Selecting Root Coverage Techniques for Autogenous Soft Tissue Grafts at Isolated and Multiple-Adjacent Mandibular Anterior Recession Defects

It is widely acknowledged that multiple anatomic and technical factors hinder successful root coverage in the mandibular anterior region. Nevertheless, the contemporary periodontal plastic surgeon is presented with a panoply of options when deciding upon the most appropriate technique to apply in a particular clinical scenario. Although many techniques are applicable at a variety of defect types, several are particularly well suited for specific situations commonly encountered in the mandibular anterior. The purpose of this report is to describe rationale for preferred autogenous soft tissue grafting techniques at isolated mandibular incisor sites, isolated mandibular canine sites, and multiple-adjacent recession defects in the mandibular anterior. Four cases are presented to exemplify the recommended procedures.

Impact of Different Orthodontic Appliances on Mandibular Condyle, Anterior Region of Mandibular, and Maxillary Sinus Using Panoramic Radiography.

BACKGROUND This retrospective study of 44 patients requiring removable functional dental appliances aimed to compare the use of a maxillary expansion device, twinblock/monoblock, face mask, and chincup on maxillary sinus dimensions, mandibular anterior region bone density (MABD), and mandibular condyle head bone formation (MCBF) using panoramic radiography. MATERIAL AND METHODS The patients were divided into 4 groups based on the treatment modalities used: group A, maxillary expansion device (n=11); group B, twinblock/monoblock (n=11); group C, face mask (n=11); and group D, chincup (n=11). MABD and MCBF were assessed using the fractal analysis method, while maxillary sinus dimensions were compared using the ImageJ program. Different groups of patients were tested by the Mann-Whitney U test, with Bonferroni correction. RESULTS Group A showed an increase in MABD, MCBF, and average maxillary sinus indentation base length (MSIL) and a decrease in average planar base length (MSPL) and maxillary sinus area (MSA) (P<0.05). Group B showed an increase in MABD, MSIL, MSPL, MSA, and MCBF (P<0.05). Group C showed an increase in MABD, MSIL, MSPL, and MCBF, with a decrease in MSA (P>0.05). Group D showed an increase in MCBF, MABD, MSIL, and MSA, with a decrease in MSPL at the end of treatment, compared with pretreatment (P<0.05). CONCLUSIONS Orthodontic treatment with different removable functional dental appliances has varying effects on the mandible and maxilla. Possible changes in the maxilla and mandible should be considered before treatments with removable functional dental appliances.

TRATAMENTO CIRURGICO DE CERATOCISTOS ODONTOGÊNICOS: RELATO DE CASO CLÍNICO

Introduction: The keratocyst or keratocyst is an odontogenic tumor characterized as a lesion that affects the maxillary and mandibular regions. It is considered a benign lesion, reaching a prevalence of 3% to 11% among all odontogenic cysts, resulting in major bone loss. It can be asymptomatic and is usually diagnosed through routine radiographs or histopathological findings. Objective: To present a clinical case concerning the diagnosis and surgical treatment of an odontogenic keratocyst in the anterior mandibular region. Clinical case: Male, normosystemic, melanodermic patient. An increase in extraoral volume was observed on the right side of the face in the anterior region of the mandible. Panoramic radiography revealed well-defined, unilocular radiolucent images in the areas of the right lower first premolar, left lower third molar and bilateral upper third molars. On intraoral physical examination, bone bulging was observed in the region of the tooth in question. A computerized tomography scan was then requested, in which extensive expansive bone lesions were detected. Marsupialization and local decompression of the two upper lesions and the lower right lesion were performed, detecting the presence of liquid content. Firstly, an incisional biopsy was carried out and a decompression drain was installed in the three lesions. The histopathological report was of an odontogenic keratocyst. The decompression device remained in place for 30 days in the lower right lesion and approximately 15 days in the upper lesions, showing a significant reduction in the lesions. The lower right lesion was then enucleated. The 6-month post-operative image showed bone formation with no signs of recurrence. Conclusion: The proposed treatment proved to be effective in treating odontogenic keratocysts, but follow-up imaging is extremely important in view of the possibility of recurrence.

Periapical cemento-osseous dysplasia masquerading as asymptomatic chronic apical periodontitis in a Chinese woman: A case report.

