Rationing of nursing care and assessment of work safety and a healthy work environment in intensive care units: A cross-sectional, correlational study

IntroductionRationing of nursing care, whichrefers to the aspects of care not delivered by nurses in an intensive care unit (ICU), has implicationsfor patient outcomes and experiences. ObjectivesThis study aimed to identify the extent to which nursing care is rationed in intensive care units, as well as asses quality of nursing care, and the level of job satisfaction and its correlation with an assessment of the climate of work safety, teamwork, and a healthy work environment. MethodologyA cross-sectional, correlational study was conducted. The study included 226 ICU nurses. It was conducted with the use of three instruments: the Perceived Implicit Rationing of Nursing Care (PRINCA) questionnaire on the rationing of nursing care, assessment of patient care quality and job satisfaction, American Association of Critical-Care Nurses Healthy Work Environment Assessment Tool (HWEAT) and the Safe Attitudes and Behaviours Questionnaire questionnaire in the version: Teamwork and Safety Climate (BePoZa). SettingsIntensive Care Units in Warmia and Mazury Region in Poland. Main Outcome MeasuresLevel of Nursing Care Rationing in Intensive Care Units. ResultsThe majority of participants were women (89.82 %) with a mean age of 42.47 years. The average score for nursing care rationing across all groups was 0.58. The mean score for the HWEAT was 2.7 and BePoZa was 3.72. The scores from the questionnaires were negatively correlated with the nursing care rationing scores, being −0.36 for the HWEAT and −0.45 for BePoZa. All correlation coefficients were statistically significant at a p-value of less than 0.05. ConclusionsIt is important to monitor work safety, teamwork climate, and standards of a healthy work environment in ICUs to minimise the risk of rationing nursing care. Implications for Clinical PracticeInterventions that enhance work organisation and teamwork can elevate nursing quality and job satisfaction in ICUs, while underestimating patient care tasks; thus, highlighting the need for further research on the factors influencing nursing performance.

Determining the effect of environmental factors, work postures and mental workload on musculoskeletal disorders in rescue workers and accidents of one of the water and sewage companies in Iran.

Musculoskeletal disorders are one of the most important problems faced by ergonomists today. This study was conducted with the aim of investigating the effect of weather conditions, working conditions and mental workload on musculoskeletal disorders of rescuers and water and sewage company accidents in hot and cold seasons. The present study was a longitudinal study that was performed on all employees of rescue and disaster teams of Shiraz Water and Sewerage Company in Iran (184 people) by census. Demographic and Nordic questionnaires, NASA Load Index (NASA-TLX), work environment assessment with temperature screening checklist and workplace ergonomic risk factor (WERA) assessment method were performed by the researcher. The collected data were analyzed using SPSS version 23 software and parametric and non-parametric statistical tests. The results showed that the highest and lowest prevalence of musculoskeletal disorders related to knee and back in winter and thigh in summer with prevalence of 57.1, 55.4 and 6% . The comparison of exposure to heat stress factors in summer and winter seasons showed that the temperature factors and work clothes of the workers in both seasons are in worse conditions than other factors (P < 0968). The results of the mental workload of people in two seasons showed that the highest and lowest mental workload is related to physical changes in the winter season and frustration in the summer season, and this relationship was significant (P < 0.001). Using multiple logistic regression model, the potential risk factors for musculoskeletal injuries including age, height, BMI, level of education and work experience in both hot and cold seasons were compared. The results showed that in summer, work experience (OR = 1.16 (1.06-1.28.)) and height (OR = 1.03(0.98-1.08) had a direct effect on the prevalence of pain and discomfort arranged in the back areas. One year of increase in work experience significantly increases the chance of a disorder in the back by 16% and a one centimeter increase in height increases the chance of a disorder in the lumbar region by 3%, although this relationship is significant. It is recommended that suitable solutions such as using appropriate and up-to-date mechanical tools, reducing activity time, increasing rest time, increasing the number of personnel in order to reduce mental workload, using work clothes suitable for the season and type of work, and ergonomics training for workers are on the agenda of company officials. Evaluating the Health of the Work Environment and the Perception of New Nurses During the Transition to Practice in Critical Care.

