The influence of hospital accreditation on nurses’ perceptions of patient safety culture

ObjectivesHospitals’ accreditation process is carried out to enhance the quality of hospitals’ care and patient safety practices as well. The current study aimed to investigate the influence of hospitals’ accreditation on patient safety culture as perceived by Jordanian hospitals among nurses.MethodsA descriptive cross-sectional correlational survey was used for the current study, where the data were obtained from 395 nurses by convenient sampling technique who were working in 3 accredited hospitals with 254 nurses, and 3 non-accredited hospitals with 141 nurses, with a response rate of 89%.ResultsThe overall patient safety culture was (71.9%). Moreover, the results of the current study revealed that there were no statistically significant differences between the perceptions of nurses in accredited and non-accredited hospitals in terms of perceptions of patient safety culture.ConclusionThe current study will add new knowledge about nurses’ perceptions of patient safety culture in both accredited and non-accredited hospitals in Jordan which in turn will provide valid evidence to healthcare stakeholders if the accreditation status positively affects the nurses’ perceptions of patient safety culture or not. Continuous evaluation of the accreditation application needs to be carried out to improve healthcare services as well as quality and patient safety.

The UEMS experience in continuous medical education accreditation process: a ‘quo vadis’ analysis of our global database

Background: The authors systematically appraise a large database of continuous professional development (CPD) and continuous medical education (CME) events against the European Accreditation Council for Continuous Medical Education (EACCME) framework. Methods: The authors performed a retrospective observational study of all CPD or CME events within the European Union of Medical Specialists (UEMS) database between 2017 and 2019, including 91 countries and 6034 events. Assessment of event design, quality and outcomes was evaluated against a validated, expert-derived accreditation framework, using thematic analysis to extract distinct themes, and subsequent quantitative analysis. Results: The authors included 5649 live educational events (LEEs) and 385 e-learning materials (ELMs). Three thousand seven hundred sixty-two [3762 (62.3%)] of the events did not report clear justification in their needs assessment process. Most accreditation applications claimed covering a single educational need [1603/2277 (70.3%)]. Needs assessments were reported to be similar across conferences, courses and other types of events (P<0.01); 5642/6034 events (93.5%) had clearly documented expected learning outcomes; only 978/6034 (16.2%) reported a single expected learning outcome while the rest report 2–10 outcomes. Providers who declared more than one educational need also declared multiple learning outcomes (ρ=0.051, P<0.01). Conclusions: Despite EACCME providing a robust framework for the CPD/CME accreditation process, reporting quality can still be improved, as more than 1 in 2 events fail to provide a clear description of their needs assessment. To the authors’ knowledge, this is the largest educational LEE/ELM database, which can be a starting to revisit the CME/CPD accreditation process.

Concretization of Online Accreditation to Improve the Quality of Digital Technology-Based Education Services in the New Normal Era

This article aims to investigate the implementation of accreditation to enhance the quality of technology-based education services in the new normal era. The primary focus of the research is on the concrete application of online accreditation based on digital technology in the field, associated with the improvement of educational service quality. The research approach utilizes school accreditation guidelines as a reference, collecting data through interviews and document analysis, which is then qualitatively analyzed. The findings indicate that the successful implementation of online accreditation is contingent upon schools providing facilities, infrastructure, and human resources professionals in using digital technology in educational services. Furthermore, the quality of educational services is reflected in the school's output, particularly in the superior quality of graduates from accredited A-level schools compared to non-accredited ones. By employing digital technology in the accreditation process, schools face the challenge of cultivating habits of using digital technology in their operational procedures. Ultimately, this positively contributes to the overall improvement of the quality of educational services. With efficient administrative management and well-managed learning processes, technology-based accreditation encourages schools to continually adapt and enhance themselves to achieve optimal educational service quality.

New Graduate Nurse Residency: Taking Rehabilitation Nurses' Transition to Practice to a New Level.

