Contextualizing guidelines for the health system of Cyprus: Experiences and lessons learnt

AbstractBackgroundCyprus is undergoing a major health reform with the recent establishment of the General Healthcare System (GHS). The GHS offers equal healthcare access through one primary insurer (Health Insurance Organization [HIO]) and benefits from a wide collaborative network of public and private healthcare providers. However, unwanted variation in practice makes this transition challenging. Healthcare guidelines could decrease these variations in practice, but Cyprus lacks the capacity to develop them de novo. Through a collaboration with the National Institute for Health and Care Excellence (NICE) in the United Kingdom, the contextualization of NICE guidelines and the derivation of local quality indicators are carried out. This study presents the methodology and experience of contextualizing the first three NICE guidelines and deriving associated quality indicators in Cyprus.MethodsHIO managed the guideline contextualization with the support of a Guidelines Secretariat. For each guideline, a local topic expert committee (TEC) was recruited. Through a series of meetings, followed by public consultation, each TEC made contextual changes to the guideline and derived relevant quality indicators. During this process, NICE assured quality by overseeing several elements of the contextualization procedure such as TEC membership, proposed changes and justification and derived quality indicators.ResultsBetween 2022 and 2024, three NICE guidelines, NG196 on Atrial fibrillation (AF), NG230 on Thyroid cancer (TC) and NG203 on Chronic kidney disease (CKD), were contextualized through the modification of several individual guideline recommendations (21/79 [26.6%] in AF, 37/67 [55.2%] in TC and 62/217 [28.6%] in CKD). In parallel, NICE quality indicators were screened for applicability and feasibility in Cyprus while additional indicators were developed if required.ConclusionIn a country with limited experience in guideline development, a supervised and systematic process supported by an established organization ensures quality, is less resource intensive and builds capability for the sustainability of the process.

Regulatory Compliance, Ethical Considerations, Audit Quality And Transparency In Modern Auditing And Assurance

This study is a conceptual review that investigates regulatory compliance, ethical considerations, audit quality and transparencyonthe dynamics of Auditing and Assurance in the Nigerian contextin the modern world by studying the challenges hindering their effective implementations. With a methodology based on critical review of related literatures through content analysis to identify key areas of development, patterns, trends and problems facing significant concepts in the auditing profession, the research seeks to point out how technology and new discoveries have influence the auditing profession. Findings reveal that while integrity and objectivity are prioritized, external pressures, inadequate training, and resource constraints impede the full implementation of regulatory compliance, ethical consideration and transparent quality audit. The study therefore,endorsedincreased auditor training, stronger enforcement of ethical guidelines, and improved resource allocation to enhance compliance, financial transparency and stakeholders’ trust

Structural Basis for the Pathogenicity of Parkin Catalytic Domain Mutants

Mutations in the E3 ubiquitin ligase parkin cause a familial form of Parkinson’s disease (PD). Parkin and the mitochondrial kinase PINK1 assure quality control of mitochondria through selective autophagy of mitochondria (mitophagy). Whereas numerous parkin mutations have been functionally and structurally characterized, several PD mutations found in the catalytic Rcat domain of parkin remain poorly understood. Here, we characterize two pathogenic Rcat mutants, T415N and P437L. We demonstrate that both mutants exhibit impaired activity using autoubiquitination and ubiquitin vinyl sulfone assays. We determine the minimal ubiquitin binding segment and show that both mutants display impaired binding of ubiquitin charged on the E2 enzyme. Finally, we use AlphaFold 3 to predict a model of the phospho-parkin:phospho-ubiquitin:ubiquitin-charged E2 complex. The model shows the repressor-element of parkin (REP) and the N-terminal residues of the catalytic domain form a helix to position ubiquitin for transfer from the E2 to parkin. Our results rationalize the pathogenicity of the parkin mutations and deepen our understanding of the active parkin-E2∼Ub complex.

Implementing FMEA (Failure Mode and Effect Analysis) in a canned corn manufacturing industry: a case study

Assuring quality and food safety is challenging for many companies, as they affect the sustainability of the business. it was identified lack of studies focused on applying risk assessment tools in the food industry by reviewing the literature. This study aims to describe the implementation of Failure Mode and Effect Analysis (FMEA) to identify potential risks and how to prevent them, to assure quality requirements in a corn food plant. This paper follows a case study approach, based on a qualitative methodology. During the execution of FMEA, the hazards and mitigation controls were identified. The results provide evidences of how FMEA can be effective by showing the vulnerabilities of production flow, controls to implement and how to prioritize actions. FMEA contributed to managing risks found for the company where the case study was conducted. This case study can be followed by other companies and different processing technologies, extrapolating the food industry. Key words: FMEA; quality; hazard; corn; food industry.

