Pandemic preparedness and health system resilience in 14 European countries.

To assess national pandemic preparedness and response plans from a health system perspective to determine the extent to which implementation strategies that support health system performance have been included. We systematically mapped pandemic preparedness and response implementation strategies that improve resilience to pandemics onto the Health System Performance Assessment Framework for Universal Health Coverage. Using this framework, we conducted a document analysis of 14 publicly available national influenza pandemic preparedness plans, submitted to the European Centre for Disease Prevention and Control, to assess how well health system functions are accounted for in each plan. Implementation strategies found in national influenza pandemic preparedness plans do not systematically consider all health system functions. Instead, they mostly focus on specific aspects of governance. In contrast, little to no mention is made of implementation strategies that aim to strengthen health financing. There was also a lack of implementation strategies to strengthen the health workforce, ensure availability of medical equipment and infrastructure, govern the generation of resources and ensure delivery of public health services. While national influenza pandemic preparedness plans often include provisions to support health system governance, implementation strategies that support other health system functions, namely, resource generation, service delivery, and in particular, financing, are given less attention. These oversights in key planning documents may undermine health system resilience when public health emergencies occur.

Investigating the Challenges, Successes, and Strategies of Implementing Advanced Neonatal Care Interventions in Low-Resource Settings: A Clinical Study.

Neonatal care interventions are crucial for reducing infant mortality rates, particularly in low-resource settings where access to advanced medical facilities is limited. Implementing these interventions poses significant challenges due to resource constraints and infrastructural limitations. This clinical study investigated the challenges, successes, and strategies involved in implementing advanced neonatal care interventions in a low-resource setting. A retrospective analysis was conducted on the data collected from a neonatal care unit in a resource-limited area over a period of two years. The study assessed the availability of medical equipment, healthcare personnel training, and the efficacy of interventions in improving neonatal health outcomes. The analysis revealed that despite resource constraints, significant strides were made in implementing advanced neonatal care interventions. Availability of essential medical equipment increased by 30%, and healthcare personnel received targeted training programs resulting in a 25% improvement in neonatal survival rates. Strategies such as task-shifting and community outreach programs played a pivotal role in overcoming infrastructural limitations. Implementing advanced neonatal care interventions in low-resource settings is challenging but feasible with targeted strategies. While resource constraints remain a barrier, innovative approaches such as task-shifting and community involvement can significantly improve neonatal health outcomes. Continued investment in infrastructure, training, and community engagement is essential for sustainable progress in reducing neonatal mortality rates in resource-limited areas.

Higher level of achievement of the community in the local area, this is also a service in the area which is a world-class public and individual service, at the same time prioritized in promotion and prevention efforts. Decree of the Minister of Health Number 129 of 2008 concerning Minimum Service Standards in Emergency Installations, Response Time, also known as "emergency service response time", is the time needed by patients from the time they arrive at the Emergency Installation until they are given first aid in minutes. The maximum response time is five minutes. The results of the study showed that the number of qualified nurses, the availability of medical equipment, and emergency room employees are the main factors affecting response time. An important factor in evaluating the quality of health services is patient satisfaction, which is measured by how patients act after receiving health services. Patient satisfaction also depends on how well the service offers patient expectations. Patient service has a significant impact on patient satisfaction and is an important component in the Puskesmas system that must always be evaluated. The experience and ability of a doctor in handling emergency patients quickly and accurately correlates with patient satisfaction. Patients who receive good health services will indirectly promote and recommend the health facility to their families.

Stress level and associated factors among nurses working in the critical care unit and emergency rooms at comprehensive specialized hospitals in Southern Ethiopia, 2023: explanatory sequential mixed-method study

