Not All Flat-Panel C-Arms Are Created Equal: A Comparison of Three Major Manufacturers.

Introduction: Flat-panel detector C-arms (FCs) are reported to reduce radiation exposure and improve image quality compared with conventional image intensifier C-arms (CCs). The purpose of this study was to compare radiation exposure and image quality between three commonly used FCs. Materials and Methods: A cadaver model was placed in the prone position to simulate percutaneous nephrolithotomy. We compared the following three FCs: OEC Elite CFD from GE HealthCare, Zenition 70 from Philips, and Ziehm Vision RFD from Ziehm Imaging. To measure the radiation dose, optically stimulated luminescence dosimeters (OSLDs) were utilized during five 300-second trials, conducted under three settings: automatic exposure control (AEC), AEC with low dose (LD), and LD with the lowest pulse rate (LDLP). Ten blinded urologists evaluated the image quality. Data were statistically analyzed using the analysis of variance (ANOVA) and Tukey's B post hoc tests. Results: In the AEC setting, the Philips C-arm demonstrated lower ventral OSLD exposure (42,446 mrad) compared with both the GE (51,076 mrad) and Ziehm (83,178 mrad; p < 0.001) C-arms. Similarly, in the LD setting, the Philips C-arm resulted in less ventral OSLD exposure (25,926 mrad) than both the Ziehm (30,956 mrad) and GE (38,209 mrad; p < 0.001) C-arms. Meanwhile, in the LDLP setting, the Ziehm C-arm showed less ventral OSLD exposure (4019 mrad) than both the GE (7418 mrad) and Philips (8229 mrad; p < 0.001) C-arms. All three manufacturers received adequate image quality ratings at the AEC and LD settings. However, at LDLP, the Ziehm C-arm received inadequate ratings in 8% of images, whereas both the GE and Philips C-arms received 100% adequate ratings (p = 0.016). Conclusions: Radiation produced by flat-panel C-arms varies dramatically, with the highest exposure (Ziehm) being almost double the lowest (Philips) in AEC. Improved picture quality at the lowest settings may come at the cost of increased radiation dose. Surgeons should carefully select the machine and settings to minimize radiation exposure while still preserving the image quality.

Optimal dual-energy computed tomography scan parameters to detect small-sized urinary stones and their composition

To investigate the optimal scanning parameters of dual-energy computed tomography (DECT), which can accurately determine sensitivity (the detectability of urinary stones) and accuracy (the composition matching of urinary stones), and to apply them to clinical trials. Fifteen urinary stones were chemically analyzed, and their chemical compositions were considered a reference standard with which we compared the uric acid (UA) and non-UA compositions determined using DECT. The urinary stones were placed inside a bolus and scanned with a dual-source CT scanner under various selected dual-energy conditions (A to X) using various solid water phantom thicknesses. These datasets were analyzed using the Siemens syngo.via software tool (integrated into the CT system) for matching the sensitivity and accuracy assessments. This study showed that 80% of the highest sensitivity (detection of urinary stones) and 92% of the highest accuracy (composition matching of urinary stones) were achieved under condition A (a collimation beam width setting of 2 × 32× 0.6mm, an automatic exposure control setting of 80/sn140 peak kilovoltage, and a slice thickness of 0.5/0.5mm) (P < 0.05). Application of the DECT energy parameters presented in the study will help identify the sensitivity and accuracy of UA and non-UA stone analysis, even in patients with small-sized urinary stones and in conditions difficult for analysis.

Lung Cancer Screening Using Clinical Photon-Counting Detector Computed Tomography and Energy-Integrating-Detector Computed Tomography: A Prospective Patient Study.