Cemento-osseous dysplasia (COD) belongs to a group of fibrous osseous disorders that can masquerade as periapical inflammatory conditions in the jawbones. We present a rare case of COD occurring in a patient who also had periapical periodontitis concurrently. When faced with a patient exhibiting no symptoms, diagnosis may be challenging. COD is a fibrous osseous disorder similar to periapical chronic inflammatory conditions and other cysts in the jaw bones on the radiograph. There is a rare case of COD occurring in a 49-year-old Chinese woman who also had chronic periapical periodontitis concurrently. The lesions were incidentally discovered in the mandibular anterior tooth region during the patient's imaging examination. The patient exhibited no symptoms, and diagnosing the conditions may be challenging due to the confusingly similar radiological features that present as a radiolucent lesion in the periapical region. The final diagnosis was made through pulp vitality tests, cone beam computed tomography (CBCT), and clinical examinations. The COD was recommended for annual follow-up. Root canal therapy has been performed for chronic apical periodontitis. According to the diagnostic process of COD, a thorough history check, multiple clinical examinations, and imaging studies should be emphasized to prevent misdiagnosis and avoid unnecessary or inappropriate therapies. The patient was followed up for 6 years. The recovery of chronic apical periodontitis in tooth #31 and the transformation of osteolytic and osteogenesis of COD had been observed in CBCT.

Mesiodistal tip expression of mandibular anterior teeth in patients with mandibular incisor extraction treated with Invisalign aligners

Extraction of a single mandibular incisor may be prescribed in conjunction with clear aligner orthodontic treatment. The accuracy of therapeutic mesiodistal tooth positioning for the remaining mandibular anterior teeth for such patients has not been comprehensively evaluated. This retrospective study compared the predicted and achieved mesiodistal tooth movements in the mandibular anterior region for patients treated with Invisalign (Align Technology, Santa Clara, Calif), along with the influence of aligner attachments and the frequency of aligner change. A sample of 83 subjects who had undergone Invisalign treatment with a single mandibular incisor extraction was collected from an existing database. The pretreatment tooth positions, the predicted mesiodistal tooth movement, and the achieved outcomes were measured for the remaining mandibular anterior teeth using Geomagic Control X metrology software (3D Systems, Rock Hill, SC). The predicted and achieved mesiodistal tip movements were analyzed and compared. Regression analysis revealed significant differences (P<0.001) between the predicted and achieved mesiodistal tooth positioning for incisors and canines. For every degree of predicted incisor mesiodistal tip, 78.89% of this tip was achieved (R2= 0.4901). For every degree of predicted canine tip, 54.16% was clinically expressed (R2= 0.5965). The accuracy of mesiodistal tip expression was not significantly affected by the prescribed number of aligners, 1-week vs 2-week aligner change protocols, or the presence of attachments (P>0.05). In addition, incisors were less likely to express the correct direction of the mesiodistal tip predicted than canines (P= 0.020). A shortfall between the predicted and clinically achieved expression of mesiodistal tooth movement with Invisalign was found for the anterior teeth after a mandibular incisor extraction. The number of aligners, duration of aligner wear, and specific attachment designs were not found to significantly influence the accuracy of the achieved mesiodistal tip movements.

Strategies for prevention and treatment of vascular and nerve injuries in mandibular anterior implant surgery

Important anatomical structures such as mandibular incisive canal, tongue foramen, and mouth floor vessels may be damaged during implant surgery in the mandibular anterior region, which may lead to mouth floor hematoma, asphyxia, pain, paresthesia and other symptoms. In severe cases, this can be life-threatening. The insufficient alveolar bone space and the anatomical variation of blood vessels and nerves in the mandibular anterior region increase the risk of blood vessel and nerve injury during implant surgery. In case of vascular injury, airway control and hemostasis should be performed, and in case of nerve injury, implant removal and early medical treatment should be performed. To avoid vascular and nerve injury during implant surgery in the mandibular anterior region, it is necessary to be familiar with the anatomical structure, take cone-beam computed tomography, design properly before surgery, and use digital technology during surgery to achieve accurate implant placement. This article summarizes the anatomical structure of the mandibular anterior region, discusses the prevention strategies of vascular and nerve injuries in this region, and discusses the treatment methods after the occurrence of vascular and nerve injuries, to provide clinical reference.