It began with a challenge from our Chief Nursing Officer. As a Magnet hospital, it was time for our nurse residency program to be recognized as an accredited program for new graduate nurses. This article shares our experience using the Association of Rehabilitation Nurses competency statements in a creative way when applying for recognition as the first American Nurses Credentialing Center-accredited specialty hospital nurse residency program. Although these competency statements were used initially to meet the requirements of the accreditation application process, they also led to the creation of a document that serves two purposes. First, it can be used to instruct new graduate nurses on the structure and processes of a nurse residency program in which competency statements are used to guide curriculum development. Second, it can be used to inform nurse residency program managers on new graduate nurses' progress throughout the nurse residency program by tracking competency completion. The end result of this accredited program is a new graduate nurse who understands not only what it means to be a nurse but also what it means to be a rehabilitation specialty nurse.

Can a self-inspection programme improve compliance with professional standards in a hospital pharmacy?

Abstract Introduction Pharmacies must comply with professional standards to provide high-quality services. However, gaining assurance of compliance can be challenging and self-inspection was investigated as a means of consistently achieving this. Aim This project aimed to develop and implement a self-inspection programme to assess compliance with professional standards in a hospital pharmacy. Through the self-inspections, we aimed to identify required improvements and maintain achieved compliance through effective reporting, evaluation, and communication of results. Methods Standards from Royal Pharmaceutical Society professional guidance1, standards2 and General Pharmaceutical Council standards3 were used to create 41 self-inspection questions. These assessed different aspects of medicines management, including the receipt, storage, dispensing, delivery, disposal, and security of medicines. They also assessed compliance with health and safety measures and service quality indicators taken from the monthly quality report, health and safety report, cleaning logs, expiry date checks and stock checks in the department. Self-inspections were conducted monthly over a 12-month period by members of the Pharmacy Senior Leadership Team. Self-inspections were recorded using the Tendable accreditation application. Training in using the application and a standard operating procedure was in place to support this. Questions in the self-inspection were only answered as “Yes” or “No”. Additional information, images or actions taken were required for any question that was negatively scored. Following self-inspections, results were calculated by the application and a summary generated. A full report was also produced by the application. These were analysed by the Pharmacy Quality Assurance Team and included in the monthly departmental quality report for review by the Pharmacy Senior Leadership Team. Any identified actions were communicated by e-mail to the relevant team leader and an inspection summary later displayed in the department. This study did not require ethics approval. Results Compliance with standards increased steadily from 75% on the first self-inspection to 95%, with an average of 89% over the 12 months. During the self-inspections, non-compliance with 14 different standards was identified. 3 concerned non-compliance of staff with processes, which was addressed and not repeated. 11 concerned non-compliance that was repeated, including delays in updating essential documents, delays in completing essential tasks, medicines storage not complying with requirements, a correct responsible pharmacist notice not displayed and building work disrupting services. All of these have now been resolved, except deficiencies in the storage of bulk fluids, where risks have been mitigated and a long-term plan is in place. Discussion/Conclusion Compliance with professional standards in the pharmacy has been improved through the implementation of a self-inspection programme. Questions in the self-inspection defined the required standards and guidance on conducting self-inspections ensured consistent assessment, enabling both compliance and non-compliance to be identified. Processes for reporting, evaluating and communicating results from the self-inspections directed how non-compliance was resolved. This promoted awareness of required standards and helped maintain achieved compliance. Limitations of the study included failing to assess compliance with standards not covered in the self-inspection questions. A regular review of self-inspection questions, standards and results is therefore required to maintain reliability and continued improvement.

