This article explores contemporary dissection techniques, their applications in dental practice, materials used, and the results achieved through these methods.

NANOTECHNOLOGY IN DENTISTRY: DIAGNOSTICS AND TREATMENT

The rapid development of nanotechnology in medicine and dentistry has led to an exponential increase in the scope of research in this area. Aspects of nanotechnology that have already brought benefits include advanced drug delivery platforms, molecular imaging, and the development of treatment materials in dental practice. The aim of the research. To investigate the current possibilities of using nanotechnology in dentistry for diagnosis and treatment. Materials and methods of the research. The materials were used scientific publications by authors conducting their scientific and practical research in the field of dentistry and nanotechnology. The search was conducted in the scientometric databases PubMed, Scopus, Web of Science, Google Scholar using the terms “nanoparticles”, “oral cavity”, “nanomaterials”, “nanodentistry”, “diseases”, “biomaterials”, “treatment”, “diagnostics”. The research was conducted using such scientific methods as theoretical generalization and grouping, formalization, analysis, synthesis, and generalization of the results. The search resulted in the selection of publications for the period 2019-2024 that studied nanotechnology and its application in dental practice in diagnosis and treatment and will be discussed in this review. Results. The article shows a unique feature of nanotechnology, which is that the properties of a material at the nanoscale can differ from those at the macro level. This can be explained by two reasons: first, nanomaterials have a large surface area to volume ratio, which causes their high reactivity and, accordingly, affects their mechanical and electrical properties. Secondly, at the nanoscale, quantum effects dominate the material's behavior, giving rise to interesting observations of its electromagnetic and optical properties. Knowing the beneficial aspects of nanomedicine, scientists are trying to use nanotechnology to diagnose, treat and prevent dental diseases. Nanotechnology is used for sensitive medical procedures. They also demonstrate successful and useful applications for precise pain management, tooth restoration, and reduction of tooth hypersensitivity. Nanomaterials can plug open dentinal tubules, improving patient comfort and overall oral health. Orthodontic therapy has also been revolutionized by nanomaterials with shape memory properties, which enables faster and more efficient tooth movement. The development of revolutionary products and therapeutic alternatives is supported by ongoing research efforts, enabling the creation of dental implants, fillings and prostheses that closely mimic the characteristics of natural teeth. Nano-delivery systems are being developed to deliver drugs precisely into the oral cavity, providing optimal therapeutic outcomes with minimal side effects. This review highlights the multifaceted aspects of nanomedicine and how nanotechnology is proving to be useful in the diagnosis and treatment of dental diseases. The integration of nanotechnology into dentistry represents a groundbreaking evolution that goes beyond conventional oral care methods, allowing for the development of innovative diagnostic methods and improved oral health. Prospects for further research. Nanotechnology has revolutionized dentistry by changing the way oral care is conceptualized, delivered, and maintained. To develop effective methods for the diagnosis and treatment of dental diseases, it is necessary to continue research on the optimization of the mechanical properties and cytotoxicity of dental composite materials based on nanotechnology.

Silver modified atraumatic restorative treatment: a paradigm shift in dental caries management.

Silver diamine fluoride (SDF) and atraumatic restorative treatment (ART) are treatment modalities that are minimally invasive and used for the prevention and control of dental caries. The amalgamation of these two techniques has led to the development of silver modified atraumatic restorative treatment (SMART). In this approach the carious lesion is restored with glass-ionomer cement (GIC) after SDF application. SMART effectively arrests caries without removing additional tooth structure, making it a promising option for caries management in children who lack cooperative ability.This article reviews the literature on SMART, the evidence regarding its applications in dental practice, its advantages, drawbacks, the scope for research, and clinical use.

