A phenome-wide association and factorial Mendelian randomization study on the repurposing of uric acid-lowering drugs for cardiovascular outcomes

Uric acid has been linked to various disease outcomes. However, it remains unclear whether uric acid-lowering therapy could be repurposed as a treatment for conditions other than gout. We first performed both observational phenome-wide association study (Obs-PheWAS) and polygenic risk score PheWAS (PRS-PheWAS) to identify associations of uric acid levels with a wide range of disease outcomes. Then, trajectory analysis was conducted to explore temporal progression patterns of the observed disease outcomes. Finally, we investigated whether uric acid-lowering drugs could be repurposed using a factorial Mendelian randomization (MR) study design. A total of 41 overlapping phenotypes associated with uric acid levels were identified by both Obs- and PRS- PheWASs, primarily cardiometabolic diseases. The trajectory analysis illustrated how elevated uric acid levels contribute to cardiometabolic diseases, and finally death. Meanwhile, we found that uric acid-lowering drugs exerted a protective role in reducing the risk of coronary atherosclerosis (OR = 0.96, 95%CI: 0.93, 1.00, P = 0.049), congestive heart failure (OR = 0.64, 95%CI: 0.42, 0.99, P = 0.043), occlusion of cerebral arteries (OR = 0.93, 95%CI: 0.87, 1.00, P = 0.044) and peripheral vascular disease (OR = 0.60, 95%CI: 0.38, 0.94, P = 0.025). Furthermore, the combination of uric acid-lowering therapy (e.g. xanthine oxidase inhibitors) with antihypertensive treatment (e.g. calcium channel blockers) exerted additive effects and was associated with a 6%, 8%, 8%, 10% reduction in risk of coronary atherosclerosis, heart failure, occlusion of cerebral arteries and peripheral vascular disease, respectively. Our findings support a role of elevated uric acid levels in advancing cardiovascular dysfunction and identify potential repurposing opportunities for uric acid-lowering drugs in cardiovascular treatment.

Association of uric acid with length of stay and mortality in pediatric hospitalized population

PurposeElevated uric acid (UA) levels have been associated with acute and chronic diseases, which could affect the prognosis of pediatric hospitalized patients. However, the association of UA levels with length of hospital stay (LOS) and mortality in hospitalized children and adolescents remains unknown. Therefore, the aim of this study was to evaluate the association of serum UA levels with in-hospital mortality and prolonged LOS in hospitalized children and adolescents. MethodsA retrospective cohort study was conducted, involving 128 patients under 18 years of age, admitted to a tertiary-care hospital between January 2014 and December 2018. UA levels were assessed with an average of 3 days before the in-hospital outcome (discharge or death). Logistic regression was used to determine the association of UA with prolonged LOS (defined as over 30 days of hospitalization), while Cox regression multivariate analysis was employed to assess UA as a predictor of in-hospital mortality. ResultsUA levels showed an inverse association with prolonged LOS. Specifically, for every 1 mg/dL increase in UA level, the odds of experiencing prolonged LOS decreased by 31% (OR=0.69; 95% CI: 0.50 – 0.95). Additionally, individuals with elevated UA levels had lower odds of prolonged LOS (OR = 0.23; 95% CI: 0.08 - 0.66). However, UA levels were not associated with in-hospital mortality (HR = 1.63; 95% CI: 0.94 – 2.82). ConclusionSerum UA is inversely associated with LOS among children and adolescents, but no association was observed with in-hospital mortality.

Correlation of serum uric acid levels with certain anthropometric parameters in prediabetic and drug-naive diabetic subjects.