Three-dimensional assessment of alveolar bone changes and root resorption in extraction patients after fixed orthodontic treatment: A prospective study

The aim of this study was to evaluate the alveolar crestal bone height, alveolar bone thickness, and root resorption in mandibular incisors after fixed orthodontic treatment. A total of 35 patients (mean age 17.02 ± 3.24 years) with moderate to severe mandibular anterior crowding requiring first premolar extraction were included. Cone beam computed tomography scans of the mandibular anterior region were obtained from the patients before and after orthodontic treatment. The alveolar crestal bone height, cortical bone thickness (CBT) (at apex and midroot level), labial and lingual alveolar bone thickness, and root resorption were assessed for all mandibular incisors. Paired Student's t test was used to compare pretreatment and post-treatment alveolar bone changes and root resorption. A significant decrease in lingual crestal bone height was observed in all mandibular incisors after orthodontic treatment (P < 0.001). Lingual CBT decreased at the midroot level significantly for all mandibular incisors after completion of orthodontic treatment (P < 0.001). A significant decrease in total lingual bone thickness was observed in all mandibular incisors (P < 0.001). Root lengths of mandibular incisors decreased significantly in the sagittal and coronal sections after orthodontic treatment (P < 0.001). Fixed orthodontic treatment with premolar extraction results in a significant reduction of alveolar crestal bone height, CBT (at midroot level), and alveolar bone thickness on the lingual aspect of mandibular incisors. Significant orthodontically induced inflammatory root resorption was observed in all mandibular incisors after orthodontic treatment.

Assessment of anterior loop of mental foramen with cone beam computed tomography-An institutional based study

Cone beam computed tomography (CBCT) is a specialized imaging technology designed to create accurate 3D images of craniofacial anatomy while using significantly less radiation than traditional CT scans. This advancement allows clinicians to closely examine dental and maxillofacial structures, forensic analysis, and modern therapeutic techniques.To evaluate several anatomical variants of the anterior loop of the mental foramen using cone-beam computed tomography (CBCT).The study involved 30 randomly selected patients aged 20-60 year from the departmental OPD, who underwent CBCT of the anterior mandibular region to assess the mental foramen. After obtaining a detailed history, a diagnosis was made and explained to the patients. Data were analyzed according to the classification by Solar et al. (1994).Type 1 mental nerve loop presence was 6.6% greater on the right side (13.3%) than on the left side (6.7%), while Type 3 mental nerve loop presence was 6.6% higher on the left side (63.3%) than on the right side (56.7%) based on a comparison of the anterior loop of the mental foramen.However, Type 2 was similar on both the right (30.0%) and the left (30.0%) sides. The anterior loop of the mental foramen is an important anatomical feature and was comparable (Type 1/Type 2/Type 3) in patients on the right and left side (p &amp;#62; 0.05) (χ2=0.78, p = 0.678), showing no significant difference. Understanding its anatomy and occurrence has a significant impact on therapeutic strategies.

Peripheral giant cell granuloma in a child with ectrodactyly-ectodermal dysplasia-cleft lip/palate syndrome: a case report

BackgroundEctrodactyly-ectodermal dysplasia-cleft lip/palate (EEC) syndrome mainly affects ectodermal and mesodermal tissues. It is usually manifested as split hands and feet, ectodermal dysplasia, and orofacial clefting, along with other signs and symptoms. A multidisciplinary approach to treatment is required, in which dentists play an important role in identifying and treating various oral conditions that may be genetically linked to or may be the result of EEC syndrome.Case presentationThe present case describes the oral condition of a young child suffering from EEC syndrome and presenting with peripheral giant cell granuloma (PGCG) in the mandibular anterior region. After obtaining a thorough medical and family history and a clinical examination, the lesion was surgically excised under local anesthesia. The patient was followed up at periodic intervals for the next twenty four months, during which no recurrence of the lesion was observed.ConclusionThis report highlights the role of a dentist in the management of the oral conditions of patients suffering from EEC syndrome.

A Single-Step Root Coverage Procedure Using Modified Bridge Flap Technique and De-epithelialized Gingival Unit Graft: A Prospective Clinical Study.

The mucogingival conditions associated with the recession defects at the mandibular anterior region, not only make plaque control difficult to be performed by the patient but also pose a problem in passively advancing the tissues at the recipient site. This surgical technique is used to treat multiple recession defects in the mandibular anterior region. Twenty-five sites in 17 subjects (11 women and 6 men) underwent modified bridge flap and de-epithelized gingival unit graft (d-GUG) as a single-step treatment modality to cover the denuded root surface, as well as increase the width of keratinized gingiva in recession type 1 gingival recession defects in the mandibular anterior region. Nearly 60% of sites in our study showed complete root coverage, with a mean root coverage percentage of 88.08%. Furthermore, keratinized tissue width, keratinized tissue thickness as well as vestibular depth showed statistically significant increases as compared to baseline levels. Thus, the modified bridge flap technique using d-GUG within the limitations of our study design, showed promising results in terms of recession coverage as well as management of the difficult mucogingival conditions at the mandibular anterior region.