The purpose of regulatory departments or pro-fessionals is to develop and promote heavily controlled therapeutic or pharmaceutical goods and build a platform for marketing approval. In addition to this, RA in the pharmaceutical industry is comparatively a new field to safeguard public well-being by monitoring the safety and efficacy of the products [3]. Furthermore, in the late 1950s, multiple tragedies resulted in an increase in legislation for pharmaceutical goods’ quality, safety, and efficacy. As a part of this, strict norms were made for Marketing Authorization (MA) and Good Manufacturing Practice (GMP) [4]. The present article covers the regulatory affairs’ responsibilities and importance in the context of pharmaceutical industries.

The findings are robust to various robustness and endogeneity checks and are of great importance for researchers, regulators and business practice. Originality/value This analysis mainly contributes to previous studies by using a combined sustainable board governance score as a major driver of corporate carbon assurance practices for the first time.

A large-scale dataset for korean document-level relation extraction from encyclopedia texts

Document-level relation extraction (RE) aims to predict the relational facts between two given entities from a document. Unlike widespread research on document-level RE in English, Korean document-level RE research is still at the very beginning due to the absence of a dataset. To accelerate the studies, we present TREK (Toward Document-Level Relation Extraction in Korean) dataset constructed from Korean encyclopedia documents written by the domain experts. We provide detailed statistical analyses for our large-scale dataset and human evaluation results suggest the assured quality of TREK . Also, we introduce the document-level RE model that considers the named entity-type while considering the Korean language’s properties. In the experiments, we demonstrate that our proposed model outperforms the baselines and conduct qualitative analysis.

The Therapeutic Potential of Essential Oils in Managing Inflammatory Skin Conditions: A Scoping Review.

Conventional therapy is commonly used for the treatment of inflammatory skin conditions, but undesirable effects, such as erythema, dryness, skin thinning, and resistance to treatment, may cause poor patient compliance. Therefore, patients may seek complementary treatment with herbal plant products including essential oils (EOs). This scoping review aims to generate a broad overview of the EOs used to treat inflammatory skin conditions, namely, acne vulgaris, dermatitis and eczema, psoriasis, and rosacea, in a clinical setting. The quality, efficacy, and safety of various EOs, as well as the way in which they are prepared, are reviewed, and the potential, as well as the limitations, of EOs for the treatment of inflammatory skin conditions are discussed. Twenty-nine eligible studies (case studies, uncontrolled clinical studies, and randomized clinical studies) on the applications of EOs for inflammatory skin conditions were retrieved from scientific electronic databases (PubMed, Embase, Scopus, and the Cochrane Library). As an initial result, tea tree (Melaleuca alternifolia) oil emerged as the most studied EO. The clinical studies with tea tree oil gel for acne treatment showed an efficacy with fewer adverse reactions compared to conventional treatments. The uncontrolled studies indicated the potential efficacy of ajwain (Trachyspermum ammi) oil, eucalyptus (Eucalyptus globulus) oil, and cedarwood (Cedrus libani) oil in the treatment of acne, but further research is required to reach conclusive evidence. The placebo-controlled studies revealed the positive effects of kānuka (Kunzea ericoides) oil and frankincense (Boswellia spp.) oil in the treatment of psoriasis and eczema. The quality verification of the EO products was inconsistent, with some studies lacking analyses and transparency. The quality limitations of some studies included a small sample size, a short duration, and the absence of a control group. This present review underscores the need for extended, well-designed clinical studies to further assess the efficacy and safety of EOs for treating inflammatory skin conditions with products of assured quality and to further elucidate the mechanisms of action involved.

Analysis of the Quality of Employee Performance on Customer Decision-Making on Bank Syariah Indonesia Murabahah Financing

This study aims to determine the effect of service quality on customer decisions at Indonesian Sharia Banks in the city of Lhokseumawe. The sampling technique used was purposive sampling technique. The number of samples is 100 people in the city of Lhokseumawe. Research data were analyzed using PLS-SEM (Partial Least Square – Structural Equation Modeling) with SmartPLS 3 software. The results showed that service quality of compliance, assurance, reliability, physical evidence, empathy, and responsiveness had a positive effect on customer decisions of Indonesian Sharia Commercial Banks in the city of Lhokseumawe. Only two variables had a positive and significant effect, namely compliance and responsiveness, the remaining four variables, namely assurance, reliability, tangibles, and empathy, had a positive but not significant effect. The implication of this research is to focus on compliance, assurance, reliability, physical evidence, empathy, and responsiveness which can influence customer decisions at Bank Syariah Indonesia in the city of Lhokseumawe.

