Evidence synthesis of health policy and systems research in rehabilitation: a protocol for Cochrane overviews of systematic reviews on delivery, governance, financial arrangements, and implementation strategies.

Cochrane Rehabilitation and the World Health Organization (WHO) Rehabilitation Program are collaborating to produce four Cochrane overviews of systematic reviews that synthesize the current evidence from health policy and systems research (HPSR) in rehabilitation. They will focus on the four pillars of HPSR identified by the Cochrane Effective Practice and Organization of Care (EPOC) taxonomy: delivery arrangements, financial arrangements, governance arrangements, and implementation strategies. The protocol describes why HPSR is currently needed in rehabilitation, provides detailed information on the four EPOC pillars in interaction with rehabilitation and reports the Cochrane methods that will be followed to produce the overviews. 1. Conundrums in Implementing Consumer Direction in Home-Based Care: Perspectives of Older Adults and Family Members in China

ABSTRACT Consumer-directed care (CDC) for older people enables clients to arrange personalized services and improves their well-being. However, little is known about clients’ preferences for policies in collectivist cultures. We investigate the views of older clients and family members about policies that promote consumer direction in a collectivist cultural setting – Guangzhou, China. Using semi-structured interviews, we recruited older persons and their family members (n = 24) in 2021. Inductive thematic analysis was employed. Two themes emerged. The first theme, the need for flexibility in utilizing benefits, includes the need for: 1) flexibility in selecting care workers; 2) autonomy in choosing budget management agents; 3) flexibility in selecting care-related goods; and 4) adequate and equitable benefits. The second theme, the need for professional support, contains the need for: 1) information and support; and 2) individualized training for care workers. Similar to those in individualistic cultures, people in collectivist environments desire autonomy and choice in service arrangements. However, the welfare and preferences of family members, rather than clients, may be prioritized by family members when making care decisions. Adapting CDC to collectivist cultures requires navigating between client preferences and family preferences.

Experiences of Turkish nurses volunteering in the disaster zone following the 2023 Türkiye-Syria earthquake.

The study aimed to determine the experiences of Turkish nurses who volunteered in the disaster zone following the 2023 Türkiye-Syria earthquake. The earthquake devastated 11 cities in Türkiye, which required a comprehensive humanitarian response. Despite the crucial role of nurses in crisis management, limited research exists on their experiences in earthquake zones. Nurses experience difficulties in disaster areas, such as physical conditions, lack of disaster plans, and inadequate disaster training before being deployed to earthquake areas. This study used a qualitative research model and case study design. Data were collected through in-depth qualitative interviews. Nine participants who had worked in earthquake zones for at least 5 days were included in the study. The discussions were recorded, transcribed verbatim, and analyzed thematically. The purposive sampling method was used, and the Consolidated Criteria for Reporting Qualitative Research guidelines were followed. As a result, 4 themes, namely physical conditions, psychosocial conditions, professional conditions, and organization, and their 14 subthemes were determined. This study revealed that nurses encountered difficulties related to physical conditions in earthquake zones. Moreover, the emotional burden expressed by the nurses highlighted the profound psychological impact of disaster response. This study determines that disaster preparedness and support for nurses should be enhanced to improve the arrangement of health services in future calamities. The results of this study can be used by nurse managers and healthcare policymakers to create nursing training programs that focus on disaster-related competencies.

Data stewardship: case studies from North American, Dutch and Finnish universities

PurposeAs national legislation, federated national services, institutional policies and institutional research service arrangements may differ, data stewardship programs may be organized differently in higher education institutions across the world. This work seeks to elaborate the picture of different data stewardship programs running in different institutional and national research environments.Design/methodology/approachUtilizing a case study design, this study described three distinct data stewardship programs from Purdue University (United States), Delft Technical University (Netherlands) and Aalto University (Finland). In addition, this work investigated the institutional and national research environments of the programs. The focus was on initiatives led by academic libraries or similar services.FindingsThis work demonstrates that data stewardship programs may be organized differently within varying national and institutional contexts. The data stewardship programs varied in terms of roles, organization and funding structures. Furthermore, policies and legislation, organizational structures and national infrastructures differed.Research limitations/implicationsThe data stewardship programs and their contexts develop, and the descriptions presented in this work should be considered as snapshots.Originality/valueThis work broadens the current literature on data stewardship by not only providing detailed descriptions of three distinct data stewardship programs but also highlighting how research environments may affect their organization. We present a summary of key factors in the organization of data stewardship programs.

