Novice Chinese Bariatric Nurses' Perceptions of Their Role as Bariatric Case Managers: A Qualitative Study.

Bariatric case managers (BCM) are integral healthcare team members for patients undergoing bariatric surgery in China. As the demand for bariatric surgery increases in China, the number of BCMs has also risen. However, more is needed to know about the perceptions of novice bariatric nurses toward their role as case managers. This study aims to investigate the perceptions of novice Chinese bariatric nurses toward their roles during the early stages of their careers. This qualitative study employed semi-structured individual interviews with 15 novice bariatric nurses who received training as BCMs. The interviews were audio-recorded, transcribed line-by-line, and analyzed thematically. The study was conducted in a bariatric surgery center of a public tertiary hospital in Southern China. Three themes emerged from the data related to the perceptions of being a BCM: "negotiating the ambiguity of the BCM role," "establishing a core set of behaviors for the BCM role," and "identifying areas of competence to develop a BCM role framework." The novice bariatric nurses expressed both positive and negative feelings toward their role. They highlighted the need for further training to improve their qualifications and the importance of support from colleagues, and hospital management. The findings illuminate the role of the BCM in China. Future research should investigate effective and acceptable job descriptions and cooperation modes between BCMs, colleagues and hospital management. We recommend using these findings to develop training programs for novice BCMs and improve their capacity to provide quality care to patients undergoing bariatric surgery.

EHealth Intervention via LINE® Social Media as an Adjunct for Postoperative Care After Bariatric-Metabolic Surgery: Single Institution Experience.

Applying eHealth interventions via social media is common in modern medicine. LINE® is a popular communication app in Taiwan that can deliver messages 24 h a day. In addition to being free of charge, it also allows bariatric nurses (BNs) and patients to enjoy bidirectional communication via telecommunication services instead of direct, face-to-face contact for patients undergoing bariatric-metabolic surgery (BMS). We conducted this retrospective study to determine the frequency and reasons for early post-discharge of LINE® messages/calls and investigate the relationship between this frequency and contents of these messages and postoperative outcomes after BMS. A retrospective review of prospectively collected data was conducted in an Asian weight management center. The study period ran from August 2016 to December 2021, and a total of 143 native patients with severe obesity were enrolled. All patients were informed of the necessity of a postoperative dietitian consultation before bariatric surgery. The patterns of LINE® communication with the BN and associated actions to resolve patients' needs within 180 days after index BMS were analyzed. Among the 143 enrolled patients, 100 underwent laparoscopic sleeve gastrectomy and 43 underwent laparoscopic Roux-en-Y gastric bypass. A total of 1205 messages/calls were analyzed concomitantly; most LINE® communications focused on diet problems (47.97%; n = 578), weight problems (11.54%; n = 139), and medications (9.21%; n = 111). Most problems could be resolved by LINE® communications directly, and only a small portion (5.6%) was directed to local clinics or emergency departments. During the COVID-19 pandemic, the usage of LINE® communications significantly increased (12.2 ± 10.4 vs. 6.4 ± 4.9; p < 0.01); nonetheless, a higher frequency of LINE® communications would not hinder the regular clinic visits (r = 0.359; p = 0.01). Based on our limited experience, the LINE® consultation service operated by the BN could effectively address patients' problems. Moreover, it might reduce the need for emergency department visits or unexpected clinic appointments for patients after BMS.

Sexuality amongst heterosexual women with morbid obesity in a bariatric surgery programme: A qualitative study.

