Antibiotic Resistance Profile of Bacteria Isolated from Stream Water in Bokkos Local Government Area of Plateau State, Nigeria

Water from selected streams in Bokkos local government area of Plateau State Nigeria was obtained and analysed for the presence of antibiotic resistant bacteria and the resistance pattern of the isolates against several antibiotics. A total of 25 stream water samples were collected for testing from five (5) different study sites (Tudu, Mabel, Dairy Farm, Maihakorin Gold and Kuba). Heterotrophic and coliform bacteria were isolated from the samples using 10-fold serial dilution and the pour plate method. The isolated bacteria were Proteus vulgaris, Salmonella spp., Escherichia coli, Pseudomonas spp., Shigella spp., Bacillus spp., Streptococcus spp., Staphylococcus aureus. Antibiotic susceptibility of isolates was determined using the disc diffusion method. Among the eight bacterial isolates, Salmonella spp. was the most frequently occurring bacterium 18(22.8%), followed by E. coli 16(20.3%), while the least occurring bacterium was Staphylococcus aureus 5(6.3%). The highest antibiotic resistance was recorded for Salmonella spp., E. coli, and Pseudomonas spp. against ceftazidime 12(67%), Ampicillin 12 (75%), and ceftazidime 11 (92%), respectively. However, E. coli, Proteus spp., Staphylococcus spp., and Shigella spp. showed high antibiotic susceptibility against cefuroxime 0(0%) and Gentamicin 0(0%). The results suggests that there are antibiotic resistance bacteria in stream water in Bokkos local government area of Plateau State. Educational awareness regarding the proper use of safe drinking water and the treatment of water before use should be encouraged.

Bacterial contamination of single and multiple-dose parenteral injection vials after opening and antibiotic susceptibility of isolates at Jimma Medical Center, Jimma, Southwest Ethiopia

Single- or multiple-dose vials are prone to bacterial contamination after improper handling and can be potential reservoirs of microorganisms that could be transmitted to the patient through the parenteral route. The present study aims to assess the magnitude of the problem and associated factors at Jimma Medical Center (JMC), Jimma, Southwest Ethiopia. A cross-sectional study design was conducted at JMC from July 2021 to October 2021. A total of 384 parental medications and nurse interviews that were administered in 11 wards and 3 intensive care units were included. Samples were processed and identified by conventional bacterial culture methods. The overall prevalence of vial contamination due to aerobic bacteria was 21 (5.5%) among multiple-dose vials and none of the single-dose vials. The highest level of contamination (8, 38.1%) was found in the paediatric ward. Pseudomonas aeruginosa and Klebsiella pneumoniae were the most common microorganisms identified vial contamination, 6 cases (28.5%) and 5 cases (23.8%) respectively Multidrug resistance was identified in 95.2% of the isolates, with all Gram-negative isolates showing a multidrug resistance against the tested antibiotics. In multivariate logistic regression analysis, vial contamination was strongly associated with reuse of syringe and/or needle, the environment where medication was handled, and the storage conditions. In this study, the prevalence of vial contamination was high. The bacterial isolates from vials were also resistant to commonly prescribed antimicrobial drugs. Healthcare professionals must strictly adhere to basic infection control practices as per standard guidelines to reduce the risk of infection from contaminated vials.

Characterization of lactic acid bacteria isolated from street pickles of Dhaka, Bangladesh

Traditionally fermented pickles are a popular street food in Bangladesh famous for their unique flavors and health benefits. Pickles are often prepared by fermentation using lactic acid bacteria (LAB) that can act as probiotics. The study was aimed to isolate and characterize lactic acid bacteria from pickle samples collected from streets of Dhaka city, as well as assess the microbial quality of pickles for food safety.A total of 30 pickle samples of different kinds were collected from streets of Dhaka city. Isolation and identification were conducted using conventional cultural and biochemical tests, followed by molecular confirmation of identity. Antibiotic susceptibility of isolates was investigated against 7 antibiotics of different groups. Antimicrobial activity of LAB isolates was analyzed by well-diffusion assay and phenotypic enterocin activity assay. Physiological characterizations of LAB were performed to determine their tolerance to temperature, salt, pH, bile, carbohydrate fermentation pattern, proteolytic activity and biofilm formation.Fifty isolates were obtained from pickle samples, of which 18% was identified as LAB, including Enterococcus faecalis (6) and Enterococcus faecium (3). The rest included S. aureus (18), E. coli (11), Klebsiella spp. (5), Salmonella (3), Shigella (3) and Pseudomonas aeruginosa (1). Antibiotic resistance pattern revealed higher occurrence of resistance against azithromycin among the non-LAB isolates, but none of the LAB isolates were found to resist any of the antibiotics used. Antimicrobial activity of LAB isolates was not observed against the foodborne isolates. All LAB isolates fermented a wide range of carbohydrates and showed adequate tolerance to salt, pH, temperature and bile. Out of 9 isolates, 5 displayed proteolytic activity, and 6 were found as strong biofilm producer.These results suggest that although the LAB isolates from street pickles collected from Dhaka does not have antimicrobial activities, they still have potential to be used as probiotics. It also shows high occurrence of antibiotic resistant foodborne pathogens in pickles, indicating that consumption of such street food can be serious health hazard.

