Rapid assessment of cosmic radiation exposure in aviation based on BP neural network method.

Cosmic radiation exposure is one of the important health concerns for aircrews. In this work, we constructed a back propagation neural network model for the real-time and rapid assessment of cosmic radiation exposure to the public in aviation. The multi-dimensional dataset for this neural network was created from modeling the process of cosmic ray transportation in magnetic field by geomagnetic cutoff rigidity method and air shower simulation by a Monte Carlo based Geant4 code. The dataset was characterized by parameters including cosmic ray energy spectrum, Kp-index, coordinated universal time, altitude, latitude, and longitude. The effective dose and dose rate was finally converted from the particle fluxes at flight position by the neural network. This work shows a good agreement with other models from International Civil Aviation Organization. It is also illustrated that the effective dose rate by galactic cosmic ray is <10μSvh-1 and the value during ground level enhancement (GLE) 42 is 4~10 times larger on the routes calculated in this work. In GLE 69, the effective dose rate reaches several mSvh-1 in the polar region. Based on this model, a real-time warning system is achieved.

Assessment of radiation exposure in the phosphate mining area of Gafsa (Southeastern Tunisia)

ABSTRACT This study investigates the radiological health implications of sixty-three (63) samples of soil, food, and drinking water collected from the phosphate mining area of Gafsa in Southeastern Tunisia. Additionally, it assesses the exposure to radon concentrations in houses located around the phosphate mining sites. The activity concentrations in soil samples were found to be 382.1, 36.1 and 16.3 Bq kg−1 respectively for 40K, 232Th and 226Ra. The average activity of 232Th was higher than worldwide average value of 30 Bq kg−1. Eleven (11) radiological hazard parameters were computed based on various exposure scenarios including external gamma dose rate (GDR), annual effective dose (AED) and risk assessment (RA). The calculated GDR values were of 3.2 nGy h−1 in food and 45.4 nGy h−1 in soil samples, while the average AED values were of 3.9 µSv y−1 in food and 83.5 µSv y−1 in soil samples. Internal AED due to ingestion of radionuclides was found to have a mean value of 0.1 µSv y−1 in soil, 264.4 µSv y−1 in food and 10 µSv y−1 in drinking water samples. The AED values due to inhalation of soil-dust and radon were assessed to be 2.1 and 1.2 µSv y−1, respectively. The RA average values in food and drinking water samples were found to be 3 × 10−5 and 2.4 × 10−4, respectively. Based on the exposure pathway, inhalation of radioactivity through soil and radon resulted in highest total AED values, which remained below the reference limit. This was followed by internal exposure, with total AED values of 274.5 µSv y−1.

In-depth exploration of the radiation exposure to staff performing endoscopic retrograde cholangiopancreatography procedures (ERCP) through RANDO phantom and TLDs.

This study provides a comprehensive evaluation of the occupational radiation exposure faced by healthcare professionals during Endoscopic Retrograde Cholangiopancreatography (ERCP) procedures. Utilizing an anthropomorphic RANDO phantom equipped with Thermoluminescent Dosimeters (TLDs), we replicated ERCP scenarios to measure radiation doses received by medical staff. The study meticulously assessed radiation exposure in various corresponding body regions typically occupied by medical staff during ERCP, with a focus on eyes, thyroid, hands, and reproductive corresponding organ regions. The findings revealed significant variations in radiation doses across different body parts, highlighting areas of higher exposure and underscoring the need for improved protective measures and procedural adjustments. The effective radiation doses were calculated using standard protocols, considering the varying levels of protection offered by lead aprons and thyroid shields. The results demonstrate the substantial radiation exposure experienced by healthcare staff, particularly in regions not adequately shielded. This study emphasizes the necessity for enhanced radiation safety protocols in clinical settings, advocating for advanced protective equipment, training in radiation safety, and the exploration of alternative imaging modalities. The findings have crucial implications for both patient and staff safety, ensuring the continued efficacy and safety of ERCP and similar interventional procedures. This research contributes significantly to the field of occupational health and safety in interventional radiology, providing vital data for the development of safer medical practices.

Primary care clinic visits in formerly evacuated areas due to radiation disaster following the Great East Japan Earthquake: A retrospective descriptive study.

