Abstract 98: Midlife Vascular Risk Factors and Incident Dementia in the ARIC Cohort

Background: Vascular risk factors have been associated with risk of cognitive decline, with increasing evidence that midlife exposure to these risk factors may be most important in conferring late-life risk of cognitive impairment. We explored associations between vascular risk factors measured in midlife and the development of dementia over 25 years in the biracial Atherosclerosis Risk in Communities (ARIC) cohort. Methods: Participants in the ARIC study were recruited from four U.S. communities in 1987-1989, at ages 45-64, with four additional in-person visits, surveillance for hospitalizations, annual phone calls, and repeated cognitive evaluations over a 25-year period. In 2011-2013, ARIC participants were seen for the ARIC Neurocognitive Study (ARIC-NCS), and underwent a detailed neurocognitive battery and informant interviews. Through adjudicated review, dementia cases were defined. Additional dementia cases were identified through the telephone interview for cognitive status (TICS) or informant interview for those participants not attending ARIC-NCS; or by a prior dementia ICD-9 code during a hospitalization. Results: Of 15,744 participants in the cohort, 1516 cases of dementia were identified, of whom over one third were among black participants. Risk of dementia was highest in individuals of black race, with less than a high school education, older age, APOE ε4 carriage, and who, at ARIC baseline, had hypertension, diabetes, or were current smokers (table). APOE ε4 and smoking were each stronger risk factors for dementia in whites than in blacks. Discussion: Vascular risk factors measured in midlife are associated with increased risk of dementia in this biracial cohort, in black and white participants. The risk associated with diabetes nears the increased risk associated with having an APOE ε4 allele. Further studies are needed to evaluate the mechanism of and opportunities for prevention of the cognitive sequelae of these risk factors in midlife.

Factor structure of the ARIC-NCS Neuropsychological Battery: An evaluation of invariance across vascular factors and demographic characteristics.

Neuropsychological test batteries are designed to assess cognition in detail by measuring cognitive performance in multiple domains. This study examines the factor structure of tests from the ARIC-NCS battery overall and across informative subgroups defined by demographic and vascular risk factors in a population of older adults. We analyzed neuropsychological test scores from 6,413 participants in the Atherosclerosis Risk in Communities Neurocognitive Study (ARIC-NCS) examined in 2011-2013. Confirmatory factor analysis (CFA) was used to assess the fit of an a priori hypothesized 3-domain model, and fit statistics were calculated and compared to 1- and 2-domain models. Additionally, we tested for stability (invariance) of factor structures among different subgroups defined by diabetes, hypertension, age, sex, race, and education. Mean age of participants was 76 years, 76% were White, and 60% were female. CFA on the a priori hypothesized 3-domain structure, including memory, sustained attention and processing speed, and language, fit the data better (comparative fit index [CFI] = 0.973, root mean square error of approximation [RMSEA] = 0.059) than the 2-domain (CFI = 0.960, RMSEA = 0.070) and 1-domain (CFI = 0.947, RMSEA = 0.080) models. Bayesian information criterion value was lowest, and quantile-quantile plots indicated better fit, for the 3-domain model. Additionally, multiple-group CFA supported a common structure across the tested demographic subgroups, and indicated strict invariance by diabetes and hypertension status. In this community-based population of older adults with varying levels of cognitive performance, the a priori hypothesized 3-domain structure fit the data well. A SAS macro for the joint modeling of longitudinal outcomes and multiple competing risk dropouts

Background and objectivesThe joint modeling of longitudinal and survival data to assess effects of multiple informative dropout mechanisms on longitudinal outcomes inference has received considerable attention during recent years; related statistical programs to apply these methods have been lacking. This paper provides a SAS macro implementation of a shared parameter model to accommodate the analysis of longitudinal outcomes in the presence of multiple competing survival/dropout events. MethodsIn this macro, we assumed that the associations between the survival and the longitudinal submodels are linked through a set of shared random effects. The submodel for the longitudinal outcome takes the form of a linear mixed effects model, with specifications for the random intercept and/or random slope. The survival submodel allows up to three different competing causes for dropout, each allowing either an exponential or Weibull parametric baseline hazard function. In addition, information criterion fit statistics AIC and BIC are provided to assist with parametric baseline hazard function selection. ResultsWe illustrate the SAS Macro in a cognitive decline study sensitivity analysis using data from the Atherosclerosis Risk in Communities Neurocognitive Study (ARIC-NCS). In addition, we also conduct a simulation study to show that the joint model provides unbiased parameter estimates when informative dropout exists compared against separate model approach which assumes missing at random dropout mechanisms. ConclusionsWe have presented a SAS macro to implement a shared parameter model for a longitudinal outcome and multiple cause-specific dropouts and made the macro code freely available for download.