Relationship of OqxAB efflux pump to antibiotic resistance, mainly fluoroquinolones in Klebsiella pneumoniae, isolated from hospitalized patients.

This research was designed to study the prevalence of OqxAB efflux pump genes and also to investigate the relationship between efflux pump and resistance to antibiotics, especially to fluoroquinolones, evaluate the expression levels of OqxAB genes, and molecular typing of Klebsiella pneumoniae isolated from hospitalized patients in Hamadan hospitals, west of Iran. In a cross-sectional study, 100 clinical strains of K. pneumoniae were isolated from hospitalized patients in three major teaching hospitals from January to June 2021. The antibiotic susceptibility of isolates was evaluated by the disk-diffusion agar method. The frequency of genes encoding oqxA and oqxB of efflux pump genes was investigated by PCR, and the expression of the oqxA efflux pump gene was investigated by the Real-time PCR method. The genetic relationship of K. pneumoniae isolates was analyzed by the Enterobacterial Repetitive Intergenic Consensus (ERIC)-PCR technique. According to our results, the multi-drug resistance phenotype (MDR) in 65% and high prevalence resistance to ciprofloxacin in 89% of K. pneumoniae isolates was detected. The higher prevalence of oqxA (95%) and oqxB (98%) was also detected. There was a significant relationship between ciprofloxacin resistance and the oqxB gene as well as between ceftriaxone and chloramphenicol resistance and the oqxA gene. The expression of the oqxA gene was higher in ciprofloxacin-resistant isolates. The results of this study suggest a potential reservoir for the spread of OqxAB genes among hospital-acquired bacteria. Infection control strategies should be used prudently to reduce the spread of resistant strains of K. pneumoniae in hospitals.

ALCOHOL DISRUPTED AND INCREASED ANTIBIOTICS RESISTANT PROFILES OF SELECTED BACTERIA RESIDENT IN GASTRO-INTESTINAL TRACT OF WISTER RAT

The study is aimed at investigating the effect of alcohol intake on gut microbiota and antibiotic susceptibility of some of the microbial genera isolated. Twenty-four wistar rats were administered orally with branded dry gin (40% alcohol) and one was picked randomly every 48hrs and sacrificed by cervical decapitation. The intestine was collected aseptically after laparotomy and placed in a sterile petri dish. The intestinal bacteria were enumerated, isolated and identified using standard microbiological methods. The antibiotic susceptibility was done using the disc agar diffusion method. The Total Heterotrophic (x 104 CFU/g) and Total Coliform Counts ranged from 2.0 – 65 and 1 – 28 respectively. Bacteria genera isolated are; Escherichia, Proteus, Citrobacter, Lactobacillus, Enterococcus, Enterobacter, Salmonella, Klebsiella and Bacillus. E. coli was the most (100%) prevalent. Enterococcus, Lactobacillus and Enterobacter were also highly prevalent. The occurrence of the genera was in the order Lactobacillus > Citrobacter > Klebsiella and Salmonella > Proteus. All isolates were present in all the intestines few days after administration of the gin, but there was a decline in the occurrence till the 21st day, after which there was an increase. The susceptibility of Escherichia to conventional antibiotics was between 0–90% and there was no particular trend with the length of exposure but there was substantial decrease in the susceptibility to the antibiotics. The susceptibility of Enterococcus to the antibiotics decreased with the length of exposure to the alcohol. Intake of alcohol has a pronounced effect on the gut microbiome and the antibiotic susceptibility of isolates.