Practical Considerations for Obtaining Perioperative Transesophageal Echocardiography Accreditation: Collective Experiences at Early-Adopting Centers

The Intersocietal Accreditation Commission (IAC) is a non-profit accrediting organization committed to ensuring the quality of diagnostic imaging and related procedures. It is comprised of a collaboration of stakeholders, spanning numerous medical professionals and specialties. In a recent initiative, IAC Echocardiography introduced a new accreditation specifically for Perioperative Transesophageal Echocardiography (PTE). This accreditation process is anchored in rigorous clinical peer review to ensure diagnostic quality and report accuracy, thus maintaining high standards of medical care.The authors represent the inaugural four sites to achieve IAC accreditation for PTE who have collaborated to share their experiences in achieving this accreditation. This review endeavors to offer actionable insights and proven solutions to navigate the accreditation journey for others. Mirroring the IAC Standards and Guidelines for PTE accreditation, this review is divided into three pivotal sections: 1) organization of a perioperative echocardiography service, including stakeholder engagement to facilitate the application for accreditation; 2) performance of examinations and reporting; and 3) instituting quality improvement strategies and establishing a robust program.The pursuit of accreditation in PTE is to transcend a mere compliance exercise. It signifies a dedication to excellence, continual growth, and above all to the well-being of patients.

Benefits, mechanisms and challenges of international accreditation for teacher education: ASEAN academics’ perspectives

Purpose This study aims to explore some benefits and challenges of establishing an international accreditation for teacher education institutions (TEIs) by AsTEN Quality Assurance Agency. This specific accreditation agency is expected to improve the quality of teaching, learning and research at TEIs in ASEAN region. Design/methodology/approach The qualitative study generates data from questionnaires and online semi-structured interviews among ASEAN academics. They work as teacher educators in Indonesia, Singapore, Thailand and Malaysia. Findings As findings show, participating academics from Indonesia, Singapore, Malaysia, the Philippines, Thailand and Brunei Darussalam believe that it is necessary to have a specific international accreditation agency that can properly accommodate particular standards of teacher education. This accommodation is expected to increase the governance of quality teaching, learning and researching to prepare more competent and professional future teachers. Participating academics also acknowledge some potential challenges this specific accreditation agency may have, including local acceptance by national governments in ASEAN region and global acknowledgement from international accrediting agencies, mostly based in Global North countries. Research limitations/implications The study only involves academics in five ASEAN countries of Indonesia, Malaysia, Singapore, Thailand and the Philippines so that it may have less international acceptance. Practical/social implications The study also identifies aspects and mechanisms of blended online-onsite international accreditation application for TEIs, which grows its significance because of technological advancement, efficiency and prolonged COVID-19 pandemic. Originality/value The study provides a digital accreditation system for TEIs, particularly in ASEAN region. This originality is important in this era of Internet of Things.

Due to the earthquakes that have occurred in recent years, many lives and property have been lost in the world. After these earthquakes, national and international regulations used in the world have updated with various changes. Especially the regions where the population and industry in Turkey are dense are under the risk of heavy earthquakes. When the damages in the buildings after the earthquakes were examined, it has seen that they are in poor quality and uncontrolled structures. The inspection of the buildings has built in Turkey is carried out by the relevant municipalities and ministries. The most important part of these inspections is carried out by laboratories that inspect the suitability of the materials used. The importance of public and private control laboratories in the field of construction organized under the ISO/IEC 17025 standard in Turkey is important for the safety of buildings. In this context, the effectiveness of the construction materials laboratories in Turkey according to the ISO 17025 standard has investigated. In this context, the establishment processes of the civil engineering laboratory, which is in the process of establishment and for which an accreditation application will be made, have examined in accordance with the ISO 17025 standard. In addition, the compliance of the laboratory's quality management system documents with the ISO 17025 standard has investigated. Factors affecting compliance with national accreditation essential safety standards in the Kingdom of Saudi Arabia