Radiation disasters pose distinctive medical challenges, requiring diverse care approaches. Beyond radiation exposure assessment, addressing health impacts due to lifestyle changes, especially among vulnerable populations, is vital. Evacuation orders issued in radiation-affected areas introduce unique healthcare dynamics, with their duration significantly influencing the recovery process. Understanding evolving patient demographics and medical needs after lifting evacuation orders is crucial for post-disaster care planning. Minamisoma Municipal Odaka Hospital, located 13 to 20 km from Fukushima Daiichi Nuclear power plant in a post-evacuation zone, was greatly affected by the Great East Japan Earthquake and subsequent radiation disaster. Data were retrospectively collected from patient records, including age, gender, visit date, diagnoses, and addresses. Patient records from April 2014 to March 2020 were analyzed, comparing data before and after the July 2016 evacuation order lift. Data was categorized into pre and post-evacuation order lifting periods, using International Classification of Diseases, Tenth Edition codes, to identify the top diseases. Statistical analyses, including χ-square tests, assessed changes in disease distributions. Population data for Odaka Ward and Minamisoma City fluctuated after lifting evacuation orders. As of March 11, 2011, Odaka Ward had 12,842 residents (27.8% aged 65+ years), dropping to 8406 registered residents and 2732 actual residents by April 30, 2018 (49.7%). Minamisoma City also saw declines, with registered residents decreasing from 71,561 (25.9%) to 61,049 (34.1%). The study analyzed 11,100 patients, mostly older patients (75.1%), between 2014 and 2020. Post-lifting, monthly patient numbers surged from an average of 55.2 to 213.5, with female patients increasing from 33.8% to 51.7%. Disease patterns shifted, with musculoskeletal cases declining from 23.8% to 13.0%, psychiatric disorders increasing from 9.3% to 15.4%, and trauma-related cases decreasing from 14.3% to 3.9%. Hypertension (57.1%) and dyslipidemia (29.2%) prevailed post-lifting. Urgent cases decreased from 1.3% to 0.1%. This study emphasizes the importance of primary care in post-evacuation zones, addressing diverse medical needs, including trauma, noncommunicable diseases, and psychiatric disorders. Changing patient demographics require adaptable healthcare strategies and resource allocation to meet growing demands. Establishing a comprehensive health maintenance system tailored to these areas' unique challenges is crucial for future disaster recovery efforts.

Global research trends in Total Body Irradiation: a bibliometric analysis.

This manuscript presents a bibliometric and visualization analysis of Total Body Irradiation (TBI) research, aiming to elucidate trends, gaps, and future directions in the field. This study aims to provide a comprehensive overview of the global research landscape of TBI, highlighting its key contributions, evolving trends, and potential areas for future exploration. The data for this study were extracted from the Web of Science Core Collection (WoSCC), encompassing articles published up to May 2023. The analysis included original studies, abstracts, and review articles focusing on TBI-related research. Bibliometric indicators such as total publications (TP), total citations (TC), and citations per publication (C/P) were utilized to assess the research output and impact. Visualization tools such as VOS Viewer were employed for thematic mapping and to illustrate international collaboration networks. The analysis revealed a substantial body of literature, with 7,315 articles published by 2,650 institutions involving, 13,979 authors. Full-length articles were predominant, highlighting their central role in the dissemination of TBI research. The authorship pattern indicated a diverse range of scholarly influences, with both established and emerging researchers contributing significantly. The USA led in global contributions, with significant international collaborations observed. Recent research trends have focused on refining TBI treatment techniques, investigating long-term patient effects, and advancing dosimetry and biomarker studies for radiation exposure assessments. TBI research exhibits a dynamic and multifaceted landscape, driven by global collaboration and innovation. It highlights the clinical challenges of TBI, such as its adverse effects and the need for tailored treatments in pediatric cases. Crucially, the study also acknowledges the fundamental science underpinning TBI, including its effects on inflammatory and apoptotic pathways, DNA damage, and the varied sensitivity of cells and tissues. This dual focus enhances our understanding of TBI, guiding future research toward innovative solutions and comprehensive care.