DETERMINATION OF UREAPLASMA UREALYTICUM AND ANTIBIOTIC SUSCEPTIBILITY IN SEXUALLY ACTIVE WOMEN WITH DIFFERENT METHODS

Objective: In this study, the presence of Ureaplasma urealyticum (U.urealyticum) in vaginal and urine samples taken from sexually active women was investigated and antibiotic susceptibilities were determined with culturing bacteria and using kits. Material and Method: The vaginal and urine samples taken from 110 women who applied to Başkent University Ankara Gynecology and Obstetrics Clinic were included in this study. U. urealyticum was investigated by the culture method with using two different test kits [Mycoplasma IES (IES-Autobio, China) and Mycoplasma IST2 (IST2) Biomereux, France] and Mycoplasma Agar. Antibiotic susceptibilities of isolates were also determined. Result and Discussion: We compared the results of (IES) and (IST2) used for U. urealyticum detection in 220 clinical samples. U. urealyticum was found to be positive in 82 (74,5%) by classical culture method. U. urealyticum was detected at a rate of 46.8% with the IST 2 and 53.6% with the IES. As a result of the comparison of culture method and commercial kits,it was determined that the IES kit provides a fast and accurate identification in the detection of U.urealyticum,a fast and reliable result in the detection of antibiotic resistance.A significant difference was found in the detection rate of U. urealyticum of the IES compared to the other commercial kit.It was observed that the detection rate of U. urealyticum with the kit from vaginal were significantly higher than urine samples.

Characterization of antibiotic resistance of Staphylococcus aureus isolated from patients with diabetic foot ulcers in Wasit Province

Diabetic foot ulcers are one of the most common public health issues worldwide, putting a social strain on those who suffer from them; chronic ulcers that lead to amputation are a severe disease that can reduce diabetic patients' quality of life. A total of seventy-five samples were collected from diabetic foot ulcer with age (30 - 89) years, during the period from 1st October 2020 to15th January 2021, admitted to Al-Karama teaching hospital and private clinics in Wasit province. The results revealed that the frequency among males 50 (67 %) more than female 25 (33 %).The sample distribution according to age it appears high (42.7 %) with group (50-59) years. The results shows that Gram negative bacteria reveals a high rate 57.4% (n=35) which includes K. pneumoniae that show a high percentage 24% (n=18), then E.coli 13.2% (n=10). followed, P.aeruginosa 4.0% (n=3) and P.mirabilis 2.7(n=2) and A.baumannii 2.7% (n=2). While Gram-positive bacteria recorded 42.6% (n=26) ,which include S. aureus was the most isolated bacteria in this study with percentage 33.3 % (n=25) followed by streptococcus group B 1.4% (n=1). Methicillin resistant Staphylococcus aureus was recorded with 92.3 % in all isolates and results of MRSA isolates antibiotic susceptibility for S. aureus as shown: the maximum resistance level to the Oxacillin (100 %), penicillin (100 %), cefoxitin (92.3 % ), vancomycin (61.5 % ), clindamycin (61.5 % ), followed by gentamicin (53.8 %), erythromycin (53.8 %), while the lowest resistance with ciprofloxacin (38.4 %). The maximal S. aureus sensitivity has been to Trimethoprim-Sulfamethoxazole (57.8 %) , vancomycin (38.5 %) , ciprofloxacin (30.7 %) and gentamicin (34.6 %). DNA of twenty-five isolates S. aureus were extracted, also purity and concentration were confirmed with Nanodrop, the purity of the nucleic acid in the samples ranged between 1.8-2, while its concentration ranged from (50-360 mg / μl). Resistance genes possessed by the S. aureus isolates were: mecA (96%), ermC (40%), msrA (8.0 %) and aac(6′)-aph(2″) (16 %). while ermA and vanA genes were absent among all isolates. In conclusion, the findings of the present study revealed that most studied isolates which had multiple antibiotic resistances.

Bacteriological Profile and Antibiotic Resistance Pattern at the Yaounde University Teaching Hospital: A Retrospective Study