Accreditation is a widespread culture internationally and nationally. The effectiveness of compliance with accreditation standards was positively correlated with health care settings’ performance in multiple aspects: leadership, professional performance, patient safety and organizational culture. There is limited knowledge of the national compliance rate with accreditation standards. Therefore, it is important to assess the hospital compliance with accreditation rate in the Kingdom Saudi Arabia (KSA) and its related factors. This paper presents a quantitative cross-sectional study. Data were extracted from the annual Essential Safety Requirement (ESR) survey database from the Central Board for Accreditation of Health care Institutions (CBAHI) research center during the period 2016 to 2018. Hospitals that started their operation after the first ESR survey round in 2016 or shut down during the study period were excluded. The hospital scoring was on a scale of 0 to 100 and classified as follows: score 2 if the hospital satisfactory compliance (Fully Met) was ≥ 80% and score 1 if particular compliance (Partially Met) was ≥ 50% to < 80%. Then, a score of 0 indicated insufficient compliance (Not Met) when < 50% and a score of not applicable (NA) if the standard does not apply to the hospital. A total of 437 hospitals were surveyed in 20 regions in the KSA and had an overall compliance rate on average that was higher among private hospitals than among public hospitals (77% vs. 66%). Overall, private hospitals had a significantly better compliance rate than public hospitals (mean rate = 84% vs. 68%, respectively, P = 0.019). Large hospitals had more compliance with some standards than smaller hospitals. After adjusting for the year of the survey report, the private hospital type was more compliant than the public hospital. This study supports mandatory accreditation programs for both public and private health sectors, with increased monitoring by the concerned parties (i.e., CBAHI and the Ministry of Health). The authors encourage the application of accreditation for specialized and independent health services.

The AOA/ACGME Single Accreditation System and Its Immediate and Future Impact on Family Medicine.

The American Osteopathic Association (AOA) agreed to combine its graduate medical education programs with the Accreditation Council for Graduate Medical Education (ACGME) between July 1, 2015 and June 30, 2020 in an initiative called the Single Accreditation System (SAS). The objective of our study was to identify the impact the SAS had on the ACGME, family medicine (FM), and implications for the future of FM. We collected and compiled data from the AOA, ACGME, and the National Residency Matching Program (NRMP). Analysis reveals the effects that the addition of former 122 AOA-accredited FM residencies had on the ACGME and FM programs. Several osteopathic FM programs encountered challenges meeting ACGME accreditation standards. As of June 1, 2020, 89 of 122 accreditation applications received initial or continuing accreditation; the others had accreditation issues to resolve. The Osteopathic Recognition program emphasizing training in osteopathic principles and practices was a popular option in FM residencies. Fewer DOs serve as program directors in former AOA-accredited FM residencies. The SAS has shifted the balance in the percentages of MDs, DOs, and international medical graduates (IMGs) in FM. Trends in FM show that as more DOs enter the NRMP the percent of MDs and IMGs decreases. In the future, it is projected that DOs will outnumber MDs and IMGs in ACGME FM residencies. The 51 new medical schools started between 2010 and 2020 will generate a test for the integration of their graduates into GME. Increased competition for FM residencies is expected.

S126: AN EU -- ARISE INITIATIVE GAP ANALYSIS APPROACH TO IMPROVING THE QUALITY OF LABORATORY SYSTEMS IN SUB-SAHARAN AFRICA.