Uric acid is produced during the metabolism of nucleotide and adenosine triphosphate and contains the final product of human purine metabolism. It acts both as an antioxidant and pro-inflammatory marker and has a positive association with visceral fat in overweight subjects. The aim of the present study is to find an association of uric acid level with certain anthropometric parameters in subjects having type 2 diabetes. The study included 124 urban drug-naive diabetic Indian subjects above 18 years of age from the general population of the city of North India. Uric acid concentrations were estimated by the uricase method. Fasting plasma glucose (FPG) concentrations were estimated by the glucose oxidase-peroxidase method. Anthropometric measurements and information on lifestyle factors and disease history were collected through in-person meeting. All participants of the study subjects had a body mass index (BMI) of more than 23.5. BMI, waist-to-hip ratio (WHR), waist-to-height ratio, waist circumference, neck circumference, weight, age, sagittal abdominal diameter (SAD), skinfold thickness, and body roundness index were positively correlated with the serum uric acid level. The correlation of weight, BMI, SAD, and WHR was statistically significant. We found that serum uric acid level increases as body fat content increases. Statistical data show remarkable results for a significant correlation of uric acid level with BMI, WHR, SAD, and FPG. Hypertrophy occurs as a result of inflammatory processes and oxidative stress when the supply of energy starts to exceed the storage capacity of adipocytes, as a result, adipokines such as interleukin (IL)-1, IL-6, and tumor-necrosis factor-alpha are released more frequently which lead to low-grade chronic inflammation. Uric acid levels are much lean toward visceral obesity than overall body fat content.

Association between uric acid and depressive symptoms in older adults: The Korea National Health and Nutrition Examination Survey.

Uric acid is known for its antioxidant capacity; however, whether uric acid is independently associated with depression in the older population remains controversial. The current study aimed to investigate the association between uric acid and depressive symptoms in older adults according to sex, using a large national sample population. Data from the 2016, 2018, and 2020 Korean National Health and Nutrition Examination Surveys were analyzed, and 5609 participants older than 60years were included in this study. We defined a Patient Health Questionnaire-9 score ≥5 as indicating the presence of depressive symptoms. The prevalence of depressive symptoms was higher in women with lower uric acid levels than in those with higher uric acid levels. In women, the lower levels of uric acid were significantly associated with depressive symptoms in the multivariable logistic regression analysis (odds ratio, 1.36; 95% confidence interval, 1.10-1.68; p=0.005). However, no significant association between uric acid levels and depressive symptoms was observed in men. The findings of this study suggest that uric acid is associated with depressive symptoms in older women, but not in men. Relatively lower serum uric acid levels in women compared to men and sex differences in oxidative stress may explain the significant association of uric acid levels and depressive symptoms in older women. Further research is needed on sex differences in the relationship between serum uric acid levels and depressive symptoms.

Role of Serum Uric Acid in Metabolic Syndrome: A Cross-sectional Study in Central Karnataka

Abstract Background: Elevated levels of uric acid are the end product of purine metabolism. Uric acid levels are regulated by xanthine oxidase enzyme. Increased xanthine oxidase activity results in the generation of free radicals. Hence, elevated uric acid levels are the manifestations of oxidative stress. Meanwhile, raised uric acid can trigger release of inflammatory mediatory and create the inflammatory state. Thus, inflammation and oxidative stress are the important events in the pathogenesis of metabolic syndrome. This study aimed to find the association of uric acid levels with metabolic syndrome and the individual components of metabolic syndrome. Methodology: A comparative, descriptive, cross-sectional study was conducted among 50 metabolic syndrome cases and 50 controls. Clinical examination followed by laboratory investigations for the estimation of serum uric acid, fasting blood sugar, postprandial blood sugar, glycated hemoglobin, high sensitivity C-reactive protein (Hs-CRP), fasting insulin, and lipid profile. Insulin resistance (IR) was calculated by homeostatic model assessment-IR (HOMA-IR). Results: In the metabolic syndrome patients’ group, serum uric acid was found to have strong positive correlation with body mass index, blood pressure, glycemic indices, IR, lipid profile, and Hs-CRP. It was found that there was 2.5 times higher risk of development of metabolic syndrome with every unit increase in serum uric acid levels and this association was found to be statistically significant. Conclusion: Uric acid can be used as the marker for the early identification of metabolic syndrome or as a prognostic marker in the diagnosed cases of metabolic syndrome.