Aims: Over the decades, antibiotic resistance has become a cross-border public health problem. This calls for the profiling of microorganisms, particularly bacteria implicated in antibiotic resistance, in order to improve clinical practice and reduce the incidence of therapeutic failure in the treatment of infectious diseases.
 Study Design: We conducted a retrospective cross-sectional study.
 Place and Duration of the Study: The study was made at the bacteriology laboratory at the Yaounde University Teaching Hospital, Cameroon during the period between January 2016 and June 2021.
 Methodology: All bacterial strains from the following biological fluids were included: blood, stool, urine, suppurations, probe tip and catheter tip. The antibiotic susceptibility of isolates was collected from the registers of the said laboratory. The data were encoded in Censuses and Survey Procession Software (CSPro) version 7.3 and analysed using the Statistical Package for Social Science (SPSS) version 25. Graphs and figures were made using Excel 2016 spreadsheet software.
 Results: A total of 1071 bacteria were enrolled in 955 patients. The age group most represented was 0-5 years (34.6%). Most of the isolates came from a blood sample. Among the isolates, Coagulase-negative Staphylococci (18.5%), Escherichia coli (17.7%), Staphylococcus aureus (14%) and Klebsiella pneumoniae (11.2%) were the most common. A total of 1071 bacteria were enrolled in 955 patients. The age group most represented was 0-5 years (34.6%). Most of the isolates came from a blood sample. Among the isolates, Coagulase-negative Staphylococci (18.5%), Escherichia coli (17.7%), Staphylococcus aureus (14%) and Klebsiella pneumoniae (11.2%) were the most represented. Between 2016 and 2020, almost remarkable resistance was observed to the class of penicillins (78% to 83%), cephalosporins (44% to 61%) and quinolones (43% to 100%) for Escherichia coli. For Staphylococcus aureus, resistance changes range from 68% to 77% for the penicillin class. Klebsiella pneumoniae showed an evolution ranging from 11% to 19% for aminosides.
 Conclusion: Although not all isolates showed a change in the level of resistance to all antibiotics that are frequently used in our study population, Nevertheless, it is important for national public health actors to establish active surveillance of antibiotic and even antimicrobial resistance and to implement a guide to the proper use of antibiotics for health professionals, and the community.

Characterization of Lactic Acid Bacteria Isolated from <em>Idli</em> Batter and their Susceptibility to Antibiotics

<em>Idli</em> is a common flour based fermented food prepared using lactic acid bacteria which bear probiotic properties, consumed in Sri Lanka and some parts of India. The objective of the present study was to characterize the lactic acid bacteria from <em>idli</em> batter, identify the changes of acidity during fermentation and to determine the antibiotic sensitivity of isolates. Lactic acid bacteria were isolated and characterized up to genus level. Subsequently, antibiotic susceptibility of isolates against four antibiotics was studied. Changes of pH, lactic acid bacterial count and titratable acidity with fermentation were determined. The pH value dropped steadily from 6.28 to 3.72 while titratable acidity increased from 0.24 to 0.92% during the fermentation period of 0 to 32 h. With fermentation, the lactic acid bacterial count was increased up to 9.88 log10cfu/g after 12 hours of fermentation and the count reduced with increasing acidity. A bacterial count of 5.91 log10cfu/g was recorded at the end of 32 hours of fermentation. Ten lactic acid bacteria isolates were identified from <em>idli</em> batter, of which six isolates were rod shaped and other four were cocci. All isolates were Gram positive, non-motile, no n-spore formers and catalase negative. According to the above tests, it was confirmed up to the genus level that all strains be longed to Lactic acid bacteria. All isolates were susceptible to more than two antibiotics. The diameter of the zone of inhibition ranged from 1-23 mm. The highest zone of inhibition was observed in isolates I-6 against chloromphenicol (23 mm) while the lowest zone of inhibition was observed in isolates I-2 against erythromycin (1 mm). With fermentation the biochemic al and sensory qualities of <em>idli</em> batter were changed. Lactic Acid Bacteria isolates showed different sensitivity towards antibiotics tested.

Healthcare Workers’ Mobile Phones as a Possible Vehicle of Nosocomial Pathogens and the Role of Different Disinfectants in their Decontamination

Background: As Mobile Phones (MPs) aren’t cleaned routinely and have been touched during patient’s examination, they may become contaminated with hospital pathogens. Objectives: Screen MPs of Health care workers (HCWs) for pathogens and verify the effect of disinfectants in their decontamination. Methods: A questionnaire was submitted by 160 HCWs in Tanta University Hospitals. Samples were taken from their MPs and subjected to pour plate counting before and after disinfection. Standard identification and antibiotic susceptibility of isolates were done. Results: Colony count was greater in MPs used while caring for patients or inside restroom, and was less in regularly cleaned MPs. All tested disinfectants reduced the colony count significantly. Pathogens were isolated from 84.38% of samples and 36.25% of them were Multi-Drug Resistant Organisms (MDROs). Conclusion: Using MPs at critical care areas and restroom may contribute to their contamination with pathogens. Regular disinfection of MPs can reduce this contamination.

