Relationship of Illness Perception with Asthma Symptoms Control in Adolescents

Asthma has a high prevalence in Indonesia. Good self-management is necessary for therapy and greatly affects the level of asthma control symptoms. The patient's perception plays an important role in the success of asthma therapy, especially among students, because education affects a person's mindset and perception of disease conditions. The purpose was to determine the relationship between illness perception and control of asthma symptoms in adolescents. The method was cross-sectional and was conducted from November 2019 to January 2020. The variables were asthma control and illness perception. This research was conducted on a campus in Surabaya city. Respondents were asked to fill out the Illness Perception Questionnaire (IPQ) and asthma control test (ACT) questionnaire and analyze by calculating each answer point. This study used a chi-square test to analyze the relationship between ACT and Illness perception. This study involved 75 respondents. Of most of the respondents, 58 people have their asthma partially controlled. Most respondents experienced shortness of breath and wheezing symptoms, and most understood that they were related to their asthma. There was a relationship between asthma control scores and illness perception (p>0.05) on the timeline (p=0.03), personal control (p=0.03), and causal representation (p=0.01). Illness perception can affect asthma symptoms. Asthma will be experienced for a long time, and respondents will better adjust their lifestyle and have confidence that it was controlled. Material factors were also considered. If their asthma was controlled, it was hoped not to affect their finances.

Factors associated with asthma expression in adolescents.

ABSTRACTObjective:To evaluate risk factors associated with asthma symptoms in adolescents in the 13- to 14-year age bracket. Methods:This was a cross-sectional study involving adolescents enrolled in randomly selected public schools in the city of Belo Horizonte, Brazil, and conducted with the use of the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire and its supplementary module for risk factor assessment. The ISAAC questionnaire was completed by the students themselves, whereas the supplementary questionnaire was completed by their parents or legal guardians. Variables showing p ≤ 0.25 in the univariate analysis were included in the multivariate analysis. Stepwise regression with backward elimination was used for variable selection. Results:We evaluated 375 adolescents, 124 (33.1%) of whom had asthma symptoms. The final multivariate analysis model revealed that asthma symptoms were associated with birth weight < 2,500 g (p < 0.001), day care center or nursery attendance (p < 0.002), maternal history of asthma (p < 0.001), contact with animals during the first year of life (p < 0.027), current contact with animals outside the home (dogs, cats, or farm animals; p < 0.005), and more than 20 cigarettes per day smoked by parents or other household members (p < 0.02). Conclusions:Exposure to animals in and outside the home is associated with asthma symptoms, as is environmental tobacco smoke exposure. Families, health professionals, and administrators of health care facilities should take that into account in order to prevent asthma and reduce asthma morbidity.

Visual analogue scale (VAS) as a monitoring tool for daily changes in asthma symptoms in adolescents: a prospective study

BackgroundSuccess in asthma management hinges on patients’ competency to detect and respond to ever-changing symptom severity. Thus, it is crucial to have reliable, simple, and sustainable methods of symptom monitoring that can be readily incorporated into daily life. Although visual analogue scale (VAS) has been considered as a simple symptom assessment method, its utility as a daily symptom monitoring tool in adolescents is unknown. This study was to determine the concurrent validity of VAS in capturing diurnal changes in symptoms and to examine the relationships between VAS and asthma control and pulmonary function.MethodsForty-two adolescents (12–17 years old) with asthma completed daily assessment of symptoms twice per day, morning and bedtime, for a week using VAS and 6-item symptom diary concurrently. Asthma control was measured at enrollment and 6 month later, and spirometry was conducted at enrollment. Pearson correlations, multilevel modeling and regression were conducted to assess the relationships between VAS and symptom diary, asthma control and FEV1.ResultsMorning and evening VAS was positively associated with symptom diary items of each corresponding time frame of the day (r = 0.41–0.58, p < 0.0001). Morning VAS was significantly predicted by morning diary data reflecting nocturnal wakening (β = 2.13, p = 0.033) and morning symptoms (β = 4.09, p = 0.002), accounting for 57% of the total variance of morning VAS. Similarly, changes in four evening diary items, particularly shortness of breath (β = 2.60, p = 0.028), significantly predicted changes in evening VAS, accounting for 55% of the total variance. Average VAS scores correlated with asthma control (r = 0.65, p < 0.001) and FEV1 (r = −0.38, p = 0.029), and were predictive of asthma control 6 months later (β = 0.085, p = 0.006).ConclusionsVAS is a valid tool capturing diurnal changes in symptoms reflected in a multi-item symptom diary. Moreover, VAS is a valid measure predicting concurrent and future asthma control. The findings suggest VAS can be a simple alternative to daily dairies for daily symptom monitoring, which can provide invaluable information about current and future asthma control without substantially increasing self-monitoring burdens for adolescent patients.Clinical Trial Registration NCT01696357. Assessing Asthma Symptoms in Adolescents and Adults: Qualitative Research Supporting Development of the Asthma Daily Symptom Diary