BackgroundStress is a pervasive occurrence within certain professions, including nurses working in emergency and intensive care unit environments. Nurses in these settings often confront various stress-inducing factors, such as unsupportive management and distressing events like patient mortality, and experience notably higher levels of stress. Nevertheless, information is scarce regarding the precise level of stress in Ethiopia, particularly within southern hospitals.ObjectiveTo assess stress levels and associated factors among nurses working in the critical care unit and emergency rooms at comprehensive specialized hospitals in southern Ethiopia, 2023.MethodsA facility-based cross-sectional explanatory sequential mixed-method study was undertaken, involving a total of 239 nurses. For the quantitative component, all nurses working in intensive care units and emergency rooms were included as participants, while a purposive sampling technique was employed to select participants for the qualitative aspect. Data for the quantitative study were gathered through the utilization of self-administered questionnaires, while interviews were conducted using a structured interview guide for the qualitative portion. Quantitative data entry and analysis were performed using EpiDataV4.6 and the Statistical Package for the Social Sciences software, respectively. Thematic analysis of the qualitative data was conducted using the OpenCode software.ResultsThe level of stress among nurses in the emergency and intensive care units was low (19.3%), moderate (55.9%), and high (24.8%). Workload (Adjusted odds ratio (AOR) = 3.51, 95% confidence interval (CI) (1.17–10.56) and time constraints (AOR = 2.5, 95% CI (1.03–6.07) were significantly associated with moderate stress level, while duty demands (AOR = 3.03, 95% CI (1.17–7.14), availability of medical equipment and supplies (AOR = 1.42, 95% CI (1.18–4.97), and witnessing death and dying (AOR = 2.34, 95% CI (1.13–5.88) were significantly associated with high-stress level. The qualitative data analysis revealed that the participants underscored the significant impact of organizational factors, individual factors, and profession-related factors on the stress levels experienced by nurses in emergency and critical care settings.Conclusion and recommendationBased on the findings, the participants in this study experienced some level of stress, to varying degrees. Therefore, it is crucial to implement effective strategies such as optimizing staffing and workflow, improving communication and collaboration, providing adequate support and resources, leveraging technology and innovation, emphasizing patient-centered care, and implementing data-driven quality improvement to alleviate the burden.

AVAILABILITY OF MEDICAL EQUIPMENT IN PUBLIC OBSTETRIC AID ORGANIZATIONS IN THE REPUBLIC OF KAZAKHSTAN

One of the main fields of state policy in the field of healthcare is the development of infrastructure. The infrastructure in the state’s healthcare system is associated with huge investments in the development of new treatment methods and the construction of new medical facilities, as well as the provision of up-todate medical devices. The actuality of the chosen topic lies in the fact that practice shows the development of domestic healthcare, as well as the provision of medical care to the population, being determined by the availability level of medical equipment in medical organizations. The purpose of the review: To study the availability of medical devices in public obstetric aid organizations in the Republic of Kazakhstan. Methodology: The paper used analysis of the data from public obstetric aid organizations in the Republic of Kazakhstan concerning the availability of medical equipment for 2022 for 9 types of medical devices. Results: This review provides analytical information on the availability of medical equipment in public obstetric aid organizations in the Republic of Kazakhstan. Conclusions: As a result of the analysis, the insufficiency of medical equipment in public obstetric aid organizations in the Republic of Kazakhstan is noted. Therefore, in the course of implementing new projects for the construction of healthcare facilities, equipment issues are solved by increasing the current percentage of medical devices and availability of new medical devices through the introduction of innovative technologies, which in turn positively affects the quality of medical care to the population of the Republic of Kazakhstan.

Triage practices for emergency care delivery: a qualitative study among febrile patients and healthcare workers in a tertiary care hospital in Nepal

BackgroundImproving screening and triage practices is essential for early severity assessments at the first point of contact and ensuring timely attention by healthcare workers (HCWs). The main objective of this study was to explore the triage process among febrile patients and HCWs in the emergency department (ED) of a tertiary care hospital in a resource-constrained setting.MethodsThis qualitative study was conducted from March to May 2023 at the ED of Tribhuvan University Teaching Hospital (TUTH), Nepal. The study included in-depth interviews with febrile patients (n = 15) and HCWs (n = 15). Additionally, direct observation notes (n = 20) were collected to document the triage process and patients’ experiences in the ED. Data underwent thematic analysis using the Interpretative Phenomenological Analysis (IPA) approach.ResultsThe ED of TUTH offered comprehensive triage services with clear delineation for the severity of febrile patients in line with the World Health Organization (WHO) guidelines. Nonetheless, challenges and constraints were identified. In the ED, evenings were generally the busiest period, and the triage process was not thorough during night shifts. Perception of triage was limited among patients and variable among HCWs. Digitalizing recordings of patient information including payment was deemed necessary for effective management of patients’ waiting times at the triage station. High patient throughput added pressure on HCWs and had a potential influence on the delivery of services. Availability of medical equipment and space were also identified as challenges, with patients sometimes compelled to share beds. There were constraints related to waste disposal, hygiene, cleanliness, and the availability and maintenance of washrooms. Febrile patients experienced delays in receiving timely consultations and laboratory investigation reports, which affected their rapid diagnosis and discharge; nonetheless, patients were satisfied with the overall healthcare services received in the ED.ConclusionsImproving current triage management requires resource organization, including optimizing the waiting time of patients through a digitalized system. Urgent priorities involve upgrading visitor facilities, patient consultations, laboratory investigations, hygiene, and sanitation. HCWs’ recommendations to resource the ED with more equipment, space, and beds and a dedicated triage officer to ensure 24-hour service, together with training and incentives, warrant further attention.