Abstract MP100: Arterial Stiffness and Pressure Amplification Associated with Markers of Cerebral Microvascular Disease: The Atherosclerosis Risk in Communities Neurocognitive Study (ARIC-NCS)

Introduction: Small vessel disease is associated with decreased cognitive function, possibly differential by race. Age-related central arterial stiffening increases pulsatility resulting in hypoperfusion, microvascular damage and remodeling in the brain, potentially impairing cognition. We examined if arterial stiffness and pressure amplification are associated with lacunar infarcts and greater volumes of white matter hyperintensities (WMH) in a sample of Caucasian and African American (AA) older adults. Methods: We analyzed a cross-sectional sample of ARIC participants aged 67-90 years (n=1486) from visit 5 (2011-2013), with brain magnetic resonance imaging (MRI). The Omron VP-1000 Plus was used to measure aortic stiffness (carotid-femoral pulse wave velocity [cfPWV]) and pressure amplification measures (pulse pressure amplification [PPA], central pulse pressure [cPP], and estimated central systolic blood pressure [cSBP]). Aortic stiffness and pressure amplification were dichotomized at race-specific 25th percentile cut points. Brain MRI using 3D-1.5T equipment quantified the presence of lacunar infarcts and volumes of WMH following a standardized protocol. Logistic regression, adjusted for age, sex, education, ApoE4, heart rate, smoking and body mass index, was used to quantify the odds of lacunar infarcts in participants with high vs. low cfPWV, cPP, cSBP, and low vs. high PPA. Linear regression models, additionally adjusted for intracranial volume, estimated the difference in log-transformed volumes of WMH among participants with high vs. low cfPWV, cPP, cSBP, and low vs. high PPA. Probability sampling weights for an MRI were included to allow for generalizability to the full visit 5 cohort. Results: Among the 1486 participants with a brain MRI (mean age: 76, 41% male, 26% AA), measures of aortic stiffness and pressure amplification were associated with lacunar infarcts in Caucasians, but not in AAs. Caucasian participants with a high cfPWV had greater odds of lacunar infarcts (Odds Ratio [OR] =2.02, 95% confidence interval [CI]: 1.23, 2.20). Caucasians with high cSBP had higher odds of lacunar infarcts (OR=1.72, 95% CI: 1.10, 2.69). In Caucasians, high cfPWV was associated with a 21% (95% CI: 6, 38) greater volume of WMH as compared to a low cfPWV; high cSBP was associated with a 28% (95% CI: 14, 45) greater volume of WMH compared to a low cSBP. In AAs, high cfPWV was associated with a 32% (95% CI: 7, 62) greater volume of WMH as compared to low cfPWV. Cerebral microvascular imaging markers did not differ quantitatively with measures of PPA and cPP. Conclusions: Central arterial stiffening and pressure amplification are plausible microvascular contributors to cognitive aging, providing new information on modifiable pathways for previously observed associations between cardiovascular disease risk factors and the rates of cognitive decline and dementia among older adults.

Impact of Differential Attrition on the Association of Education With Cognitive Change Over 20 Years of Follow-up: The ARIC Neurocognitive Study

Studies of long-term cognitive change should account for the potential effects of education on the outcome, since some studies have demonstrated an association of education with dementia risk. Evaluating cognitive change is more ideal than evaluating cognitive performance at a single time point, because it should be less susceptible to confounding. In this analysis of 14,020 persons from a US cohort study, the Atherosclerosis Risk in Communities (ARIC) Study, we measured change in performance on 3 cognitive tests over a 20-year period, from ages 48-67 years (1990-1992) through ages 70-89 years (2011-2013). Generalized estimating equations were used to evaluate the association between education and cognitive change in unweighted adjusted models, in models incorporating inverse probability of attrition weighting, and in models using cognitive scores imputed from the Telephone Interview for Cognitive Status for participants not examined in person. Education did not have a strong relationship with change in cognitive test performance, although the rate of decline was somewhat slower among persons with lower levels of education. Methods used to account for selective dropout only marginally changed these observed associations. Future studies of risk factors for cognitive impairment should focus on cognitive change, when possible, to allow for reduction of confounding by social or cultural factors.