Isodose mapping of terrestrial gamma radiation in Sarawak-Borneo: Insights from surface-weathered soil analysis

Presentation of baseline data on terrestrial gamma radiation (TGR) levels is crucial for assessing the annual effective dose received by the public due to natural radiation exposure. Cumulative doses from various sources can become significant, warranting a spatial understanding of TGR distribution. Few countries have comprehensively mapped TGR on a national scale, often facing challenges due to remote or inaccessible regions. This study investigated the influence of weathered soil groups on TGR dose rates in Sarawak-Borneo, Malaysia, to facilitate insights for TGR projection and isodose mapping. A total of 1044 TGR dose rate measurements were collected using NaI (Tl) scintillation detector survey meters, with a mean of 100 nGy h−1 and a range of 8–375 nGy h−1. Non-parametric statistical analyses of variance using Welch's ANOVA, Brown-Forsythe, and Kruskal-Wallis validated (P-sig.=.000) notable dissimilarities among six categories of superficial-weathered soil. Graphical analysis using Sinclair's cumulative plot revealed deviations at intervals of 50, 80, 100, 120, 175, and 205 nGy h−1. These deviations indicate distinct lithological influences. Skeletal soil (entisols) and podzolic soils had high mean dose rates (148 and 113 nGy h−1, respectively) due to limited development, thus preserving abundant uranium (U) and thorium (Th). Meanwhile, gleysols and thionic soils exhibited compatible means (90 and 82 nGy h−1, respectively), while alluvial (or transported soils) and organic soils displayed lower dose rate ranges (mean of 76 and 47 nGy h−1, respectively), reflecting rapid hydrolysis weathering processes. Simple linear regression analysis revealed a strong relationship between TGR dose rate and mean value of weathered soil groups (y = 0.851x + 0.141 nGy h−1), signifying the significance and magnitude of weathered soil groups' impact on TGR dose rates. The obtained R-value is 0.704, indicating a strong linear correlation among soil group variables, and a Durbin-Watson statistic of 1.41, suggesting positive autocorrelation among residuals, thus positive relationships. An isodose map was successfully developed using the Kriging technique, aligning with lithological features of the study area. Semivariogram analysis reveals spatial dependence within a range of 1.47°, supporting the Kriging technique's suitability for spatial inference. In conclusion, this study has successfully revealed the relationship between TGR dose rates and superficial-weathered soil in Sarawak-Borneo. While the linear relationship is applicable to the Sundaland-Borneo tectonic block, it has potential to be used as a valuable tool for spatial inference of TGR dose rates in isodose development with similar lithologial characteristics, aiding in radiation exposure assessment and environmental monitoring.

Assessing Radiation Exposure and Contrast Agent Utilization: A Comparative Analysis of the Woven EndoBridge Device and Stent-Assisted Coil Embolization for Managing Unruptured Wide-Neck Bifurcation Aneurysms.

In this study, we determined whether there were significant differences in procedure time, radiation dose, fluoroscopy time, and total contrast media dose when unruptured wideneck bifurcation aneurysms (WNBAs) were treated with the Woven EndoBridge (WEB) device and stent-assisted coil (SAC) embolization. The WEB device and SAC embolization (14:17) were used to treat 31 cases of internal carotid artery bifurcation, anterior communicating artery, middle cerebral artery bifurcation, and basilar bifurcation aneurysms between August 2021 and December 2022. The procedure time, radiation dose, fluoroscopy time, and total contrast medium dose between the 2 treatment groups were compared and analyzed. In the WEB device group, the results between operators were compared, and the follow-up radiologic outcomes were investigated. The procedure and fluoroscopy times were significantly shorter in the WEB device group. Radiation and total contrast media dose were also significantly smaller in the WEB device, but there was no significant difference in results between operators. The follow-up radiological outcome showed adequate occlusion in 83.3% (10/12) of cases. The WEB device can be used as an alternative treatment method among the available endovascular treatment methods for WNBAs to reduce radiation exposure and the dose of contrast media when used adequately with appropriate indications.

Assessment of fetal radiation exposure in pregnant women undergoing computed tomography and rotational angiography examinations for pelvic trauma.

