Patient safety culture awareness among healthcare providers in a tertiary hospital in Riyadh, Saudi Arabia.

Patient safety is a serious concern in the health care industry. To enhance patient safety, healthcare providers are expected to minimize accidental harm to patients and enhance the quality of patient-centered care. The main objective of this study is to explore the awareness of the patient safety culture among healthcare providers. It is further intended to assess key fields and factors that hinder patient safety adoption and determine the effects of demographic factors on healthcare providers' awareness of patient safety culture. This study applied a cross-sectional quantitative design. It was conducted in a tertiary hospital in Riyadh, Saudi Arabia. The participants consisted of all healthcare providers working in a specific tertiary hospital in Saudi Arabia. A random sampling technique was applied in this study. The study sample size was 409 participants. A valid and reliable questionnaire was used to collect the required data. The T-tests, ANOVA. And regression was used. The study found that there is a moderate level of patient safety culture awareness among healthcare providers. Moreover, the findings also revealed that the age group “31–40” showed statistically different awareness levels as compared to the “more than 50 years' age group” (p = 0.012). Also, this study has found that gender and education have a significant influence on the awareness level of patient safety culture while position and work area have no influence on the awareness level of patient safety culture among healthcare providers. Managers in healthcare institutions should develop speeder response plans and make them part of the patient safety culture. Institutions offering bachelor's degrees and postgraduate in nursing should pay more attention to the subject of patient safety. The government healthcare sector, together with the private healthcare sector, should continuously train healthcare providers on patient safety procedures to improve the patient safety culture. Healthcare providers should be encouraged to report errors made during diagnosis or treatments to avoid them in the future.

Effects of Awareness of Patient Safety Culture, Emotional Labor and Job Stress on Patient Safety Nursing Activities by Comprehensive Nursing Care Medical Service Ward Nurses

Purpose This study aimed to examine the effects of the awareness of patient safety culture, emotional labor, and job stress on patient safety nursing activities by comprehensive nursing care medical service ward nurses. Methods The participants were 112 nurses from the comprehensive nursing care service wards from six hospitals. Data were collected using structured questionnaires and analyzed using a t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression analysis using SPSS version 20.0. Results The factors that significantly influenced patient safety nursing activities in the comprehensive nursing care medical service ward nurses included job stress (β=−0.18, p<.032) and awareness of patient safety culture (β=0.54, p<.001). The explanatory power of these factors was 31.0% (F=13.43, p<.001). Conclusion This study showed that the factors that influenced patient safety management activities of comprehensive nursing care medical service ward nurses included job stress and awareness of patient safety culture. Influence of Nurses' Awareness of Patient Safety Culture on Patients Satisfaction

Background: Patient safety culture is crucial in quality care, and could improve patient satisfaction. The aim of this study was examining the relationship between nurses‘ awareness of patient safety culture and patient satisfaction through Assessing nurses' awareness of patient safety culture, Assessing patient satisfaction level with the quality of the care, finding out the relation between nurses' awareness of patient safety culture and patient satisfaction. Design: across-sectional analytical. Setting: The study was carried out at Dar-El Shifa Hospital, this hospital is affiliated to ministry of health, specialized medical center Subjects: Two groups of subjects were included in this study, namely staff nurses working in inpatient departments and patients under their care in the same departments. Tools of data: Two different self-administered questionnaires were used for data collection, one for the staff nurses and the other one for the patients. Results: Nurses‘ age ranged between 18 and 54 years. The most of them were diploma degree. In total, more than third had high awareness of patient safety culture. The awareness score had a significant positive correlation with current experience years (r=0.232). In multivariate analysis, working shifts was a negative predictor of this score. Overall, slightly less than third, of the patients were satisfied. Satisfaction decreased with higher education, and among rural residents. Patients‘ age was a positive predictor of the satisfaction score. In ecologic analysis, negative correlations were revealed between patient satisfaction with support staff and nurses‘ scores of awareness of hospital-level and outcome level patient safety culture. Conclusion: Nurses in the study setting lack awareness of patient safety culture. Their awareness is only influenced by working in shifts. Patients‘ satisfaction is also low, especially with support staff, and seems to be not correlated to nurses‘ awareness of patient safety culture Recommendations: Nurses in the study setting lack awareness of patient safety culture, and their patients have low satisfaction. The study recommends more efforts to foster the concept of patient safety culture