New nurses are the major source of hiring for most hospitals. They fill vacancies left by seasoned nurses creating an experience-complexity gap. Turnover among new-graduate nurses is as high as 85% in the first 2 years. The purpose of this study was to provide a deeper understanding of the factors contributing to turnover. The specific aims were (1) to better understand novice nurses' perceptions of their transition to critical-care practice and (2) to describe all eligible critical-care nurses' perceptions of a healthy work environment. A quantitative descriptive design was used. Data were collected over 3 weeks in February 2022 from novice nurses (n = 12) who completed the Casey-Fink Nurse Experience Survey and from novice and experienced nurses (n = 47) who completed the American Association of Critical-Care Nurses' Healthy Work Environment Assessment Tool. Data were analyzed using descriptive statistics. Respondents to the Casey-Fink Nurse Experience Survey (n = 12) indicated feeling well supported by family, friends, and preceptors, although they felt less positive about prioritizing care, organizing patient needs, and recommending changes to the plan of care. The overall mean score on the Health Work Environment Assessment Tool was 2.99 (n = 47), indicating the need for improvement. Responses varied by unit, years of experience, and certification status. This study contributes to the evolving body of nursing knowledge and has provided a more detailed understanding of the health of the work environment and perceptions of new nurses during the transition to practice.

Resilience index and mental stress of nursing staff working in intensive care units during the SARS-CoV-2 coronavirus pandemic

Background. Resilience, or mental toughness, is the ability to effectively cope in challenging circumstances. High resilience levels allow for faster adaptation in extreme situations. Aim of the study. An assessment of resilience, mental stress, and self-reported work environment among nursing staff in intensive care units (ICUs) during the SARS-CoV-2 pandemic. Material and methods. The study involved 102 nursing staff working in ICUs. Data collection used the Resilience Coping Scale (RCS-25), Meister’s psychological strain scale, and an original work environment self-assessment scale. Results. The overall mean score of the respondents on the resilience scale was 74.11 points out of a possible 100, while they scored an average of 24.36 points out of 50 on the mental stress scale. Staff working in hospitals converted into infectious disease sites (p = 0.000) or in infectious hospitals with a third level of referral (p = 0.012) assessed their working environment significantly better in the context of the coronavirus disease 19 (COVID-19) pandemic. Medical staff who worked longer at their current workplace, those with longer seniority in the profession, and those who were older scored higher in the resilience areas. The younger the respondent, the more mentally stressed they were in general (rho= -0.200), in non-specific areas (rho= -0.217), and in monotony (rho= -0.211). Those who did not feel well equipped with personal protective equipment showed lower resilience. Conclusions. Respondents were characterized by average resilience and second-level mental stress, while those with higher resilience suffered lower mental stress. We recommend organizing training to strengthen mental toughness, the identification of resilience factors, and the assessment of mental stress in the work environment in the context of the ongoing pandemic. The data obtained could be used to adjust working conditions during the next pandemic. Keywords: nurse, intensive care unit, resilience, mental stress, mental strain, COVID-19.

Assessment of the Relationship Between Intention to Migrate With Workload and a Healthy Work Environment of Nurses

Background: Like other countries in the world, Iran is facing the problem of a shortage of nurses. Meanwhile, many Iranian nurses migrate to other countries every year. The present study aims to assess the relationships between migration intention, workload, and a healthy work environment for nurses. Methods: This cross-sectional correlational study was conducted in 2020 using a proportional stratified sampling of 360 nurses working in the Iran University of Medical Sciences (IUMS) teaching hospitals. The study data were collected through a demographic questionnaire, the nurses’ intention to migrate questionnaire, the NASA task load index, and the American Association of Critical-Care Nurses Healthy Work Environment Assessment. In this study, we used the independent t-test, analysis of variance, and the Pearson correlation coefficient for data analysis at a significance level of &lt;0.05 in SPSS software, version 16. Results: The Mean±SD intention to migrate score was 65.59±33.51. A weak but significant positive correlation existed between the intention to migrate and workload (r=0.166, P=0.002). There was an inverse and weak relationship between the intention to migrate and a healthy work environment (r=-0.160, P=0.002). There was also a significant relationship between the intention of nurses to migrate and their work experience (P=0.048) and employment status (P=0.001). Conclusion: The prevention of nurses’ migration must be investigated thoroughly to determine the reasons. But according to the results of this study, providing a healthy work environment and maintaining the current workforce by providing the indicators desired by nurses may partly prevent excessive migration of nurses.