This study aimed to assess fetal radiation exposure in pregnant women undergoing computed tomography (CT) and rotational angiography (RA) examinations for the diagnosis of pelvic trauma. In addition, this study aimed to compare the dose distributions between the two examinations. Surface and average fetal doses were estimated during CT and RA examinations using a pregnant phantom model and real-time dosemeters. The pregnant model phantom was constructed using an anthropomorphic phantom, and a custom-made abdominal phantom was used to simulate pregnancy. The total average fetal dose received by pregnant women from both CT scans (plain, arterial and equilibrium phases) and a single RA examination was ~60mGy. Because unnecessary repetition of radiographic examinations, such as CT or conventional 2D angiography can increase the radiation risk, the irradiation range should be limited, if necessary, to reduce overall radiation exposure.

Assessment of Radio-Frequency Radiation Exposure from Selected Mobile Base Stations in Yenagoa, Bayelsa State, Nigeria

The antennas from mobile base stations radiate non-ionizing radio frequency radiation into space. The human body can experience negative health effects from prolonged exposure to radiofrequency (RF) radiation. Because base station installations and telecommunication networks are growing, there is a growing need to address the health risks connected with exposure to telecommunication masts. Using the Electrosmog Meter Cornet Model, power density from many telecom masts owned by various network operators was measured in this investigation. At 10-meter intervals, the RF radiation in seven (7) settlements in Yenagoa town, located in the Yenagoa Local Government Area of Bayelsa State, was measured within radial distances of 100 meters. With a mean value of 4.0569mWm-2, the measured values vary from 0.650mWm-2 to 9.810mWm-2. Still, values evolved. Nevertheless, additional factors, such as adjacent electromagnetic sources' wave interference or reference base station interference, caused readings to fluctuate. It was discovered that the greatest power density level was 9.810 mWm-2, and it was measured 20 meters away from the base station at site MBS1. As per the International Commission on Non-ionizing Radiation Protection (ICNIRP) and other regulatory authorities, the standard limit of 4.5Wm-2 for the 900 MHz system is not exceeded by the radiation exposure level, as the findings show. This shows that the exposure levels in these areas are low enough to not seriously jeopardize the health of those living in the research area.

RADIATION EXPOSURE IN ROUTINE MAMMOGRAPHY SCREENING: A LARGE OBSERVATIONAL CROSS-SECTIONAL STUDY IN MOROCCO

This observational cross-sectional study aimed to assess radiation exposure during mammography for breast cancer screening in Morocco and highlight the importance of dose optimization.&#x0D; Patient data from 1100 mammographic examinations conducted in five units in the Rabat-Salé-Zemmour-Zaër region were collected. The mean glandular dose (MGD) and technical parameters, such as exposure factors and breast thickness, were analyzed. The data were compared with international studies to provide context.&#x0D; The overall mean MGD was 1.34 ± 0.36 mGy for craniocaudal (CC) and 1.48 ± 0.38 mGy for mediolateral oblique (MLO) incidences. The MGD varied among different units and anode/filter combinations. The Rh/Rh anode/filter combination was most commonly used, resulting in the lowest radiation dose. The study findings also highlighted the relationship between MGD and breast thickness, with higher doses observed for thicker breasts.&#x0D; This study provides valuable insights into radiation exposure during mammography breast cancer screening in Morocco. The results underscore the importance of dose optimization strategies to ensure patient safety without compromising diagnostic image quality. Implementing optimized technical parameters, conducting regular quality assurance programs, and promoting education and awareness are essential in achieving dose reduction and minimizing radiation risks. Collaboration among healthcare professionals, regulatory bodies, and international organizations is crucial for sharing best practices and advancing radiation dose optimization in mammography.&#x0D;

Long-Term Science Goals with In Situ Observations at the Sun-Earth Lagrange Point L4

The Korean heliospheric community, led by the Korea Astronomy and Space Science Institute (KASI), is currently assessing the viability of deploying a spacecraft at the Sun-Earth Lagrange Point L4 in collaboration with National Aeronautics and Space Administration (NASA). The aim of this mission is to utilize a combination of remote sensing and in situ instruments for comprehensive observations, complementing the capabilities of the L1 and L5 observatories. The paper outlines long-term scientific objectives, underscoring the significance of multi-point in-situ observations to better understand critical heliospheric phenomena. These include coronal mass ejections, magnetic flux ropes, heliospheric current sheets, kinetic waves and instabilities, suprathermal electrons and solar energetic particle events, as well as remote detection of solar radiation phenomena. Furthermore, the mission’s significance in advancing space weather prediction and space radiation exposure assessment models through the integration of L4 observations is discussed. This article is concluded with an emphasis on the potential of L4 observations to propel advancements in heliospheric science.