Mandibular Intercanine Width at Three Stages of Mixed Dentition in Children at Namakkal District: A Cross-sectional Study.

To assess and compare the mandibular intercanine width (ICW) of children at three stages of mixed dentition in children at Namakkal district. An analytical cross-sectional study was conducted among 135 schoolchildren aged 7-11 years, with 45 children in each group. Group I included children with completely erupted permanent mandibular central incisors, group II included children with completely erupted permanent mandibular central and lateral incisors, and group III included children with completely erupted permanent mandibular central incisors, lateral incisors, and canines. Impressions were made in the mandibular anterior region, and study casts were prepared. Measurements of the mandibular ICW were taken using a digital vernier caliper. The mean mandibular ICW after the complete eruption of permanent mandibular central incisors, lateral incisors, and canines was 23.7, 25.5, and 25.7 mm, respectively. There was a statistically significant difference in the ICW between the three groups (p < 0.001). Post hoc tests showed that mandibular ICW was statistically significant between groups I and II and group III (p < 0.001). Linear regression analysis showed that age does not contribute to the changes in mandibular ICW. The average mandibular ICW found can be a standard comparison for treatment planning in the western Tamil Nadu population. Mandibular ICW reaches its maximum after the eruption of permanent mandibular lateral incisors. Comparison Between Autologous Albumin Gel and Liquid Platelet-Rich Fibrin Mixture Versus Connective Tissue Graft to Modify the Gingival Phenotype: A Randomized Controlled Trial.

Objectives To assess changes in gingival thickness (GTH) and the width of keratinized gingival tissue (KTW) following treatment with either connective tissue graft (CTG) or an albumin gel-platelet-rich fibrin mixture (Alb-PRF). Materials and methods Twenty treatment sites were included in a split-mouth design involving 10 patients with a thin gingival phenotype in the mandibular anterior region. The sample was randomly divided into two groups, with the Alb-PRF applied to the experimental group and CTG used for the control group. GTH and KTW were measured at baseline and after one, three, and six months. Results GTH increased in both groups during all follow-up periods. However, no statistically significant differences (p > 0.05) between the groups were observed at baseline and six months. At three months, the experimental group exhibited significantly higher GTH (p < 0.001). Additionally, at three and six months, the CTG group showed a superior increase in KTW (p < 0.05). Conclusion Within the constraints of this study, Alb-PRF application for modifying thin gingival phenotypes proved to be an effective therapeutic option, potentially serving as an alternative to CTGs. Although Alb-PRF resulted in thicker gingiva, CTG demonstrated a greater enhancement in KTW.

Aesthetic correction by making anterior adhesive bridge mesh design: A case report

Objective: The article aims to present an aesthetic correction for tooth loss in the anterior region of the mandible by making an anterior adhesive bridge. Methods: The patient complained of decreased self-assurance due to the absence of teeth in the lower right central incisor area following an accident. They desire a permanent denture solution but are reluctant to undergo extensive tissue removal. The treatment carried out was by making an anterior adhesive bridge using a mesh design. The mesh was designed to increase the retention of the metal wing surface in contact with the abutment teeth. Results: The patient was happy that they could use their teeth for chewing as well as for improving his appearance when smile. Conclusion: It was concluded that the mesh design adhesive anterior bridge construction was a conservative treatment option for aesthetic correction of missing teeth in the mandibular anterior region.