Effect of Supplemental Oxygen on Apgar Score of Newborns in Pregnant Women Undergone Cesarean Section with Spinal Anesthesia

Background: In obstetrics, there have been discussions and changes in procedures surrounding the provision of extra oxygen during cesarean sections performed under spinal anesthesia. Despite its recognized benefits in maintaining maternal cerebral oxygenation, concerns persist about its impact on neonatal health, especially regarding newborn oxygen saturation levels. This gap in understanding the direct effects on immediate neonatal outcomes, particularly Apgar scores, underscores the need for further research. This study aimed to determine the effectiveness of supplement on Apgar scores at one and five minutes post-delivery in newborns of mothers undergoing cesarean sections with spinal anesthesia. Methodology: Through randomized sampling, 100 patients from two hospitals in Iraq were included in this interventional trial. Following approval for research and establishment of inclusion and exclusion criteria, the researcher gathered patient sociodemographic information and average score. The sample is divided into two groups: (50) participants as a study group and (50) participants considered as a control group. With a significance threshold 0.05, the gathered data were examined using descriptive and inferential statistics in Statistical Package for the Social Sciences software version 22. Results: The results demonstrated that the most represented age group in both study and control groups was 25-30 years, and the least represented was 35-40 years. The highest BMI category was 'overweight' in both groups, while 'obese' was the least common. Regarding birth weights, the study group had a higher percentage of normal birth weights than the control group. In APGAR scores after 5 minutes, a significantly higher percentage of newborns in the study group had reassuring scores (7 to 10) compared to the control group. Furthermore, a relevant difference in outcomes was shown by a significant link between the study group and the impact of O2 supplement on APGAR score at (p-value. 0). Sociodemographic factors such as age, education level, occupation, BMI, and gestational age showed no significant correlation with APGAR scores, underscoring the complexity of these factors' influence on neonatal outcomes. Conclusion: The study found that supplemental oxygen positively affects newborns' APGAR scores during Cesarean sections with spinal anesthesia, with the study group showing better scores than the control group. It suggests that improving prenatal care, training in neonatal care, ensuring medical equipment availability, and enhancing obstetric research and education are vital for better neonatal and maternal care in Iraq, ultimately leading to improved health outcomes for mothers and newborns.

Perceptions of youth-friendly sexual and reproductive health services in selected Higher and Tertiary Education Institutions of Zambia: A qualitative study on the perspectives of young people and healthcare providers.

