X monosomy and balanced Robertsonian translocation in a girl with Turner Syndrome

We describe a case of X monosomy associated with a maternally inherited t(13;14) Robertsonian translocation in a girl with Turner syndrome. The girl's X chromosome was demonstrated to be maternally inherited, ruling out the hypothesis that the translocation exerted an interchromosomal effect on the origin of the monosomy. Chromosomes 13 and 14 showed biparental inheritance.

Review and meta-analysis of systematic searches for uniparental disomy (UPD) other than UPD 15.

All systematic searches for uniparental disomy (UPD) so far published and comprising clinically defined populations (Silver-Russell syndrome/primordial growth retardation (SRS/PGR) (n = 14), multiple malformations (n = 2), or rare syndromes (n = 12)) or situations at risk (confined placental mosaicism (CPM) (n = 13), spontaneous abortions (n = 6), additional marker chromosomes (n = 15), balanced non-Robertsonian translocations (n = 3), or balanced Robertsonian translocations (n = 15)) were reviewed. In many studies clinical and/or cytogenetic information on fluorescent in situ hybridization (FISH) results was very scarce. Meta-analysis concerning an adequate number of cases was possible for SRS/PGR, CPM, additional marker chromosomes, and balanced Robertsonian translocations only. As expected, the highest risk for UPD was found in cases with translocations between homologous acrocentric chromosomes (11 cases with UPD of 15 investigated) and in CPM due to a meiotic error (25 of 51 cases). In prenatal investigations or in cases with a normal phenotype, translocations between nonhomologous acrocentric chromosomes implied a risk for UPD of less than 0.5%. The risks for maternal UPD 7 in cases with SRS/PGR, for UPD 15 in cases with an additional inv dup(15) marker chromosome, and for UPD of any chromosome in cases with multiple malformation/mental retardation were approximately 5.5%, and approximately 1.3%, respectively. Searches for UPD in well-defined syndromes (Brachmann-De Lange syndrome, Sotos syndrome, Rett syndrome, Weaver syndrome, or XX true hermaphroditism) were disappointing. Not a single case was found.

Uniparental disomy in fetuses diagnosed with balanced Robertsonian translocations: risk estimate.

Forty-two fetuses with non-homologous Robertsonian translocations were analyzed for uniparental disomy (UPD). One fetus with a de novo translocation t(13q;14q) had maternal isodisomy of chromosome 14. In a summary of the published data (including the present study), 315 cases were analyzed for UPD after prenatal diagnosis of balanced Robertsonian translocations, of these two fetuses had UPD, giving a risk estimate of 0.65% (CI 0.2-2.3). This risk justifies the recommendation of UPD analysis in fetuses diagnosed prenatally with Robertsonian translocations, with the emphasis on the chromosomes known to contain imprinted genes, such as 14 and 15. We also discuss the possibility of UPD in offspring of Robertsonian translocation carriers with normal karyotype. Based on the risk for UPD in fetuses with Robertsonian translocation we suggest to test these fetuses for UPD and to do so on amniocytes rather than chorionic villi when the risk for unbalanced karyotype is approximately 1%, comparable to the risk for UPD.

PGD in female carriers of balanced Robertsonian and reciprocal translocations by first polar body analysis.

Preimplantation genetic diagnosis (PGD) using the first polar body (1PB) is a modality of PGD that can be used when the woman is the carrier of a genetic disease or of a balanced chromosomal reorganization. PGD using 1PB biopsy in carriers of balanced chromosome reorganizations has not become generalized. Here, we describe our experience based on the analysis of unfertilized or fresh, non-inseminated control oocytes, by fixing separately the 1PB and the corresponding oocyte, and on the study of six clinical cases of PGD using 1PB biopsy (four Robertsonian translocations and two reciprocal translocations). In fresh oocytes, the chromosome morphology of the 1PB was well preserved, and the results were always concordant for each oocyte-1PB pair. This indicates that the 1PB can be reliably used for the diagnosis of chromosome reorganizations. In these studies the technical problems encountered when performing PGD using 1PB biopsies for chromosome studies are also addressed. Three different strategies of 1PB biopsy (laser beam, partial zona dissection and acid Tyrode's) and two different protocols (intracytoplasmic sperm injection before or after 1PB biopsy) and their effect on the percentage of oocytes diagnosed and the fertilization rate, are discussed. In reciprocal translocation cases, published in the literature or studied by us, in which at least nine oocytes had been diagnosed, a correlation has been found between the frequency of nondisjunction observed and the theoretical recombination rate. To date, PGD by 1PB analysis alone or combined with blastomere biopsies in female carriers of chromosomal rearrangements has been used in 18 cases, with a further six cases reported here. A total of 325 cumulus-oocyte complexes have been obtained, of which 294 were biopsied and 224 were diagnosed. A total of 52 embryos was transferred, 19 of which implanted and 17 produced full-term pregnancies.