An injectable dental pulp-derived decellularized matrix hydrogel promotes dentin repair through modulation of macrophage response

Maintaining the viability of damaged pulp is critical in clinical dentistry. Pulp capping, by placing dental material over the exposed pulp, is a main approach to promote pulp-dentin healing and mineralized tissue formation. The dental materials are desired to impact on intricate physiological mechanisms in the healing process, including early regulation of inflammation, immunity, and cellular events. In this study, we developed an injectable dental pulp-derived decellularized matrix (DPM) hydrogel to modulate macrophage responses and promote dentin repair. The DPM derived from porcine dental pulp has high collagen retention and low DNA content. The DPM was solubilized by pepsin digestion (named p-DPM) and subsequently injected through a 25G needle to form hydrogel facilely at 37 °C. In vitro results demonstrated that the p-DPM induced the M2-polarization of macrophages and the migration, proliferation, and dentin differentiation of human dental pulp stem cells from deciduous teeth (SHEDs). In a mouse subcutaneous injection test, the p-DPM hydrogel was found to facilitate cell recruitment and M2 polarization during the early phase of implantation. Additionally, the acute pulp restoration in rat models proved that injectable p-DPM hydrogel as a pulp-capping agent had excellent efficacy in dentin regeneration. This study demonstrates that the DPM promotes dentin repair by modulating macrophage responses, and has a potential for pulp-capping applications in dental practice.

Rapid Deposition of the Biomimetic Hydroxyapatite-Polydopamine-Amino Acid Composite Layers onto the Natural Enamel.

In this work, we developed a technology that enables rapid deposition of biomimetic composite films onto natural enamel slices (known as biotemplates). These films are composed of polydopamine (PDA) and nanocrystalline carbonate-substituted hydroxyapatite (nano-cHAp) that have been functionalized with amino acid l-Arginine. We utilized atomic force microscopy (AFM) and scattering scanning near-field optical microscopy (s-SNOM) combined with infrared (IR) synchrotron to achieve nanoscale spatial resolution for both IR absorption and topography analyses. This combined analytical modality allowed us to understand how morphology connects to local changes in the chemical environment on the biotemplate surface during the deposition of the bioinspired coating. Our findings revealed that when using the proposed technology and after the deposition of the first PDA layer, the film formed on the enamel surface nearly covers the entire surface of the specimen whose thickness is larger on the surface of the emerging enamel prisms. Calculation of the crystallinity index for the biomimetic layer showed a multiple increase compared with natural enamel. This indicates regular and dense aggregation of nano-cHAp into larger crystals, imitating the morphology of natural enamel rods. The microhardness of the formed PDA-based biomimetic layer mineralized with nano-cHAp functionalized with amino acid l-Arginine deposited on natural enamel was practically the same as that of natural enamel. The characterization of nano-cHAp-amino acid-PDA layers using IR and Raman microspectroscopy showed that l-arginine acts as a conjunction agent in the formation of mineralized biomimetic composite coatings. The uniformity of the mechanisms of PDA layer formation under different deposition conditions and substrate types allows for the formation of coatings regardless of the macro- and micromorphology of the template. Therefore, the results obtained in this work have a high potential for future clinical applications in dental practice.

Analysis of anatomical variations in the frontal and maxillary sinuses and their relevance in dentistry

Introduction: Paranasal sinuses can be defined as air-filled cavities lined with mucosa. Although there is a limited number of previous studies that have addressed the relationship between anatomical variations in these sinuses and dentistry, it is essential to emphasize the significance of morphofunctional knowledge of the maxillary sinus (MS) and frontal sinus (FS) for various dental specialties. Objectives: To investigate and analyze anatomical variations in selected paranasal sinuses (MS and FS) from the embryonic period to advanced age, and to correlate them with their diverse applications in dental practice. Methods: Electronic databases PubMed, Lilacs (via Bvsalud), and Scielo were consulted using the descriptors: "anatomical variation," "maxillary sinus," "frontal sinus," "dentistry," and their English counterparts. Initially, a total of 334 published articles were found, and after reading the abstracts and applying inclusion and exclusion criteria, 19 articles were analyzed. Results: The main morphological variations found included changes in dimension, number, characteristics of septations, and physiological pneumatization, which vary with age, sex, and facial sides. Among the dental implications, notable aspects include associated pathologies, techniques for maxillary sinus elevation in implantology, craniofacial profile relationships, connections with orthodontics and forensic dentistry, imaging techniques, and general surgical treatments. Conclusion: The literature reviewed suggests that the morphology of the sinuses analyzed directly influences dental practice, contributing to the improvement of clinical, forensic, surgical, and other procedures. However, studies involving the subject matter are still scarce, thus requiring further investigations.