Isolation and antibiotic susceptibility of bacteria from milk of apparently healthy goats in Abeokuta, Ogun State, Nigeria

Milk contaminated with antibiotic resistant bacteria can be a major threat to public health. This study wasconducted toinvestigate the antibiotic susceptibility of bacterial isolates from goat's milk in Abeokuta. Nine (9) bacteria species comprising 182 isolates were identified in 73 milk samples collected from six (6) different places located some 2 - 50 kilometers apart in Abeokuta. Isolation and identification of the bacteria species were carried out using standard microbiological procedures. The bacteria species were Pseudomonas spp (22.28%), Micrococcus spp (21.74%), Staphylococcus aureus (19.02%), Staphylococcus saprophyticus (15.22%), Enterobacter spp (8.70%), Bacillus spp (5.43%), Pasteurella spp (4.89%), Escherichia coli (2.17%), and Citrobacter spp (0.54%). Antibiotic susceptibility of the isolates and minimum inhibitory concentration were determined using a panel of 10 antibiotics by disc diffusion method and standard guidelines. The bacterial load from milk samples obtained in various locations are as follows:-DUFARMS,0.1-1.2x10 cfu/mL;Eweje, 0.6-1.2x10 cfu/mL; Odeda, 0.6-3.5x10 cfu/mL; Elite, 1.0-5.5x10 cfu/mL; Elega, 0.1- 2.52x10 cfu/mL; and Obantoko 0.16-1.04x10 cfu/mL. The mean counts were 0.54 ± 0.40x10 cfu/mL; 0.78 ±0.13x10 cfu/mL; 1.83±1.23x10 cfu/mL; 2.58 ±1.45x10 cfu/mL; 8.51 ±5.60x10 cfu/mL and 4.14 ±3.90 x10 cfu/mL respectively. Antibiotic susceptibility results showed that the organisms were 100% resistant to Amoxicillin,86.26% to Ceftriazone, 84.62% to Streptomycin, 82.42% to Chloramphenicol and 78.02% to Cotrimoxazol. However, the isolates were only 6.04% resistant to Ofloxacin and 11.54% to Pefloxacin suggesting that these might just be the only two antibiotics that the pathogens might respond to. In conclusion, microbes that are ordinarily commensals may be highly resistant to commonly used Antibiotics. This could pose serious problems in managing outbreaks associated these microbes. Reservoirs for bacterial resistance may be present in healthy animal populations and research is needed to accurately quantify the problem, propose and evaluate practicable solutions. There is the need to clarify the role of environmental factors, agents, and transmission of bacterial resistance in apparently healthy livestock.

ANALYSIS OF SELECTED BOREHOLE FILTERED SAMPLES IN IJEBU-ODE, OGUN STATE, NIGERIA

This study was carried out to investigate the microbial and physiochemical quality of common borehole water filter installations. Ten (10) borehole filter samples were collected from private homes in Ijebu-Ode, Southwestern Nigeria. These were analyzed using standard procedures. Antibiotic susceptibility of isolates was determined using the standard disc diffusion method. The temperature o oand pH range of filter samples recorded was 27.6 – 28.9C and 6.09 – 6.90C respectively. Total dissolved solid, total suspended solid and total dissolved oxygen of filter samples recorded ranged 3 3between 10 - 55mg/L, 0.5 x10 - 17.14 x 10mg/L and 4.7 - 9.7mg/L respectively. Chemical oxygen demand ranged from 9.5 - 13.25mg/L while biochemical oxygen demand ranged from 4.5 - 8.5mg/L. Total heterotrophic count, Salmonella-Shigella count and Escherichia coli count ranged from 16 x 3 5 3 5 3 510 - 52 x10, 0 x 10 - 12 x 10 and 0 x 10 - 12 x10 CFU/mL respectively. Faecal coliform count 3 5ranged from 0 x 10 - 12 x10 CFU/mL. Nine bacteria isolated and identified includes: Escherichia coli, Klebsiella species, Staphylococcus aureus, Pseudomonas species, Pseudomonas aeruginosa, Proteus species, Bacillus cereus, Enterobacter species and Salmonella species. Escherichia coli was sensitive to all antibiotics, except augmentin. The highest level of resistance was exhibited by Enterobacter sp. This study revealed that the borehole water filter from Ijebu-Ode is of poor microbial and physiochemical quality. Further work needs to evaluate the filter composition and quality. Disinfection of water in storage tanks before distribution is strongly recommended as a short term solution.