Impact of Achieving Hospital Accreditation on Assuring Quality in Clinical Laboratories: Experience of an Oncology Hospital in low middle income country

Abstract Introduction/Objective Shefaa Al-Orman Oncology Hospital (SOH) is a leading cancer care facility in Upper Egypt dedicated to providing specialized treatment to cancer patients for free. The General Authority of Healthcare Accreditation and Regulation (GAHAR, accredited from the International Society for Quality in Health Care) is the regulatory and accreditation body that ensures that healthcare facilities in Egypt comply with the highest standards of quality and safety. Here, we aim to assess the impact of GAHAR accreditation process on: Streamlining workflow, contributing to staff professional development and knowledge, increasing loyalty, and creating a more productive and fair work environment. - Optimizing turnaround time (TAT). - Compliance with the minimum retesting interval (MRI) guidelines. - Healthcare providers' satisfaction with the laboratory services. - Patients’ satisfaction. Finally, we aim to emphasize the role of building an integrated lab quality management system that addresses different lab processes/procedures as a basis for la accreditation. Methods/Case Report laboratory personnel, healthcare providers and patients responses were measured using questionnaire method to assess the impact of accreditation on various aspects of laboratory services. Comparative lab statistics were collected from laboratory information system. Results (if a Case Study enter NA) As shown by questionnaires, 95% of lab staff reported positive impact on streamlining of workflow, 100% reported in professional development and improvement in knowledge, and increased loyalty and created a fair work environment (80% in each). Healthcare providers satisfaction reached 88%, and patient satisfaction reached 93%. TAT optimization increased from 84.1% to 98.1%. Prior to accreditation, the lab had an effective internal quality control program. GAHAR mandated enrollment in External Quality Assurance Services (EQAS) to ensure reliable results. In addition, GAHAR mandated a policy/procedure to enforce minimum retesting interval. Application of this policy led to better utilization. Finally, pre and post-analytical processes were streamlined, leading to improved overall laboratory performance. Conclusion Achieving GAHAR accreditation had positive impact on laboratory workflow, staff professional development, compliance with guidelines, ensure reliable laboratory results, and overall laboratory performance. Laboratories should consider accreditation and implementing quality assurance measures to enhance the delivery of high-quality laboratory services and patient safety.

This research discusses the application of Business Model Canvas (BMC) on Seventeen Voucher service business before and after implementation, using SWOT analysis to identify strengths and weaknesses. The study aims to provide insights for business owners in developing effective business strategies and improving customer satisfaction. The data was collected through observation, in-depth interviews, and documentation, and analyzed using qualitative and SWOT analysis methods.The results show that the target customer segment is students in and around the campus, the value proposition is providing convenient services for internet-related needs, and the channels used are WhatsApp Business andFacebook. The study also identifies areas for improvement in customer relationship management and cost structure.The research discusses the evaluation of the Business Model Canvas (BMC) implementation in Seventeen Voucher service business and its analysis using SWOT analysis. The study found that affordable prices, promotions, and social media usage for promotion are factors that attract customers. Quality of service, reliability, responsiveness, and assurance are also important factors in customer satisfaction. Pricing is determined by observing the market and adjusting to customerdemand. The BMC helps simplify and communicate business strategies effectively. Overall, the study provides valuable insights for business owners in developing effective business strategies and improving customer satisfaction. The use of the BMC and SWOT analysis methods can help businesses identify their strengths and weaknesses, and develop strategies to improve their performance. Understanding Hotel’ S Buffet Restaurant Assurance After Covid-19: A Case of Thailand