Background: Accurate and reliable laboratory results are critical to diagnostics in sickle cell disease (SCD) newborn screening. In sub-Saharan Africa, due to neglect of some national health laboratories, there has been persistently high levels of laboratory error, lack of functioning quality management systems and laboratory accreditation. ARISE (African Research and Innovative Initiative for Sickle Cell Education) is a research project funded by the EU. It involves a secondment plan across 8 work packages between institutions in EU (Italy, France, UK and Cyprus) and non-EU (Nigeria, Lebanon, Kenya and USA) countries, creating an interagency and multidisciplinary staff exchange programme to share best practices in newborn screening, diagnosis and treatment of SCD, leading to improved disease outcomes. Secondments include medical, nursing, laboratory, administrative, academic and research professionals. Led by the RCPath and INSERM, WP3 is tasked with improving laboratory diagnostics, capacity, and quality assurance systems for population screening, diagnosis, treatment and monitoring of SCD through assessments, training and mentoring of laboratory health professionals. This is facilitated by baseline, interim and final gap assessments Aims: To compare a baseline gap analysis at the commencement of the project in 2019 with an interim gap assessment 2 years into the project, following secondments to UK laboratories from Nigeria To identify areas of increased focus and support during planned secondments, workshops, and virtual lectures Methods: 6 partner organisation laboratories participated in the baseline gap assessment in 2019, while 10 laboratories participated in the interim gap assessment in 2021, due to new institutions joining the ARISE initiative. An electronic online questionnaire (40 questions) was sent to each laboratory lead. Results: Survey results of the 6 laboratories that undertook both the baseline and interim gap assessment were analysed. A sample of comparative results between 2019 and 2021 include the following: Working towards accreditation - 2/6 versus 3/6: There is the need for each laboratory to progress towards an application for accreditation. SOP’s available for tests and processes - 38 versus 32: This might suggest standardisation and rebranding of documents previously labelled as SOP’s. No schedule of SOP revisions in 0/6 laboratories versus 4/6 laboratories: This clearly demonstrates an understanding that SOP’s need to be updated via a document management system. Scientist certified competences - 22 versus 22. Self -assessed improvement targets - 18 versus 15. Laboratory lead for QA - 3/6 versus 6/6: The importance of QA leadership for each laboratory identified prior to interim assessment. Summary and Recommendations: Early evidence suggests that ARISE secondments have facilitated improved approaches to laboratory governance procedures and QA. Mentoring by EU partner laboratories will further enhance this. ARISE related workshops and train - the- trainer programmes will support capacity building and skills transfer. A final assessment will be undertaken at the end of the ARISE initiative. Conclusion: The ARISE initiative will build on the early success of improvement in laboratory systems, to further support service development and capacity building. The target for each partner laboratory will be a timeline of application to full accreditation

The competent and experienced staff is one of the criteria for assessment the technical competence of the laboratory. This paper presents the AHP analysis of competent and experienced laboratory accreditation staff. The following sub-criteria were analyzed: training SRPS ISO 17025:2017, training ISO 9001, training staff to work on a particular machine using a valid method, responsibility, impartiality, confidentiality, authority and interpersonal cooperation of the laboratory staff. The criterion for competent and experienced staff should meet all the requirements for the technical equipment of the laboratory to submit an application for accreditation.

Community participation in advancing education can be seen from the proliferation of establishment of PAUD and PNF institutions in the community. The application for accreditation is also carried out as an effort to obtain quality recognition from the government and the community. However, institutional managers often do not have enough information regarding the application procedure to improve the quality of the institution based on the accreditation criteria. This community service program is carried out in order to provide assistance to PAUD and PNF managers to apply for and obtain superior accreditation results. This program is intended for. The target of this activity is aimed at managers of PAUD institutions and other non-formal education institutions such as PKMB and LKP, especially those who do not yet have institutional accreditation or plan to reaccredit. This community service program is attended by representatives from 32 institutions, which are currently and will apply for accreditation. The mentoring process is carried out through seminars on urgency, stages, assessment criteria, and strategies for obtaining superior accreditation. As well as consulting assistance during the application process.

PAUD accreditation is a tangible form of assessing the feasibility of the quality of the program run by the PAUD unit. This study aims to examine various literatures on the accreditation of PAUD units based on Sispena 3.1. The research method used is literature review, researchers collect data from various literatures to solve a problem. The results of the study obtained are (1) in attaching the assessment process documents in accordance with the Accreditation Prerequisite Assessment (PPA) instrument through SISPENA 3.1; (2) the assessor conducts an assessment process for the classification of applications for accreditation (KPA); (3) Education units in fulfilling the PPA can be assisted by the integration of an external database with the Sispena 3.1 application; (4) The data collection procedure for Sispena 3.1 starts from the PPA filling stage to the Validation and Verification stage to determine the accreditation results