Probiotic potential of lactic acid bacteria obtained from fermented curly kale juice

The aim of the paper was to analyse changes in lactic acid bacteria (LAB) populations during spontaneous fermentation of green curly kale juice (Brasicca oleracea L. var. acephala L.) and to determine the probiotic potential of LAB isolates. The analyses revealed that changes in LAB populations were specific for spontaneously fermented vegetable juices. The initial microbiota, composed mostly of Leuconostoc mesenteroides bacteria, was gradually replaced by Lactobacillus species, mainly Lactobacillus plantarum, Lactobacillus sakei, and Lactobacillus coryniformis. Screening tests for the antimicrobial properties and antibiotic susceptibility of isolates allowed for the selection of 12 strains with desirable characteristics. L. plantarum isolates were characterized by the widest spectrum of antimicrobial interactions, both towards Gram-positive and Gram-negative bacteria. Also, L. plantarum strains exhibited the best growth abilities under low pH conditions, and at different NaCl and bile salt concentrations. All strains showed different levels of antibiotic sensitivity, although they were resistant to vancomycin and kanamycin. The present study has shown that bacterial isolates obtained from spontaneously fermented kale juice could constitute valuable probiotic starter cultures, which may be used in fermentation industry.

Microbiological patterns of endophthalmitis in a tertiary level hospital of Kathmandu, Nepal.

Endophthalmitis is a serious ocular infection that can result in blindness and therefore is a major ophthalmic concern. The study was conducted to find out the microbial etiology in clinically diagnosed cases of endophthalmitis and to determine the antibacterial susceptibilities of bacterial isolates in vitro. A retrospective analysis was carried out of all patients presenting between 15th January 2012 to 15th January 2013 with clinically diagnosed Endophthalmitis at Tilganga Institute of Ophthalmology (TIO). Intraocular specimens (aqueous or vitreous fluids) were collected from 102 patients. Along with intraocular aspirates, blood and urine specimens, chest X-ray from endogenous endophthalmitis and corneal scrapes from corneal ulcer leading to endophthalmitis were also collected. The intraocular specimens were then subjected to microbiological evaluation. Antibiotic susceptibility of isolates were determined for different ocular antibiotics using the Kirby-Bauer disk-diffusion test. Samples from 102 patients diagnosed with endophthalmitis underwent microbiological analysis, of which culture positive 34(33.33%). Out of 34, 29 (85.29%) had bacterial growth and the remaining 5 (14.70%) had fungal growth. Most of them (40.8%) were secondary to cataract surgery(postoperative). A total of 29 bacterial culture reports, 72.41% were gram-positive and 27.58% were gram-negative. Streptococcus pneumoniae (37.93%) was the most frequently isolated organism. The antimicrobial sensitivity for isolates was as follows: Ofloxacin - 86.2%, Chloramphenicol - 82.75%,Cefazolin- 82.7%, Gentamicin - 75.8%, Ceftazidime - 58.6%, Amikacin - 55.17% and Vancomycin - 51.7%. The data highlights low culture positivity and a predominance of gram positive bacteria as the major causes of infectious endophthalmitis, usually following cataract surgery. The most common isolate was Streptococcus pneumoniae. Ofloxacin and Chloramphenicol demonstrated greatest efficacy against these bacterial isolates.

Application of bacteriophages isolated from sewage to control urinary O157:H7 Escherichia coli and several bacterial uropathogens

Urinary tract infections (UTI) are global bacterial infections. Since the spread of antibiotic resistance, it is necessary to find alternative antimicrobial agents. The aim of this work was to investigate the effect of waste water bacteriophages on the growth of some multi-drug resistance Escherichia coli and other bacterial uropathogens. Urine samples were collected from 30 UTI patients at Minia University Hospital, Minia, Egypt. Twenty Escherichia coli strains were isolated from UTI patients. The Kirby-Bauer disc diffusion method was used to determine the isolates antibiotic susceptibility. The isolates showed resistance to ciprofloxacin and levofloxacin by 70%. Five E. coli bacteriophages were isolated from sewage water samples, tested for their host range and then examined by Transmission electron microscopy (TEM). The TEM examination revealed T4 like bacteriophages. The bacteriophages demonstrated lytic activities against the tested multidrug resistant clinical uropathogenic O157:H7 and non-O157 E. coli isolates, E. coli O157:H7 ATCC 43894, E. coli NRRL B-3008 and, Pseudomonas aeruginosa ATCC 27853 strain, but showed no activity against Klebsiella pneumoniae ATCC10031 and Staphylococcus aureus ATCC 6538. This study revealed that bacteriophages could act as effective alternatives of antibiotics especially against multidrug resistant bacteria; however, further in-vivo and shelf stability studies are needed.

Antibiotic Susceptibility, Clonality, and Molecular Characterization of Carbapenem-Resistant Clinical Isolates of Acinetobacter baumannii from Washington DC.