Status quo of Intensive Care Unit nurses' awareness of patient safety culture and its influencing factors

Objective To understand the status quo of Intensive Care Unit (ICU) nurses' awareness of patient safety culture and its influencing factors, and to provide reference for nursing managers to formulate effective nursing measures to improve ICU nursing service quality. Methods Using convenience sampling method, 318 nurses in ICU Departments of 5 general hospitals in He'nan province from March to May 2018 were selected as subjects. General Information Questionnaire, Hospital Survey of Patient Safety Culture (HSOPSC) , Professional Self-Concept of Nurses Instrument (PSCNI) and Conditions of Work Effectiveness Questionnaire-Ⅱ (CWEQ-Ⅱ) were used to investigate them. The influencing factors of patient safety culture awareness in the ICU nurses were analyzed. Results The ICU nurses' patient safety culture score was (154.34±29.02) and the average score of the items was (3.67±0.69) . Regression analysis showed that the ICU nurses' title, professional attitude, job assignment and the number of night shifts per month were the influencing factors of patient safety culture (P<0.05) . Conclusions ICU nurses need to improve the awareness of patient safety culture. It is suggested that nursing managers should intervene in the aspects of ICU nurses' title, professional attitude, work assignment and number of night shifts per month to improve ICU nurses' awareness of patient safety culture. The psychometric properties of "Hospital Survey on Patient Safety Culture" in a Czech environment

Aim: To analyze the psychometric properties of the Hospital Survey on Patient Safety Culture (HSOPS) in internal care units at a specific teaching hospital in the Czech Republic. Design: The paper has the character of a cross-sectional validation study. Methods: The study was conducted between November 2015 and January 2016. The convenience sample consisted of 207 registered nurses working in internal care units at a specific faculty hospital in the Czech Republic. Analysis of the psychometric properties of the HSOPS was conducted using Principal Component Analysis (PCA) and reliability testing. Results: Acceptable psychometric properties of the HSOPS were verified in our study. We confirmed 12 components of the HSOPS, the same number stated in the original version from the Agency for Healthcare Research and Quality (AHRQ). The reliability of the instrument was evaluated using the Cronbach alpha coefficient, and is considered acceptable. Conclusion: These findings can help healthcare institutions to raise awareness of patient safety culture and to gain a comprehensive overview of the individual dimensions of patient safety culture. In the future, the instrument could help to compare the components of patient safety culture, not only at a national but also at an international level.

Structural Analysis of Variables related to Fall Prevention Behavior of Registered Nurses in Small-to-Medium Sized Hospitals

Purpose The purpose of this study was to build a model to predict the fall prevention behavior of nurses in small and medium sized hospitals. Methods Participants were 382 nurses from 13 hospitals who responded to the structured self-reported questionnaire. The research model was based on previous study of fall prevention, theory of planned behavior, and the health belief model. Results The modified model generally showed higher levels than recommended level of model fit indices and acceptable explanation. Of 17 hypothetical paths, 14 were supported. Predicting variables explained 51.6% of fall prevention behavior. The fall prevention behavior of nurses showed a direct influence of fall prevention expectations, fall prevent threats, perceived behavioral control for fall prevention, and intention to prevent falls and an indirect of influence of patient safety culture, attitude toward fall prevention, and the subjective norm. Conclusion Findings show a need to identify a range of barrier factors to increase the benefits of fall prevention behavior and enhance the perceived control of fall prevention so that nurses will be able to promote fall prevention behavior in hospitals. Self-Learning Package Related to Patient Safety Goals and its effect on Nurse Interns' Performance and Awareness of Patient Safety Culture