To evaluate the diagnostic quality of photon-counting detector (PCD) computed tomography (CT) in patients undergoing lung cancer screening compared with conventional energy-integrating detector (EID) CT in a prospective multireader study. Patients undergoing lung cancer screening with conventional EID-CT were prospectively enrolled and scanned on a PCD-CT system using similar automatic exposure control settings and reconstruction kernels. Three thoracic radiologists blinded to CT system compared PCD-CT and EID-CT images and scored examinations using a 5-point Likert comparison score (-2 [left image is worse] to +2 [left image is better]) for artifacts, sharpness, image noise, diagnostic image quality, emphysema visualization, and lung nodule evaluation focusing on the border. Post hoc correction of Likert scores was performed such that they reflected PCD-CT performance in comparison to EID-CT. A nonreader radiologist measured objective image noise. Thirty-three patients (mean, 66.9 ± 5.6 years; 11 female; body mass index; 30.1 ± 5.1 kg/m 2 ) were enrolled. Mean volume CT dose index for PCD-CT was lower (0.61 ± 0.21 vs 0.73 ± 0.22; P < 0.001). Pooled reader results showed significant differences between imaging modalities for all comparative rankings ( P < 0.001), with PCD-CT favored for sharpness, image noise, image quality, and emphysema visualization and lung nodule border, but not artifacts. Photon-counting detector CT had significantly lower image noise (74.4 ± 10.5 HU vs 80.1 ± 8.6 HU; P = 0.048). Photon-counting detector CT with similar acquisition and reconstruction settings demonstrated improved image quality and less noise despite lower radiation dose, with improved ability to depict pulmonary emphysema and lung nodule borders compared with EID-CT at low-dose lung cancer CT screening.

Conversion from dose length product to effective dose for the CT component of whole-body PET/CT.

For dose management of CT, the ratio of effective dose (ED) to dose length product (DLP) is often used to convert DLP to ED. We evaluated this ratio in the CT component of whole-body PET/CT performed under various imaging conditions to determine a practical method for ED estimation applicable to PET/CT. In total, 400 patients who underwent whole-body PET/CT were enrolled. The imaging conditions were variable in terms of the scanner model, setting of automatic exposure control (AEC) setting and arm positioning. The scan range was divided into six anatomical regions. DLP was calculated for each region, and multiplied by the conversion factor for the respective region to determine regional ED. The six regional EDs were summed together to determine ED by the regional DLP method (EDrDLP). Additionally, regional ED was assessed using CT-Expo, software dedicated to CT dose estimation, and the total of six regional EDs were defined as ED by the CT-Expo method (EDCT-Expo). EDrDLP/DLP and EDCT-Expo/DLP were calculated using DLP automatically provided by the scanner. EDrDLP/DLP ranged from 0.0121 to 0.0128mSv/mGy/cm with the arms up and from 0.0127 to 0.0134mSv/mGy/cm with the arms down. Putting the arms down slightly increased EDrDLP/DLP, presumably due to an increased contribution of the chest and abdomen to total radiation exposure. The AEC setting and scanner model also influenced EDrDLP/DLP significantly but slightly. EDCT-Expo/DLP showed apparent scanner dependence, which appeared mainly attributable to differences in the constants used for DLP calculation between the scanner and CT-Expo. Multiplication of scanner-derived DLP by a conversion factor of 0.013mSv/mGy/cm provides acceptable ED estimates.

ESTIMATION OF RADIATION DOSE IN CT VENOGRAPHY OF THE LOWER EXTREMITIES: PHANTOM EXPERIMENTS USING DIFFERENT AUTOMATIC EXPOSURE CONTROL SETTINGS AND SCAN RANGES.

We performed phantom experiments to assess radiation dose in computed tomography (CT) venography of the lower extremities. CT images of a whole-body phantom were acquired using different automatic exposure control settings and scan ranges, simulating CT venography. Tube current decreased in the lower extremities compared to the trunk. The scout direction and dose modulation strength affected tube current, dose length product (DLP) and effective dose. The middle and distal portions of the lower extremities contributed substantially to DLP but not to effective dose. When effective dose was estimated by multiplying DLP by a single conversion factor, overestimation was evident; this became more pronounced as the scan range narrowed. In CT venography of the lower extremities, the scout direction and modulation strength affect radiation dose. Use of DLP severely overestimates radiation dose and underestimates effects of scan range narrowing.

