Tunneled Flaps: An Ingenious Reconstructive Option for Anterior Auricle Defects.

Background:The anterior surface of the auricle is especially susceptible to a variety of traumatic insults and cutaneous malignancies. However, given the unique anatomy of this structure and the lack of local tissue available, the reconstruction of these defects is still a challenge. Despite of their positive outcomes, tunneled flaps have not been thoroughly explored in the literature.Objectives:To review the clinical utility of tunneled flaps in reconstructing anterior auricle defects.Materials and methods:A review of the literature on surgical reconstruction of the anterior auricle surface was performed in PubMed, with specific reference to tunneled flaps.Conclusions:When designed and executed properly, tunneled flaps offer unrivalled opportunities to reconstruct anterior auricle defects, with maximal preservation of the symmetry.

Inferiorly Based Preauricular Flap for Anterior Ear Reconstruction.

The anterior aspect of the auricle is a complex 3-dimensional structure. Each anatomical component in this region has an essential role in the aesthetic appearance of the ear and face. The reconstruction of defects in this region is challenging because of the lack of mobile, excess skin for primary closure, and the inability to skin graft overexposed cartilage. The aim of the study was to present the planning and surgical technique of a simple, reproducible, 1-stage flap, for the reconstruction of the anterior aspect of the auricle. A series of 11 patients, who underwent reconstruction of the anterior aspect of the auricle with 1-stage, inferiorly based, preauricular, cutaneous flap. All reconstructions were conducted under local anesthesia, and the defects were mainly due to tumor resections or skin necrosis after otoplasty. The average defect size was 1.5 cm. The flap was applied to different sites of the anterior auricle. All flaps survived except one, where there was partial flap loss. The aesthetic results were excellent, with no auricular deformity. The inferiorly based, preauricular flap is a versatile flap for a safe, simple, and reproducible, 1-stage reconstruction for almost every region of anterior ear defect, with excellent aesthetic results.

Adverse event associated with auriculotherapy

ABSTRACT CaseA 76‐year‐old man with tympanic membrane perforation and hearing loss in the right ear since childhood was hospitalized in September 2015 due to deterioration of renal function. On cranial magnetic resonance imaging (MRI) to evaluate possible cerebrovascular disorder, an artifact was observed. Further, cranial computed tomography (CT) showed seven metallic objects in the region extending from the right external ear canal to the right middle ear.OutcomeThe objects were determined to be acupressure pellets measuring 1 mm in diameter.ConclusionTaking careful history of any prior ear disease is essential, as is the conducting of thorough examination for tympanic membrane perforation. Use of acupressure pellets on the anterior auricle should be avoided in patients with tympanic membrane perforation.

Pseudocyst of the auricle in a very young child

Abstract Pseudocyst of the auricle (AP) is a rare, benign, cystic lesion that often arises asymptomatically along the anterior auricle typically in adolescent and adult Asian males with no history of antecedent trauma. This is a case report of a 19-month-old male who spontaneously developed AP. Treatment consisted of pressure bolstering of both auricles after excisional biopsy of right AP and aspiration with steroid injection of the left lesion. Follow-up at three months revealed resolution of all lesions and no auricular deformity. This is only the second case reported within literature of AP presenting in a very young child.

Olenekian (Early Triassic) bivalves from the Salt Range and Surghar Range, Pakistan

Abstract: Based on newly collected material from the uppermost Smithian and lower to middle Spathian (Olenekian, Lower Triassic) of the Salt Range and Surghar Range (Pakistan), 15 bivalve species belonging to 11 genera are described, including two new genera, Eobuchia and Dimorphoconcha, and one new species, Palaeoneilo? fortistriata. Eobuchia gen. nov. is placed in a new subfamily, the Eobuchiinae, which differs from the Buchiinae in having an almost planar and only moderately inclined or offset right anterior auricle. Inclination of the right anterior auricle is proposed as a synapomorphy of the revised suborder Monotidina, which includes the Buchiidae, Monotidae, Oxytomidae and, tentatively, the Dolponellidae. The Pseudomonotidae, Chaenocardiidae and Claraiidae are discussed as candidate ancestors of the Monotidina. Dimorphoconcha gen. nov., provisionally placed in the Limidae, is a morphologically unusual genus characterized by a globose shell centre and a strongly plicate fringe. Permophorus costatus, which was previously known exclusively from Permian strata, is reported from the Spathian of the Surghar Range. This record extends the range of P. costatus for at least 8 Myr and makes it the first reported Lazarus species, with an outage of more than 2 Myr after the end‐Permian mass extinction. Ten of 15 species recognized in this study have not been reported from other regions, which may indicate increasing provincialism towards the end of the Early Triassic, or, alternatively, reflect the still insufficient knowledge of benthic faunas from the epoch that followed the greatest crisis in the history of life.

Surgical management of polyotia

Polyotia is an extremely rare type of congenital external ear malformation, which is defined as an accessory ear that is large enough to resemble an additional pinna. The terms 'mirror ear' or 'accessory ear' are sometime used. We present our methods in correcting this malformation and summarise the aetiology. The posterior part of the polyotia may presents with a normal ear, a constricted ear or a microtic ear. Free auricular composite tissue transplantation was used to correct the constricted ear. Ear reconstruction was applied in cases of microtia. The anterior auricle was mainly used to form the tragus. 7 cases polyotia were treated between 2004 and 2008. After free auricular composite tissue transplantation the size of the constricted ear and the contralateral ear was similar. In microtia cases the reconstructed ears were natural looking and had a satisfactory three-dimensional contour. The extra tissue of the anterior ear was excised and the tragus was reconstructed. Through operative intervention tailored to the individual case natural-looking and symmetric ears were acquired. The aetiology of polyotia probably relates to abnormal migration of neural crest cell.

A primitive Halobia (Bivalvia: Halobioidea) from the Triassic of northeast British Columbia

Halobia daonellaformis new species is described from the lowermost Carnian of northeast British Columbia. Halobia daonellaformis n. sp. is regarded as a primitive Halobia characterized by external ornamentation similar to Daonella lommeli, but with a poorly developed anterior auricle. Morphologic characters of H. daonellaformis n. sp. suggest that Halobia may be not a natural taxon but a polyphyletic group with one or more ancestors from Daonella and Aparimella and/or other posidoniid(s). The sudden appearance of Halobia throughout the marine Triassic suggests a rapid dispersal mechanism following a Ladinian origin. Larval shell morphology indicates a planktotrophic developmental strategy for H. daonellaformis n. sp., and by extension to other halobiids, which may explain the widespread distribution of many halobiid species.

Pseudocyst of the Auricle Associated With Trauma

A pseudocyst of the auricle (benign idiopathic cystic chondromalacia) is an intracartilaginous cystic swelling of the anterior auricle. The cause is uncertain, and most patients deny any history of inflammation or trauma. A patient had antecedent trauma and a fracture in the conchal cartilage; a simple surgical procedure was used to treat this patient.