BackgroundDespite the widespread availability of patient-reported asthma questionnaires, instruments developed in accordance with present regulatory expectations are lacking. To address this gap, the Patient-Reported Outcome (PRO) Consortium’s Asthma Working Group has developed a patient-reported asthma daily symptom diary (ADSD) for use in clinical research to assess outcomes and support medical product labeling claims in adults and adolescents with asthma. ObjectivesTo summarize the qualitative research conducted to inform the initial development of the ADSD and to provide evidence for content validity of the instrument in accordance with the Food and Drug Administration’s PRO Guidance. MethodsResearch informing the initial development and confirming the content validity of the ADSD is summarized. This comprised a review of published qualitative research, semi-structured concept elicitation interviews (n = 55), and cognitive interviews (n = 65) with a diverse and representative sample of adults and adolescents with a clinician-confirmed diagnosis of asthma in the United States to understand the asthma symptom experience and to assess the relevance and understanding of the newly developed ADSD. ResultsFrom the qualitative literature review and concept elicitation interviews, eight core asthma symptoms emerged. These were broadly categorized as breathing symptoms (difficulty breathing, shortness of breath, and wheezing), chest symptoms (chest tightness, chest pain, and pressure/weight on chest), and cough symptoms (cough and the presence of mucus/phlegm). Conceptual saturation was achieved and differences in the experience of participants according to socio-demographic or clinical characteristics were not observed. Subsequent testing of the ADSD confirmed participant relevance and understanding. ConclusionsThe ADSD is a new patient-reported asthma symptom diary developed in accordance with the Food and Drug Administration’s PRO Guidance. Evidence to date supports the content validity of the instrument. Item performance, reliability, and construct validity will be assessed in future quantitative research.

Identify the factors associated with asthma symptoms in adolescents. Cross-sectional study with the participation of 1,176 students between the ages of 11 and 17 years old, in Salvador, Bahia, Brazil. The asthma symptoms were identified by using the standardized questionnaire from The International Study of Asthma and Allergies in Childhood Program (ISAAC), phase III. The subjects' food intake data were collected by a semiquantitative food frequency questionnaire (FFQ) comprised of 97 food items. All of the information was collected during a single interview. Anthropometric, socioeconomic, and demographic data were collected from the study population. The robust Poisson model was adopted to obtain the raw and adjusted PR (prevalence ratio). From the total number of adolescents, 57.6% are female and 83.71% of them were between 13 and 17 years old. The prevalence of asthma symptoms was of 7.6%, being higher among males (9.62%). The prevalence of wheezing in the last 12 months was 8.6%. Among the adolescents with rhinitis and eczema, the prevalence of asthma symptoms was of 15.2 and 14.3%, respectively. After an adjusted analysis, the following factors were identified: females PR = 0.64 (95%CI 0.42 - 0.96), rhinitis PR = 3.23 (95%CI 2.17 - 4.83) and the 2nd tertile of the healthy pattern (moderate consumption) PR = 0.61 (95%CI 0.37 - 0.99). Rhinitis and eczema were presented as risk factors for asthma symptoms. The healthy food intake pattern that had been stratified in tertiles, presented the 2nd tertile as a protector for asthma symptoms.

The Associations of Asthma Symptoms With Active and Passive Smoking in Hong Kong Adolescents

Tobacco smoke has detrimental effects on the respiratory system. This study investigated the associations of active and passive smoking with asthma symptoms in Hong Kong adolescents. A total of 6,494 Hong Kong secondary school students, with a mean ± SD age of 15.0 ± 1.21 years, participated in the Health Related Behavior General Survey in 2000-2001. They reported their demographic factors (sex, age, housing type, district of living), lifestyles (smoking, drinking, extracurricular sports, eating), and asthma symptoms (exercise-induced bronchospasm [EIB] and nocturnal cough) in the questionnaire. In addition, number of smoking parents (none/one/both) and presence of a smoking best friend (yes/no) were assessed. Logistic regression models were used to determine the odds ratios (OR) of frequently having the asthma symptoms for different smoking status of students, parents, and best friend, with adjustment for demographic factors and lifestyles. The prevalence of former, light, and heavy smokers was 17.5%, 7.7%, and 1.0%, respectively. Moreover, 35.1% of the participants had one and 3.8% had 2 parents who smoked. Heavy smokers were more likely to experience EIB with OR (95% CI) of 2.27 (1.30-3.97) and nocturnal cough with OR (95% CI) of 3.45 (1.52-7.81), as well as both symptoms with OR (95% CI) = 4.69 (1.88-11.73) when compared to those who never smoked. The corresponding OR (95% CI) for having at least one smoking parent and a smoking best friend was 1.45 (1.17-1.81), 1.61 (1.06-2.42), and 2.43 (1.37-4.31), when compared with those without a parent or best friend who smoked. Adolescents who are heavy smokers and having parents and a best friend who smoke are more likely than others to have asthma symptoms. Both active and passive smoking are related to asthma symptoms in adolescents.