111. Dose optimization in mammographic screening: results from ten years invigilation

Purpose Mammographic screening represents one of the most significant medical expose to radiation for women. The method proposed from European Reference Organisation for Quality Assured Breast Screening and Diagnostic Services (EUREF) evaluate average glandular dose (AGD) considering the breast glandularity (BG) a function of breast thickness (BT). VOLPARA software (Volpara Solutions Limited) compute BG from acquired images using a multi-parametric algorithm. The purpose of this study is to evaluate the difference between AGD computed in both method and to assess the applicability of automatic tools for patient dose optimisation and mammographic unit QCs. Methods Mammographic screening exams from last 10 years performed in our institution have been considered. More than 350’000 exams overall (total 1.5 M of expositions), from seven mammographic units (of three different vendors), were included in this study. Exams have been performed using automatic exposure control settings. Data was extracted from DICOM images and processed using VOLPARA software (Volpara Solutions Limited) and analysed with a custom Matlab software. For each exposition difference between Diff = AGD EUREF - AGD VOLPARA was determinated. Exposure data are retrieved to identify patient dose optimisation strategies and to track device performances. Results Average Diff is 0.07 ± 0.13 mGy (min/max −1.7/+1.8 mGy), mean breast thickness is 55 mm (SD 13 mm min/max 10/117 mm). In figure is reported percent distribution of Diff as function of BT. Large part of exams shows Diff > 0 (71% of total), the remains have Diff 0. These results are generally reproducible among seven mammographic units considered. Conclusions Presented results indicate EUREF approach overestimate AGD, the proposed method allow to assess personal dosimetry and could be implemented in order to monitor in live time AGD difference from EUREF evaluation. Use of automatic dose software allow to monitor possibly variation in exposition parameters.

Observer Performance with Varying Radiation Dose and Reconstruction Methods for Detection of Hepatic Metastases.

Purpose To estimate the ability of lower dose levels and iterative reconstruction (IR) to display hepatic metastases that can be detected by radiologists. Materials and Methods Projection data from 83 contrast agent-enhanced CT examinations were collected. Metastases were defined by histopathologic analysis or progression and regression. Lower radiation dose configurations were reconstructed at five dose levels with filtered back projection (FBP) and IR (automatic exposure control settings: 80, 100, 120, 160, and 200 quality reference mAs [QRM]). Three abdominal radiologists circumscribed metastases, indicating confidence (confidence range, 0-100) and image quality. Noninferiority was assessed by using jackknife alternative free-response receiver operating characteristic (JAFROC) analysis (noninferiority limit, -0.10) and reader agreement rules, which required identification of metastases identified at routine dose, and no nonlesion localizations in patients negative for metastases, in 71 or more patient CT examinations (of 83), for each configuration. Results There were 123 hepatic metastases (mean size, 1.4 cm; median volume CT dose index and size-specific dose estimate, 11.0 and 13.4 mGy, respectively). By using JAFROC figure of merit, 100 QRM FBP did not meet noninferiority criteria and had estimated performance difference from routine dose of -0.08 (95% confidence interval: -0.11, -0.04). Preset reader agreement rules were not met for 100 QRM IR or 80 QRM IR, but were met for doses 120 QRM or higher (ie, size-specific dose estimate ≥ 8.0 mGy). IR improved image quality (P < .05) but not reader performance. Other than 160 QRM IR, lower dose levels were associated with reduced confidence in metastasis detection (P < .001). Conclusion For detection of hepatic metastases by using contrast-enhanced CT, dose levels that corresponded to 120 quality reference mAs (size-specific dose estimate, 8.0 mGy) and higher performed similarly to 200 quality reference mAs with filtered back projection. © RSNA, 2018 Online supplemental material is available for this article.

PROSPECTS FOR LOWERING DOSES DURING PREVENTIVE DIGITAL CHEST RADIOGRAPHY

Objective: to search for the possibility of lowering a dose burden on a patient during preventive chest radiography. Material and methods. The investigation was carried out in two hospitals (City Mariinsky Hospital, Saint Petersburg, Russia) and (Skane University Hospital, Malm ° ö, Sweden) in 2013–2015. The existing protocols for this type of investigation were corrected by changing the anode voltage, the thickness of complete filtration, and the refusal to use a raster to determine the optimal dose/image quality ratio. Results. The investigation showed the possibility of reducing the values of the products of the dose per area (PDA) by 47% and lowering the effective dose (ED) by 33%, by increasing the tube voltage (up to 125 or 150 kV) and decreasing the thickness of full filtration of direct beam radiation (to 3 mm Al). The refusal to use a raster could further reduce the value of PDA and ED to 75%. Conclusion. Comparison of the dose values of Russian and Swedish X-ray apparatuses showed that the absolute values of PDP and ED are 10 times lower for Swedish devices in the same modes of chest radiography. This fact can be explained by the use of various image receivers and automatic exposure control settings.