Effects of Uric Acid on Disease Severity and Mortality in Hospitalized Covid-19 Patients

Objective: High and low uric acid (UA) levels in the general population are associated with mortality. Information on the association of UA levels with clinical outcomes in COVID-19 patients is contradictory. We investigated the relationship between UA levels and clinical endpoints in COVID-19 patients.Methods: Laboratory and clinical parameters, including UA at the admission of hospitalized COVID-19 patients, were recorded retrospectively. Binary logistic regression analysis determined risk factors for mortality and the intensive care unit (ICU) needs. Results: This study included 708 patients (57.1% men), and the median age was 63 (18-98) years. Two hundred and three (28.7%) patients needed ICU, and 107 (15.7%) died. Uric acid levels were significantly higher in the deceased (6.5 vs. 4.9; p<0.001). Uric acid levels were similar in patients who needed ICU and those who did not (5 vs. 5.1; p=0.348). High UA (>median value 5.1 mg/dL) group have higher mortality rate (22.4% vs. 9.5%; p<0.001). In multivariate analyses, a high UA level was a risk factor for mortality [OR 1.93 (1.08 – 3.44); p=0.026]. In addition, age [OR 1.03 (1.01 – 1.05); p=0.004], albumin [OR 0.30 (0.17 - 0.52); P<0.001], neutrophil-to-lymphocyte ratio [OR 1.04 (1.01 – 1.06); p=0.003] and procalcitonin [OR 1.06 (1.0 – 1.11); p=0.048] was associated with mortality. A high UA level was not a risk factor for ICU need (p=0.780).Conclusion: High serum UA level affects mortality in COVID-19 patients. Risk assessment for the prognosis of patients can be made according to the UA levels at admission.

A systematic review and meta-analysis of the association between uric acid levels and chronic kidney disease

The function of uric acid (UA) in the genesis and evolution of chronic kidney disease (CKD) has motivated numerous studies, but the results remain inconclusive. We sought to conduct a systematic review and meta-analysis of cohort studies aiming to analyze the association of UA levels with the incidence and progression of CKD. Pubmed/Medline, Lilacs/Bireme and Web of Science were searched to identify eligible studies, following the PRISMA protocol. Data were presented for CKD incidence and progression separately. For the meta-analysis, studies with data stratified by subgroups according to serum UA levels were selected. The inverse variance-weighted random effects model was used to generate a combined effect estimate. Meta-regressions were performed to identify the causes of heterogeneity. The Newcastle–Ottawa Scale was used to assess the risk of bias. The publication bias was tested by funnel plot and Egger’s test. Eighteen CKD incidence studies (n = 398,663) and six CKD progression studies (n = 13,575) were included. An inverse relationship was observed between UA levels and protection from CKD incidence and progression. Lower UA levels were protective for the risk of CKD incidence (RR 0.65 [95% CI 0.56–0.75]) and progression (RR 0.55 [95% CI 0.44–0.68]). UA seems to be implicated both in the genesis of CKD and its evolution.

Uric Acid: Friend or Foe?

Introduction. Adverse Pregnancy outcomes (APO) continue to trouble physicians worldwide, and uric acid has become a marker of interest in its prediction. Despite this, the correlation of uric acid in the third trimester is rarely studied, and the reference value for normal uric acid levels in pregnancy has yet to be established. This study was done to evaluate the association of uric acid levels in the third trimester of pregnancy and the development of APO, specifically gestational hypertension (GH), pre-eclampsia (PE), gestational diabetes mellitus (GDM), small for gestational age (SGA) and to determine a cut-off value of serum uric acid levels for screening of the APO. Materials and methods. A retrospective observational study was conducted among 374 women who delivered at Pusat Perubatan Universiti Kebangsaan Malaysia (PPUKM) and developed the APO of interest and had serum uric acid levels taken during the third trimester of pregnancy. A cut off value of 309 µmol/L was used to assess the sensitivity and specificity in predicting the APO. Results. Associations are observed between 309 µmol/L and the development of GH (p<0.0005), PE (p<0.0005), GDM (p<0.0005), SGA (p=0.0264). Plotted ROC curves with cutoff point of 309 µmol/L demonstrated moderate predictive value of serum uric acid and the development of GH (AUC 0.639), PE (AUC 0.713), SGA (AUC 0.649) and low predictive value for GDM (AUC 0.4). Conclusion. Serum uric acid level of 309 µmol/L in the third trimester of pregnancy is a moderate predictor of GH, PE, SGA and a poor predictor of GDM.

