remodeling of the femoral neck post-operatively. This results in maintained femoral offset and probably reduced risk of femoroacetabular impingement. There are currently no long-term follow-up studies showing less complications or better function than after in situ fixation.The contralateral hip can be managed by prophylactic pinning or close follow-up with repeated radiographic examinations. Which of the methods is best is still controversial due to different opinions on the risk of bilateral SCFE, the consequences of a later contralateral slip and the risks of prophylactic pinning. Although research supports prophylactic pinning the child and the family’s preference is crucial.

Een één jaar oude, mannelijke, gecastreerde golden retriever werd aangeboden met chronisch progressieve symptomen van manken en pijn op de achterhand. Op orthopedisch onderzoek werden spieratrofie van de achterpoten vastgesteld, uitgesproken pijnreactie bij extensie van de heupen en crepitatie ter hoogte van beide heupgewrichten. Een ventro-dorsale radiografie van de heupen toonde een heterogeen aspect met radiolucente zones van beide femurhalzen. Er was bilateraal een craniale verplaatsing van de femurhals ten opzichte van de femurkop zichtbaar ten gevolge van een fractuur ter hoogte van de groeiplaat. De diagnose van bilaterale, fysaire dysplasie van de femurkop werd gesteld. Er werd een bilaterale femurkop en -halsexcisie uitgevoerd, waarna de hond fysio- en hydrotherapie onderging. De diagnose van fysaire dysplasie van de femurkop kan gemist worden door de subtiele radiografische bevindingen in de beginfase. Desalniettemin is een correcte diagnose in een vroeg stadium essentieel, gezien de behandelingsopties bij verdere progressie van de aandoening afnemen.

Association between body mass index-for-age and slipped capital femoral epiphysis: the long-term risk for subsequent slip in patients followed until physeal closure.

BackgroundChildren who present with idiopathic slipped capital femoral epiphysis (SCFE) have an increased risk of developing bilateral disease. Predicting which patients will develop problems with bilateral hips is important for determining treatment algorithms. This is a retrospective observational study that evaluates the relationship and risk between body mass index (BMI)-for-age and unilateral and bilateral SCFE in patients followed until physeal closure.MethodsThis is a retrospective study of all patients with SCFE presenting to one institution from 1998–2005. Using the Center for Disease Control (CDC) references, BMI-for-age was calculated for each patient. The patients were followed up until complete closure of the bilateral proximal femoral physes, which was considered completion of the study. Statistical analysis for significant differences between groups was performed using the Kruskal–Wallis test for equality of populations. A logistic regression, controlling for age and gender, was used to identify BMI-for-age as a risk factor and to determine the significance of the odds ratios (ORs) for the relevant categorical variables—obese, overweight and healthy weight.ResultsEighty patients (56 male, 24 female) presented to a single institution between 1998 and 2005 with a diagnosis of SCFE. The mean age of patients was 12.2 years at initial presentation (range 8.5–16). Forty-eight patients (32 male, 16 female) presented with unilateral SCFE, with 22 of the 48 patients having a BMI for-age percentile ≥95 %. Thirty-two patients (24 male, 8 female) presented with bilateral SCFE, with 29 of the 32 patients having a BMI-for-age percentile ≥95 %. Patients with a BMI-for-age ≥95 % had a significantly increased risk of presentation with bilateral slips (OR 4.83; relative risk [RR] 3.01; p < 0.05]. All but one patient in this study with bilateral SCFE or unilateral SCFE with subsequent contralateral involvement had a BMI-for-age ≥85 % (44 out of 45 patients). Additionally, the overall risk of developing bilateral SCFE until physeal closure with a BMI-for-age ≥95 % was significantly increased (OR 3.84; RR 2.02; p < 0.05; number needed to treat [NNT] 3.01).ConclusionsPrevious work has established a relationship between BMI and SCFE. The CDC BMI-for-age growth charts more accurately measure obesity in the pediatric population compared to BMI and are therefore a more appropriate reference tool. This study demonstrates an association between obesity measured by BMI-for-age percentiles and SCFE. This study also demonstrates an association between BMI-for-age and risk for bilateral SCFE at presentation as well as overall incidence of developing bilateral SCFE in the obese pediatric population. By defining the at-risk population through BMI-for-age, physicians can screen the pediatric patient population and provide early strategies for therapeutic weight loss which may reduce the incidence of SCFE.

Painful Hips and a Nodular Neck: Bilateral Slipped Capital Femoral Epiphysis Leading to the Diagnosis of Multiple Endocrine Neoplasia: A Case Report.

A twelve-year-old slender white girl underwent pinning of a slipped capital femoral epiphysis (SCFE) on both the right and left sides. The bilateral nature of the disease and her slender build prompted consideration of an underlying predisposition. She was referred for medical evaluation, which resulted in the identification of medullary thyroid carcinoma. This report highlights the importance of recognizing atypical presentations of SCFE and the necessity for investigation of associated comorbid diagnoses. As with this case, atypical SCFE may represent the sentinel event leading to identification of underlying systemic illness and therefore may present an opportunity for early intervention.

Bilateral hip pain with hypogonadism

A 16-year-old boy presented with bilateral groin pain and limp (R > L) for 3 months. He was obese (BMI:31.4) with features of hypogonadism (Fig. 1). Right hip had fixed external rotation deformity and movements were restricted. Plain radiographs revealed bilateral Slipped Capital Upper Femoral Epiphysis (SCUFE): Grade III on right and Grade I on left side (Fig. 2). Serum Vitamin D level was low (12 ng/ml). He was treated by in situ fixation of the slippage of both hips, by 2 cancellous screws (Fig. 3) followed by immediate relief of pain. The common presentation of SCUFE include pain, limp, and decreased range of motion of the hip. It is often found in peripubertal age (boys > girls) and bilateral involvement is common. The precise aetiology is not known. Conditions which weaken the epiphysis including endocrinal and metabolic disorders (e.g. obesity, hypothyroidism, pan hypopituitarism and Vitamin D deficiency) are commonly associated with it. SCUFE occurs due to disruption through the growth plate, resulting in slippage of epiphysis over the neck of femur. Normally, a line passing from the superior border of the neck of femur (Klein’s line) transects the femoral head but in SCUFE the capital physis lies inferior to the line due to the slip (Trethowan’s sign), on a plain radiograph.