Generation and content validation of physical and vocational rehabilitation in paraplegics: An interview guide

BACKGROUND: As the mainstay treatment for paraplegics is rehabilitation, patient satisfaction and experience are important predictors of the care being provided. To understand whether the current arrangements of rehabilitation services are sufficient to help an individual with paraplegia return to a productive life. Patient-centered interview guides to explore their rehabilitation experience are lacking, which calls for a generation of the same. OBJECTIVE: To generate and validate an interview guide (PVR-PIG), which aims at understanding and exploring the experience of physical and vocational rehabilitation for individuals with paraplegia. METHODS: After an extensive literature search, an initial 27-item interview guide was formulated under three domains: general experience and expectations from rehabilitation care; physical rehabilitation experience, barriers, and facilitators; and vocational rehabilitation. Later, it was sent for content validation to a multidisciplinary expert panel with prior experience in spinal cord injury (SCI) management. RESULTS: After suggestions were received from nine experts with an average of 14 years of experience in dealing with SCI, a 15-item interview guide was finalized, with a scale-level and item-level content validity index of 0.897 and 0.78–1, respectively. CONCLUSION: The generated interview guide (PVR-PIG) aimed to explore physical and vocational rehabilitation experiences in paraplegics was found to be valid.

A Revisionist History Approach to the Study of Emotional Labor: Have We Forgotten Display Rules and Service Contexts?

We take a revisionist approach to study emotional labor—commoditization of emotions for a wage—to delineate how organizational scholars must “revive and resubmit” two crucial elements of the emotional labor phenomenon that have been left behind as research within this space evolved. First, we argue that scholars have not paid enough recent attention to display rules that prescribe what emotions are acceptable within service interactions, instead assuming classic conceptualizations (i.e. show positive emotions and hide negative emotions) still prevail. Second, we highlight that the shift away from service occupations to more white-collar occupations may have minimized our understanding of the complexity of emotional labor in modern service arrangements, such as multiple job holders, and employees in the gig economy. We hope that future emotional labor scholarship will dig into several taken-for-granted assumptions about the phenomenon moving forward to help “go back to the basics” regarding display rules of those in service work.

Analysis of the Economic Value of Environmental Services of Mangrove Ecosystem in Cendi Manik, Sekotong District, West Lombok Regency

The area of mangrove ecosystems in West Nusa Tenggara Province continues to decline each year. One of the threatening factors that substantially contribute to such condition is socioeconomic activities of human beings in fulfilling their needs. An effort that can be made primarily to preserve the mangrove ecosystem is to quantify the economic value of environmental services provided by the mangrove ecosystem as a whole, so that the existence and sustainability of the mangrove ecosystem could be maintained. This research aims to calculate the total economic value of mangrove ecosystem services which indicate the four categories of ecosystem services including supporting services, provisioning services, regulatory services and cultural services. The research was carried out using a survey method by collecting data and information from location of this study to obtain actual data regarding the existence of the mangrove ecosystem. The results of the analysis show that the total economic value of mangrove ecosystem environmental services is 963,888,060/year. This value is divided into provider services (supply services) amounting to Rp. 153,002,250, regulatory services (arrangement services) Rp. 598,339,702, supporting services (support services) Rp. 2,243,250, and cultural services (cultural services) Rp. 210,308,857. The magnitude of the total economic value of mangrove ecosystem environmental services can be a reference for the government in determining compensation for services if there is degradation to mangrove ecosystem resources in the area.