Female Rangers in Latin America: An Assessment of Role, Recruitment, and Work Environment

Abstract There has been increased research examining conservation rangers. Little is known, however, about female rangers, including their primary roles and workplace environment. Few studies have examined the profession within a Latin American context. This qualitative study explores ranger respondents’ perceptions towards the role, recruitment, and working environment of female rangers in Latin American countries. Based on data obtained from interviews with 28 respondents across 11 countries, findings suggest female rangers’ roles are largely consistent with that of their male counterparts. However, different strengths were highlighted. Additionally, female involvement in the profession was attributed to national employment-based quotas, increased education and exposure, and the presence of women in leadership. Factors that led to a supportive work environment for female rangers included camaraderie, opportunities for professional development and growth, and attentive supervisors and subordinates. Conversely, the persistence of a machismo culture and discriminatory policies and practices were viewed as obstacles. Findings have several implications regarding gender diversity within the ranger profession. Discussion on academic-practitioner collaborations is also provided.

Adapting the Healthy Work Environment Assessment Tool for French-Canadian Intensive Care Nurses.

Self-administered instruments are used to measure components of work environments that cannot be measured directly. The Healthy Work Environment Assessment Tool (HWEAT) of the American Association of Critical-Care Nurses is a promising instrument. However, it is available only in English and Japanese, precluding its use in other populations and cross-national comparisons. To describe the Canadian French translation and cross-cultural adaptation of the HWEAT (F-HWEAT) and to explore its factor structure and psychometric properties. Cross-cultural adaptation of the HWEAT and collection of evidence of validity via an electronic cross-sectional survey. A total of 564 intensive care unit nurses participated in the validation study. Confirmatory factor analysis supported the presence of a single overarching factor measured by the F-HWEAT. The Cronbach α for the instrument was 0.89 (95% CI, 0.88-0.91). The mean and median interitem correlations were both 0.32, and item-partial total correlations ranged from 0.33 to 0.64. The overall F-HWEAT score indicated that nurses believed their work environment needed improvements. Moderate positive correlations were found between the overall F-HWEAT score and nurses' perceptions of care quality (r = 0.45 [95% CI, 0.38-0.51]) and safety (r = 0.48 [95% CI, 0.40-0.55]). The results support the use of the F-HWEAT in French-speaking populations. Using the F-HWEAT will help elucidate areas needing improvement and expand global dialogues about healthy critical care work environments. With this information, nurse leaders and researchers can develop and implement modern strategies to improve the work conditions of intensive care unit nurses.

Evaluation of Agricultural Work Environment with Ergonomic Checkpoints in Demangan, Siman, Ponorogo

An ergonomic checkpoint is a method for evaluating the current work environment and implementing a more suitable work environment. When workers do work activities, the work environment is a critical component. Ergonomics is described as studying human factors in the workplace, including anatomy, physiology, psychology, engineering, management, and design. Based on preliminary observations, several activities allow potential hazards to arise, such as farmers’ lack of understanding of Personal Protective Equipment (PPE) when spraying pesticides and farmers’ use of t-shirts as a mask. Farmers repeated movements with a bent body position when planting rice seeds, which can cause potential health problems such as musculoskeletal. Using descriptive observational research, this study tries to objectively establish the conditions of the work environment concerning the circumstances. The Ergonomic Checkpoints method was used for data collecting. The results of the work environment assessment using the Ergonomic Checkpoints method in agriculture in Demangan Village revealed that five aspects that required corrective action, consisting of materials storage, and handling, had two points of incompatibility, machine safety had one moment of incompatibility, premises/workspace had one moment of incompatibility, and welfare facility had one moment of mismatch. Farmers in Demangan village should use Ergonomic Checkpoints to pay attention to the work environment in keeping with agricultural standards.

Assessment of the Critical Care Work Environment of Intensive Care Unit Nurses in Saudi Arabia.