The Relative Effects of Manual Versus Automatic Exposure Control on Radiation Dose to Vital Organs in Total Hip Arthroplasty.

Technologic advances have reduced medical radiation exposure while maintaining image quality. The purpose of this study was to determine the effects of the presence of total hip arthroplasty implants, compared with native hips, on radiation exposure of the most radiosensitive organs when manual and automatic exposure control settings are used. Detection probes were placed at six locations (stomach, sigmoid colon, right pelvic wall, left pelvic wall, pubic symphysis, and anterior pubic skin) in a cadaver. Radiographs were obtained with the use of manual and automatic exposure control protocols, with exposures recorded. A total hip arthroplasty implant was placed in the cadaver, probe positioning was confirmed, and the radiographs were repeated, with exposure values recorded. The control probe placed at the stomach had values ranging from 0.00 mSv to 0.01 mSv in protocols with and without implants. With the manual protocol, exposures in the pelvis ranged from 0.36 mSv to 2.74 mSv in the native hip and from 0.33 mSv to 2.24 mSv after implant placement. The increases in exposure after implant placement, represented as relative risk, were as follows: stomach, 1.000; pubic symphysis, 0.818; left pelvic wall, 1.381; sigmoid colon, 1.550; right pelvic wall, 0.917; and anterior pubic skin, 1.015. With automatic exposure control, exposures in the pelvis ranged from 0.07 mSv to 0.89 mSv in the native hip and from 0.21 mSv to 1.15 mSv after implant placement. With automatic exposure control, the increases in exposure after implant placement, represented as relative risk, were as follows: stomach, 1.000; pubic symphysis, 1.292; left pelvic wall, 1.476; sigmoid colon, 2.182; right pelvic wall, 3.000; and anterior pubic skin, 1.378. The amount of radiation to which patients are exposed as a result of medical procedures or imaging, and whether exposure is associated with an increased risk of malignant transformation, are the subject of ongoing debate. We found that after insertion of a total hip arthroplasty implant, exposure values increased threefold at some anatomic locations and surpassed 1 mSv, the generally accepted threshold for concern. Radiation exposure to radiosensitive organs increased up to threefold after total hip implantation with automatic exposure control and up to approximately 1.5 times with the manual protocol. Doses were greater with manual exposures than with automatic exposure control (except at the control probe on the stomach, where exposure was negligible, as expected). However, after implant placement, doses increased more with automatic exposure control than with manual exposure. This difference can be attributed to increased scatter and the difficulty of dose modification because of the density of the implant. Current radiographic protocols should be reassessed to determine if the benefits of frequent radiographs outweigh the newly demonstrated risks.

Low-dose computed tomography scans with automatic exposure control for patients of different ages undergoing cardiac PET/CT and SPECT/CT.

This study aimed to evaluate the efficacy of automatic exposure control (AEC) in order to optimize low-dose computed tomography (CT) protocols for patients of different ages undergoing cardiac PET/CT and single-photon emission computed tomography/computed tomography (SPECT/CT). One PET/CT and one SPECT/CT were used to acquire CT images for four anthropomorphic phantoms representative of 1-year-old, 5-year-old and 10-year-old children and an adult. For the hybrid systems investigated in this study, the radiation dose and image quality of cardiac CT scans performed with AEC activated depend mainly on the selection of a predefined image quality index. Multiple linear regression methods were used to analyse image data from anthropomorphic phantom studies to investigate the effects of body size and predefined image quality index on CT radiation dose in cardiac PET/CT and SPECT/CT scans. The regression relationships have a coefficient of determination larger than 0.9, indicating a good fit to the data. According to the regression models, low-dose protocols using the AEC technique were optimized for patients of different ages. In comparison with the standard protocol with AEC activated for adult cardiac examinations used in our clinical routine practice, the optimized paediatric protocols in PET/CT allow 32.2, 63.7 and 79.2% CT dose reductions for anthropomorphic phantoms simulating 10-year-old, 5-year-old and 1-year-old children, respectively. The corresponding results for cardiac SPECT/CT are 8.4, 51.5 and 72.7%. AEC is a practical way to reduce CT radiation dose in cardiac PET/CT and SPECT/CT, but the AEC settings should be determined properly for optimal effect. Our results show that AEC does not eliminate the need for paediatric protocols and CT examinations using the AEC technique should be optimized for paediatric patients to reduce the radiation dose as low as reasonably achievable.