Evaluation of Radiation Exposure and Influential Factors in Cone-Beam Computed Tomography (CBCT) of the Head and Abdomen during Interventional Procedures.

Cone-beam computed tomography (CBCT) is a widely used imaging technique in interventional radiology. Although CBCT offers great advantages in terms of improving comprehension of complex angioarchitectures and guiding therapeutic decisions, its additional degree of radiation exposure has also aroused considerable concern. In this study, we aimed to assess radiation exposure and its influential factors in patients undergoing CBCT scans of the head and abdomen during interventional procedures. A total of 752 patients were included in this retrospective study. Dose area product (DAP) and reference air kerma (RAK) were used as measures of patient dose. The results showed that the median values of DAP were 53.8 (50.5-64.4) Gy⋅cm2 for head CBCT and 47.4 (39.6-54.3) Gy⋅cm2 for that of the abdomen. Male gender and body mass index (BMI) were characterized by increased DAP and RAK values in both head and abdominal CBCT scans. Larger FOV size was associated with a higher DAP but a lower RAK value, especially in head CBCT scans. Exposure parameters under automatic exposure control (AEC) also varied according to patient BMI and gender. In conclusion, the patients received slightly higher radiation doses from head CBCT scans than from those applied to the abdomen. BMI, gender, and FOV size were the key factors that influenced the radiation dose administered to the patients during CBCT scans. Our results may help to define and minimize patients' exposure to radiation.

Assessment of occupational external radiation exposure of workers in the Southwest of Libya using portable NaI detector

In this study, gamma dose rates generated from the naturally occurring radioactive materials (NORM) were measured in the waste streams of a large scale and sludge onshore petroleum operations. Measurements conducted in this work involved: sludge recovery from separation tanks, sludge forming, NORM storage, scaling in oil tubulars, scaling in gas production and sedimentation in produced water evaporation ponds. Field work was carried out in many places of different terrain of an operation oil exploration and production in Murzuq basin in the Southwest of Libya. The radiation dose rates were measured using portable InSpector-1000. A total of 400 dose rates were acquired. The highest dose rate was 70 μSv/h acquired in sludge stored in barrels. The estimated mean annual equivalent doses in this field were in the range of 0.2–2.8 mSv/y in the first scenario, while in the second scenario the calculated mean annual equivalent doses were in the range of 0.04–0.68 mSv/y. It is assumed that workers may face various exposures in the field where measurements took place, considering the total annual effective dose to be 1.53 mSv/y. The main radioisotopes detected in these samples indicated by the display of the measuring device were 226Ra and 228Ra but detecting both radioisotopes in the same run is not achievable by using the display of the screen.

Assessment of dosimetric approaches in evaluating radiation exposure for interventional cardiologists in Sri Lanka.

Interventional cardiologists face significant radiation exposure during interventional cardiology procedures. Therefore, this study focuses on assessing radiation exposure among interventional cardiologists during their procedures. Specifically, it aims to determine the effectiveness of both single and double dosimeter methods in estimating annual occupational radiation doses. This research holds pioneering significance as it represents the very first study undertaken in Sri Lanka. Thirteen interventional cardiologists performed 486 interventional cardiology procedures over three months in three different healthcare institutes. Active Hp(10) dosimeters were placed to measure radiation exposure. Effective doses were calculated using single and double dosimetric algorithms. Annual occupational doses were assessed on an operator basis. Statistical analyses were conducted to assess algorithmic differences and dose variations using the Kruskal-Wallis test and linear regression. The highest annual occupational dose for each dosimetric algorithm received as 2.00 ± 0.24 mSv, 2.29 ± 0.48 mSv, 3.35 ± 0.71 mSv, and 2.64 ± 0.42 mSv, respectively, and remained below the recommended safety limit of 20 mSv/year. The Kruskal-Wallis test revealed no significant differences in the effective doses among double dosimetric algorithms, as well as between single and double dosimetric algorithms (p > 0.05). Linear regression showed strong correlations among various algorithms, demonstrating consistency. The findings of this study hold significant effects on interventional cardiology practice in Sri Lanka, enhancing radiation safety and monitoring.