Implementation of administrative service policy at the Makassar city population and civil registry service

The problem in this study is that implementing policies at the Makassar City Population and Civil Registry Office could be more optimal. This study aims to analyze the implementation of administrative service policies at the Makassar City Population and Civil Registry Office. This type of research is qualitative research. Primary data sources were obtained through interviews with 18 informants and supported by observation and documentation review at the Makassar City Population and Civil Registry Office. Data collection techniques in this study used interviews, observations, and documentation review. The results showed that implementing administrative service policies had interpreted the above policies. However, the employee placement arrangement could be more effective because it only places two employees at the file verification counter, so its implementation results in long queues. This shows a need for coordination by implementers of existing service arrangements and procedures and a lack of socialization in the community, resulting in a weak ability to adapt to the procedures and service innovations that have been carried out.

In carrying out the activity, an evaluation of the activity and evaluation of the presenters was carried out in the form of a questionnaire. The results of the questionnaire showed that 96 percent said that the presenters and the material presented were very satisfying, and 4 percent were satisfactory. Meanwhile, 85 percent of activity evaluations said they were very satisfactory, 7 percent said they were satisfactory and 8 percent said they were quite satisfactory. Based on the evaluation results of the training, participants said that this activity was very useful and should become a routine agenda for improvement quality service health.

Exploring the reasons for defaulting from childhood immunization: a qualitative study in Pakistan

BackgroundChildhood vaccination is widely recognized as the most effective means to prevent various diseases. However, a considerable amount of children still miss out on their vaccination schedules. Therefore, this study explores the reasons for defaulting from the expanded program on immunization in district Swat, Pakistan.MethodsA qualitative phenomenological approach was used. Data collection took place from March to September 2022. Thirty-six in-depth interviews were conducted with participants who had defaulter children. The collected qualitative data were analysed thematically to identify key patterns and themes related to the reasons for defaulting from childhood vaccination schedules.ResultsSix themes emerged, i.e., illness of the defaulter child at the scheduled time, perceived side effects of the vaccination, factors related to caregivers, myths and misconceptions, vaccinators attitudes and crowed vaccination centres, as well as poor immunization service arrangements. Four subthemes arose related to caregivers, such as lack of clear understanding about completion of vaccination, least priority for child’s vaccination, cultural restriction on mothers, and the loss of vaccination card.ConclusionAccording to the study’s findings, caregivers have their own perceptions regarding the non-completion of their children’s vaccination schedule. The childhood immunization defaulting arises from various factors including child illness, Adverse Events Following Immunization (AEFIs) concerns, misconceptions, improper injection techniques, and negative vaccinator attitudes. The vaccination completion rate may be increased if the concerns of the caregivers are appropriately addressed.

“If we're wrong, take us to court”: An analysis of special police services (SPS) arrangements for the policing of football in England and Wales

Under the Police Act 1996, Special Police Services (SPS) allow the police in England and Wales to partially cost recover for resources deployed to police events such as football, horse racing and music festivals. Historically, there have been numerous legal cases about SPS charges involving police forces and event organisers. Most recently Ipswich Town Football Club Company Limited vs. The Chief Constable of Suffolk Constabulary [2017] EWCA Civ 1484, resulted in clarification on points of law and a more restricted area within which police forces can charge for SPS. This qualitative research interviews police officers engaged in policing football, football club representatives and other key stakeholders involved in the SPS negotiation process. It finds that there are divisive approaches by both parties, which can result in compromising public safety. The notion of SPS being abolished is explored, despite potential benefits of this, further research is recommended to examine this further.