Parent-of-origin studies were performed using molecular markers for both chromosomes involved in the translocation. No UPD cases were identified out of 23 analysed cases. The results presented here, combined with other available data, provide preliminary elements for genetic counselling in these common chromosomal rearrangements. Chromosome analysis of human spermatozoa from an oligoasthenozoospermic carrier for a 13;14 Robertsonian translocation by their injection into mouse oocytes.

We present a case of a 46,XY der(13;14) Robertsonian translocation carrier whose spermatozoa were karyotyped after injection into mouse oocytes. Fresh semen samples as well as recovered samples were used. There was no significant difference in the survival rate of mouse oocytes (fresh: 78.1% versus frozen: 81.7%), activation rate (fresh: 84.0% versus frozen: 90.6%), fertilization rate (fresh: 72.0% versus thawing of frozen: 76.5%) between fresh or frozen spermatozoa. Metaphase chromosome spreads from 45 spermatozoa were analysed. The frequency of spermatozoa that were chromosomally unbalanced with respect to the translocation was 8.9%, and the frequency of abnormalities unrelated to translocation was 4.4%. An excess of spermatozoa with balanced chromosomes was observed: compared with normal, 23 (51.1%) versus 16 (35.6%) respectively; but this segregation difference was not statistically significant (chi(2) = 0.9, P > 0.3). After genetic counselling with the carrier and his partner, intracytoplasmic sperm injection treatment was performed. Healthy female and male infants were delivered at 36 weeks gestation via a Caesarean section. Both babies were carriers for the balanced Robertsonian translocations detected for prenatal diagnosis at 16 weeks gestation. The present study demonstrates that patients can be given further information about the proportion of the spermatozoa which carry a chromosomal abnormality.

Incidence and significance of chromosome mosaicism involving an autosomal structural abnormality diagnosed prenatally through amniocentesis: a collaborative study.

Among 179,663 prenatal diagnosis cases collected from ten institutions and two publications, 555 (0.3 per cent) were diagnosed as having chromosome mosaicism. Of these, 57 (10.3 per cent) were mosaic for an autosomal structural abnormality, 28 (5 per cent) for a sex chromosome structural abnormality, and 85 (15.3 per cent) were mosaic for a marker chromosome. Ninety-five cases of prenatally diagnosed mosaicism with a structural abnormality in an autosome and a normal cell line, and with a known phenotypic outcome, were collected for karyotype-phenotype correlations through our collaboration (40 cases), a prior survey (26 cases), and published reports (29 cases). They included 13 balanced reciprocal translocations, one unbalanced reciprocal translocation, four balanced Robertsonian translocations, four unbalanced Robertsonian translocations, four inversions, 17 deletions, three ring chromosomes, 19 i(20q), seven +i(12p), six other isochromosomes, and 17 partial trisomies resulting from a duplication or other rearrangement. All cases mosaic for a balanced structural rearrangement resulted in a normal phenotype. All cases of 46/46,i(20q) resulted in normal liveborns. Five of seven cases with 46/47,+i(12p) had an abnormal phenotype compatible with Killian-Pallister syndrome. The overall risk for an abnormal outcome for a mosaic case with an unbalanced structural abnormality, excluding 46/46,i(20q) and 46/47,+i(12p), is 40.4 per cent. In the same category, the study also suggested a correlation between the percentage of abnormal cells and an abnormal phenotype. For mosaicism involving a terminal deletion, the possibility of a familial fragile site should be considered.

Prenatal ultrasound detection of isolated neural tube defects: is cytogenetic evaluation warranted?

To determine the value of cytogenetic evaluation in fetuses with isolated neural tube defects diagnosed by prenatal ultrasound. Fifty-five thousand two hundred sixty obstetric ultrasounds performed for various indications at the Indiana University Prenatal Diagnostic Center from July 1988 to March 1994 were reviewed using a computerized data base. Excluding all cases of anencephaly, fetuses with isolated neural tube defects were identified. Maternal demographic data, pregnancy outcomes, level of defect, and fetal karyotype when available were obtained. Statistical analysis was performed using the χ2 test, when appropriate. P ≤ .05 was considered statistically significant. Seventy-seven medical record charts of women with prenatally diagnosed isolated fetal neural tube defects were reviewed. Nineteen pregnancies were terminated, 42 pregnancies were delivered, and 16 pregnancies were lost to follow-up. Karyotypes were available in 43 of the 77 cases (55.8%). The theoretical risk of chromosomal anomalies in this sample population based solely on maternal age was 0.3%. Of the 43 documented karyotypes, seven chromosomal anomalies were discovered (16.3%). The difference was statistically significant (P = .012). Detected chromosomal anomalies included two trisomy 18, two triploid 69,XXX, one triploid 69,XXY, one balanced Robertsonian translocation t(13q14q), and one inversion in the q arm of the X chromosome. The prevalence of karyotypic abnormalities is significantly increased in fetuses with isolated neural tube defects; therefore, prompt antenatal genetic evaluation should be considered in such cases.