DTR-Net: Dual-Space 3D Tooth Model Reconstruction From Panoramic X-Ray Images.

In digital dentistry, cone-beam computed tomography (CBCT) can provide complete 3D tooth models, yet suffers from a long concern of requiring excessive radiation dose and higher expense. Therefore, 3D tooth model reconstruction from 2D panoramic X-ray image is more cost-effective, and has attracted great interest in clinical applications. In this paper, we propose a novel dual-space framework, namely DTR-Net, to reconstruct 3D tooth model from 2D panoramic X-ray images in both image and geometric spaces. Specifically, in the image space, we apply a 2D-to-3D generative model to recover intensities of CBCT image, guided by a task-oriented tooth segmentation network in a collaborative training manner. Meanwhile, in the geometric space, we benefit from an implicit function network in the continuous space, learning using points to capture complicated tooth shapes with geometric properties. Experimental results demonstrate that our proposed DTR-Net achieves state-of-the-art performance both quantitatively and qualitatively in 3D tooth model reconstruction, indicating its potential application in dental practice.

Knowledge of Saudi dental students and interns towards luting cements and their applications in fixed prosthodontics

BackgroundEnhancement of students’ knowledge is essential in improving their clinical skills and performance. Thus, the curriculum should be prepared to achieve a better outcome. The current study aimed to determine the dental students’ and interns’ basic knowledge towards dental luting cements and their application in dental practice to improve the theoretical and clinical training sections.Materials and methodsA cross–sectional study was conducted among dental students and interns at three Colleges of Dentistry in the Kingdom of Saudi Arabia between September 2019–June 2020. An online questionnaire was used which included demographic data, questions about luting cement usage, cementation techniques, and commonly used cements in dental clinics. Descriptive analysis and chi–square test were used to show the association between level of dental education and the use of dental cements using SPSS software. The significance level was set at 5%.ResultsThe total respondents were 626 dental students/interns of whom 78.8% were undergraduate dental students. Participants who reported undergraduate studies as the source of information were 79.7%. The type of restoration was the main factor in luting cement selection (62.6%). Concerning the isolation technique in cementing laminate veneers, 49.7% used dri–angles, cotton rolls and saliva ejectors. Dual–cure resin cement was the most common cement used in all the mentioned restorations except in pressed porcelain laminate veneers and cement–retained implant–supported restorations.ConclusionsStudents’ knowledge and practice in managing dental implants and porcelain laminate veneers need to be improved. The selection of a luting agent for a given restoration by students and interns was based on the basic knowledge, available cement, and the type of restoration. Awareness towards the management of short prepared teeth and custom-made cast posts and cores is also limited.

A Parental Behavior Scale in Pediatric Dentistry: The Development of an Observational Scale

Children's treatment means a triangular relationship between the child, practitioner, and parent, with specific interactions influencing the procedure. The objective was to create and validate a hetero-rating scale of parental behavior and verify the correlation between child and parental behavior during pediatric dentistry sessions. Treatment sessions were recorded and evaluated, including 60 children representing three age groups. Two raters interpreted the resulting video clips using the modified Venham scale for children and the new hetero-rating scale for parents. They analyzed the videos twice and attributed scores at different time points of the appointment. The correlation between parental behavior upon entrance and the children's behavioral at the dental office in the treatment stage was significantly positive in both raters (Kendall Tau: 0.20-0.30). Furthermore, a panel of 20 dental practitioners scored a randomized selection of five recordings per age group. The level of agreement between the two experts was higher than that between the 20 clinicians. Venham types of scale involving multiple aspects can be used in research, but their application in dental practice requires further development. The link between parental anxiety and child anxiety is confirmed, but further research is required to incorporate specific aspects of treatment and parental behavior.