Background: The International Patient Safety Goals (IPSGs) are used to improve the quality of care. Nurse interns’ proper understanding of patient safety is essential ingraining this concept. Aim: The aim of this study was to assess the effect of a self-learning package related to patient safety goals on nurse interns' performance and awareness of patient safety culture. Subjects and Methods: The study was conducted at Ain-Shams University Hospitals using a quasi-experimental design with pre-post assessment on 110 nurse interns. The data collection tools were Patient safety knowledge questionnaire, Patient safety culture awareness questionnaire, and an observation checklist for nurse interns' safety performance. A Self-Learning Package was prepared and distributed to nurse interns. Its impact was evaluated after one and three months. Results: The nurse interns’ median age was 23.0 years, with a majority of females (67.3%). Before the intervention, 40.0% had satisfactory knowledge, which rose to 100.0% at the postintervention phase, and declined to 94.5% at the follow-up phase (p<0.001). Their awareness increased from 40.9% to 88.2% in the post-intervention phase, and slightly declined to 80.0% at follow-up (p<0.001). Overall, 50.0% had total adequate performance of IPSGs, which increased to 96.4% in the post-intervention phase, and slightly declined to 93.6% at the follow-up phase (p<0.001). Significant positive correlations were revealed among knowledge, awareness, and performance scores. The study intervention was a statistically significant independent positive predictor of all three scores. Conclusion and recommendations: The application of the developed self-learning package is effective in improving nurse interns’ knowledge of patient safety, awareness of patient safety culture, and performance of patient safety goals. The study recommends its use.

Radiation therapist (RT) perceptions and practices related to safety culture (SC) and error reporting in radiation oncology.

42 Background: The delivery of radiotherapy is complex and demands careful management, high vigilance, and precise coordination of clinical personnel. RTs are at the frontline and are often first to discover or report an error. However, few studies have examined RT patient safety practices. We conducted a national survey to explore the attitudes, training, and experiences of RTs related to safety in radiation oncology. Methods: In 2016, an electronic survey was sent in June and July to a random sample of 1,500 RTs (~10% of licensed RTs in the US). The survey assessed department SC, error reporting, knowledge, and practices. Questions were multiple choice or recorded on a 5-point Likert scale. Results were summarized using descriptive statistics and analyzed using multivariate logistic regression. Results: A total of 702 RTs from 48 states (47% response rate) completed the survey. Respondents represented a broad distribution across practice and geographic settings with varying levels of work experience (see table). Most RTs gave their department a “Patient Safety Grade” of Excellent (61%) or Very Good (32%), especially if they had an Incident Learning System (ILS) (OR: 2.0). Most departments (58%) had an ILS; however 59% of RTs had not reported an event in the previous year. When an error occurred but did not result in patient harm, 39% of RTs said they would report it and 37% said they would not. Bullying was reported among 17% of respondents; 40% reported that burnout, stress, and anxiety negatively impacted their ability to effectively treat patients. RTs reported errors were more likely to occur with longer work days (&gt;8hrs) and poor multidisciplinary communication during handoffs and transitions. Most (62%) reported they could benefit from additional patient safety education and training, indicating significant interest in this topic. Conclusions: The majority of RTs reported excellent SC practices within their treatment facilities. Most facilities had systems in place to report errors. The obstacles identified, the results of this study may inform future educational efforts and safety initiatives directed towards RTs and should help increase awareness of patient SC in radiation oncology.

치과병원 종사자의 환자안전문화인식과 환자안전관리활동의 관련성

Objectives: This research was conducted to recognize dental health care employees' awareness of patient safety culture and to examine the association between awareness of patient safety culture and patient safety activities. Methods: The subject of this research includes the dental health care employees from 11 different dental hospitals in the nation, dental offices in the 6 different general hospitals in Seoul, and 4 different private dental offices in 2014. A self-reported questionnaire was completed by 485 dental health care employees from March 17 to April 3, 2014. Multiple regression models were used in the analysis. Results: First, the dental health employees' awareness of patient safety culture was 3.25 on average. The extent of patient safety activities was 2.75. The behavior of the supervisor/manager and the adequate explanation showed positive correlation with r=0.213 (p<0.001). The process of communication and the preventative inspection of the medical equipment showed negative correlation with r=-0.258 (p<0.001), and especially the awareness of patient safety culture and the infection control exercise showed the most significant correlation (r=0.293, p<0.001). Second, hospital environment factor of patient safety perceptions positively were related to most of patient safety management activities after controlling several covariates. Conclusions: The result of this research showed that most of the patient safety culture perceptions positively were correlated with patient safety activities. We also found that proper hospital environment, simple communication process, and positive perceptions for patient safety level were more likely to perform patient safety management activities after controlling several covariates. Improving the patient safety perceptions of dental hospital workers is an important consideration.