ASSESSMENT OF AEC SYSTEM RESPONSE IN GE 16 SLICES SCANNER

Computed Tomography scanners equipped with system for Automatic Exposure Control (AEC) have been recently installed into clinical practice in Macedonia. Assessment of their AEC settings and performances is important task from patient doses and images quality point of view. This study was done by analyzing of CT examinations in patients in the City Hospital "8 September" in Skopje. The examinations were carried out by GE Bright Speed 16 slices scanner equipped with AEC system. In all patients were applied the same protocol with constant acquisition parameters was applied, and images were reconstructed by standard mode. Patient dimensions and image noise were measured from the scouts and axial images. From DICOM header the information related to dose, TCM and slice position were extracted. It was found that scanner automatic exposure system adjusts exposure mainly according to maximal patient lateral dimension (LR) and applying the same Noise Index (NI) value in patients with different size does not provides necessarily the same image noise level. In patients which LR dimension was less than 30 cm it was found that AEC adjusts tube current at the minimum of mA interval with no modulation throughout different body parts.

Background Detection of small renal calculi has benefitted from recent advances in computed tomography (CT) scanner design. Information regarding observer performance when using state-of-the-art CT scanners for this application is needed. Purpose To assess observer performance and the impact of radiation dose for detection and size measurement of <4 mm renal stones using CT with integrated circuit detectors and iterative reconstruction. Material and Methods Twenty-nine <4 mm calcium oxalate stones were randomly placed in 20 porcine kidneys in an anthropomorphic phantom. Four radiologists used a workstation to record each calculus detection and size. JAFROC Figure of Merit (FOM), sensitivity, false positive detections, and calculus size were calculated. Results Mean calculus size was 2.2 ± 0.7 mm. The CTDIvol values corresponding to the automatic exposure control settings of 160, 80, 40, 25, and 10 Quality Reference mAs (QRM) were 15.2, 7.9, 4.2, 2.7, and 1.3 mGy, respectively. JAFROC FOM was ≥ 0.97 at ≥ 80 QRM, ≥ 0.89 at ≥ 25 QRM, and was inferior to routine dose (160 QRM) at 10 QRM (0.72, P < 0.05). Per-calculus sensitivity remained ≥ 85% for every reader at ≥ 25 QRM. Mean total false positive detections per reader were ≤ 3 at ≥ 80 QRM, but increased substantially for two readers ( ≥ 12) at ≤ 40 QRM. Measured calculus size significantly decreased at ≤ 25 QRM ( P ≤ 0.01). Conclusion Using low dose renal CT with iterative reconstruction and ≥ 25 QRM results in high sensitivity, but false positive detections increase for some readers at very low dose levels (≤ 40 QRM). At very low doses with iterative reconstruction, measured calculus size will artifactually decrease.

Assessing task performance in FFDM, DBT, and synthetic mammography using uniform and anthropomorphic physical phantoms.