Nurses play a major role in the delivery of complex and challenging critical care in intensive care units (ICUs). Assessment of work environment is essential indicators of hospital management and can be applied to workforce planning and identifying nursing profession needs. The American Association of Critical-Care Nurses (AACN) recognized six standards for a healthy work environment and developed the Healthy Work Environment Assessment Tool (HWEAT). The aim of this study was to assess the work environment of ICU nurses in Jazan, Saudi Arabia. This cross-sectional study was conducted at public and private hospitals. Data were collected using a self-administered questionnaire that included the sociodemographic characteristics and the AACN HWEAT. Data were analyzed to obtain descriptive and inferential statistics. The Mann-Whitney U and Kruskal-Wallis tests were employed to compare demographic data, on the basis of the overall mean HWEAT score. The study participants were 238 ICU nurses, who were predominantly female (83%). The mean overall HWEAT score was 3.55±1.03, which is within the "good" range, and was higher for male nurses (3.66), nurses aged ≥41 years (3.76), and nurses with postgraduate education (4.04), work experiences of >10 years (3.63), and alternate work shifts (3.6). Nurses in private hospitals had significantly higher overall scores than nurses in public hospitals (3.83 vs 3.19, P<0.001). All HWEAT standards ("effective decision-making", "authentic leadership", "appropriate staffing", "true collaboration", "skilled communication", and "meaningful recognition") were rated as good (mean range, 3.43-3.63). The study results could assist hospitals in prioritizing the adoption of AACN HWE standards. A target benchmark of "good" was established for both the overall score and each standard, which indicates a good work environment as perceived by ICU nurses. In Addition, interpersonal differences should be considered when developing improvement initiatives.

The characteristics and factors associated with omitted nursing care in the intensive care unit: A cross-sectional study

IntroductionPrior research showed that work environment features in acute care settings influence nurses’ capacity to provide care and impacts patient outcomes (e.g., falls). However, little is known about this phenomenon in the intensive care unit. The objectives of this study were to describe the characteristics of omitted nursing care, and to examine the associations between work environment features, omitted nursing care and nurse-reported outcomes in the intensive care unit. MethodsAn electronic cross-sectional correlational study was conducted in the province of Quebec, Canada. Over September 2021, nurses were asked to complete the Healthy Work Environment Assessment Tool (HWEAT), the Intensive Care Unit Omitted Nursing Care instrument (ICU-ONC) and to report their perceptions of nurse-reported outcomes (e.g., quality of care). The associations between these variables were estimated using multivariable cluster-robust regression models, adjusted for nurse and hospital characteristics. ResultsA total of 493 nurses from 42 distinct hospitals participated to this study. On average, nurses felt that their work environment was acceptable, and that the quality and safety of patient care was good. Basic care activities (e.g., mobilisation) were most frequently reported as omitted as opposed to those related to surveillance and medical interventions. In multivariable analyses, higher work environment scores were associated with reduced omitted nursing care scores (p < 0.001) and better ratings for nurse-reported outcomes (p < 0.001). Also, higher omitted nursing care scores were associated with more negative perceptions about the quality and safety of care (p < 0.001). ConclusionOur study portrays the characteristics and some factors associated with omitted nursing care in the intensive care unit. Further research should determine whether intensive care nurses’ reports of organisational features and omitted nursing care are associated with objectively captured patient outcomes.

Interprofessional Team Collaboration and Work Environment Health in 68 US Intensive Care Units.

Safe, reliable, high-quality critical care delivery depends upon interprofessional teamwork. To describe perceptions of intensive care unit (ICU) teamwork and healthy work environments and evaluate whether perceptions vary by profession. In August 2015, Assessment of Interprofessional Team Collaboration Scale (AITCS) and the American Association of Critical-Care Nurses Healthy Work Environment Assessment Tool (HWEAT) surveys were distributed to all interprofessional members at the 68 ICUs participating in the ICU Liberation Collaborative. Overall scores range from 1 (needs improvement) to 5 (excellent). Most of the 3586 surveys completed were from registered nurses (51.2%), followed by respiratory therapists (17.8%), attending physicians (10.5%), rehabilitation therapists (8.3%), pharmacists (4.9%), nursing assistants (3.1%), and physician trainees (4.1%). Overall, respondents rated teamwork and work environment health favorably (mean [SD] scores: AITCS, 3.92 [0.64]; HWEAT, 3.45 [0.79]). The highest-rated AITCS domain was "partnership/shared decision-making" (mean [SD], 4.00 [0.63); lowest, "coordination" (3.67 [0.80]). The highest-scoring HWEAT standard was "effective decision-making" (mean [SD], 3.60 [0.79]); lowest, "meaningful recognition" (3.30 [0.92]). Compared with attending physicians (mean [SD] scores: AITCS, 3.99 [0.54]; HWEAT, 3.48 [0.70]), AITCS scores were lower for registered nurses (3.91 [0.62]), respiratory therapists (3.86 [0.76]), rehabilitation therapists (3.84 [0.65]), and pharmacists (3.83 [0.55]), and HWEAT scores were lower for respiratory therapists (3.38 [0.86]) (all P ≤ .05). Teamwork and work environment health were rated by ICU team members as good but not excellent. Care coordination and meaningful recognition can be improved.

