The Role of Voluntary Counselling and Testing Points (VCTs) in HIV Diagnostics – Analysis of VCTs Activity in Poland in the Context of Local and European Epidemiology

Abstract Screening tests are now readily accessible, quick and highly sensitive. Diagnostics of HIV infections involves serological testing followed by confirmation tests by molecular biology methods (NAAT). Fast, accurate, anonymous and free HIV testing has been provided by Voluntary Counselling and Testing Points (VCTs) for over 20 years to any adult in Poland, regardless of nationality. Additionally, the educational activities are conducted aimed at informing people from various social backgrounds about HIV prevention and diagnostic possibilities, as well as other sexually transmitted infections (STIs). The aim of the article was to present the procedures for the operation of VCTs in Poland, their location, testing scheme and a summary of the activities that promote such points in the medical community. Between 2019 and 2023, 157,833 people registered for HIV testing in VCTs in Poland, of which 10,177 (6.45%) were foreigners. In the analyzed period, the number of tests among foreigners visiting VCTs in Poland almost doubled, from 1,552 in 2019 to 2,827 in 2023. In addition, a higher percentage of positive results was recorded among foreigners – 5.08% compared to Poles – 1.24%. The frequency analysis of positive results detection reported in VCTs vs. National Institute of Public Health NIH – National Research Institute showed that, on average 26% (2,342/8,891) of positive results in Poland from 2019–2023 were detected in the framework of tests carried out in VCTs. The operation of VCT points in Poland enables the reaching of a larger group of people, especially key adult populations. It is a valuable complement to routine diagnostics outside the health care system.

Estimation of the prevalence of HIV, syphilis and hepatitis C among prisoners in Tajikistan

Aim. To assess the prevalence of HIV, syphilis and hepatitis C in prisons in the Republic of Tajikistan.Materials and methods. The biobehavioural study included anonymous testing for 3 infections (HIV, syphilis and hepatitis C) and a survey. Participation was voluntary. Each participant was able to access the results of the study through their unique code, which was assigned prior to the start of the study. Data entry and analysis were performed using Epi-Info-3.5.2 analysis software.Results and discussion. The prevalence rates of HIV, syphilis and hepatitis C in prisons were HIV - 3.4%, syphilis - 2.8%, hepatitis C - 8.9%. According to the results, drug use by injection is prevalent in prisons. The presence of homosexual contacts in prisons is also confirmed.Conclusion. HIV prevalence among prisoners in Dushanbe has increased from 2.2 to 2.9 per cent. In Dushanbe, it increased from 2.5 per cent in 2020 to 3.5 per cent in 2023, while in Khujand it decreased (3.8 per cent and 3.3 per cent). Prisoners’ awareness of HIV/AIDS and use of preventive measures is low. In 2023, only 56 per cent of prisoners had been tested for HIV.

Effectiveness of integrating HIV prevention within sexual reproductive health services with or without peer support among adolescents and young adults in rural KwaZulu-Natal, South Africa (Isisekelo Sempilo): 2 × 2 factorial, open-label, randomised controlled trial

Approximately 200 000 South Africans acquired HIV in 2021 despite the availability of universal HIV test and treat and pre-exposure prophylaxis (PrEP). The aim of this study was to test the effectiveness of sexual and reproductive health services or peer support, or both, on the uptake of serostatus neutral HIV services or reduction of sexually transmissible HIV. We did an open-label, 2 × 2 randomised factorial trial among young people in a mostly rural area of KwaZulu-Natal, South Africa. Inclusion criteria included being aged 16-29 years, living in the mapped geographical areas that were accessible to the area-based peer navigators, being willing and able to provide informed consent, and being willing to provide a dried blood spot for anonymous HIV testing and HIV viral load measurement at 12 months. Participants were randomly allocated by computer-generated algorithm to one of four groups: those in the standard-of-care group were referred to youth-friendly services for differentiated HIV prevention (condoms, universal HIV test and treat with antiretroviral therapy, and PrEP if eligible); those in the sexual and reproductive health services group received baseline self-collected specimens for sexually transmitted infection (STI) testing and referral to integrated sexual and reproductive health and HIV prevention services; those in the peer support group were referred to peer navigators for health promotion, condom provision, and facilitation of attendance for differentiated HIV prevention services; and those in the final group received a combination of sexual and reproductive health services and peer support. Coprimary outcomes were linkage to clinical services within 60 days of enrolment, proportion of participants who had sexually transmissible HIV at 12 months after enrolment, and proportion of sampled individuals who consented to participation and gave a dried blood spot for HIV testing at 12 months. Logistic regression was used for analyses, and adjusted for age, sex, and rural or peri-urban area of residence. This study is registered with ClinicalTrials.gov (NCT04532307) and is closed. Between March 2, 2020, and July 7, 2022, 1743 (75·7%) of 2301 eligible individuals were enrolled and followed up. 12-month dried blood spots were collected from 1168 participants (67·0%). The median age of the participants was 21 years (IQR 18-25), 51·4% were female, and 51·1% had secondary level education. Baseline characteristics and 12-month outcome ascertainment were similar between groups. 755 (43·3%) linked to services by 60 days. 430 (49·8%) of 863 who were in the sexual reproductive health services group were linked to care compared with 325 (36·9%) of 880 who were not in the sexual and reproductive health services group (adjusted odds ratio [aOR] 1·68; 95% CI 1·39-2·04); peer support had no effect: 385 (43·5%) of 858 compared with 370 (43·1%) of 885 (1·02, 0·84-1·23). At 12 months, 227 (19%) tested ELISA-positive for HIV, of whom 41 (18%) had viral loads of 400 copies per mL; overall prevalence of transmissible HIV was 3·5%. 22 (3·7%) of 578 participants in the sexual and reproductive health services group had transmissible HIV compared with 19 (3·3%) of 590 not in the sexual and reproductive health services group (aOR 1·12; 95% CI 0·60-2·11). The findings were also non-significant for peer support: 21 (3·3%) of 565 compared with 20 (3·3%) of 603 (aOR 1·03; 95% CI 0·55-1·94). There were no serious adverse events or deaths during the study. This study provides evidence that STI testing and sexual and reproductive health services create demand for serostatus neutral HIV prevention in adolescents and young adults in Africa. STI testing and integration of HIV and sexual health has the potential to reach those at risk and tackle unmet sexual health needs. Past-Year HIV Testing, Current Antiretroviral Therapy Use, and Participation in Services for People Who Inject Drugs.

