Cystic fibrosis and Russell‐Silver syndrome in a child with maternal isodisomy of chromosome 7

A 6-year-old Asian girl was diagnosed with cystic fibrosis at 3 months of age, following investigations for failure to thrive. She had intrauterine growth retardation and continued to have restricted postnatal growth, despite adequate caloric intake and enzyme replacement therapy. Further investigations were initiated when she was 5 years old, as her growth was not responding to the usual treatment measures. These tests revealed that she had maternal isodisomy of chromosome 7.

Cultural barriers to diabetes care in South Asians: arranged marriage – arranged complications?

AbstractSocial and cultural differences that are present in South Asian diabetic patients may affect their diabetes care. We present a case concerning a young South Asian girl with type 2 diabetes mellitus who, despite persistent attempts at education and encouragement by health care professionals, was unwilling to accept treatment of her diabetes. It was felt by both herself and her family that acceptance of the diagnosis of diabetes would adversely affect her marriage prospects. As a result she developed severe microvascular complications before she finally agreed to treatment. Understanding potential cultural barriers to diabetes care may be vital in managing South Asian diabetic patients and it is important to establish education programmes specifically aimed at removing the perceived stigma of diabetes in this population. Must a face be disfigured by a terrible tropical disease, in order to make us realise that the well being of our gesture is aesthetically acceptable? Must our reparative motivation remain the only trace of our skill and our humanity? During a plastic surgery mission, so-called humanitarian mission, can this surgery be aesthetic? Can aesthetic surgery be humanitarian? We learnt from aesthetic surgery on humanitarian missions, that aesthetic is only a technic to assist the reparation aspect. There is no need to justify aesthetic by reconstruction, because aesthetic belongs, in its entirety, to reparation. There is no need either to feel guilty, when our technical gesture is aesthetic, since what really motivates our intervention is not for us to decide. Is the reparation of the cleft lip of a little Asian girl, a reparation gesture or only an aesthetic approach? You may think that this will be a reparation gesture, because it will be more “popular” or because you will reconstruct the muscular belt. This child will most probably look for a more beautiful or aesthetic lip (in any case, she ignores the intricacies of facial growth principles). Therefore, it will be an aesthetic intervention. Never mind the motivation, since your technic will be the same, and you will try to do your very best and the most aesthetically you can. Only the very intention behind an intervention will decide whether it is an aesthetic or a plastic intervention, whereas your technical gesture will remain the same. All this illustrates that our definitions are of a minor importance, compared with the mere satisfaction of our little Asian girl, which, hopefully, will be a “beautiful” satisfaction. In fact, we have learnt not to do aesthetic surgery, but to give an aesthetic approach to our surgery, even during our humanitarian missions. The form aesthetic surgery will take is very subjective and only the final result will have the last say. Hopefully this surgical result will be aesthetically and humanly acceptable.

An Asian girl with a ???milder??? form of the Hydrolethalus syndrome

Hydrolethalus syndrome is an autosomal recessive disorder characterized by hydrocephalus, micrognathia, limb anomalies and several other abnormalities, mostly in the midline structures. The syndrome was first described in Finland, where the incidence is approximately 1 in 20000. All of the Finnish patients were stillborn or died during the first day of life. Only three non-Finnish cases have survived beyond the neonatal period. Here, we report the first Oriental girl with a 'milder' form of hydrolethalus syndrome. The patient died at age 44 days making her the fourth reported case surviving beyond the neonatal period. The case supports the concept of a 'milder' form of the syndrome. Whether this spectrum is due to allelism or locus heterogeneity awaits molecular analysis.

Primary intracerebral mesenchymal chondrosarcoma with rhabdomyosarcomatous differentiation: case report and literature review

Intracranial chondrosarcoma (Ch-S) is a slow-growing, locally recurrent, malignant cartilaginous tumour of the skull base. Intracranial mesenchymal chondrosarcoma (MsCh-S) is a rarer, more malignant variant associated with the supratentorial meninges. Only seven cases of Ch-S, and six of MsCh-S, that were primarily intraparenchymal in origin have been reported. Moreover, no case of intracranial Ch-S or MsCh-S has been reported in which rhabdomyosarcomatous differentiation was prominent. A 17-year-old Asian girl presented with a 4-week history of occipital headache, vomiting and paraesthesia in the left hand. She was drowsy with a left hemiparesis and had a dilated right pupil with bilateral papilloedema. CT demonstrated a large, partly calcified, contrast-enhancing mass in the right temporo-parietal region with oedema and midline shift. Through a large craniotomy, a tense brain was encountered with no apparent cortical abnormality. Despite a radical tumour excision, with excellent initial clinical recovery, a local recurrence rapidly occurred within weeks prior to the administration of any radiotherapy. Initial histopathological examination revealed a primary MsCh-S with osseous and rhabdomyosarcomatous differentiation, with an indistinct margin. After a second radical excision, a second recurrence rapidly occurred; however, this proved excessively vascular and inoperable. Radiotherapy was declined and death followed within 3 weeks. This is the seventh case of primary intracerebral MsCh-S to be reported and the first to demonstrate rhabdomyosarcomatous differentiation. It was characterized clinically by rapid, local recurrence with increased vascularity.