Influence Of Accreditation Readiness In Improving The Quality Of Teacher Teaching In PAUD Banguntapan Bantul Subdistrict

Recognition of educational institutions is demonstrated through accreditation and quality of teaching. This research aims to explain the influence of PAUD accreditation readiness on the quality of teaching in 40 PAUDs in Banguntapan. Research methodology uses quantitative descriptive. The results showed the existence of six criteria for the willingness of accreditation mechanisms that must be met, among others: first, general requirements; Second, special requirements; third, classification of accreditation applications; fourth, the completeness of the document; fifth, validation and verification; and sixth, accreditation results. The results showed X2h&gt;X2t, where there was a significant influence on accreditation readiness to the quality of teaching in PAUD Banguntapan. Meanwhile, six schools focus on general requirements, and six schools also on specific needs. Twelve schools prepare for the early stages of the school's survival. Furthermore, eight schools in the preparation stage of accreditation application classification; seven schools in the readiness stage of the process and awaiting visit; ten schools in terms of validation and verification; And three schools are fully prepared for accreditation.

Brief Review of Accreditation Policy For X-Ray Compliance Testing Laboratory The Compli¬ance Test Program has been Implemented Since 2011 and The Updated Regulation was in 2018. According to the implementation progress, adjustments are needed to the dynamics of the problems that arise. In 2021, it is planned to prepare a draft amendment to BAPETEN Regulation Number 2 of 2018; therefore, an analysis of the implementation profile of the regulation is carried out to identify existing obstacles. One of the problems identified is the polemic of the urgency of accreditation obligations for the Compliance Testing Laboratory, which is the institution appointed by the Head of BAPETEN to carry out compliance tests and issue compliance certificates. Currently, 78% of laboratories are not accredited, most of which are importing companies. Therefore, it will have implications for reducing laboratory availability and constraining the compliance test process in health facilities. This paper aims to identify aspects that can hinder the application for laboratory accreditation, especially for importing companies, and the proposed solution that can recommend. This paper uses a regulatory impact analysis approach. The study results show that laboratory accreditation is essential to maintain the quality of services, competencies, and outputs of the laboratory so that four options that LUK can take are proposed in dealing with obstacles in applying for accreditation. This paper can provide contributions in preparing the draft amendments to BAPETEN Regulation Number 2 of 2018.

FACTORS AFFECTING USER BEHAVIOR OF TECHNOLOGY APPLICATION OF MANAGEMENT INFORMATION SYSTEM

This research aimed to determine the factors influencing the behavior of management information system users at the National Accreditation Committee. The theory used was based on the Technology Acceptance Model (TAM) theory extension, and the analytical method used was the Structural Equation Model (SEM). The factors were according to Davis's modified theory, where external factors were the quality of internet connection and leadership support. Internal factors included perceived ease of use and perceived usefulness, attitudes toward using, behavioral intention to use, and actual system use. This research was conducted on 128 respondents representing the Conformity Assessment Bodies (CABs). This research showed that leadership support had a significant effect on perceived ease of use and perceived usefulness. Meanwhile, the quality of the internet connection only affected the perceived usefulness. Leadership support, quality of internet connection, perceived ease of use, perceived usefulness, and attitudes toward using were factors that affected user behavior intention to use. This research's managerial implication is to strengthen the server and increase the leadership's motivation towards the CAB employees by socializing accreditation applications. Accreditation, which guarantees quality, embodies a response to the global development in many areas of the social life. Education is an analogous field. Therefore, the educational institutions awarded a certificate constitute a reliable place for acquiring new competences. The article attempts to present accreditation issues on the ground of the applicable law. Through analysis of the legal norms, the application of accreditation and the process of obtaining it, is presented in the paper. This leads to the conclusion that in the context of the value of accreditation, it should have the obligatory aspect rather than - as it is under currently binding law - only optional character.