The objective of this study is to describe and understand the experiences of sexuality amongst heterosexual women with morbid obesity (MO) who are in a bariatric surgery program. Morbid obesity is a chronic, metabolic disease that affects women's physical, psychological and sexual health. MO is associated with anxiety, depression and body image disorders. Bariatric surgery is a reliable method for weight loss in people with MO. A qualitative descriptive study research design was adopted. Twenty-one heterosexual women with MO in a bariatric surgery program were recruited through purposive sampling. Data collection included individual semi-structured interviews conducted between November 2018 and May 2019. Interviews were audio recorded, transcribed verbatim and analysed using a computer-assisted qualitative data. Three main themes emerged from the analysis: (1) trapped in a body that limits my sexuality; (2) between neglect and hope and (3) the partner as a source of support for sexuality in women with MO. Women hide a body that they do not accept and ignore their own sexuality, focusing on that of their partner. Although the women have doubts about their partners' desire for them, they share the decision-making process with them whilst waiting for bariatric surgery, on which they place all of their hopes for improved sexuality and quality of life. The findings highlight the importance of exploring the experiences and sexual issues faced by heterosexual women with MO in a bariatric surgery program. Bariatric nurses have a privileged position to assess these women's sexuality, recommend alternatives to sexual intercourse or refer them to sexologists. As part of the multidisciplinary team, nurses can contribute to managing the expectations of women with MO and their partners in relation to the improvement of their sex lives following bariatric surgery.

Sexuality amongst heterosexual men with morbid obesity in a bariatric surgery programme: A qualitative study.

To describe and understand the experiences of sexuality amongst heterosexual men diagnosed with morbid obesity (MO) who are in a bariatric surgery programme. Morbid obesity is a chronic metabolic disease that affects men's physical, psychological and sexual health. Evidence suggests that MO could be linked to anxiety, depression, low self-esteem, sexual life and social disorders. Bariatric surgery is a reliable method for weight loss in patients with MO. This is a qualitative study based on Merleau-Ponty's phenomenology, and the COREQ checklist was employed to report on the current study. Convenience and purposive sampling was carried out and included 24 in-depth interviews with heterosexual men with MO in a bariatric surgery programme between October 2018-March 2019 in Spain. Two main themes emerged from the analysis: (a) a corporality which is judged and condemned; and (b) adapting sexual practices to bariatric surgery. Men with MO reject a body that limits their physical, social and sexual life. A negative body image and low self-esteem, which do not respond to the traditional masculine role or new masculinities, reduce men's self-concept. Men with MO feel rejection, stigma and isolation. Support from their partner is fundamental to adapt. The results of the study draw attention to how heterosexual men with MO experience their sexuality in a bariatric surgery programme and the challenges nurses face. Recognising the problem, helping to develop coping strategies or referring to specialists in sexuality could improve the quality of life in patients and their partners.

Australian bariatric nurses: What do they do and what do they need?

BackgroundMetabolic/bariatric surgery is the surgical treatment of obesity and obesity-related comorbidities.As one of the most obese nations in the world, the demand for continuing professional development (CPD) in this specialty is growing, however, it is unknown what form and content of education this group of specialist nurses need. AimThis quantitative descriptive study sought to define metabolic/bariatric nursing practice in Australia and to identify the professional development needs of metabolic/bariatric nurses in Australia. MethodsA total of 37 metabolic/bariatric nurses from across Australia responded to an online survey. Data gathered enabled researchers to describe the demographics of the nurses, daily tasks in their nursing role, current education needs, their previous education, and their current learning needs. FindingsThis study confirmed there is a gap in available education for metabolic/bariatric nurses in Australia. A third (n = 10, 32 %) of metabolic/bariatric nurses did not attend any relevant Continuing Professional Development in the last 12 months. All 34 were requesting more postgraduate education opportunities. ConclusionAustralian Metabolic/bariatric nurses are asking for more education to achieve the Continuing Professional Development required for nursing registration, so they may provide the specialised evidence-based care that metabolic/bariatric patients require. This study provides a list of favoured education topics and preferred modes of education delivery. The results from this study may be used to: assist in the development of education for metabolic/bariatric nurses at the workplace, conference and university level; and supports research on the specialised practice of metabolic/bariatric nursing.