The purpose of this study is to quantify the differences in detectability between full field digital mammography (FFDM), digital breast tomosynthesis (DBT), and synthetic mammography (SM) for challenging, low contrast signals, in the context of both a uniform and an anthropomorphic, textured phantom. Images of the phantoms were acquired using a Hologic Selenia Dimensions system. Images were taken at 50%, 100%, and 200% of the dose delivered under automatic exposure control (AEC). Low-contrast disks, created using an inkjet printer with iodine-doped ink, were inserted into the phantom. The disks varied in diameter from 210 to 630 μm, and in local contrast from 1.1% to 2.8% in regular increments. Human observers located the disks in a 4 alternative forced choice experiment. Proportion correct (PC) was computed as the number of correct localizations out of the total number of tries. Overall, scores from FFDM and DBT were consistently greater than scores from SM. At an exposure corresponding to the AEC setting, mean PC scores for the largest disks with the uniform phantom were 0.80 for FFDM, 0.83 for DBT, and 0.66 for SM, with the same rank ordering at other doses. Scores were similar but lower for the nonuniform background. At an exposure twice the AEC setting, however, the difference between uniform and nonuniform scores was most pronounced for DBT alone. Differences between scores for FFDM and SM were statistically significant, while those between FFDM and DBT were not. Scores were used to compute the minimum contrast level needed to reach 62.5% detection rate. The minimum contrast for SM was 36%-81% higher compared to FFDM or DBT, in either background. This study shows that an anthropomorphic phantom and lesions inserts may be used to conduct a reader study. Detectability was significantly lower for synthetic mammography than for FFDM or DBT, for all conditions. Additionally, observer performance was consistently lower for the anthropomorphic phantom, indicating the greater challenge due to anatomical background. Because of this, it may be important to use realistic phantoms in observer studies in order to draw conclusions that are more clinically relevant.

Impact of compressed breast thickness and dose on lesion detectability in digital mammography: FROC study with simulated lesions in real mammograms.

The aim of this work was twofold: (1) to examine whether, with standard automatic exposure control (AEC) settings that maintain pixel values in the detector constant, lesion detectability in clinical images decreases as a function of breast thickness and (2) to verify whether a new AEC setup can increase lesion detectability at larger breast thicknesses. Screening patient images, acquired on two identical digital mammography systems, were collected over a period of 2 yr. Mammograms were acquired under standard AEC conditions (part 1) and subsequently with a new AEC setup (part 2), programmed to use the standard AEC settings for compressed breast thicknesses ≤49 mm, while a relative dose increase was applied above this thickness. The images were divided into four thickness groups: T1 ≤ 29 mm, T2 = 30-49 mm, T3 = 50-69 mm, and T4 ≥ 70 mm, with each thickness group containing 130 randomly selected craniocaudal lesion-free images. Two measures of density were obtained for every image: a BI-RADS score and a map of volumetric breast density created with a software application (VolparaDensity, Matakina, NZ). This information was used to select subsets of four images, containing one image from each thickness group, matched to a (global) BI-RADS score and containing a region with the same (local) volpara volumetric density value. One selected lesion (a microcalcification cluster or a mass) was simulated into each of the four images. This process was repeated so that, for a given thickness group, half the images contained a single lesion and half were lesion-free. The lesion templates created and inserted in groups T3 and T4 for the first part of the study were then inserted into the images of thickness groups T3 and T4 acquired with higher dose settings. Finally, all images were visualized using the ViewDEX software and scored by four radiologists performing a free search study. A statistical jackknife-alternative free-response receiver operating characteristic analysis was applied. For part 1, the alternative free-response receiver operating characteristic curves for the four readers were 0.80, 0.65, 0.55 and 0.56 in going from T1 to T4, indicating a decrease in detectability with increasing breast thickness. P-values and the 95% confidence interval showed no significant difference for the T3-T4 comparison (p = 0.78) while all the other differences were significant (p < 0.05). Separate analysis of microcalcification clusters presented the same results while for mass detection, the only significant difference came when comparing T1 to the other thickness groups. Comparing the scores of part 1 and part 2, results for the T3 group acquired with the new AEC setup and T3 group at standard AEC doses were significantly different (p = 0.0004), indicating improved detection. For this group a subanalysis for microcalcification detection gave the same results while no significant difference was found for mass detection. These data using clinical images confirm results found in simple QA tests for many mammography systems that detectability falls as breast thickness increases. Results obtained with the AEC setup for constant detectability above 49 mm showed an increase in lesion detection with compressed breast thickness, bringing detectability of lesions to the same level.