Homologous Robertsonian translocation (21q21q) and abortions

Instances of balanced Robertsonian translocations between the homologues of chromosome 21 were observed in two couples with a history of repeated abortions. The male partner of one couple and the female partner of another couple exhibited this anomaly. The translocation (21q21q) was found to be transmitted to their live children with Down's syndrome.

Chromosomal basis of recurrent fetal losses.

Chromosome abnormalities constitute the single most etiological factor in spontaneous abortions and other fetal losses and include sporadic chromosome abnormalities such as monosomy, trisomy, triploidy, tetraploidy and chromosomal mosaicism. These errors either originate during gametogenesis or after fertilization during the early zygotic divisions. Based upon the information now available it is apparent that if a couple has had two fetal losses, the karyotypes of the abortuses are most likely to be concordant either both being normal or both being abnormal. Fetal losses may also be due to a chromosome abnormality carried by a parent in a clinically silent form such as a balanced translocation. A compilation of cytogenetic data from 17,112 parents (8,743 females and 8,369 males) revealed 517 instances of chromosome abnormalities, an incidence of 3 per cent (6% of couples). Approximately 50 per cent of all chromosome abnormalities detected were balanced reciprocal translocations, followed by balanced Robertsonian translocations (23%) and sex chromosome mosaicism in females (14%). Parental chromosome abnormalities are known to be factors in abnormal gametogenesis and zygote formation, and, therefore, prenatal diagnosis should be considered in future pregnancies. Further, even when parental karyotypes are normal, prenatal diagnosis should be offered to couples who have had two or more fetal losses due to their increased risk of having a fetus with a chromosome abnormality.

A cytogenetic survey of 14,069 newborn infants. III. an analysis of the significance and cytologic behavior of the Robertsonian and reciprocal translocations.

A study of the chromosomes of 14,069 consecutive liveborn infants revealed 11 infants with balanced reciprocal and 13 with balanced Robertsonian translocations. A detailed physical assessment revealed no increase in major or minor malformations over control infants except for an increase in frequency of upslanting palpebral fissures and some possible minor dermatoglyphic changes. A preliminary psychologic assessment of six heterozygotes and 11 controls at 6 years of age revealed no detectable differences. The results of detailed family studies revealed only one child with an unbalanced karyotype, suggesting that where families are ascertained at random through balanced translocation heterozygotes, the frequency of children with unbalanced karyotypes is low in both the t(DqDq) and reciprocal translocation families. Fetal wastage was shown to be slightly increased in reciprocal but not in Robertsonian translocation families. In both the reciprocal and Robertsonian translocation families the segregation ratio for the translocation was not significantly different from 1:1. A significant excess of males was found among reciprocal but not among Robertsonian translocation heterozygotes. A study of the chromosomes involved in 60 reciprocal translocations ascertained at random by conventional staining in the present and two other series suggested that chromosome 11 and perhaps 13 were involved significantly more frequently than might have been expected by chance. However, with the possible exception of band ll(ql3), no “hot” spots for chromosome breakage were observed in these series. Breaks were, however, found to occur significantly more frequently in pale fluorescent, light-staining than in bright fluorescent, dark-staining bands. The importance of these findings in genetic counselling is discussed.

The formation of the abnormal chromosome in balanced homologous Robertsonian translocation carriers

A balanced carrier of a 13/13 translocation had the karyotype 45,t(13q13q) in several tissues sampled (blood, skin from three sites, and three abdominal structures). On the basis of absence of mosaicism in this and the other reported balanced homologous Robertsonian translocation carriers, it is suggested the translocation occurred immediately after fertilization.

Dicentric and monocentric Robertsonian translocations in man.

5 balanced Robertsonian translocations in man were identified by fluorescence studies. Orcein staining showed two distinct centromeres in 4 of these cases (tdic(13;13), tdic(13;14), tdic(15;21), tdic(21;22)) indicating breaks in the short arms of the involved chromosomes. The dicentric translocation chromosomes were rather stable but monocentrics were noticed in each case. Fluorescence- and measurement studies seemed to indicate that an invisible centromere and part of the short arms were present in these monocentric chromosomes. One case, t(14q21q), was monocentric in all metaphase plates but measurement studies were very suggestive of a visible 21 centromere and incorporation of the invisible 14 centromere (and short arm material) in the long arm of the translocated 14 chromosome, indicating that this translocation originally might have been a real dicentric. Heterochromatin staining was carried out in all cases. The tdic(15;21) showed 6 heterochromatin blocks; 2 of these blocks were probably satellites from chromosome No. 21, visible too in fluorescence. The 4 other translocations showed 4 separated blocks. No differences were observed between monocentrics and dicentrics supporting the theory of a preserved, but invisible centromere in monocentrics.