Uric Acid and P-Wave Dispersion in Subjects with Heart Failure

Uric acid used as a marker of cardiovascular risk, which is associated with oxidative stress and inflammation. P-wave dispersion (PWD) is an electrocardiographic measure, which shows heterogeneity of atrial depolarization. It has demonstrated that there is an association of increased PWD with atrial fibrillation. This article's goal was to investigate the relationship between PWD and uric acid in subjects with heart failure (HF). Methods This descriptive, cross-sectional study included a total of 315 stable HF outpatients. The subjects were classified into two groups by their PWD: the normal PWD group consisted of subjects with a PWD lower than 40 ms (n = 201), and the increased PWD group consisted of subjects with a PWD higher than or equal to 40 ms (increased PWD) (n = 114). Results Significantly higher uric acid levels determined in the increased PWD group, in comparison with the normal PWD group (7.4 ± 1.6 mg/dL, vs. 6.5 ± 1.6 mg/dL p < 0.001). Univariate analyses revealed an association between uric acid, blood urea nitrogen, systolic pulmonary artery pressure, left atrial diameter, and increased PWD. In multivariate logistic regression analysis, there was an association of uric acid level (OR: 1.293; 95% CI: 1.106-1.511, p: 0.001), systolic pulmonary artery pressure (OR: 1.027; 95% CI: 1.011-1.044, p: 0.001), and left atrial diameter (OR: 1.754; 95% CI: 1.028-2.992, p: 0.039) with PWD. The receiver operating characteristics (ROC) curve analysis demonstrated that the optimal cut-off level of uric acid for predicting increased PWD was ≥ 7.1 mg/dL with a specificity of 77 percent and a sensitivity of 81 percent. Conclusion Our study results suggest that serum uric acid levels independently correlated with PWD in subjects with HF.

Association of Uric Acid Level With Severe Preeclampsia in Pregnancy Women in RSUD Dr. H Abdul Moeloek Province Lampung

 Pendahuluan: Preeklampsia adalah suatu sindrom kehamilan setelah umur kehamilan 20 minggu dan merupakan salah satu kegawatdaruratan obstetrik. Dibutuhkan suatu deteksi dini untuk diagnosis preeklampsia sehingga dapat menghindari prognosis yang buruk dari preeklampsia. Asam urat selama kehamilan merupakan salah satu biomarker preeklampsia yang dapat digunakan sebagai penilaian diagnosis preeklampsia. Tujuan penelitian ini untuk mengetahui hubungan kadar asam urat yang dengan kejadian preeklampsia berat di RSUD Dr. H. Abdul Moeloek Provinsi.
 Metode: Rancangan pada penelitian ini adalah case control. Populasi kasus adalah ibu hamil dengan preeklampsia dan populasi kontrol adalah ibu hamil tanpa preeklampsia. sampel kasus sebanyak 31 responden dan kasus kontrol sebanyak 31 responden. Data preeklampsia didapatkan pemeriksaan sedangkan kadar asam uat didapatkan dari pemeriksaan laboratorium. Analisis bivariat menggunakan Chi Square.
 Hasil: Pada ibu hamil dengan preeklampsia memiliki kadar asam urat tinggi (74,2%) dibandingkan kelompok kontrol (tanpa preeklampsia) yaitu (35,5%). Berdasarkan analisis bivariat, didapatkan nilai p =0,0005 dan OR 5,227 (CI 95% =1,7-15,5).
 Pembahasan: Kegagalan remodeling arteri spiralis yang abnormal pada preeklampsia menyebabkan iskemik seluler plasenta yang akan meningkatkan aktivitas Xantin Oksidase yang dapet meningkatkan produksi asam urat dari jaringan ibu, janin atau plasenta.
 Simpulan: Terdapat hubungan kadar asam urat yang tinggi selama kehamilan dapat meningkatkan risiko preeklampsia sebesar 5 kali bila dibandingkan dengan kadar asam urat normal selama kehamilan.