Through the Looking Glass: Harnessing the Power of African American Children's Literature

The following article explores the topic of culturally relevant curriculum materials in relation to the use of African American children's literature as a teaching tool. The author provides a rationale, authentic examples of instruction taken from actual classrooms, and reflections of personal experience from childhood and her professional career. A significant number of African American children's books are reviewed in relation to their value in teaching concepts which span the curriculum. The use of African American children's literature throughout the school curriculum provides a powerful means to greatly enhance the educational experience of African American children by (a) immersing them in the stories of their people and (b) developing a cultural mirror image which would reflect their place in the world as valid, valuable, and voiced. My professional interest in this area spans over 20 years of teaching in the K-12 system and in higher education. My interest is also personal given my experiences as an elementary student engaged in a curriculum that reflected someone other than me. I searched for some reflection of my experience and me. Yet, like a mirror with a set image, what I saw were pictures of another's story. I believe that as educators we must place our classroom instruction within a context that supports the success of our students. That context requires reliance upon the familiar and meaningful experiences drawn from the students' lives and the lives of those who share their cultural background. My goal is to give them a mirror where the reflections unequivocally recognize their right to be, their unique qualities, and rich cultural heritage. This paper will explore several areas in which the use of African American children's literature can benefit the lives of all students, particularly those of African American descent. IDENTITY FORMATION Roethler (1998) submits that the transition into adulthood requires that each individual wrestle with the challenge of identity formation. This process requires a separation from the primary caregiver and development of a set of rules to govern our lives as adults. This undertaking is described as particularly tenuous for African American children for two reasons. First, they are challenged to define their character as adults. Secondly, they must come to terms with both their cultural and national heritage. This complex duality is what W.E. B. Dubois termed double-consciousness. African American children's literature provides one way to support children of African descent through the process of developing a sense of identity given its authentic perspectives, flavor, and illustrations. One of my most cogent memories as a classroom teacher involved a young Asian girl enrolled in the second grade of the school to which I was then assigned. I was using a book by Patricia Polocca (1992) entitled Chicken Sunday, a story of a Black grandmother adopted by a White female friend of her grandsons. This story contained illustrations of real family photographs on several pages. During one of our sessions on Chicken Sunday, the Asian student displayed intense excitement towards a photograph she saw on a particular page from the book. That's me, that's me! she cried, eyes shining with the delight of self-discovery. Her response caught me off-guard until I looked at the page and discovered that the photograph which drew her reaction reflected the features of a very fair complexioned African American female with dark, shoulder length hair, and somewhat slanted eyes. At a quick glance, one could assume, in fact, that the nationality of the individual was Asian. I quickly responded to the student that although we knew the story involved a family different from her own, the photograph did indeed look like her. The child's need for self-discovery was so strong that she continued to affirm that the picture was her own image. I decided at that moment to leave the child's joy of selfdiscovery intact, deeming that more constructive to enhancing her skill and love for reading than a continued effort to correct her mistaken identity. …

Ultrasound biomicroscopy in the diagnosis and management of limbal dermoid

PURPOSE: To report the use of ultrasound biomicroscopy in the diagnosis and management of limbal dermoid. METHOD: We used ultrasound biomicroscopy to examine an 18-month-old Asian girl with a congenital limbal mass with corneal extension. A biopsy of the mass was obtained. RESULTS: Clinical examination, ultrasound biomicroscopy, and biopsy confirmed the diagnosis of limbal dermoid. CONCLUSION: Ultrasound biomicroscopy may serve as a useful diagnostic adjunct for limbal dermoids. Additionally, it may be helpful in delineating the extent of these lesions.

Early onset of diabetic nephropathy

A 12 year old Asian girl with a four year history of poorly controlled insulin dependent diabetes mellitus developed overt diabetic nephropathy. There were no clinical features of Rabson-Mendenhall syndrome....

Case report: Multiple neonatal fractures — Dietary or deliberate?

A case of multiple fractures in a 3-month-old Asian girl with rickets is presented, in which difficulty in excluding child abuse was encountered. No firm conclusions were reached, and the child later died suddenly at 5 months of age. Post-mortem confirmed rickets histologically and the principal cause of death was recorded as sudden infant death syndrome. Three years later evidence of child abuse was found against a subsequent sibling in the same family. If the typical injuries associated with child abuse are present, these must be taken as strong points in favour of this diagnosis, even in the presence of metabolic bone disease.

Sociocultural Correlates of Eating Disorders among Asian Schoolgirls in Bradford

A survey of 204 south-Asian and 355 Caucasian schoolgirls was conducted in Bradford using the EAT-26 and the BSQ. At interview, seven Asian girls and two Caucasian girls met DSM-III-R criteria for bulimia nervosa, yielding a prevalence of 3.4% and 0.6% respectively. One Asian girl met DSM-III-R criteria for anorexia nervosa. Factor analyses of the EAT and BSQ supported their cross-cultural conceptual equivalence in this south-Asian population. Among the Asians, high EAT and BSQ scores were associated with a more traditional cultural orientation and not with greater Westernisation. It is probable that these findings reflect the cultural and familial difficulties faced by these Asian girls growing up in Britain.

Uric acid excretion in children with urolithiasis

Urinary uric acid excretion was assessed in 38 children to determine whether hyperuricuria was a risk factor in children with urolithiasis. Uric acid excretion (measured per deciliter glomerular filtration rate), and fractional excretion of uric acid were similar in 27 children with hypercalciuria and calcium oxalate urinary stones, in six children with idiopathic calcium oxalate urolithiasis, and in five with uric acid urolithiasis, of whom four were white boys and one was an Asian girl. One boy with a urate stone had cystinosis. Serum uric acid concentrations exceeded 6.0 mg/dl (360 mumol/L) in two children with hypercalciuria and in two patients with idiopathic calcium oxalate urolithiasis. None of the children with calcium urolithiasis had excessive urinary excretion of uric acid. In children with hypercalciuria, uric acid excretion did not change significantly when dietary sodium was increased from 1.0 to 5.0 gm/1.73 m2. We conclude that excessive urinary uric acid excretion is seldom an additional risk factor in children with calcium urolithiasis and that dietary sodium chloride does not have a strong influence on urinary excretion of uric acid in children with hypercalciuria.