Objectives: This study was conducted as descriptive correlation research in order to survey healthcare personnel(HCP)`s awareness of patient safety culture and their recognition of standard precautions, and to examine the correlation between the two factors. Methods: The subjects were 400 HCPs including 80 doctors, 240 nurses, and 80 medical technicians from two general hospitals. The questionnaire used in the survey consisted of 9 questions on general characteristics, 44 on the perception of patient safety culture, and 21 on the recognition of standard precautions. Results: According to the subjects` general characteristics, the score was significantly higher in those aged over 40 than in those aged 30-39. In addition, it was significantly higher in managers than in practitioners, in those with work experience of less than a year than in those with 5-9 years. The score was also significantly higher in those working 8 hours a day than in those working over 10 hour a day. The number of medical accident reports according to the subjects` general characteristics was significantly larger in nurses than in doctors, in managers than in practitioners, and in those with 10 years` or longer experiences than in those with less than a year. In the awareness of standard precautions according to the subjects` general characteristics, the score was significantly higher in female workers than in male workers, and in managers than in practitioners. As a whole, the subjects` awareness of patient safety culture and their recognition of standard precautions showed a significant correlation with each other. Conclusion: From the result of this study, sufficient work force and the promotion of organizational culture for safety is needed in order to guarantee patient safety. Likewise, these results suggest that experience, job skill, and adequate working hours have a positive effect on the awareness of patient safety culture and the recognition of standard precautions.

A survey of nurses' awareness of patient safety culture in neonatal intensive care units.

Background:Patient safety is considered as the most important quality for healthcare. One of the main factors that play an important role in the promotion of healthcare institutes is patient safety. This study describes the nurses’ awareness of patient safety culture in neonatal intensive care units (NICUs).Materials and Methods:In this descriptive study, 83 nurses working in neonatal intensive care units of hospitals affiliated to Isfahan University of Medical Sciences, Iran, were selected using purposive sampling. Data collection tools consisted of the demographic characteristics questionnaire and the Hospital Survey on Patient Safety Culture. Data were analyzed by using SPSS software.Results:The dimension that received the highest positive response rate was “expectations and actions of the supervisor/manager in promoting safety culture.” The dimension with the lowest percentage of positive responses was “frequency of error reporting.” 21.70% of the NICU nurses reported one or two incidents in their work units in the previous 12 months.Conclusions:In order to create and promote patient safety, appropriate management of resources and a correct understanding of patient safety culture are required. In this way, awareness of dimensions which are not acceptable provides the basic information necessary for improving patient safety.

The Influences of the Awareness of Patient Safety Culture on Safety Care Activities among Nurse in small-medium Sized General Hospitals

The study was conducted to identify the perception of patient-safety and the level of safety care activity among nurses in small-medium sized general hospitals. Data were collected at three hospitals among 344 nurses on April 2012 and administered questionnaire regarding the perception of patient-safety and safety-care-activity. Data analyzed by using descriptive statistics, inferential statistics was used to determine comparison, correlation and association (Pearson correlation, t-test, ANOVA, Scheffe test, multiple regression). The results revealed that there were significant differences in the level of perception of patient-safety according to the nurses' career, time of work, and work department in their hospitals. And also there were significant differences in the level of safety-care-activity according to the nurses' age, position, marriage, the nurses' career, time of work in their hospitals. Nurses with higher perceived level of patient-safety performed more safety-care-activities. The findings of the study suggests that in order to improve the nurse's perceived level of patient-safety and safety-careactivities, the hospitals need to establish the policies that support patient-safety, improvement the environmental system, proper working atmosphere to ensure appropriate work time, regulation nurse-patient ratio are also required.