The recognition of the need for Adolescent and Youth-Friendly Health Services (AYFHS) is growing. It is important to ensure the provision of high-quality sexual and reproductive health (SRH) services that cater to the unique needs of adolescents and young people (AYP). Adolescents and young people spend a significant amount of time in Higher and Tertiary Education Institutions (HTEIs) where adolescent friendly services are needed. However, there is limited evidence on the availability of sexual and reproductive health services for young people in HTEIs in Zambia. Using the Human Rights Based Approach to healthcare availability, accessibility, acceptability, and quality of care (AAAQ) framework, this study explores young people's perceptions of youth-friendly sexual and reproductive health services in selected HTEIs in Zambia. Between March and June 2021, a qualitative case study was conducted in 12 selected HTEIs located in Lusaka, Central, and Copperbelt provinces of Zambia. The study employed In-depth Interviews (IDIs) and Focus Group Discussions (FGDs) with AYPs, as well as Key-informant Interviews (KIIs) with healthcare providers. The healthcare providers at health facility, district and provincial levels were interviewed to provide insights about the services provided in the HTEIs. A total of 112 interviews were conducted. Data analysis was performed using thematic analysis in NVivo version 11. In the study, young people reported the availability of primary health services like malaria, HIV, and pregnancy testing, as well as screening for STIs. However, their awareness of SRH services was limited. Contraception, STI testing and treatment, and HIV and pregnancy screening and testing were the most accessed services. Equipment and commodity shortages were common, hindering care provision. Young people found healthcare services in educational institutions inaccessible, with limited comprehensive care and understanding from providers. Services lacked tailoring for key populations and privacy/confidentiality. Health care providers also reported inadequate youth-friendly spaces, equipment, medication and trained workers which compromised the quality of care. Peer educators and provider training were seen as essential for improving accessibility and acceptability of services. The findings indicate significant barriers to the accessibility, availability, and acceptability of SRH services for AYP in HTEIs in Zambia. There is a pressing need to enhance AYSRH programming by increasing awareness of services and ensuring their availability and accessibility for young people. Sufficient funding for AYFHS can address challenges related to service quality, including inconsistent availability of medical equipment and supplies. Building the capacity of service providers can potentially enhance the uptake of services by AYP. It is crucial to target services to address the specific vulnerabilities of students with disabilities and key populations, aligning with the goal of achieving universal health coverage and leaving no one behind.

Implementation of a decentralised maintenance model with a measurable impact on the functionality and availability of medical equipment in healthcare facilities in Burundi

PurposeThis study measures the impact of implementation of the Improved Decentralized Maintenance Model on the functionality and availability of medical equipment in Burundi’s resource constrained health care system with hypothesis that the model has a measurable impact.MethodsThe initial model was tested in 2 provinces before being improved and extended to the provinces of Bujumbura and Rumonge. It is composed by 4 interrelated main axes (Maintenance management, Human, Financial and Material resources,). This study analyses the effects of this implementation between October 2020 and February 2022.ResultsThe rate of medical equipment functional in service has increased substantially, respectively +8% in the province of Bujumbura and 2% in the province of Rumonge, and the rate of medical equipment requiring maintenance has fallen by 9.7% (Bujumbura) and 2% (Rumonge). The provinces implementing the model have convincing results in terms of functionality with scores of at least 80%, while the national average is 64%.ConclusionsThere is a notable positive change between 2020 and 2022 in the functionality of medical equipment and a decrease in equipment awaiting maintenance, and therefore in its availability. This study proves that a sustainable implementation of a decentralized maintenance model is feasible and highly useful in low-resource settings due to its affordability. The context of a low-resource country is a challenge in itself, but the model developed shows that by tackling the low-resource levels of preventive maintenance, we can have some impact on the functionality of the equipment and therefore on the quality of care.

Therefore, implementing strategic management is very important in dealing with this pandemic. Strategic management is the process of planning, organizing, controlling, and making decisions in achieving long-term organizational goals. In the context of the Covid-19 pandemic, strategic management must be directed at efforts to minimize the negative impact of the pandemic, while maximizing the potential benefits that can be gained from this situation. The implementation of strategic management in dealing with the Covid-19 pandemic can be done in several ways, among others, the first is Strengthening the health system: The first step in dealing with the Covid-19 pandemic is to strengthen the existing health system. This includes increasing hospital capacity, increasing the availability of medical equipment, and increasing the number of available medical personnel.

Disaster Medical Responder’s Course for Training of Field Medical Teams in Singapore

Introduction:Casualties need to be triaged, stabilized and treated before they can be evacuated to the hospital. However, when Field Medical Teams (FMTs) arrive at the First Aid Post (FAP), the staff has to perform outside of their usual settings. There are also differences in the conception of medical operations, organization of the FAP, availability of medical equipment and supply, as well as means of communication, command, and control which can affect their performance and eventually the optimal survival of casualties during a mass casualty incident.Method:Guided by Kern’s model for curriculum development, Disaster Medical Responder's Course (DMRC) was developed. The curriculum focused on disaster response operations and processes; roles and responsibilities; command, control and communication; as well as supplies and resources. The content was taught through interactive lectures and skill stations. Course evaluation was based on the Kirkpatrick Model. A feedback form evaluated the reaction of the participants as to whether the course was relevant, if they learnt new knowledge and skills, and if they could apply these to their roles as FMTs. A tabletop exercise evaluated learning with participants working collaboratively.Results:DMRC has been sustainable since 2013 with six to eight courses per year. There had been numerous revisions of the content and delivery to keep up-to-date with the latest concept of operations, best practices from the literature, as well as educational methodologies. The last update was in 2020 in response to the COVID-19 pandemic where course schedule and mode of delivery were adjusted to comply with the safe management measures.Conclusion:FMTs will require training so they can function to their maximum capacity and capability. In Singapore, DMRC is the course for this unique and important training of FMTs. DMRC plays a pivotal role in ensuring the preparedness and operational readiness of FMTs for mass casualty incidents.

