Efficacy of antiobesity medications among breast cancer survivors taking aromatase inhibitors.

Aromatase inhibitors (AI) block estrogen synthesis and are used as long-term adjuvant treatment for breast cancer in postmenopausal women. AI use can be associated with weight gain that can lead to increased cardiometabolic risk. The response to anti-obesity medications (AOM) in patients using AI has yet to be studied. We sought to investigate weight loss outcomes of AOM in patients taking AI for breast cancer treatment. This is a matched retrospective cohort study of breast cancer survivors on AI using AOM (AOM/AI group). We compared their weight loss outcomes with a group of female patients with obesity, without a history of breast cancer or AI use, on AOM (AOM group). The primary endpoint was total body weight loss percentage (TBWL %) at the last follow-up. We performed mixed linear regression models, including diabetes status at baseline, to assess associations between use of AOM with/without AI with total body weight loss percentage (TBWL%). We included 124 patients: 62 in the AOM/AI group (63.6 ± 10years, body mass index [BMI] 34.3 ± 7.1kg/m2) and 62 in the AOM group (62.8 ± 9.9years, BMI 34.6 ± 6.5kg/m2). The mean time of follow up was 9.3 ± 3.5months, with no differences among the two groups. The AOM/AI group had a lower TBWL% compared to the AOM group at the last follow-up -5.3 ± 5.0 vs. -8.2 ± 6.3 (p = 0.005). The results remained significant after adjusting for diabetes status (p = 0.0002). At 12months, the AOM/AI group had a lower TBWL% compared to the AOM group 6.4 ± 0.8% vs. 9.8 ± 0.9% (p = 0.04). The percentage of patients achieving ≥ 5%, ≥ 10%, and ≥ 15% of weight loss at 12months was greater in the AOM compared to the AOM/AI group. Although the weight loss response was suboptimal, patients in the AOM/AI group had improvement in fasting glucose, glycated hemoglobin, systolic blood pressure, and low-density lipoprotein cholesterol. The use of AI in breast cancer survivors is associated with less weight loss response to AOM compared to patients without breast cancer history and who do not take AI. Studies are needed to assess the mechanisms behind the differential weight loss response to AOM in women taking AI.

The role of anti-obesity medications (AOMs) in patients using AIs has not been studied. We sought to investigate weight loss outcomes to AOMs in patients taking AIs for breast cancer treatment. Methods: This is a matched case-control retrospective cohort study of breast cancer survivors on AI using FDA-approved AOMs (AI/AOM group). We compared their weight loss outcomes with a group of female patients with obesity, without a history of breast cancer or AI use, on AOMs (AOM group). We matched patients by age and body mass index (BMI). Exclusion criteria: AOM initiation prior to AI therapy, ≤3 months of AOM use, history of bariatric surgery, active malignancy, or pregnancy. Primary endpoint: total body weight loss % (TBWL%) at 12 months. Secondary endpoints: TBWL% at 3 and 6 months and % of patients achieving categorical TBWL ≥5%, ≥10% and ≥15% at 12 months. We used Pearson χ2 and unpaired t-test to compare baseline characteristics and outcomes between groups, and univariate analyses to estimate the contribution of variables to TBWL% in the AI/AOM group. We present results as mean ± standard deviation. Results: We included 99 patients: 63 in the AI/AOM group (63.5±10 years, BMI 34.4±7.0 kg/m2) and 36 in the AOM group (60.4±8.7 years, BMI 36.3±6.9 kg/m2). Liraglutide, semaglutide and phentermine were the only AOMs used. There was no difference in the frequency of use of these three AOMs among both groups (p= 0.1). AI/AOM group had a lower TBWL% compared to the AOM group during follow-up: 3.7± 4.3% vs. 5.6 ± 4.1% (p= 0.03) at 3 months, 3.9± 4.3% vs. 9.5± 4.7% (p< 0.0001) at 6 months, and 5.2± 5.3% vs. 10.5± 6.8% (p< 0.0001) at 12 months. Although the AI/AOM group was more likely to use lower doses of AOMs (p= 0.0004), TBWL% differences persisted among groups at 12 months after adjusting for AOM dosing (p< 0.0001). There was no difference in nutritional and behavioral modification sessions among groups (p=0.06 and p=0.9, respectively). At 12 months, the percentage of patients achieving ≥5%, ≥10%, and ≥15% TBWL was greater in the AOM group compared to the AI/AOM group: ≥5% TBWL, 85.7% vs. 50.0% (p< 0.0001); ≥10%, 57.1 vs. 20.0% (p< 0.0001); and ≥15%, 28.6 vs. 5.0% (p< 0.0001); respectively. We found no predictors of TBWL% at 12 months in the AI/AOM group. Conclusion: Weight loss outcomes in breast cancer survivors on AIs taking AOMs are poorer compared to patients without breast cancer history and not taking AIs. Studies are needed to assess the mechanisms behind the different weight loss response to AOMs in women taking AIs, which we hypothesize may relate to AIs’ anti-estrogenic effect on lean and fat mass. Electroretinographic and optical coherence tomography findings in breast cancer patients using aromatase inhibitors

Background: The present cross-sectional study has the purpose to investigate the impact of aromatase inhibitors (AIs) on retinal nerve fiber layer (RNFL) thickness, optic nerve and macular function in patients using AIs for breast cancer treatment.Methods: Participants in our study were 41 hormone-receptor-positive earlystage breast cancer patients who were treated with AIs in the adjuvant setting. Moreover, 40 age- and gender-matched control subjects, having neither ocular nor systemic disorders, were included in this study. All participants underwent a complete ophthalmological examination, including best corrected visual acuity (BCVA) assessment, RNFL thickness and central foveal thickness (CFT) measurement, visual evoked potentials (VEP) recording and multifocal-electroretinogram (mf-ERG) recording. Univariate and multiple regression analyses were performed.Results: At the multiple regression analyses, patients receiving AIs presented with lower average RNFL and inferior RNFL. Moreover, similarly to the univariate analysis, intake of AIs was associated with lower amplitude P100, lower retinal response density in ring 1 and ring 2, longer peak time P100 and longer P1 time in ring 1.Conclusion: Our study is the first in the literature investigating the potential effect of AIs on RNFL thickness, optic nerve and macular function in patients using AIs for breast cancer treatment. The principal message of our study is that patients using AIs exhibited a significant decrease in RNFL thickness (average, superior and inferior), retinal response density and visual acuity compared to healthy controls, while VEP findings (both amplitude and peak time of P100) differ significantly as well.

The Effect of Grape Seed Extract on Estrogen Levels of Postmenopausal Women: A Pilot Study

ABSTRACTThe role of estrogens in breast cancer (BC) development is widely accepted, leading to the development of selective estrogen receptor modulators and aromatase inhibitors for BC treatment and prevention. However, because of potential adverse effects, healthy women with high risk of BC are hesitant to take them. Preliminary evidence from animal studies shows that grapes may have an aromatase-inhibiting effect, decreasing estrogen synthesis and increasing androgen precursors. We conducted a randomized, double-blind, dose-finding early-phase trial on the effect of grape seed extract (GSE) on estrogen levels. Postmenopausal women who met study inclusion criteria (N = 46) were randomly assigned to daily GSE at a dose of 200, 400, 600, or 800 mg for 12 weeks. Primary outcome was change in plasma levels of estrogen conjugates from baseline to 12 weeks posttreatment. Thirty-nine participants (84.8%) completed the study. GSE in the 4 daily doses did not significantly decrease estrogen or increase androgen precursors.