Dento-maxillary disharmony class I malocclusion with severe crowding, crooked canine, midline shifting and lip incompetence management: A case report

Objective: This case report aimed to provide treatment sequential DDM to meet the patient’s expectations. Case Report: Before starting the treatment, 2 first upper premolar needed to be extracted to gain space for severe anterior crowding teeth and 2 second lower molar because the teeth were not in good condition. After extraction, bracket placement with fixed appliance 0,22” MBT prescription and NiTi wires sequences were done for initial leveling. When it has already leveled, start for canine retraction using an elastomeric chain in wire SS 0,016x0,022-inch. Then, T-loop bends were used for anterior retraction in the same wire. Finishing detailing using SS 0,017x0,025-inch. The last one, Hawley Retainer, was used as a retainer. Results: The final results were as expected by the patient. She gained confidence through her new smile, and the orthodontist obtained what it needed for ideal occlusion except for Class I molar relationship because mesialization of the second lower mandibular tooth, and distalization of the anterior mandibular region but Class I canine were obtained, no rotation, no space, flat occlusal plane, and excellent crown angulation and inclination. Also, the patient’s lip became competent. Conclusion: This case concludes that the patient was happy with the overall treatment results and was satisfied with the pleasant and esthetic smile achieved at the end of the treatment.

Evaluation of Lingual Foramen in the South Indian Population Using Cone-Beam Computed Tomography: A Prospective Cross-Sectional Study.

Surgical procedures in the anterior mandibular region require a profound understanding of the complex anatomical structures to ensure patient safety. The lingual foramen (LF) and its intra-osseous canal are pivotal structures, supplying vascular and neural support to the mental region. The prevalence of LF is widely recognized, with variations among populations. The study involved 650 adult participants. Data were collected using high-resolution cone-beam computed tomography (CBCT), and statistical analysis was conducted, establishing inter-rater agreement. The population had a varied age distribution, with males comprising 53.23% and females comprising 46.77%. The median lingual canal was found in the Supra and Infra spinosum (65.38%). Most participants had lateral lingual canals (60.92%). The most prevalent lingual foramina was 4 (30.62%). The study found no significant associations between gender and LF diameter, distribution in different travel directions, or LF locations. However, males had a slightly larger diameter. The findings provide valuable insights into LF variations based on anatomical locations, and there were no statistically significant differences in measurements across different age groups. This knowledge contributes to the evolving field of oral and maxillofacial surgery, ensuring better patient outcomes.

Modification on Window Technique in Full Edentulous Patient with Hyperplasia

Complete denture is often used by patients that have edentulism. However, a long-term denture wearers commonly showed the hyperplastic growth of tissue called hyperplasia. This condition will present some difficulties for impression and retention. A 77 year old male patient reported to the RSGM UMY with a loose-fitting denture. He has been using it for 10 years. His old denture has suction cup on the palatal which no longer retentive and stable. The clinical examination showed the maxillary and mandibular complete edentulous ridge with flabby ridge area in the maxillary and mandibular anterior region. Patient is given treatment complete denture with acrylic resin material. In this case, window technique is used as the impression technique for a flabby ridge. Denture-induced hyperplasia is a condition of enlarged of fibrous tissue that could leads to poor denture retention, support, and stability. In the presence of hyperplasia, fabrication of as stable denture can be very arduous challenge. Special impression technique, like window technique frequently used on a case with flabby ridge. Window technique is a technique which use medium body impression material for the un-displaceable tissue whereas the flabby tissue impressed by using light body elastomer impression that must be freed from pressure. In patient with flabby ridge or hyperplasia due to denture, fabrication of impression need special technique like window technique which can be proven to ensure a final impression of the flabby ridge to improve denture stability, retention, support, and patient comfort.

Interproximal reduction in the refinement phase of Invisalign treatment: A quantitative analysis

Interproximal reduction (IPR) is a common adjunct to contemporary orthodontic treatment. This study aimed to carry out a quantitative analysis of IPR prescribed in the refinement phases of clear aligner therapy with the Invisalign appliance (Align Technology, San Jose, Calif). The digital treatment plans (DTPs) of a total of 330 patients treated by 11 orthodontists were evaluated. Relevant data regarding patient age, gender, and prescription of IPR in the initial and refined DTPs were obtained from Align Technology's digital interface, ClinCheck. Computational analyses included descriptive statistics, Mann-Whitney U, and Kruskal-Wallis tests. Most (n= 182; 75.2%) of the 242 patients who satisfied inclusion criteria were females. The median (interquartile range [IQR]) age was 29.2 (22.1-40.2) years. More than 60% of the contact sites prescribed IPR related to the initial DTP (n= 1312; 60.4%), with 39.6% (n= 859) recorded in the refinement DTPs. A median (IQR) of 1.1 (0.6-2.1) mm of IPR was prescribed per patient in the initial DTP compared with a median (IQR) of 0.6 (0.3-1.3) mm in the refinement DTPs. The most common site for prescribed IPR in all DTPs was the mandibular anterior region. Almost half (n= 108; 44.6%) of the patients were prescribed IPR at the same contact point site more than once during treatment. Almost 40% of the contact points that were prescribed IPR were in the patients' refinement DTPs. Most IPR was prescribed for the anterior region of the mandible. Almost half of the patients had IPR repeatedly prescribed at the same sites during treatment.