Hyperuricemia: A predictor of severe obstructive sleep apnea in a Tunisian population with atrial fibrillation?

Obstructive sleep apnea (OSA) is involved in initiation of atrial fibrillation (AF). Uric acid has emerged as an independent marker of morbidity and mortality in several cardiovascular pathologies. But there are few data addressing the association of uric acid levels and OSA in AF patients. The purpose of this study was to investigate whether the levels of serum uric acid were correlated with OSA severity in Tunisian patients with AF. It was a study which included 100 patients followed for non-valvular AF. Non-inclusion criteria were: gout and/or treatment with allopurinol, chronic lung disease with hypoxemia and severe chronic kidney disease. All patients underwent overnight polygraphy. Fasting blood samples were obtained to dose serum uric acid. OSA's diagnosis was retained in 90% of patients: mild in 32% of cases, moderate in 27% of cases and severe in 31% of cases. The mean value of serum uric acid was 61.05 ± 19.4 mg/L. Hyperuricemia was found in 31 patients. Forty-eight percent of patients diagnosed as having hyperuricemia, had also severe OSA. We did not find significant correlation between OSA's diagnosis and uric acid levels. But we note a significant correlation between the level of uric acid and the AHI ( P = 0.02) and there was a statistically significant association between hyperuricemia and severe OSA ( P = 0.04). According to the ROC curve for predicting severe OSA, the cut-off value of serum uric acid was about 56.5 mg/L (with a sensitivity of 61.5% and specificity of 57%). In our study conducted in a Tunisian population with AF, we did not find a significant difference in uric acid levels between patients with OSA and those without OSA. But we have demonstrated a significant correlation between the increased levels of serum uric acid and severe OSA. So, it seems to be interesting to propose a routine screening of OSA, in patients having AF and hyperuricemia, whose main goal is to detect severe forms of OSA.

Association of Uric Acid Level and Body Mass Index in Type 2 Diabetes Mellitus Patients.

Body Mass Index (BMI) or formerly called Quetelet Index is the measurement for scoring nutrient state in adults by using body weight (in kilogram) and body height (in metre). Obesity and overweight are defined as an abnormality of fat accumulation that can damage our health. Uric acid is a substance that is formed when the breaking process of purine occur. Hyperuricemia are condition that are closely related with gout. Uric acid level increased significantly towards those having obesity. It is also concluded that obesity was the usual cause in patients with recurrent gout attack. Strong relation has been found between high BMI with gout arthritis occurance. This research is a descriptive analytic research with cross sectional design included 50 diabetic subjects aged over 40 years old. Subjects picked by consecutive sampling then we examined the weight, height, waist size, blood pressure and laboratory tests for fasting blood sugar and serum uric acid level. With statistical analysis using chi square we found that there was no significant association between BMI and serum uric acid level in type 2 diabetes mellitus patients (p>0.005).