Assessment of Availability and Utilization of Medical Equipment’s in Nekemte Specialized Hospital, August, 2021

Objectives: The aim of this study was to assess the availability and utilization of medical equipment in Nekemte Specialized Hospital (NSH). Methods and Materials: A retrospective cross-sectional study design was employed. Adapted and pre-tested structured English version checklist for availability and utilization of medical equipment, document review and an in-depth interview were used for data collection. Data were collected by observation of availability of the medical equipment and interviewing selected professionals for utilization of capital item medical equipment. Microsoft excel 2013 was used for data analysis. Descriptive analysis was made to determine the availability and functional status of medical devices and qualitative part were thematically analysed. Results: This finding showed that about 60.4% (209) of medical equipment are available from the total required quantity of 348 which account only 45 (37.2%) of them were items. Among the available 209 medical equipment’s about 84.67% of them are functional and 15.33% of them were non-functional during the assessment. Besides, the Donated equipment accounts 45%. From eight-capital item three of them were properly utilized with utilization coefficient of 81.4%, 100% and 100.0% while the rest five were underutilized with utilization coefficient of 2.5%, 5.77%, 9.2% and 0%. Conclusion and Recommendation: The hospital lacks valuable, advanced and specialized medical equipment that are needed for diagnosis and treatment. Medical equipment fails due to lack of training for user, infrastructure problem like power surge and hard water, and misuse. Training that improves skill of the user and technicians should be given.

The electricity sector around the world is considered one of the most important sectors, which has a direct impact on the economic development of a country. Based on the increase in energy consumption, the crisis with the emission of high levels of harmful gases - the greenhouse effect and the expansion at the economic level of countries, electricity, namely the increase of energy capacity will remain key issues for future discussion at the global level. Electricity is the engine of the rapid economic development of a country; necessarily the well-being of citizens to access to energy sources and energy stability is a key requirement of every consumer, producer or economic operator. How much capacity do countries in the world have to respond to the increase in energy consumption in the future and what sources will be oriented so that the consequences are minimal for the environment, is a question that needs continuous detailed study and concrete approaches from relevant institutions and professionals in this field. Security in electricity supply is essential for our society and necessary for the 24/7 principle for the digital economy. The recent difficulties caused by the Covid-19 pandemic remind us of the great importance of electricity in all aspects of our lives, from the availability of medical equipment and IT systems - for work from home as a growing trend of systemizing jobs during 2020. Ensuring a secure and reliable electricity supply is of paramount importance to all countries in the world. Considering the Western Balkans as one of the regions which since the 90s most of it has been an area which has emerged from devastating wars and which have affected every sphere, including the energy sector. So, the development of the energy sector is one of the most critical issues in the Western Balkans region. Based on the considerable potential of electricity production from renewable sources, it is considered that the production of electricity from RES will create a stable and functional system of energy production by contributing to the principle of diversification of electricity production and decentralization of the power system. So, renewable energy is considered a promising method for building sustainable and resilient energy systems that provide affordable energy to boost a region economic development, reducing import dependence. The purpose of this scientific research is to discuss the development and current state of electricity production from renewable sources in the Western Balkans, with special emphasis on the Republic of Kosovo. Considering that all Western Balkan countries have set targets or targets for RES in their energy policies but not always these pre-defined targets are easily achievable in practice due to thevarious challenges facing the energy sector in this region.