The Shortened Dental Arch Concept: Awareness, Knowledge, and Practice of Dentists in Dubai and the Northern Emirates, United Arab Emirates

This is a cross-sectional study utilizing an online questionnaire anonymously to investigate the awareness and views of dentists about SDA. The questionnaire was sent to all 901 dentists registered with the Emirates Medical Association (EMA). The questionnaire consists of 17 questions, which comprise demographics, awareness, and application in dental practice, preferred treatment modality, and risks and benefits associated with SDA. The data were analyzed using SPSS Statistics. The response rate reported was 40.3%. Two-thirds of the respondents (65.8%) were aware of the SDA concept; however, it was not usually applied in clinical practice (n = 196, 54.7%). Specialists were more aware of the concept (p ≤ 0.001) and applied it more frequently in their clinical practice (p=0.041) than general dental practitioners (GDPs). Respondents agreed that SDA was associated with the risks of teeth migration (n = 211, 59.9%), tooth wear (n = 196, 55.8%), and/or temporomandibular disorder (TMD) (n = 163, 45.3%). The implant was the treatment of choice for many of the participants (n = 169, 46.6%) to replace missing molars, followed by the acrylic removal partial denture (RPD) (n = 129, 35.5%). Most dentists who responded to this survey were aware of the SDA concept and had a positive attitude about it. However, they did not apply it frequently in their clinical practice.

Design and build teledentistry applications based on the appstore playstore on the management of dental practice during the Covid-19 pandemic and adapting new habits

Health is one of the basic needs of every human being. Covid-19 pandemic has challenged existing healthcare systems around the world, so the development of information technology is increasing. This article is aimed to create a teledentistry applica-tion based on the appstore and playstore for dental practice during the Covid-19 pandemic. Implementation method includes conducting literature studies, requirements engineering, system design, system implementation anddrawing conclusions and suggestions. The result show tested functionality on application features is directly proportional to the expected result. It was concluded that the design of an android-based teledentistry application in dental practice can simplify the service process, help increase time effectiveness in the dental clinic service process, increase the efficiency of the recording system in dental clinic services quickly and accurately. And the design of this system canhelp minimize losses due to errors in recording patient data.

The environmental impact of community caries prevention - part 1: fluoride varnish application

Background Healthcare is a significant contributor to climate change and planetary health. Prevention of oral disease, such as caries, is an important part of any mechanism to improve sustainability. Caries prevention includes community schemes such as water fluoridation, toothbrushing, or fluoride varnish (FV) application. The aim of this study was to quantify the environmental impact of FV application.Materials and methods A comparative life cycle assessment (LCA) was conducted to quantify the environmental impact of a five-year-old child receiving two FV applications in a one-year period in schools and in dental practice.Results FV application in dental practice during an existing appointment had the lowest environmental impact in all 16 categories, followed by FV application in schools. FV application at a separate dental practice appointment had the highest impact in all categories, with a majority of the impact resulting from the patient travel into dental practice.Discussion FV application while a child is already attending dental practice (for example, at routine recall) is the most sustainable way to deliver FV. School FV programmes are an alternative, equitable way to reach all children who may not access routine care in dental practice.