일개 상급종합병원 의사들의 환자안전문화에 대한 인식 분석

본 연구에서는 의료기관의 환자안전문화를 이해하고, 의사들의 환자안전문화에 대한 인식분석을 위한 기초자료로 활용하고자 시도하였다. G시에 소재한 상급종합병원 의사를 대상으로 2011년 8월 1일 부터 9월 5일까지 설문조사하여 194부를 최종분석 하였다. 연구결과는 첫째, 대상자와 병동 및 병원안전문화 간의 관계는 직원배치가 성별, 연령별, 병원 근무년수, 환자 접촉여부, 1주 근무시간에 따라 인식의 차이가 있으며, 조직학습과 병동내 팀워크는 1주 근무시간에 따라, 병동 안전문화 모든 하부영역은 진료과별로 유의하게 나타났다. 둘째, 대상자와 의료사고보고체계, 환자 안전도 평가 및 전반적 의식수준과의 관계에서는 사고에 대한 피드백과 의사소통, 사고빈도보고, 전반적 안전의식이 진료과별로 유의하였으며, 전반적 안전의식은 환자접촉과 1주 근무시간에 따라 유의한 결과를 나타냈다. 셋째, 병동 및 병원 안전문화 인식 하부영역과 전반적 인식, 환자안전도 평가는 모두 정의 상관관계가 있는 것으로 나타났다. 의료사고 보고체계와는 직속상관/관리자의 태도와 병원경영진의 태도를 제외한 모든 영역에서 정의 상관관계가 있는 것으로 나타났다. 넷째, 환자안전도 영향을 미치는 환자안전문화 하부영역은 조직학습, 의사소통의 개방성, 전반적 안전인식, 부서간의 협조체계, 피드백과 의사소통, 비처벌적 대응에서 유의한 결과를 나타냈다. 결론적으로, 의사들의 병동 및 병원 환자안전문화 수준을 높이고 의료사고보고체계를 충실하게 하기 위해서는 적절한 직원배치와 근무시간을 바탕으로 병동 내 조직적 학습을 통한 팀워크을 활성화 시키고, 부서간 팀원간의 개방적 의사소통과 사고에 대한 피드백을 제공하여 환자 안전에 대한 병원경영진의 적극적인 지원과 진료과별 협조체계 구축이 필요하다. This study was designed to figure out patient safety culture of medical institutions and try to utilize the study results as basic data for analyzing doctor's awareness of patient safety culture. To this end, questionnaire survey was conducted from August 1st to September 5th, 2011, targeting doctors working at senior general hospitals located in G city, and 194 questionnaires were utilized for final analysis. The research results are as follows. First, there was a difference in awareness of deployment of staffs depending on gender, age, term of service in the hospital, contact with patients and working hours per week in relationship between subjects, wards and hospital safety culture, and organizational learning and teamwork in the ward turned out to be significant in accordance with working hours per week, and all sub-areas of the ward safety culture by departments. Second, feedback about the malpractice, communication, report on malpractice frequency and overall safety awareness were found to be significant by departments in relationship of subjects, medical incident reporting system, patient safety evaluation and overall level of consciousness, and the overall safety awareness showed significant results according to contact with patients and working hours per week. Third, there was a positive corelation in sub-areas of the ward and hospital safety culture awareness, overall recognition and patient safety evaluation, and a positive corelation with medical incident reporting system was found in all areas except for attitude of managers/immediate supervisors and that of hospital executives. Fourth, sub-areas of patient safety culture which has a effect on patient safety showed significant results in organizational learning, openness of communication, overall safety awareness, systematic cooperation between departments, feedback/communication and non-punitive response. In conclusion, to increase the level of the ward and hospital patient safety culture of doctors and implement medical incident reporting system faithfully, it is necessary to activate teamwork through organizational learning in the ward based on the adequate staffing and working hours, promote open communication between departments and provide feedback on medical malpractice, thereby establishing a cooperative system by departments and active support of hospital executives for patient safet.

Patient safety culture: an Italian experience

To understand what level of awareness health professionals working in a hospital of Northern Italy had of the patient safety culture by using the Italian version of the Hospital Survey on Patient Safety Culture questionnaire and also validate its use in this setting. The Hospital Survey on Patient Safety Culture questionnaire was developed by the Agency for Health Research and Quality in 2008 and validated by several studies. Therefore, we decided to test the Italian version of this tool to see if it could be used effectively to measure and appraise patient safety culture also in Italy. Survey. To check the validity of the questionnaire, we applied the continued comparison and the global data treatment methods. Of the 1008 questionnaires we administered, 724 (71%) were returned. The majority of the questions were answered (mean 98, SD 3·4%). From a Chi-square test on the consistency of the answers, we found that six of the 12 perspectives we explored had a high level of significance, whereas in the other six, the differences in the answers were just casual. The Italian version of the Hospital Survey on Patient Safety Culture questionnaire allowed us to assess health professionals' awareness of patient safety culture and thus validate the effectiveness of this tool. The findings of our survey could be useful to design an objective tool that would allow the five health professional groups involved in the study (directors/coordinators, physicians, nurses/midwives, physiotherapists and technicians) gain a better knowledge of the patient safety culture and thus improve the quality of clinical practice.