Radiation exposure assessment in coronary angiography computed tomography scan: Establishing diagnostic reference level for northeast Assam population

Background/Aim: Coronary angiography computed tomography (CT) scans play a pivotal role in diagnosing cardiovascular diseases, providing crucial information for treatment planning. However, concerns regarding radiation exposure have prompted the need for establishing region-specific diagnostic reference levels (DRLs) to ensure patient safety. This study aimed to assess radiation exposure during coronary angiography CT scans in the northeast Assam population and establish DRLs tailored to this demographic. Methods: A total of 380 patients were referred to the Primus Diagnostic Centre and Heath City Hospital, Guwahati Assam with coronary artery disturbances. Data on the technical parameters used in CT procedures were taken in 2021-2022. Organ and surface dose to specific radiosensitive organs (chest) estimation was done using software imPACT 1.0.4 from the National Radiological Protection Board (NRPB) SR250 Monte Carlo dataset. Results: The study population (n = 380) comprised 190 men and 190 women with an age range from 29 to 75 years. The mean body mass index (BMI) and effective dose (ED) were 22.42 ± 1.06 kg/m 2 and 21.57 ± 4.27 mSv.cm, respectively. The mean the dose-length product (DLP) was 854.67 mSv.cm and the mean ED was 21.57 mSv.cm. The ED for males was 13-27 mSv and 13-29 mSv for females. The DRL for the male population was found to be 24.26 mSv.cm 2 whereas for the female population was 24.69 mSv.cm 2 . Conclusion: This study highlights the necessity of establishing tailored DRLs for coronary angiography CT scans in the northeast Assam population. By doing so, healthcare providers can ensure optimal image quality while minimising radiation exposure, ultimately enhancing patient safety and quality of care. These findings have implications for radiological practice in the region and contribute to the ongoing efforts to standardise radiation doses in medical imaging procedures.

Assessment of Natural Background Radiation Exposure in the Federal Capital Territory of Nigeria

The detrimental effects of environmental contamination and deterioration on health are a worldwide concern and Nigerian environmental and public authorities continue to be concerned about the risk to public health. The water, the sky, construction materials and the earth's crust all release natural background radiation that contaminates the environment around us. Additionally, people are exposed to background radiation that comes from internal, cosmic, and terrestrial sources, although, the altitude determines the amount of cosmic radiation exposure, and high altitudes result in large radiation doses. Monitoring the amounts of radiation to which humans are exposed, either directly or indirectly, requires an understanding of the natural background radiation in the environment. The current study attempted to create a baseline of outdoor background radiation in FCT for exposure rate, absorbed dose rate, annual effective dose equivalent, and excess life cancer risk. The study used a very sensitive survey meter to measure the BIR. The average BIR value found in the research areas is marginally below the 0.013mRh-1 global BIR level, indicating an almost high BIR level for the FCT while the absorbed dose rates of 105.85nGy/hr was greater than the 59nGy/hr global population weighted average gamma dose rates estimate. The obtained annual effective dosage equivalent value is greater than the global average normal annual effective dosage level for outdoor environments, and the excess lifetime cancer risk values were higher than the 0.29×10-3 allowable level as reported by UNSCEAR &amp; ICRP. Therefore, the general people and those who live in environmentally sensitive areas may experience immediate health effects from contamination and radiation levels at the current rates.

RAD01: Development of a NUMTEK RAD01 Low-Cost, Portable Digital Gamma Radiation Detector

Additive manufacturing and open-source affordable instruction provide equal opportunities. Therefore, it is possible to quickly, compactly, and cheaply make expensive, scarce, and large devices in the previous century. The Geiger-Muller tube is used for the primary sensing component proposed and incorporates digital signal processing to ensure accurate and reliable measurement of gamma radiation. The performance of the developed detector is evaluated through experimental testing and comparisons with commercially available radiation detection devices. The results demonstrate the detector's ability to provide precise and consistent measurements while remaining cost-effective. Moreover, its portability and user-friendly interface make it suitable for various applications, including environmental monitoring, nuclear safety, personal radiation exposure assessment, and, most importantly, educational purposes. This low-cost portable digital gamma radiation detector contributes to the field of radiation detection by presenting an accessible solution that addresses the financial limitations associated with conventional gamma radiation detectors.