Public values and sector service delivery preferences: Public preferences on contracting from simple to complex human services

AbstractNonprofit and for‐profit providers play an increasing role in public service delivery, but we know little about what shapes public service delivery preferences. Responding to calls to put the “public” back in public values theory, we examine the influence of public values on sector service delivery preferences for government, nonprofit, or for‐profit delivery across six service areas ranging from simple services such as trash collection to complex services such as child welfare. We find equity predicts a preference for government service delivery across areas, while efficiency corresponds to a preference for for‐profit service delivery. Nonprofit sector preferences varied across service areas; equity corresponds to simple services such as street maintenance, whereas effectiveness corresponds to complex human services such as elder care. Public administrators should be cognizant of the public value trade‐offs that underlie sector preferences for public services to design and implement service arrangements in line with the preferences of the public they serve.

Transformation of Mortgage Rights Registration by Electronic System

On entering the digital era, the Ministry of Agrarian Affairs and Spatial Planning/National Land Agency introduced electronic land services, including electronic mortgage rights. To facilitate investment needs by applying for bank loans and to improve mortgage rights services in meeting the principles of transparency, timeliness, speed, convenience, effectiveness, and efficiency. This research aims to describe and analyze the arrangement of electronic mortgage rights system services, and the advantages and disadvantages faced by the organizers and users of the electronic mortgage rights system services. The type of research used is descriptive normative legal research. Secondary data were analyzed qualitatively. For the conclusion deductive logic was used. The research results showed that the types of services in the electronic mortgage rights system include registration of mortgage rights, transfer of mortgage rights, change of creditor name, and cancellation of liability electronically integrated mortgage rights service mechanisms. The land deed officials, creditors, and the land office experienced problems while registering mortgage rights in the electronic system. There are advantages and disadvantages to an electronic mortgage rights system. There are unclear settings. It is proposed that the rules regarding electronic mortgage rights must be revised regarding the improvement period for electronic mortgage rights applications. The provisions of Article 10 paragraph (3) of the Mortgage Rights Law cannot be implemented because the Electronic Mortgage Rights are based on land data that has been legalized and registered with the local land office. Data driven value creation in industrial services including remanufacturing

In the era of twin transition companies face complex challenges. Economical resource usage and efficiency are particularly important in the manufacturing sector. Product-service systems are seen a promising solution that can meet the expectations regarding efficiency in a sustainable way. However, there is a huge potential regarding the evaluation of sustainable value creation. The paper focuses on the value creation process in product-service systems including offers with remanufactured products.Against this background, the goal of this paper is to describe how data driven industrial services can create sustainable value in the meaning of the triple bottom line. Based on previous work, a quantitative model for the assessment and optimization of this value creation is extended to include additional remanufacturing strategies. The value optimization model integrates the different perspectives of provider, custumer and society. The numerical evaluation of this model for a specific application case shows that economic and ecological value creation can be jointly achieved and optimized, and which service arrangements lead to this optimization.

Health Services Availability and Readiness for Management of Hypertension and Diabetes in Primary Care Health Facilities in Ghana: a Cardiovascular Risk Management project.

Hypertension and diabetes are leading causes of adult hospital admissions and mortality across health facilities in Ghana. Timely screening and diagnosis at primary health facilities are crucial to initiate treatment and avert complications. This study explored service availability and readiness of health systems for managing hypertension and diabetes in selected district hospitals in Ghana. We adapted the World Health Organization (WHO) Service Availability and Readiness Assessment (SARA) tool to assess hypertension and diabetes management practices between June and July 2022 in four district hospitals in Ghana. Domain scores of service readiness were calculated based on the mean score of tracer item availability, transformed into percentages, and stratified by facility ownership. The mean readiness index was based on basic clinical logistics and equipment, diagnostic capacity, and first-line medications. Service availability was based on the core health workforce and specific service arrangements for the management of hypertension and diabetes. Facilities were considered 'ready' for services at a cut-off readiness score of 70%. All facilities (n = 4, 100%) provided hypertension and diabetes services, with a median of 118 nurses (IQR 103-140) and 5 physicians (IQR 2-8). Only one facility (n = 1, 25%) had conducted cardiovascular disease training in the past year. All basic equipment (weighing scales, stethoscopes, glucometers, and blood pressure monitors) were available in all 4 facilities. Antihypertensives, including ACE inhibitors (n = 3; 75%), calcium channel blockers (n = 4; 100%), centrally acting agents (n = 4; 100%), and thiazides (n = 4; 100%), were available, as were antidiabetic medications like metformin (n = 4; 100%) and insulin (n = 2; 50%). Only two facilities (n = 2; 50%) could perform the required test (Hemoglobin A1c, full blood count, renal function, serum creatinine, blood urea, electrolytes, and blood lipid tests). Overall readiness score was 75.5%, essential medications (83.5%), basic equipment (78%), clinical guidelines for the management of cardiovascular disease management (75%), and diagnostic capacity (65.5%). Mission facilities had a higher readiness score (96%) and government facilities (55%). Facilities demonstrated high readiness for basic hypertension and diabetes care, with higher availability of some essential medications and basic clinical logistics and equipment. Limited diagnostic capacity and cardiovascular disease training, highlight areas of improvement to strengthen hypertension and diabetes services in Ghana.