Optimizing quality of digital mammographic imaging using Taguchi analysis with an ACR accreditation phantom

ABSTRACTThis work demonstrated the improvement of the visualization of lesions by modulating the factors of an X-ray mammography imaging system using Taguchi analysis. Optimal combinations of X-ray operating factors in each group of level combination were determined using the Taguchi method, in which all factors were organized into only 18 groups, yielding analytical results with the same confidence as if each factor had been examined independently. The 4 considered operating factors of the X-ray machine were (1) anode material (target), (2) kVp, (3) mAs and (4) field of view (FOV). Each of these factors had 2 or 3 levels. Therefore, 54 (2×3×3×3 = 54) combinations were generated. The optimal settings were Rh as the target, 28 kVp, 80 mAs and 19×23 cm2 FOV. The grade of exposed mammographic phantom image increased from the automatic exposure control (AEC) setting 70.92 to 72.00 under the optimal setting, meeting the minimum standard (70.00) set by Taiwan's Department of Health. The average glandular dose (AGD) of the exposed phantom, 0.182 cGy, was lower than that, 0.203 cGy, under the AEC setting. The Taguchi method was extremely promising for the design of imaging protocols in clinical diagnosis.

MANAGEMENT OF PATIENT DOSES FROM DIGITAL X-RAY CHEST SCREENING EXAMINATIONS.

An anthropomorphic phantom study was carried out in 2013-14 in two hospitals, one located in Russia (Mariinsky Hospital, Saint Petersburg) and the other in Sweden (Skåne University Hospital, Malmö). The aim of the study was to investigate the possibilities to reduce the patient dose from digital X-ray chest screening examinations. The existing chest imaging protocols were adjusted by changing the tube voltage, total filtration and grid in order to determine the most dose-effective combination of the examination parameters. It was possible to achieve up to 50 % dose-area product (DAP) and 30 % effective dose reduction by raising the tube voltage from 100 to 125 or 150 kV, and simultaneously decrease the total filtration to the minimum allowed by the X-ray unit (3 mm Al). The absence of a grid allowed to further reduce the DAP and effective dose by up to 80 %. Comparison between Russian and Swedish X-ray units showed the same trend in DAP and effective dose reduction, but the absolute dose values were lower by almost a factor of 10 for the Swedish units due to different image receptors and automatic exposure control settings.

Automatic exposure control systems designed to maintain constant image noise: effects on computed tomography dose and noise relative to clinically accepted technique charts.

To compare computed tomography dose and noise arising from use of an automatic exposure control (AEC) system designed to maintain constant image noise as patient size varies with clinically accepted technique charts and AEC systems designed to vary image noise. A model was developed to describe tube current modulation as a function of patient thickness. Relative dose and noise values were calculated as patient width varied for AEC settings designed to yield constant or variable noise levels and were compared to empirically derived values used by our clinical practice. Phantom experiments were performed in which tube current was measured as a function of thickness using a constant-noise-based AEC system and the results were compared with clinical technique charts. For 12-, 20-, 28-, 44-, and 50-cm patient widths, the requirement of constant noise across patient size yielded relative doses of 5%, 14%, 38%, 260%, and 549% and relative noises of 435%, 267%, 163%, 61%, and 42%, respectively, as compared with our clinically used technique chart settings at each respective width. Experimental measurements showed that a constant noise-based AEC system yielded 175% relative noise for a 30-cm phantom and 206% relative dose for a 40-cm phantom compared with our clinical technique chart. Automatic exposure control systems that prescribe constant noise as patient size varies can yield excessive noise in small patients and excessive dose in obese patients compared with clinically accepted technique charts. Use of noise-level technique charts and tube current limits can mitigate these effects.

WE‐D‐18A‐02: Performance Evaluation of Automatic Exposure Control (AEC) Across 12 Clinical CT Systems