Purpose: The aim of this study is to examine tourist expectations and perceptions of hotel’s buffet restaurant assurance to obtain an overall understanding of restaurant assurance after COVID-19 situation. Theoretical framework: The concept of SERVQUAL, particularly assurance dimension, is employed to examine tourist expectations and perceptions of hotel’s buffet restaurant assurance. The study expects to expand the literature in the area of service quality of restaurant assurance. Design/methodology/approach: The questionnaire survey is employed to collect data from 384 participants through a convenience sampling method. Factor analysis and t-test are used to analyse the data. Findings: The results indicate that there are two factors associated with hotel’s buffet restaurant assurance, and they are identified as: 1) employee courtesy, knowledge, and service confidence and 2) sanitary restaurant environment. Both factors are related to common and new assurance attributes of buffet restaurant services. Practical & Social implications: The stuyd has expanded the knowledge of hotel’s buffet restaurant assurance based on the current situation. Assurance is a key element of several models used to assess restaurant service quality. The current finding helps to update the literature as well as assists restaurant managers to thoroughly understand tourists’ trust and confidence when dining in hotel buffet restaurants. Originality: The value of the study is the identfication of the two factors associated with hotel’s buffet restaurant assurance. This is an initial study to integrate the new normal services into restaurant’s new assurance attributes by examining tourist expectations and perceptions of hotel’s buffet restaurant assurance based on the current situation (after COVID-19).

Central obesity and its associated factors among cancer patients at the University of Gondar Comprehensive Specialized Hospital, Northwest Ethiopia.

Obesity, especially the hidden type of obesity (central obesity), has been believed to be the major risk factor for developing and progressing non-communicable diseases, including cancers. However, there are limited studies regarding the issue in Ethiopia and the study area. Therefore, this study aimed to evaluate the magnitude of central obesity and its associated factors among cancer patients visited the oncology unit of the University of Gondar Comprehensive Specialized Hospital. An institutional-based cross-sectional study was conducted from January 10 to March 10, 2021.A total of 384 study participants were enrolled using a systematic sampling technique. The data were collected using a semi-structured interviewer-administered questionnaire and were pretested to address the quality of assurance. The weight of the participants was assessed using body mass index (BMI) and central obesity. Both bivariate and multivariate logistic regressions were conducted to identify the factors associated with central obesity, and p-values less than 0.05 with multivariate were considered statistically significant associations. Most respondents (60.16%) were stage I cancer patients. The study found that about 19.27% of the participants were prevalent central obesity, and none of them were obese by body mass index (BMI) categorization criteria. However, about 12.24% and 7.03% of the participants were found to be underweight and overweight, respectively. The variables associated with central obesity were sex (AOR=14.40; 95% CI: 5.26 - 39.50), occupation (AOR=4.32; 95%CI: 1.10 - 17.01), and residency (AOR=0.30; 95% CI: 0.13 - 0.70). A significant number of the respondents (19.27%) were centrally obese. Being female, urban residency and having an occupation other than a farmer, merchant, and governmental were the factors associated with central obesity. Hence, cancer patients may be centrally obese with average body weight.

Kualitas Pelayanan Model CARTER Dan Pengaruhnya Terhadap Kepuasan Nasabah Bank Syariah Indonesia Kota Lhokseumawe Dengan Pendekatan SEM-PLS

This study aims to determine the effect of service quality with the CARTER model on customer satisfaction at Bank Syariah Indonesia in the city of Lhokseumawe. The sampling technique used was purposive sampling technique. The number of samples is 100 people who are in the city of Lhokseumawe. The research data were analyzed using PLS-SEM (Partial Least Square – Structural Equation Modeling) with SmartPLS 3 software. The results showed that the service quality of compliance, assurance, reliability, tangibles, empathy, and responsiveness had a positive influence on customer satisfaction of Islamic Banks. Indonesia in the city of Lhokseumawe. Only two variables have a positive and significant effect, namely compliance and responsiveness variables, the remaining four variables, namely assurance, reliability, tangibles, and empathy, have a positive but not significant effect. The implication of this research is to focus on compliance, assurance, reliability, tangibles, empathy, and responsiveness that can affect customer satisfaction at Bank Syariah Indonesia in the city of Lhokseumawe. The results of this study have differences and similarities with previous studies. The difference with previous research is that it uses the measurement of service quality with the Servqual dimension and the similarity that both use the CARTER dimension.

Applying Rapid Qualitative Analysis for Health Equity: Lessons Learned Using "EARS" With Latino Communities.