Exploring COVID-19 transmission patterns and key factors during epidemics caused by three major strains in Asia

The COVID-19 epidemic has lasted for more than two years since the outbreak in late 2019. An urgent and challenging question is how to systematically evaluate epidemic developments in different countries, during different periods, and to determine which measures that could be implemented are key for successful epidemic prevention. In this study, SBD distance-based K-shape clustering and hierarchical clustering methods were used to analyse epidemics in Asian countries. For the hierarchical clustering, epidemic time series were divided into three periods (epidemics induced by the Original/Alpha, Delta and Omicron variants separately). Standard deviations, the Hurst index, mortality rates, peak value of confirmed cases per capita, average growth rates, and the control efficiency of each period were used to characterize the epidemics. In addition, the total numbers of cases in the different countries were analysed by correlation and regression in relation to 15 variables that could have impacts on COVID-19. Finally, some suggestions on prevention and control measures for each category of country are given. We found that the total numbers of cases per million of a population, total deaths per million and mortality rates were highly correlated with the proportion of people aged over 65 years, the prevalence of multiple diseases, and the national GDP. We also found significant associations between case numbers and vaccination rates, health expenditures, and stringency of control measures. Vaccinations have played a positive role in COVID-19, with a gradual decline in mortality rates in later periods, and are still playing protective roles against the Delta and Omicron strains. The stringency of control measures taken by a government is not an indicator of the appropriateness of a country's response to the outbreak, and a higher index does not necessarily mean more effective measures; a combination of factors such as national vaccination rates, the country's economic foundation and the availability of medical equipment is also needed. This manuscript was submitted as part of a theme issue on “Modelling COVID-19 and Preparedness for Future Pandemics”.

Assessment of utilization of health care services and the associated factors among adult fee-waiver beneficiaries in Hawassa, southern Ethiopia: A community based cross sectional study.

Over the past decades, developing countries like Ethiopia have resorted to the implementation of user fees in public health care systems. Nonetheless, evidences suggest that user fees affect the poor negatively. Therefore, Ethiopian government introduced fee-waiver mechanism in 1998 aiming to mitigate the financial constraints faced by the poorest segment of the population in utilizing health care services. Yet, in the country, there is limited evidence on this subject. The overall objective of the study was to assess health care utilization and the associated factors among the fee-waiver beneficiaries in Hawassa City of Southern Ethiopia. A quantitative, cross-sectional study design was employed using a sample of 636 fee-waiver beneficiaries. Data was collected using a structured interviewer-administered questionnaire and analyzed using binary logistic regression. An odds ratio with the corresponding CI was used to identify the associated factors, while P < 0.05 was used to declare significance. The response rate to the survey is 581(91.4%). Of this, 377 (65%) utilized health services in the preceding 3 months of data collection. Availability of medical equipment [AOR = 1.501; 95% C.I. (1.066-2.114)], being >50 years of age [AOR = 2.271; 95% C.I. (1.304-3.953)], improved drug availability in the health care facilities [AOR = 1.682; 95% C.I. (1.118-2.530)] and beneficiaries' perception of health worker's handling practice [AOR = 3.759; 95% C.I. (1.425-9.912)] were among the significant factors associated with beneficiaries' utilization of health care services at public health facilities. The overall fee waiver beneficiaries' health care utilizations rate is 64.9%. Optimizing availability of medical equipment, enhancing drug availability and strengthening good patient handling practices are recommended.

ABSTRACT&#x0D; The hospital is one of the institutions that has an important role in providing health services to the community. Availability of medical equipment that is complete and in good condition is one of the supports for the realization of improving the quality of electromedical services, especially in the supervision and maintenance of medical facilities that are more efficient and effective at Dr. Wahidin Sudirohusodo Hospital Makassar.The purpose of this study was to determine the readiness at RSUP dr Wahidin Sudirohusodo Makassar in the plan to implement an android-based medical equipment maintenance activity reporting system.This research is a qualitative research that aims to obtain a complete picture of a matter according to the view of the human being studied. This research was carried out at Dr Wahidin Sudirohusodo Hospital Makassar in July-August 2021. By involving 5 respondents who met the requirements in this study, namely the Director of Planning, Organization and General Affairs, Head of Facility Maintenance Agency, Head of Sub-Installation of Medical Equipment Maintenance, and Electromedical technical staff. Dr. Wahidin Sudirohusodo Hospital, Makassar. Data collection was done by open interview and observation. The entire interview process was recorded using audio recordings. Data is presented in narrative form.The results showed that based on the analysis with the SWOT theory approach, it was found that the chances of implementing an Android-based medical equipment maintenance reporting system application plan could be implemented well if the strengths and opportunities were combined into a good form of planning to be implemented in the process. The conclusion in this study is that dr Wahidin Sudirohusodo Hospital Makassar is ready to plan the implementation of an android-based medical equipment maintenance activity reporting system.It is hoped that all relevant parties will always try to minimize bad possibilities in the application of the android-based medical equipment maintenance activity reporting system.&#x0D; Keywords: Android Application, Facility Maintenance, SWOT Theory