Assessment of food environment at work and its association with cardiometabolic health among employed adults in Delhi, India

Background and aimsUnhealthy food environment at work is believed to be playing a role in the burgeoning obesity, cardiometabolic risk, diabetes and cardiovascular diseases in India. Therefore, food environment assessment at work is crucial to understand the effect of food environment and to find its association with cardiometabolic risk among adults in Delhi, India. MethodologyMix-methods study to find association between worksite food environment on the food choices, dietary behavior, and cardio-metabolic health of 455 apparently healthy adults (both males and females) aged 25–55 years was done in urban India. ResultsUnhealthy food environment, poor eating pattern and sedentary lifestyle at work resulted in clustering of CMR factors among the study participants. Work environment assessment revealed that worksites with canteens had higher overall CHEW score for all its domains (Physical, nutritional, and information environment), in comparison to worksites without or no canteens. Four out of ten apparently healthy adults had metabolic syndrome indicating poor cardiometabolic health. The odds of CMR in subjects with access to canteen (unhealthy food) were estimated to be 0.74 (CI: 0.51 to 1.07; p = 0.11) times the odds of CMR in subjects without canteen (limited access to food). Females (β:0.34; 95% CI:0.23, 0.44; p = 0.00) had high CMR factors in comparison to males. Overall cardiometabolic risk factors increased with age (β:0.01; 95% CI:0.01,0.01; p = 0.00). ConclusionFindings of the present study urge the need for dietary and lifestyle intervention along with longitudinal studies to further disentangle the association of the food and work environment on the prevalence of CMR among adults.

Development and initial validation of the Japanese healthy work environment assessment tool for critical care settings.

This study aims to translate the Healthy Work Environment Assessment Tool (HWE-AT) into Japanese and evaluate its validity and reliability. The authors followed the guidelines for scale translation, adaptation, and validation in cross-cultural healthcare research. After translation and back-translation, a series of pilot studies were conducted to assess comprehensibility. Subsequently, an expert panel established the content validity. Content validity was calculated using the content validity index (CVI). Finally, we verified the construct validity and calculated the test-retest reliability. The updated HWE-AT achieved sufficient comprehensibility after conducting the two pilot tests. Content validity was calculated using the scale-level CVI/average and all the items were 1.00. The content validity indices CFI and RMSEA were 0.918 and 0.082, respectively. Intraclass correlation coefficients for all dimensions ranged from 0.618 to 0.903, indicating acceptable test-retest reliability. Our findings suggest that the Japanese version of the HWE-AT has good validity and reliability.

Such a slow, methodical process can feel frustrating and tedious, especially when the elephant represents a task like changing the culture of your workplace. Creating a healthy work environment (HWE) is challenging and can be overwhelming at the best of times. With the current negative impact of the coronavirus pandemic on the health care community, it is even more important now to take that first bite. Data from the American Association of Critical-Care Nurses (AACN) consistently show that implementing the HWE standards leads to better nurse staffing and retention and less moral distress.In “Implementing AACN’s Healthy Work Environment Framework in an Intensive Care Unit,” Kester et al describe 2 years of working toward improving the health of their work environment. Their research details their journey from unit assessment using AACN’s healthy work environment assessment tool (HWEAT) to developing, implementing, and evaluating a plan targeting interventions to improve each of the 6 HWE standards.