Radiofrequency electromagnetic radiation exposure assessment, analysis, computation, and minimization technique in 5G networks: A perspective on QoS trade-offs

Radiation exposure assessment of nuclear medicine staff administering [177Lu]Lu-DOTA-TATE with active and passive dosimetry

BackgroundThe use of lutetium-177 (177Lu)-based radiopharmaceuticals in peptide receptor nuclear therapy is increasing, but so is the number of nuclear medicine workers exposed to higher levels of radiation. In recent years, [177Lu]Lu-DOTA-TATE has begun to be widely used for the treatment of neuroendocrine tumours. However, there are few studies evaluating the occupational radiation exposure during its administration, and there are still some challenges that can result in higher doses to the staff, such as a lack of trained personnel or fully standardised procedures. In response, this study aims to provide a comprehensive analysis of occupational doses to the staff involved in the administration of [177Lu]Lu-DOTA-TATE.ResultsA total of 32 administrations of [177Lu]Lu-DOTA-TATE (7.4 GBq/session) carried out by a physician and a nurse, were studied. In total, two physicians and four nurses were independently monitored with cumulative (passive) and/or real-time (active) dosemeters. Extremity, eye lens and whole-body doses were evaluated in terms of the dosimetric quantities Hp(0.07), Hp(3) and Hp(10), respectively. It was obtained that lead aprons reduced dose rates and whole-body doses by 71% and 69% for the physicians, respectively, and by 56% and 68% for the nurses. On average, normalised Hp(10) values of 0.65 ± 0.18 µSv/GBq were obtained with active dosimetry, which is generally consistent with passive dosemeters. For physicians, the median of the maximum normalised Hp(0.07) values was 41.5 µSv/GBq on the non-dominant hand and 45.2 µSv/GBq on the dominant hand. For nurses 15.4 µSv/GBq on the non-dominant and 13.9 µSv/GBq on the dominant hand. The ratio or correction factor between the maximum dose measured on the hand and the dose measured on the base of the middle/ring finger of the non-dominant hand resulted in a factor of 5/6 for the physicians and 3/4 for the nurses. Finally, maximum normalised Hp(3) doses resulted in 2.02 µSv/GBq for physicians and 1.76 µSv/GBq for nurses.ConclusionsIf appropriate safety measures are taken, the administration of [177Lu]Lu-DOTA-TATE is a safe procedure for workers. However, regular monitoring is recommended to ensure that the annual dose limits are not exceeded.

Assessment of secondary radiation dose in radiology departments

BackgroundIn hospitals and clinics, medical imaging using ionizing radiation is one of the most essential and powerful diagnostic techniques that exposes patients, workers, and the general population to radiation through primary and secondary radiation. Thus, routine leakage testing of X-ray tubes and scattered radiation testing are important in ensuring the absence of leakage and scattered radiation. Materials and methodsThis investigation conducted background and secondary radiation exposure assessments of 17 X-ray units in the radiology departments of three selected major public hospitals. The weekly dose in both controlled and uncontrolled areas was measured using a Ludlum Model 9DP-1 ionization chamber dosimeter and expressed in μSv/h. Resultsand Discussion: The highest dose was recorded in the controlled area of the CT scan room in Hospital A, where the mean dose rate was 12.16 μSv/h (in the control door) and 8.38 μSv/h (in the main door). The dose rates in these areas were approximately ten times higher than those measured in corridors. Generally, the measurement results showed that the scattered radiation doses in the controlled and uncontrolled areas of the radiology units were within the permissible limits set in the National Council on Radiation Protection & Measurements Report No. 147. This finding may be attributed to the distance between these locations and the X-ray sources, and the lead lining of the area. ConclusionThe secondary radiation levels in the assessed diagnostic rooms were within the reference exposure level for the public and workers, indicating that appropriate barriers (doors and walls) had been installed in the investigated hospitals.