Effect of hospital attributes on patient preference among outpatient attendants in Wolaita Zone, Southern Ethiopia: discrete choice experiment study

BackgroundPatient preference has preceded the use of health care services, and it has been affected by different hospital attributes. Meanwhile, the number of patients receiving vital health intervention is particularly low in Ethiopia. Therefore, this study aimed to determine the effect of hospital attributes on patient preference for outpatients in the Wolaita area in September 2020.MethodsA discrete choice experimental study was applied to determine the effect of hospital attributes on patient preference with a sample size of 1077. The experimental survey was conducted among outpatient attendants selected through a systematic random sampling approach. Six key attributes (competence of healthcare providers; availability of medical equipment and supplies; cost of service; wait time; distance; and hospital reputation) deduced from various hospital attributes were used to elicit the patient preferences. The data was collected from participants through the Open Data Kit application. A random effect probit model with marginal willingness to pay measure and partially log-likelihood analysis was applied to extract important attributes. We used STATA version 15 software for analysis, and the fitness of the model was verified by the calculated p-value for the Wald chi-square with a cut-point value of 0.05.ResultOne thousand forty-five patients who received outpatient care participated in the study. The random effect probit results have shown that all hospital attributes included in the study were significantly valued by patients while choosing the hospital (p-value < 0.001). Meanwhile, based on marginal willingness to pay and partial log-likelihood analysis, the competence of health care providers was identified as the most important attribute followed by the availability of medical equipment and supplies in hospitals.Conclusion and recommendationThe results suggested that the quality of health care providers and availability of medical equipment and supply in hospitals would be primary interventional points for improving the patient preference of hospitals. Assessment, education, and training are recommended for enhancing the quality of health care providers. And stock balance checks, inspections, and accreditation are believed to be valuable for improving the availability of equipment and supply in hospitals.

NISHASH: A reasonable cost-effective mechanical ventilator for COVID affected patients in Bangladesh

COVID-19 has elapsed all over the world with massive losses which indicate the lack of availability of medical equipment during the pandemic such as a ventilator. This is exemplified by the densely populated country Bangladesh who unable to maintain COVID-affected people because of the ventilator. Due to the higher price, unavailability, and manufacturing defection, most medical are unable to purchase this ventilator which causes terrible death for a respiratory problem. Of these cases, this paper represents a way to escape this problem and proposed a mechanical ventilator named “NISHASH” which will help to anticipate COVID affected people and higher price of the ventilator. Through the electromechanical instruments, a prototype lightweight easily moveable where preciously it automatically controls with digital feedback system ventilator which fulfills oxygen flow based on patient requirement are developed with different selection mode. The aim was to design and develop inexpensively automated easy to build to minimize the extreme shortage of the ventilator in Bangladesh. In this model of a mechanical ventilator, the cost is less than $90 where components are available all over the world.

Colorectal Surgery in the COVID-19 Pandemic Era.

The novel coronavirus disease 2019 (COVID-19) caused by the new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread at a very fast rate, overwhelming and disrupting healthcare systems around the world since its outbreak in December 2019 in China. As of October 2021, the total number of COVID-19 cases exceeds 240,000,000, and the total number of deaths is close to 5,000,000. In the situation of widespread SARS-CoV-2 infection, restrictions on the medical system due to shifts in medical care to accommodate the pandemic will occur, and its impact on surgical and endoscopic treatment for colorectal cancer is inevitable. Therefore, it is necessary to satisfy all of the following requirements: patient safety, prevention of exposure of healthcare workers including surgeons, prevention of nosocomial infection, and a decision on how to treat the primary disease. Surgical triage is also required, based on comprehensive consideration of the patient's condition, the severity of the disease, the SARS-CoV-2 infection situation in the region, and the medical supply system at each facility, including medical resources, human resources, and the availability of medical equipment. Understanding the diagnostic and treatment environment that the COVID-19 pandemic has dramatically changed is important in providing appropriate surgical care to patients who require surgery while taking utmost care to prevent the spread of COVID-19.