The occurrence of carbapenem-resistant (CR) strains of Acinetobacter baumannii is reported to contribute to the severity of several nosocomial infections, especially in critically ill patients in intensive care units. The present study aims to determine the antibiotic susceptibility, clonality, and genetic mechanism of carbapenem resistance in twenty-eight Acinetobacter baumannii isolates from four hospitals in Washington DC. The antibiotic susceptibility of the isolates was determined by VITEK 2 analyses, while PCR was used to examine the presence of antibiotic-resistant genes and mobile genetic elements. Trilocus multiplex-PCR was used along with pulsed-field gel electrophoresis (PFGE) for strain typing and for accessing clonal relationships among the isolates. Antimicrobial susceptibility testing indicated that 46% of the isolates were carbapenem-resistant and possessed MDR and XDR phenotypes. PFGE clustered the 28 isolates into seven clonal (C1–C7) complexes based on >75% similarity cut-off. Thirty-six percent of the isolates belonged to international clone II, while 29% were assigned to Group 4 by trilocus multiplex-PCR. Although the blaOXA-51-like gene was found in all the isolates, only 36% were positive for the blaOXA-23-like gene. PCR analysis also found a metallo-β-lactamase (MBL) gene (blaVIM) in 71% of the isolates. Of the 13 CR isolates, 8 were PCR positive for both blaVIM and blaOXA-23-like genes, while 5 harbored only blaVIM gene. This study revealed the emergence of VIM carbapenemase-producing A. baumannii isolates, which has not been previously reported in the United States.

Evaluation of fosfomycin combined with vancomycin against vancomycin-resistant coagulase negative staphylococci

The pathogenic potential of coagulase-negative staphylococci (CoNS) is increasingly recognized worldwide. The aim of the present study was to evaluate different combinations of fosfomycin (FOS) with vancomycin (VAN) or oxacillin (OXA) against vancomycin-resistant clinical isolates of CoNS. Characterization of VAN resistance in selected isolates was also sought. Antibiotic susceptibility of isolates was tested by disc diffusion method, MICs of antibiotics were determined by the agar dilution method, and fosfomycin combinations were evaluated by a time-kill assay according to the guidelines of the CLSI. Moreover, oxacillin and glycopeptides (vancomycin and teichoplanin) resistances were also characterized phenotypically and genotypically in this study. Out of 258 staphylococci, 52 were CoNS (20.2%). All isolates were multidrug resistant with 75% (n = 39) oxacillin-resistant, most of them with oxacillin MIC levels of >32 mg/L. Moreover, vancomycin non-susceptibility was observed in 46.2% (n = 24) of the tested isolates with MIC range of 4-32 mg/L. Identification of selected isolates revealed that S. epidermidis was the most abundant among tested CoNS, followed by S. hominis, S. saprophyticus, and interestingly S. caseolyticus. Furthermore, Synergistic and bactericidal effects of FOS + VAN combination were observed in 3 of 9 isolates after 6 h of treatment and in all studied isolates at 24 h. On the other hand, FOS + OXA combinations were ineffective. This study provides evidence that fosfomycin combination with vancomycin could be considered as a therapeutic alternative for CoNS infections. It also sheds some light on the possible emergence of the otherwise harmless bacterium S. caseolyticus as a human pathogen.

Microbiological Isolates and Antibiotic Susceptibilities in Cases of Posttraumatic Endophthalmitis: A 15-Year Review.

Purpose The objective of this study was to evaluate the microbiological spectrum and antibiotic susceptibilities of isolates in posttraumatic endophthalmitis over a 15-year period. Methods A retrospective study of 3,163 posttraumatic endophthalmitis cases was conducted between July 2004 and July 2019. The outcome measures included the microbiological spectrum and antibiotic susceptibilities. Chi-squared tests were conducted to detect trends in changes in antibiotic sensitivity over the 15-year period. P values of <0.05 were considered statistically significant. Results Of the 3,163 cases of posttraumatic endophthalmitis, 1,003 culture-positive isolates were identified. Among these, there were 848 (84.5%) Gram-positive isolates, 109 (10.9%) Gram-negative isolates, and 46 (4.6%) fungal isolates. The most common isolates were Staphylococcal species. There was a significant increase in the percentage of fungal isolates over the 15-year period (P=0.02). Gram-positive organisms showed the greatest level of susceptibility to vancomycin (99.6%). The susceptibilities of the 109 Gram-negative isolated organisms were as follows: levofloxacin (95.8%), meropenem (95.7%), ciprofloxacin (93.5%), tobramycin (90.8%), imipenem (88.9%), trimethoprim-sulfamethoxazole (TMP-SMX) (87.7%), ertapenem (80%), and ceftazidime (79.1%). The susceptibility of Gram-positive organisms to several antibiotics, including levofloxacin (P=0.004), ciprofloxacin (P < 0.001), and chloramphenicol (P=0.001) decreased over time, whereas the susceptibility to TMP-SMX increased over time (P < 0.001). The susceptibility of Gram-negative bacilli to ceftazidime decreased over time (P=0.03). Conclusions Over the 15-year study period, most isolates were Gram-positive cocci, especially coagulase-negative staphylococci (CNS). Vancomycin seemed to be the most effective antibiotic for Gram-positive bacteria. Gram-negative bacteria appeared to be most susceptible to fluoroquinolones. A number of antibiotics showed an increasing trend of microbial resistance.