Purpose:Automatic exposure control (AEC) is not typically evaluated or monitored in CT quality assurance programs. The purpose of this study was to develop/evaluate a new AEC testing platform for the clinical physics program at our institution, and characterize AEC performance across different CT systems.Methods:The Mercury Phantom comprises three tapered and four uniform regions of polyethylene(16, 23, 30, and 37 cm in diameter); each region includes four inserts: air, Polystyrene, Acrylic, and Teflon. The phantom was imaged using AEC and a fixed tube current technique across 12 clinical CT scanners. Those included five Siemens Somatom Definition Flash, four GE Discovery CT750HD, and three GE Lightspeed VCT systems. A custom MATLAB software package provided MTF, NPS, and detectability indices for each diameter section of the phantom. Detectability indices were used to evaluate the relationship between AEC setting, patient size, and image quality. The magnitude of the power of a best fit exponential curve to the detectability indices and phantom diameter was used as a measure of AEC strength. Results were compared within/across scanner models, and as baseline values for comparison with future system performance testing.Results:For each scanner model, the percent difference in expected image quality and AEC setting was under 3%(+/−2%). The average decrease in detectability between the small and large diameter phantom sections for the Siemens Flash, GE CT750, and GE VCT was 99%(+/−10%), 42%(+/−25%), and 33%(+/−41%), respectively. The value signifying AEC strength was 0.051(+/−13%), 0.019(+/−18%), and 0.018(+/−26%), for the Siemens Flash, GE CT750, and GE VCT models, respectively.Conclusion:This study demonstrated a practical approach to test the AEC performance of clinical CT systems at a large academic medical center. The quantification and evaluation of AEC performance should be included in acceptance testing and in annual physics testing of clinical CT systems.

SU‐E‐I‐48: The Behavior of AEC in Scan Regions Outside the Localizer Radiograph FOV: An In Phantom Study of CT Systems From Four Vendors

Purpose:This review of scanners from 4 major manufacturers examines the clinical impact of performing CT scans that extend into areas of the body that were not acquired in the CT localizer radiograph.Methods:Anthropomorphic chest and abdomen phantoms were positioned together on the tables of CT scanners from 4 different vendors. All of the scanners offered an Automatic Exposure Control (AEC) option with both lateral and axial tube current modulation. A localizer radiograph was taken covering the entire extent of both phantoms and then the scanner's Chest‐Abdomen‐Pelvis (CAP) study was performed with the clinical AEC settings employed and the scan and reconstruction range extending from the superior portion of the chest phantom through the inferior portion of the abdomen phantom. A new study was then initiated with a localizer radiograph extending the length of the chest phantom (not covering the abdomen phantom). The same CAP protocol and AEC settings were then used to scan and reconstruct the entire length of both phantoms. Scan parameters at specific locations in the abdomen phantom from both studies were investigated using the information contained in the DICOM metadata of the reconstructed images.Results:The AEC systems on all scanners utilized different tube current settings in the abdomen phantom for the scan completed without the full localizer radiograph. The AEC system behavior was also scanner dependent with the default manual tube current, the maximum tube current and the tube current at the last known position observed as outcomes.Conclusion:The behavior of the AEC systems of CT scanners in regions not covered by the localizer radiograph is vendor dependent. To ensure optimal image quality and radiation exposure it is important to include the entire planned scan region in the localizer radiograph.

Computed tomography automatic exposure control techniques in 18F-FDG oncology PET-CT scanning

Computed tomography (CT) automatic exposure control (AEC) systems are now used in all modern PET-CT scanners. A collaborative study was undertaken to compare AEC techniques of the three major PET-CT manufacturers for fluorine-18 fluorodeoxyglucose half-body oncology imaging. An audit of 70 patients was performed for half-body CT scans taken on a GE Discovery 690, Philips Gemini TF and Siemens Biograph mCT (all 64-slice CT). Patient demographic and dose information was recorded and image noise was calculated as the SD of Hounsfield units in the liver. A direct comparison of the AEC systems was made by scanning a Rando phantom on all three systems for a range of AEC settings. The variation in dose and image quality with patient weight was significantly different for all three systems, with the GE system showing the largest variation in dose with weight and Philips the least. Image noise varied with patient weight in Philips and Siemens systems but was constant for all weights in GE. The z-axis mA profiles from the Rando phantom demonstrate that these differences are caused by the nature of the tube current modulation techniques applied. The mA profiles varied considerably according to the AEC settings used. CT AEC techniques from the three manufacturers yield significantly different tube current modulation patterns and hence deliver different doses and levels of image quality across a range of patient weights. Users should be aware of how their system works and of steps that could be taken to optimize imaging protocols.