A421 - A Single-Center Experience in Supporting Certified Bariatric Nurse Certification

Background: As a MBSAQIP Accredited Center, we are committed to the quality care we deliver to our metabolic and bariatric surgery patients. Our facility has a dedicated and knowledgeable nursing staff. However despite our dedication and commitment to ongoing education, none of our nurses are currently certified bariatric nurses (CBN). With nursing leadership support, we have witnessed an increase number of our nurses throughout the institution becoming certified in a variety of specialties. A recent change in nursing leadership on our bariatric surgery designated unit provided us the opportunity to begin the dialogue with our nursing staff on the benefits of certification. This resulted in 20 nurses expressing an interest to become certified bariatric nurses.

Creating a New Sense of We-ness: Family Functioning in Relation to Gastric Bypass Surgery

Background: Gastric bypass surgery (GBP) is the most common treatment for morbid obesity (body mass index ≥35). To date, the person with obesity has been the main focus in bariatric nursing and research, although the person coexists within their family. Aim: The aim of this study was to describe family functioning in relation to GBP. Methods: Open ended questions were used. Nine families were interviewed on two occasions within 2 weeks 3 months after surgery. The interviews were transcribed and analyzed according to a hermeneutic method. Main Findings: An ongoing process of alterations related to family functioning was identified. Before, GBP families experienced distance and disengagement. Immediately after GBP, there were changes within relationships. Families experienced a period of difficulty, in interactions, while adapting to the lifestyle changes and altered positions. After 3 months, families experienced enhanced closeness in inter-relations within the family and increased social interactions as a whole family with friends and relatives. Conclusions: To better support families and enhance family functioning, bariatric nursing care needs to shift focus from an individual to a family perspective. Further studies are needed to gain a longitudinal perspective and improve understanding of family functioning after GBP.

Bullying, Moral Courage, Patient Safety, and the Bariatric Nurse

In the 2000 publication, To Err is Human, experts reported that as many as 98,000 people die in any given year from medical errors that occur in U.S. hospitals. In 2012, William Charney argues that, to make matters worse, this has been a growing problem for more than a decade, with little to no systemic approach to address the problem. Charney explains that, in 2009, 788,558 deaths in the United States occurred as a result of medical errors and hospital acquired infections, representing one-third of all deaths in America that year.1 This article explores the role of the bariatric nurse in developing moral courage to address the threat of workplace bullying on patient safety.

Development of a Bariatric Patient Readiness Assessment Tool for the Emergency Department

Morbidly obese patients present safe patient handling challenges to emergency department (ED) personnel. Emergency departments are often ill-equipped to safely manage the patient of this size. The purpose of this study was to develop an assessment tool to determine ED readiness to safely manage the morbidly obese patient. Published guidelines by the Facility Guidelines Institute and the National Association of Bariatric Nurses were used for tool development. The tool assesses policy, equipment, space/structural requirements, and patient handling and -moving tasks. A 2-phase, nonexperimental, exploratory study in a convenience sample of 5 EDs was conducted, applying a usability testing model to determine the usability and utility of the tool. Four of 5 hospital EDs agreed that the tool was easy to use and helpful in determining their readiness to safely manage the morbidly obese patient. The tool was found to be usable on a limited basis. Additional application of the tool on a larger scale is recommended.

North Carolina First in Bariatric Nursing: Development of the First State Chapter of the National Association of Bariatric Nurses in North Carolina

Specialty nursing organizations, such as the National Association of Bariatric Nurses (NABN), provide a venue for the genesis of standardized practice, expectations of care and the dissemination of new knowledge. However, fostering involvement closer to the point-of-care is critical to ensuring successful outcomes for both patients and the nurses caring for them. State chapters of the National Association of Bariatric Nurses are a natural segue way to providing local resources and support to those caring for the unique needs of bariatric patients and their families. As with many types of care, local and regional differences can provide distinct opportunities and challenges for nurses and the patient care team. This article will describe the development of the first state chapter of the National Association of Bariatric Nurses and offer strategies and insight for further state chapter development.