Correlation of biofilm formation and antibiotic resistance among clinical and soil isolates of Acinetobacter baumannii in Iraq.

Acinetobacter baumannii is an opportunistic pathogen that is reported as a major cause of nosocomial infections. The aim of this study was to investigate the biofilm formation by A. baumannii clinical and soil isolates, to display their susceptibility to 11 antibiotics and to study a possible relationship between formation of biofilm and multidrug resistance. During 8months period, from June 2016 to January 2017, a total of 52 clinical and 22 soil isolates of A. baumannii were collected and identified through conventional phenotypic, chromo agar, biochemical tests, API 20E system, and confirmed genotypically by PCR for blaOXA-51-like gene. Antibiotic susceptibility of isolates was determined by standard disk diffusion method according to Clinical and Laboratory Standard Institute. The biofilm formation was studied using Congo red agar, test tube, and microtiter plate methods. The clinical isolates were 100% resistance to ciprofloxacin, ceftazidime, piperacillin, 96.15% to gentamicin, 96.15% to imipenem, 92.31% to meropenem, and 78.85% to amikacin. The soil A. baumannii isolates were 100% sensitive to imipenem, meropenem, and gentamicin, and 90.1% to ciprofloxacin. All A. baumannii isolates (clinical and soil) were susceptible to polymyxin B. The percentage of biofilm formation in Congo red agar, test tube, and microtiter plate assays was 10.81%, 63.51%, and 86.48%, respectively. More robust biofilm former population was mainly among non-MDR isolates. Isolates with a higher level of resistance tended to form weaker biofilms. The soil isolates exhibited less resistance to antibiotics than clinical isolates. However, the soil isolates produce stronger biofilms than clinical isolates.

&lt;i&gt;In-vitro&lt;/i&gt; antibacterial activity of Lantana trifolia flower extracts on bacterial isolates from wounds

This study investigated the phytochemical constituents and antibacterial activities of Lantana trifolia flower extracts against bacterial isolates from surgical and diabetic wounds of patients. Phytochemical investigation was done by standard procedure, antimicrobial screening by agar well diffusion method and antibiotics susceptibility of isolates by Kirby-Bauer disc diffusion test. Phytochemicals present include flavonoids, tannins, phenylethanoid glycosides, reducing sugars, terpenoids, saponins, anthraquinones alkaloids and steroids. Bacterial wound isolates were mainly gram +ve and include Staphylococcus aureus, S. epidermidis, Enterococcus faecalis, Corynebacterium sp., Enterobacter aerogenes, Pseudomonas aeruginosa, Mycobacterium sp. and Escherichia coli. Inhibition diameter was concentration. The range of inhibition diameter was significantly (P&lt;0.05) lower in the aqueous extract (15.2±1.5 to 35.5 ±0.9 mm) than the ethanolic (28.3± 3.4 to 49.1±8.9mm). Antibacterial activity occurred at very low concentrations (18.75mg/ml) for ethanolic extract only. The most L trifolia extract sensitive bacterial isolate was S. aureus (35.5±0.9mm and 49.1±8.9mm for aqueous and ethanolic extracts respectively at 300mg/ml), while the least sensitive were Corynebacterium sp. (15.2±1.5mm) and Mycobacterium sp. (28.3±3.4) at 300mg/ml for aqueous and ethanolic extracts respectively. The minimum inhibitory concentration was lower for ethanolic extract, 18.75/ml to 37.5mg/ml than the aqueous extract (37.5 to 150mg/ml). The minimum bacterial concentration range from 37.5mg/ml to 150mg/ml for ethanolic extract and 75 to 300mg/ml for aqueous extract. S. aureus was the most antibiotic resistant strain (72%), while P. aeruginosa and Mycobacterium sp. were the least resistant (9.1%) strains. L. trifolia was more bacteriostatic than the most effective antibiotics, Ciprofloxacin, Rocephin, Nitrofurantoin, and deserves further investigation.Keywords: Phytochemical screening, Bacteriostatic activity, Lantana trifolia, Resistance, Wounds