Developing an Operationalized Framework for Comparing Consumer-Directed Care for Older Adults: Evidence from Expert Survey and Cross-National Comparison

ABSTRACT Consumer-directed care (CDC) programs for older people aim to optimize health outcomes by offering clients control and flexibility regarding service arrangements. However, policy design features may differ due to heterogenous sociostructural systems. By operationalizing a framework with three dimensions of CDC, i.e. control and direct services, variety of service options, and information and support, we analyzed how countries vary in their policy designs to achieve consumer direction. Using an expert survey (n = 20) and cross-national document analysis, we analyzed 12 CDC programs from seven selected countries: the United States, the United Kingdom, Germany, the Netherlands, China, Australia, and Spain. Among the three dimensions, CDC programs placed more emphasis on and displayed more homogenous performance of policy designs that achieve consumer direction in the dimension of control and direct services, while less emphasis was placed on and more heterogenous performance displayed in the dimensions of variety of service options and information and support. We offer a systematically operationalized framework to investigate CDC policy designs. Findings advance our understanding of CDC policy features from a cross-national perspective. Policymakers could incorporate these findings to empower older people in their respective societies.

Individuals can use these resources for a wide range of purposes, including pursuing business possibilities, raising their standard of living, paying off other sizable debts, responding to emergencies, etc. Microfinance is the arrangement of financial services including loans, savings, insurance, money transfers and remittances offered to the lower income groups or poor entrepreneurs, who otherwise cannot avail the standard banking services. The motive behind Microfinance is to give people in poverty a privilege to become self-sufficient by offering them crucial banking services at considerable smaller monetary amounts.

STATUS OF URBAN HEALTH INFRASTRUCTURE TO COMBAT COVID-19 IN BIRBHUM DISTRICT, WEST BENGAL

The outbreak of Covid-19 was declared a Public Health Emergency of International Concern on 31st January, 2020. Every country can take at national, sub regional and local levels to reorganize essential health services in the pandemic context and maintain a safe access to every citizen. To deal with this catastrophic Corona Virus India is making preparedness with its high urban population density, deficiency in hygiene and sanitation facilities and most crucially insufficient treatment capacity. The first 2000 cases were reported in 56 days. The next 2000 in 15 days and the next 2000 cases in a week. According to the data released by the state health department, West Bengal took just 5 days from June 3 rd to June 7th to register the next 2000 cases. Till Tuesday (9th June) total cases in the state stood at 8985. With increased number of testing and the entry of migrant labourers, there was a sudden spike in the number of cases. The returning migrant workers from the highly infected zones after an unplanned lockdown delivers a major catastrophe at our doorstep. The objective of the study is to focus on the arrangement of health services for this disaster in Birbhum District, West Bengal. According to the direction of the Bengal health department, separate hospitals for Covid-19 patients are to be set up in each district in order to arrange a local set-up before the contamination expands into the community transmission stage.