Anxiety and Asthma Symptoms in Urban Adolescents with Asthma: The Mediating Role of Illness Perceptions

Twenty to 40% of adolescents with asthma experience significant symptoms of anxiety. This study examined the mediational role of illness perceptions in the relationship between anxiety and asthma symptoms in adolescents. One hundred fifty-one urban adolescents (ages 11-18) with asthma completed measures of illness perceptions, and anxiety and asthma symptoms. Using the Baron and Kenny approach and Sobel tests, we examined whether illness perceptions mediated the anxiety-asthma symptom relationship. Three illness perceptions significantly mediated the relationship between anxiety and asthma symptoms, z = 1.97-2.13, p < .05; adjusted R(2) = 0.42-0.51, p < .05. Greater anxiety symptoms were associated with perceptions that asthma negatively impacted one's life and emotions and was difficult to control. These negative illness perceptions were, in turn, related to greater asthma symptoms. Illness perceptions helped explain the anxiety-asthma symptoms link in adolescents. Results suggest that targeting illness perceptions in adolescents with asthma and anxiety may help reduce asthma symptoms.

AimOur study aimed to determine the rate of association of rhinoconjunctivitis and asthma symptoms in adolescents to analyse whether asthma symptoms are more severe and frequent in asthmatics with concomitant allergic rhinitis and assess if adolescents are aware of having rhinoconjunctivitis. MethodsA cross-sectional study, with two components: a study in prevalence and an inter-case study (rhinitis symptoms) with a comparison group (no rhinitis symptoms), based on information from questionnaires applied in phase 3 of ISAAC in Recife in 2002. ResultsAssociated rhinoconjunctivitis and probable asthma symptoms were observed in 5.1% of adolescents (48/940; CI 95%: 3.8%-6.6%), probable asthma alone in 10.9% (103/940; CI 95%: 9.1%-13.1%) and rhinoconjunctivitis alone in 9.7% (91/940; CI 95%: 7.9%-13%). Among the rhinitisbearing adolescents, almost 81.3% (39/48) had persistent probable asthma and 31.8% (48/151) of asthmatic patients rhinoconjunctivitis. 65.1% (86/132) of adolescents with diagnosed rhinitis were unaware of rhinitis symptoms. ConclusionsThe association of rhinoconjunctivitis and asthma symptoms is frequent and associated to more severe asthma symptoms. Adolescents’ unawareness of rhinitis symptoms reflects the underdiagnosis that can result in downplaying the symptoms, and the consequent undertreatment.

Symptom triggers in an adolescent asthma population

Rationale Triggers of asthma symptoms in adolescents and trigger awareness by their parent/guardians (P/G) have not been formally evaluated. Methods We conducted a cross-sectional study in 162 adolescents with asthma, recruited through the Baltimore City Schools and surrounding counties. Subjects underwent several evaluations including a questionnaire querying for asthma triggers and skin testing. The P/G independently responded to the same questionnaire. Results The most common triggers identified by the adolescents were: exercise 89%, colds/flu 83%, dust 71%, weather changes 71%, emotions 63%, grass 52%, cats 48%, cold air 47%, irritants 45% and dogs 35%. Foods were the most unusual triggers (e.g. milk 9%, seafood 8%, peanuts 3%). For the two most common triggers reported by the adolescents, concordance with the P/Gs' report was good (exercise 71%, colds/flu 70% concordance). However, concordance for dust, emotions, grass, cats, cold air, irritants, and dogs was rather poor (<53%). We also examined the concordance between the adolescents' responses and their skin testing results. Concordance between dust as a trigger and dust mite skin test was 64% (p=0.018), 71% for cat (p=0.0005), 55% for grass (p=0.41) and 44% for dog (p=0.29). The P/G responses were not better than the adolescents in predicting the skin test results. Conclusion A) Exercise and colds/flu are the most common reported triggers of asthma in adolescents and are recognized well by both patients and their P/Gs. B) With the exception of cat, allergic triggers identified by the adolescent or the PG frequently do not have a biologic basis.