Factors Affecting Uric Acid Changes in Pulmonary Tuberculosis Patients Who Received Oral Anti Tuberculosis Therapy During One Month

Background: Pyazinamide (PZA) and Ethambutol (ETB) are tuberculosis drugs that can increase uric acidlevels by decreasing excretion and increasing uric acid reabsorption. Increased levels of uric acid can causearthritis, arthralgia and gout.Objective: To evaluate the association of uric acid levels with age, sex and body mass index (BMI) andarthralgia incidence.Method: The study was conducted in the lung outpatient treatment room at the Department of Pulmonologyand Respiratory Medicine. Method were uric acid levels examination before OAT administration, at week2 and 4. Paired t tests and spearman correlation test were used to analyze the association between sex, age,BMI and arthralgia with increased uric acid levels. The incidence of hyperuricemia and arthralgia wereperformed in percentage.Result: Seventeen subjects consisting of 9 (52.9%) males and 8 (47.1%) females who received oral antituberculosiscategory 1 mostly had normal BMI prior to treatment (9 patients, 52.9%). Uric acid levelsincreased significantly (p <0.005) at week 2 and 4 than before Oral Anti Tuberculosis (OAT) treatment.It was found significant relationship (p <0.005) between increased uric acid levels and arthralgia withhyperuricemia incidence of 82.35% and arthralgia incidence of 35.29%. There was no significant association(p <0.005) between increased uric acid levels with sex, age and BMI.Conclusion: It was found a high incidence of hyperuricemia (82.3%) in ETB and PZA treatment whilesymptomatic 35.29%.

An Integrative Omics Study Reveals Association of Higher Uric Acid Levels with Poor Clinical Outcome after Lung Transplantation

Ischemia/reperfusion injury (IRI) during lung preservation and transplantation contributes to primary graft dysfunction and poor clinical outcome. It is imperative to elucidate the biological mechanisms underlying IRI that will inform the development of effective therapeutics. We investigated metabolic profiling together with associated changes in gene expression to identify biological pathways significantly altered at reperfusion vs. cold ischemic time (CIT). We analyzed metabolomics and transcriptomics profiles from 67 paired human lung tissue samples collected from 2008-11 at the end of CIT and after 2 hrs of reperfusion. Gene expression analysis was performed in R. We used MetaboAnalyst and OmicsNet for metabolomics data analysis and omics data integration, respectively. Association of the most significantly changed molecules with transplant outcome was investigated with Kaplan-Meier estimate. Purine metabolism was identified as the most significant enriched pathway at reperfusion, based on integrative analysis of the two omics datasets. The levels of 2'-deoxyguanosine (FC = 0.23) and 2'-deoxinosine (FC = 0.28), and expression of their functionally related gene, Purine Nucleoside Phosphorylase (PNP), (FC = 2.11), were significantly altered. Uric acid (FC = 3.14), the terminal product of purine metabolism, was significantly upregulated. Univariate analyses showed positive association of uric acid level with ICU length of stay (7 days cut off, p=0.03) (Fig.1). Integrative omics analysis identified increased expression of PNP gene and the production of uric acid through the purine metabolic pathway. Uric acid (urate) production is associated with oxidative stress-a prominent mechanism of IRI. To assess the potential pathological role of elevated uric acid in poor transplant outcome, and to pursue the discovery of a precise therapeutic target post-transplant, further studies are currently in progress.

Association of uric acid levels before start of conditioning with mortality after allogeneic hematopoietic stem cell transplantation - a prospective, non-interventional study of the EBMT Transplant Complication Working Party.