Alveolar Bone and Gingival Changes in Mandibular Anterior Region Following Herbst Appliance Therapy: A Systematic Review and Meta-analysis.

To assess the alveolar bone changes and gingival recession following Herbst appliance therapy. Electronic databases such as PubMed, Ovid, Cochrane Library, Lilacs, Scopus, Web of Science, and Embase were searched until August 2022. Hand-searching of major orthodontic journals was performed to identify all peer-reviewed articles potentially relevant to the review. The quality of the selected studies was ranked using the revised Cochrane risk of bias tool for nonrandomized trials-Risk of Bias In Nonrandomized Studies (ROBINS) 1. Five relevant articles (all nonrandomized studies) were considered for qualitative analysis. The risk of bias was low for four studies and moderate for one. The reduction in the vertical alveolar bone height was 0.13 ± 0.07 mm, with the Herbst appliance. The mean difference in the loss of buccal cortical thickness between the Herbst appliance and untreated control group was 0.22 mm [95% confidence interval (CI) of -0.62-0.18]. Subsequent to Herbst appliance therapy, in the mandibular anterior region 0.1 ± 0.5 mm of gingival recession was observed. Herbst appliance treatment produces a negligible reduction in the buccal cortical thickness, vertical alveolar bone height, and gingival recession. Though the changes produced by the Herbst appliance were minimal, they are clinically important considering the young age of the patients warranting periodic periodontal assessment. Incidence, treatment method and recurrence rate in giant cell granulomas: Retrospective study

Central and peripheral giant cell granulomas are benign entities mostly seen in mandibular anterior region at female individuals, usually with observed recurrence. Their etiology is still unclear, as is the optimal method for treating them. The aim of this study was to evaluate the incidence, treatment methods, recurrence rates, and initial and definitive correlation of central and peripheral giant cell granulomas.Patients who were referred to our clinic between 2013 and 2023 and who had the lesions’ definitive diagnosis as “central giant cell granuloma” (CGCG) or “peripheral giant cell granuloma” (PGCG) were included in the study. Demographic data, recurrence rates, treatment methods, lesion location, clinical behaviors, and sizes were noted on the reports.A total of 30 lesions in 23 patients (14 PGCG and 9 CGCG) were evaluated in this study. The mean follow-up time was 62.6 months; 8 of 23 patients had systemic disease. While only 1 patient was observed to have cortical bone destruction in PCGC, all patients were found to have cortical bone destruction in CGCG (p < 0.05). In both lesions, the correlation of preliminary and definitive diagnosis was evaluated, and it was found to be 50% in PGCG while it was 77.7% in CGCG. The recurrence rates were 21.4% in PGCG and 33.3% in CGCG. Curettage was applied in all patients. Additional treatments (intralesional steroid injections, denasumab applications, resection, and graft application) were performed in 5 patients who were found to have CGCG (p = 0.004). However, there was no significant relation between treatment method and recurrence in CGCG (p > 0.05).Various peripheral lesions could mimic PGCG; thus, curettage therapy could be appropriate in the treatment of PGCG. Nevertheless, in some cases of CGCG, additional treatment methods could be more effective for preventing recurrence and any other complications.

Mandibular Mesiodens: A Case Report of Rare Clinical Entity

Supernumerary teeth are those that are in excess of the normal series. Supernumerary teeth can appear as a single tooth, multiple teeth, unilaterally or bilaterally, and in the maxilla, mandible, or both. The presence of this condition, particularly in the mandibular anterior region, has been reported infrequently. The purpose of this paper is to report and review the occurrence of such a rare combination. A 9-year-old girl presented to the Department of Pediatric and Preventive Dentistry with the chief complaint of a malpositioned tooth in the lower anterior region of the jaw. A supplemental tooth resembling a permanent central incisor was discovered during a clinical examination. The presence of Mesiodens was confirmed by radiographic examination. The mesiodens was extracted, followed by a spontaneous closure of the diastema. This case report revealed that the early detection of mandibular supplemental incisors could facilitate early intervention, a better prognosis, and fewer complications.