An Application of Machine Learning Techniques to Analyze Patient Information to Improve Oral Health Outcomes

ObjectiveVarious health-related fields have applied Machine learning (ML) techniques such as text mining, topic modeling (TM), and artificial neural networks (ANN) to automate tasks otherwise completed by humans to enhance patient care. However, research in dentistry on the integration of these techniques into the clinic arena has yet to exist. Thus, the purpose of this study was to: introduce a method of automating the reviewing patient chart information using ML, provide a step-by-step description of how it was conducted, and demonstrate this method's potential to identify predictive relationships between patient chart information and important oral health-related contributors.MethodsA secondary data analysis was conducted to demonstrate the approach on a set of anonymized patient charts collected from a dental clinic. Two ML applications for patient chart review were demonstrated: (1) text mining and Latent Dirichlet Allocation (LDA) were used to preprocess, model, and cluster data in a narrative format and extract common topics for further analysis, (2) Ordinal logistic regression (OLR) and ANN were used to determine predictive relationships between the extracted patient chart data topics and oral health-related contributors. All analysis was conducted in R and SPSS (IBM, SPSS, statistics 22).ResultsData from 785 patient charts were analyzed. Preprocessing of raw data (data cleaning and categorizing) identified 66 variables, of which 45 were included for analysis. Using LDA, 10 radiographic findings topics and 8 treatment planning topics were extracted from the data. OLR showed that caries risk, occlusal risk, biomechanical risk, gingival recession, periodontitis, gingivitis, assisted mouth opening, and muscle tenderness were highly predictable using the extracted radiographic and treatment planning topics and chart information. Using the statistically significant predictors obtained from OLR, ANN analysis showed that the model can correctly predict >72% of all variables except for bruxism and tooth crowding (63.1 and 68.9%, respectively).ConclusionOur study presents a novel approach to address the need for data-enabled innovations in the field of dentistry and creates new areas of research in dental analytics. Utilizing ML methods and its application in dental practice has the potential to improve clinicians' and patients' understanding of the major factors that contribute to oral health diseases/conditions.

Learning and understanding Quechua to reduce linguistic distance in oral care in Latin America: A narrative review

Aim: It has been proven that the linguistic differences between patients and health professionals make access to health care difficult in Latin American countries with a Quechua-speaking population. Therefore, this review focusses on the importance of learning the Quechua language as a useful tool for increasing health access. Materials and Methods: Electronic databases such as Scopus, Web of Science, and PubMed were searched from January 2010 until December 2020 using the Quechua terms for rural population, language barriers, health, and services. Results: This narrative review is based on the importance of learning the Quechua language to reduce the linguistic barrier in medical–dental care and was classified as: (1) Quechua and access to health, (2) the importance of learning Quechua in dental clinical practice, (3) teaching Quechua in universities, and finally, (4) a vocabulary in Quechua is proposed for its application in dental practice. Conclusion: Knowledge of the Quechua language is key to improving medical and dental care in Andean countries; therefore, training in this language should start from undergraduate studies.

Effects of multi-components released from S-PRG filler on the activities of human dental pulp-derived stem cells.

Numerous studies have shown that the sustained release of ions from dental restorative materials have acid buffering capacity, prevents tooth enamel demineralization, and inhibits bacterial adhesion. Herein, the release behavior and bioresponsiveness of ions released from surface pre-reacted glass-ionomer (S-PRG) fillers were investigated in different types of media based on human dental pulp-derived stem cell (hDPSC) responses. The hDPSCs were cultured for 1-7 days in S-PRG eluates diluted with varying amounts of cell culture media. S-PRG released several types of ions, such as F-, Sr2+, Na+, Al3+, BO33-, and SiO32-. The balance of eluted ions differed depending on the dilution and solvent, which in turn affected the cytotoxicity, cell morphology, cell proliferation, and alkane phosphatase activity of hDPSCs, among other properties. The results suggest that tailored S-PRG filler eluates could be designed and prepared for application in dental practice.

Assessment of some mechanical properties of PMMA/silica/zirconia nanocomposite as a denture base material

In this research, nanocomposites of poly(methyl methacrylate) (PMMA) and a mixture of nano silica (SiO2) and nano zirconia (ZrO2) were prepared in different weight percentages of the nano fillers to improve some of the properties of PMMA resin to be used as a denture base material. The nano filles were surface modified with a coupling agent and added to the PMMA in different amounts. Impact strength, transverse strength, hardness and roughness were tested for both control and experimental groups. The results indicate that PMMA/silica/zirconia nanocomposites, prepared with 5% by weight of both types of fillers, had a slight increase in impact strength, transverse strength and hardness with only an insignificant increase in roughness. This kind of composite material may have the potential for application in dental practice.