Uric acid is a danger signal contributing to inflammation. Its relevance to allogeneic stem cell transplantation (alloSCT) derives from preclinical models where the depletion of uric acid led to improved survival and reduced graft-versus-host disease (GvHD). In a clinical pilot trial, peri-transplant uric acid depletion reduced acute GvHD incidence. This prospective international multicenter study aimed to investigate the association of uric acid serum levels before start of conditioning with alloSCT outcome. We included patients with acute leukemia, lymphoma or myelodysplastic syndrome receiving a first matched sibling alloSCT from peripheral blood, regardless of conditioning. We compared outcomes between patients with high and low uric acid levels with univariate- and multivariate analysis using a cause-specific Cox model. Twenty centers from 10 countries reported data on 366 alloSCT recipients. There were no significant differences in terms of baseline comorbidity and disease stage between the high- and low uric acid group. Patients with uric acid levels above median measured before start of conditioning did not significantly differ from the remaining in terms of acute GvHD grades II-IV incidence (Hazard ratio [HR] 1.5, 95% Confidence interval [CI]: 1.0–2.4, P=0.08). However, they had significantly shorter overall survival (HR 2.8, 95% CI: 1.7–4.7, P<0.0001) and progression free survival (HR 1.6, 95% CI: 1.1–2.4, P=0.025). Non-relapse mortality was significantly increased in alloSCT recipients with high uric acid levels (HR 2.7, 95% CI: 1.4–5.0, P=0.003). Finally, the incidence of relapse after alloSCT was increased in patients with higher uric acid levels (HR 1.6, 95% CI: 1.0–2.5, P=0.04). We conclude that high uric acid levels before the start of conditioning correlate with increased mortality after alloSCT.

Background : Obesity in adolescents may cause elevated uric acid levels, leading to increased blood pressure. This occurs as a result of high uric acid levels inducing arteriolopathy in preglomerular vessels. Consequently, the autoregulator response of the afferent arterioles is impaired and eventually causing hypertension. This study aimed to determine the association of uric acid levels with blood pressure in obese adolescents. Adolescent with hypertension could continue in adulthood and had an higher increased risk of mortality. Objective : To analyze correlation of serum uric acid with blood pressure in obese adolescent in SemarangMethods : This was an observational, cross-sectional study with fifty-three subjects selected through consecutive sampling. Food intakes were assessed using Semi Quantitative Food Frequency Questionare. Serum uric acid levels were measured by Uricase PAP, while physical activity levels were determined by Short International Physical Activity Questionare. Data were analyzed using Rank-Spearman test. Result : Mean serum uric acid levels in male adolescents was 4.897±1.223 mg/dL with an average systolic blood pressure of 1.219±9.544 and a mean diastolic blood pressure of 85.85±8.359. In female subjects, the mean serum uric acid levels was 4.841±0.883 mg/dL with an average systolic blood pressure of 1.175±9.653 and an average diastolic blood pressure of 81.67±8.348. Serum uric acid levels were significantly correlated with systolic blood pressure (p = 0.000; r=0,495) and diastolic blood pressure (p = 0.010, r=0,325).Conclusion : Serum uric acid levels were significantly correlated with blood pressure among obese adolescents in Semarang.

Evaluation of serum uric acid levels in psoriasis vulgaris.

Psoriasis has been accepted as a systemic disease and it is known to be associated with various disorders including metabolic syndrome. High serum uric acid levels are also associated with the components of metabolic syndrome. In this study, we aimed to determine serum uric acid levels in patients with psoriasis and the association of uric acid levels with disease activity by taking the presence of metabolic syndrome criteria into account, since it is one of the most important factors that affect serum uric acid levels. In this cross-sectional study, we evaluated 70 psoriasis patients and 70 healthy individuals who were matched with the patients according to the presence of metabolic syndrome. We evaluated the demographic features, levels of serum uric acid, Psoriasis Area Severity Index (PASI) scores, presence of psoriatic arthritis, nail involvement, and metabolic syndrome criteria of the patients. Serum uric acid levels of psoriasis patients were significantly higher than those of controls. There was a positive correlation between PASI scores and serum uric acid levels of the patients. As hyperuricemia had a close relationship with psoriasis and PASI scores, we suggest monitoring patients with psoriasis for serum uric acid levels during treatment and follow-up.