Měřicí nástroje používané pro hodnocení pracovního prostředí v ošetřovatelství

Cílem práce bylo vyhledat nástroje zaměřené na hodnocení pracovního prostředí sester a popsat jejich charakteristiku. Zdrojem informací byly články z 6 elektronických databází. Před zahájením vyhledávání byla stanovena klinická otázka: "Které nástroje jsou vhodné pro hodnocení pracovního prostředí sester v nemocnici?". Klíčová slova byla: practice/work environment, nursing, scale. Vyhledávání probíhalo pomocí booleovských operátorů od května do června 2020, pro období let 2010-2020. Zařazovacími kritérii byly: empirická studie s uvedením metodologie a dostupný full text v anglickém jazyce. Celkem bylo nalezeno 1 212 studií, pro konečné zpracování jich bylo použito 57. Ve vyhledaných studiích bylo použito celkem 23 nástrojů. Mezi nejčastěji použité nástroje patří Practice Environment Scale-Nursing Work Index a jeho varianty Practice Environment Scale, Nursing Work Index, dále pak Brisbane Practice Environment Measure a Revised Professional Practice Environment. Spolu s pracovním prostředím byly zkoumány další proměnné, např. syndrom vyhoření, pracovní zátěž apod. Z množství nalezených nástrojů, které se používají pro monitoring pracovního prostředí, byl nejčastěji použit dotazník PES-NWI. Tento dotazník je již přeložen také do českého jazyka. Z výsledků publikací pak vyplývá souvislost kvality pracovního prostředí a např. pracovní spokojenosti, syndromu vyhoření apod., což je dokladem důležitosti tématu i v českém prostředí.

Occupational Health Hazards among Pregnant Nurses in Damietta and Khartoum General Hospitals: An Overview

The pregnant nurses' exposure of the hazards daily during contact with sick patients, infectious agents, teratogenic chemicals, radiation and environmental risks. Aim of the study: To assess Occupational health hazards among pregnant nurses in Damietta and Khartoum of general hospitals: An overview. Design: A comparative descriptive research design was used to conduct this study. Setting: carried out in Damietta and Khartoum general hospitals (Egypt and Sudan). Sample: All pregnant nurses working in all departments of hospitals and outpatient clinics. It was 135 pregnant nurses in Damietta and 50 pregnant nurses in Khartoum. Tools : four tools were used for data collection, tool l: demographic characteristics for pregnant nurses, medical and obstetric history of nurses, nurses habits, nurses knowledge regarding occupational health hazed, tool II- physical assessment of studied nurses, tool III :- nurses practices regarding use of the personal protective equipment, tool IV: Assessment of work environment. Result: concerning nurses knowledge, shows that 40% of nurses had satisfactory knowledge regarding occupational health hazards in Damietta hospital, while, 70% of them had satisfactory knowledge in Khartoum hospital. Regarding nurses practices for use of personal protective equipment, shows that 70% of nurses had done practices regarding uses of personal protective equipment in Damietta hospital, while, 80% of them had done practices in Khartoum hospital. Concerning work environment it reveals a highly statistically significant differences regarding physical work environment, housekeeping, floors, stairs, mean of exit, employee facilities, fire protection, electrical, material handling, storage, machining guarding and clinic inside the factor. Conclusion: this study concluded that there were no statistically important correlation between total knowledge about occupational health hazards facing nurses in the workplace and total practices regarding use of PPE measures (P<0.05). Recommendation: Continuing nurse health education program to reduce occupational health risk in the workplace

Construct Validity and Reliability of the Work Environment Assessment Instrument WE-10.

The purpose of this study was to validate the construct and reliability of an instrument to assess the work environment as a single tool based on quality of life (QL), quality of work life (QWL), and organizational climate (OC). The methodology tested the construct validity through Exploratory Factor Analysis (EFA) and reliability through Cronbach’s alpha. The EFA returned a Kaiser–Meyer–Olkin (KMO) value of 0.917; which demonstrated that the data were adequate for the factor analysis; and a significant Bartlett’s test of sphericity (χ² = 7465.349; Df = 1225; p ≤ 0.000). After the EFA; the varimax rotation method was employed for a factor through commonality analysis; reducing the 14 initial factors to 10. Only question 30 presented commonality lower than 0.5; and the other questions returned values higher than 0.5 in the commonality analysis. Regarding the reliability of the instrument; all of the questions presented reliability as the values varied between 0.953 and 0.956. Thus; the instrument demonstrated construct validity and reliability