Local anaesthesia in pediatric dentistry — An overview

Local anaesthesia is the commonly employed technique of achieving pain control in dentistry. The vital reasons why children dislike dental treatment is the fear and anxiety related to the injection of local anaesthetics. Painful dental operations cause fear, whereas fear and anxiety increase the amount of perceived pain. This review includes the certain different aspects of administration of local anaesthesia and recent procedures and devices for LA application in dental practice to achieve painless local anaesthesia in children. Biomimetic in vitro test system for evaluation of dental implant materials

ObjectivesBefore application in dental practice, novel dental materials are tested in vitro and in vivo to ensure safety and functionality. However, transferability between preclinical and clinical results is often limited. To increase the predictive power of preclinical testing, a biomimetic in vitro test system that mimics the wound niche after implantation was developed. MethodsFirst, predetermined implant materials were treated with human blood plasma, M2 macrophages and bone marrow stromal stem cells. Thereby, the three-dimensional wound niche was simulated. Samples were cultured for 28 days, and subsequently analyzed for metabolic activity and biomineralization. Second test level involved a cell-infiltrated bone substitute material for an osseointegration assay to measure mechanical bonding between dental material and bone. Standard and novel dental materials validated the developed test approach. ResultsThe developed test system for dental implant materials allowed quantification of biomineralization on implant surface and assessment of the functional stability of mineralized biomaterial-tissue interface. Human blood plasma, M2 macrophages and bone marrow stromal stem cells proved to be crucial components for predictive assessment of implant materials in vitro. Biocompatibility was demonstrated for all tested materials, whereas the degree of deposited mineralized extracellular matrix and mechanical stability differed between the tested materials. Highest amount of functional biomineralization was determined to be on carbon-coated implant surface. SignificanceAs an ethical alternative to animal testing, the established in vitro dental test system provides an economic and mid-throughput evaluation of novel dental implant materials or modifications thereof, by applying two successive readout levels: biomineralization and osseointegration.

Correlations of linear sizes of molars with cephalometric indicators of practically healthy men of the northern region of Ukraine

The question of the relationship between the metric characteristics of cranial structures and odontometric parameters, and the possibility of building models based on them, which can later find their practical application in dental practice, remain a pressing problem of modern medical science. The purpose of the study is to establish the features of the relationship between the linear dimensions of molars with the cephalometric parameters of the brain and facial skull of practically healthy men in the northern region of Ukraine. 32 practically healthy residents of the northern region of Ukraine (from Kyiv, Zhytomyr, Sumy and Chernihiv regions) underwent computed tomography followed by odontometry of molars and cephalometry. The correlations between odontometric and cephalometric parameters were assessed using Spearman’s nonparametric statistics in the licensed package “Statistica 6.1”. The peculiarities of the correlations of the studied characteristics of molars with the indicators of the cerebral skull are established – multiple, only direct reliable (r = 0.35-0.57) and unreliable (r = 0.30-0.35) average correlation force (18.7% on the upper jaw, of which the relative majority with vestibular-lingual and mesio-distal dimensions and 24.1% on the lower jaw, of which the relative majority with the height of the teeth, their crowns and the length of the tooth roots); as well as with the indicators of the facial skull – mostly direct reliable (r = 0.35-0.51) and unreliable medium strength (r = 0.30-0.37) correlations, in most cases of a single nature (6.0% on the upper jaw and 6.6% on the lower jaw, almost uniformly with the height of the teeth, their crowns, root length, vestibular-lingual and mesio-distal dimensions). The results of correlation analysis of craniofacial structures are necessary for the correct construction of mathematical models, which can later find their practical application in dental practice and forensic medicine.