Impact of alcohol intake on the relationships of uric acid with blood pressure and cardiac hypertrophy in essential hypertension

BackgroundHyperuricemia, which is frequently associated with hypertension, can be caused by alcohol intake. To date, limited data have shown the link between alcohol intake and hyperuricemic hypertension. MethodsWe retrospectively examined the influence of alcohol intake on the relationship between the uric acid level and blood pressure or cardio-metabolic parameters in 171 untreated non-failing hypertensive patients (mean 59.3±10.7 years).Cross-sectional analysis was separately performed in regular alcohol drinkers (more than 25g/day ethanol, n=74, 82.4% men) and non-drinkers (n=97, 33.0% men). ResultsDiastolic blood pressure was significantly higher in drinkers than in non-drinkers (101.6±11.5mmHg vs. 96.8±8.2mmHg, p<0.01). Estimated glomerular filtration rate (80.4±14.7mL/min/1.73m2 vs. 80.0±17.8mL/min/1.73m2) and body mass index (BMI, 24.7±4.4kg/m2 vs. 24.8±4.2kg/m2) were similar in the two groups. In the drinker group, the uric acid level (mean 6.3±1.7mg/dL) was positively correlated with both systolic and diastolic blood pressures (r=0.270/p=0.020 and r=0.354/p=0.0020, respectively), and with the markers of cardiac hypertrophy, including electrocardiographic voltage (V1S+V5R, r=0.244/p=0.042) and echocardiographic left ventricular mass index (r=0.270/p=0.026). These correlations were also observed in the male drinker sub-group. In the non-drinkers, the uric acid level (mean 5.0±1.4mg/dL) was positively correlated with BMI (r=0.369/p=0.0002) but not with blood pressure or the markers of cardiac hypertrophy. ConclusionsThe results suggest that the role of uric acid in blood pressure might differ between hypertensive drinkers and non-drinkers. In regular alcohol drinkers, there was a positive association of uric acid level with blood pressure and the severity of cardiac hypertrophy. In non-regular drinkers, an increased uric acid level is likely to be closely associated with increased BMI.

Prognostic Significance of Uric Acid Levels in Ischemic Stroke Patients.

The importance and function of serum uric acid (UA) levels in patients with cardiovascular disease or stroke are unclear. We sought to evaluate the appropriate UA levels for stroke patients and the association between endogenous UA levels and clinical outcomes in acute ischemic stroke (AIS) patients, particularly regarding the possible interaction between gender and UA levels with respect to AIS prognosis. We examined 303 patients who had an onset of ischemic stroke within 48 h. Of those, 101 patients received thrombolytic treatment. Serum UA (μmol/L) levels were measured the second morning after admission. Patient prognosis was evaluated 90 days after clinical onset by modified Rankin Scale. Patients were divided into four groups according to serum UA quartiles. A binary multivariate logistic regression model was used to assess clinical relevance in regard to functional outcome and endogenous UA levels. Analysis of subgroups by gender and normal glomerular filtration rate were also been done. Poor functional outcome was associated with older age, history of atrial fibrillation, or higher baseline National Institutes of Health Stroke Scale scores. After adjustment for potential confounders, patients with higher UA levels (>380 μmol/L) or lower UA levels (≤250 μmol/L) were 2-3 times more likely to have a poor outcome (OR 2.95, 95% CI 1.14-7.61; OR 2.78, 95% CI 1.02-7.58, respectively) compared to the baseline group (UA level 316-380 μmol/L). The same results were observed in thrombolyzed patients. Patients with high and low UA levels were 9-18 times more likely to having poor outcomes compared to the baseline group (UA level: 316-380 μmol/L; OR 18.50, 95% CI: 2.041-167.67; OR 9.66, 95% CI 1.42-65.88, respectively). In men, patients with high UA levels were 6 times more likely to have poor outcomes compared to the baseline group (UA level: 279-334 μmol/L; OR 6.10, 95% CI 1.62-22.93). However, female patients with UA level 271-337 μmol/L were seven times more likely to perform badly compared to the baseline group (UA level >337 μmol/L, OR 7.06, 95% CI 1.00-49.81). Serum UA levels in an appropriate range were associated with better outcome in patients with AIS but may be harmful when too high or too low. The association of UA levels with AIS prognosis differed in male and female patients, which highlights the necessity of stratifying by gender in investigations of cerebrovascular risk factors.