Qualitative research amplifies the voices of marginalized communities and thus plays a critical role in shaping our understanding of health inequities and their social determinants. Traditional qualitative approaches, such as grounded theory or thematic analysis, require extensive training and are time- and labor-intensive; as such, they may not be adequately suited to address healthy equity issues that require a swift response. Rapid qualitative analysis (RQA) is an action-oriented approach to qualitative data analysis that may be used when findings are needed to quickly inform practice. RQA capitalizes on using a team to summarize key points from qualitative data into matrices to explore relevant themes efficiently and systematically. In this paper, we provide case examples from our work applying RQA to health equity research with Latino communities to address community needs, such as responses to public health emergencies and the development of service delivery and technology interventions for infectious and chronic diseases. We draw from our collective experiences to share lessons learned and provide the following specific recommendations ("EARS") to researchers interested in applying RQA for health equity research: (1) Employ RQA to address rapidly evolving, urgent, health equity challenges; (2) Assure quality and rigor throughout the RQA process; (3) Respond to barriers and problem-solve as needed; and (4) Strengthen community relationships before, during, and after using RQA. Overall, we advocate for the use of RQA to promote health equity due to its ability to integrate the vital perspectives of marginalized communities and efficiently respond to their needs.

This study aims to determine the quality of state assurance in the sustainability report of companies in Indonesia. Using agency theory, legitimacy theory, stakeholder theory, assurance statement and ISAE 3000 & AA1000AS. The object of this study is a company listed on the Indonesia Stock Exchange (IDX) participants and winners of the Asia Sustainabilty Report Rating (ASRRAT) for the 2017-2021 period which publishes a complete and sequential sustainability report. The research design used is a descriptive study. The results of the analysis show that the quality of assurance statements in Indonesia, assurance statements issued by assurance providers for winning companies and participants of the Asia Sustainability Report Rating (ASRRAT) for the 2017-2021 period and the quality of assurance statements issued by assurance providers.

Evolution of δ phase precipitates in Inconel 625 superalloy additively manufactured by laser powder bed fusion and its modeling with fuzzy logic

Experimental and modeling studies of the evolution of plate-like δ phase precipitates in Inconel 625 superalloy additively manufactured by the laser powder bed fusion process are performed. The maximum Feret diameter and the number of particles per unit area are used as parameters describing the size and distribution of the δ phase precipitates. On the basis of microstructural analysis and quantitative image analysis, the effect of time and temperature on the development of δ phase precipitates is determined. The distinct differences in the intensity of precipitation, growth, and coarsening of the δ phase precipitates during annealing at temperatures of 700 and 800 °C up to 2000 h are shown. The experimental results are compared with computational data obtained by thermodynamic modeling. Using the experimentally determined parameters of the δ phase precipitates in different variants of annealing, a fuzzy logic-based phase distribution model is designed. Since the quantity of available data was too small to train a model with the machine learning approach, expert knowledge is used to design the rules, while numerical data are used for its validation. Designed rules, as well as reasoning methodology are described. The proposed model is validated by comparing it with the experimental results. It can be used to predict the size and number density of the δ phase precipitates in the additively manufactured Inconel 625, subjected to long-term annealing at temperatures of 700–800 °C. Due to limited experimental data, the quality of assurance is not perfect, but warrants preliminary research.Graphical abstract

Financial burden and coping strategies for cancer care in India

ProblemGlobally 9.3 million deaths occurred due to cancer in 2018. Currently, India has 13.9 lakh cancer cases, which are estimated to increase by 12% by the year 2025. Treatment of cancer inflicts a heavy cost of care and may even impoverish households. MethodsUsing the national health survey data for 2017–18, this study estimates the burden of healthcare expenditure due to cancer and coping strategies to pay for treatment. Factors affecting the health care expenditure on cancer are examined using a two-part model. FindingThe monthly values (in 2022 prices) of inpatient and outpatient OOPE on cancer for 2017-18, were found to be ₹6549 and ₹8811 implying that, 37% and 49% of household's monthly consumption expenditure was spent on inpatient and outpatient cancer care, respectively. Households relied on their own income/savings to pay for care and hardship financing was faced even by the higher income quintile patients both for inpatient and outpatient care. The second richest quintile had the greatest odds of borrowing money (0.94[0.54–1.63]).The two-part model shows that the likelihood of incurring expenditure on cancer care is greater at higher age-groups and income quintiles and is lower for females and people seeking care at private facilities. However, the mean expenditure is higher for those using private facilities or belonging to richer quintiles and is lower in urban areas. ConclusionThere is a need for policies to impart financial protection and expand the screening and curative services for cancer, with an assured quality in the public sector to ameliorate the financial burden of cancer care among households in India.