Evaluating routine HIV testing and treatment and use of services for people who inject drugs (PWID) is critical to curb the ongoing HIV epidemic. We analyzed data from the 2018 National HIV Behavioral Surveillance of PWID aged 18years or older, recruited using respondent-driven sampling and offered anonymous HIV testing after survey. We performed bivariate and multivariable analyses with log-linked Poisson regression of the generalized linear models to examine the associations between demographics and PWID service use, past-year HIV testing, and current antiretroviral therapy (ART) use. Among 10,311 HIV-negative PWID, 56% reported past-year HIV testing, and of the 553 HIV-positive PWID, 69% reported current ART use. Of the HIV-negative PWID, 64% (2874/4482) in drug treatment and 62% (3386/5440) who used syringe service programs (SSPs) reported past-year HIV testing. Among HIV-positive PWID, 75% (187/248) in drug treatment and 67% (200/298) SSP participants were on ART. In the adjusted multivariable model, past-year HIV testing was associated with drug use treatment (aPR 1.26, 95% CI 1.23-1.31) and SSP participation (aPR 1.19, 95% CI 1.13-1.26) among HIV-negative PWID. Current ART use was associated with drug use treatment (aPR 1.13, 95% CI 1.00-1.28) but the link was not significant probably due to small sample size. Findings support the expansion and improvement of PWID-targeted services, into comprehensive programs, including drug use treatment, SSP, and HIV testing and treatment.

Voluntary counselling and testing points (VCTs) in Poland as a complementary and accessible alternative to public healthcare facilities: A comparative analysis of epidemiological data from the VCTs and the National Institute of Public Health NIH-National Research Institute (2015-2022).

Voluntary counselling and testing points (VCTs) offer anonymous and free HIV tests in Poland. They also play an essential role in educational initiatives focused on the prevention and diagnosis of HIV and other sexually transmitted infections. However, no comprehensive data is available that summarizes the results of the work carried out by these VCTs. Therefore, our aim was to conduct a comparative analysis of epidemiological data obtained from VCTs and data reported by the epidemiological surveillance undertaken by the National Institute of Public Health NIH-National Research Institute (NIPH NIH-NRI) covering the period from 2015 to 2022. This retrospective analysis was conducted on data from 258 071 people attending VCTs in Poland in 2015-2022. On average, 32 259 individuals underwent testing each year, with a notable increase in the number of people being tested in November. The average positivity rate was 1.39% (3576/258 071). The Masovian voivodeship conducted the most tests and had the highest number of positive results. The comparative analysis of the frequency of detecting positive results in VCTs and those reported in NIPH NIH-NRI data revealed that, on average, 31.49% (3576/11 356) of positive results in Poland between 2015 and 2022 were identified through tests conducted at VCTs. The positive results identified in VCTs constituted approximately one-third of all results reported by the National Institute of Public Health NIH-National Research Institute, highlighting the importance of VCTs. Moreover, the high availability of testing in the Masovian voivodeship resulted in better detection of HIV. The educational actions performed during European Testing Week increased the number of tests performed in November.

A rapid review of antenatal hepatitis C virus testing in the United Kingdom

BackgroundThe United Kingdom (UK) has committed to the World Health Organization’s viral hepatitis elimination targets. New case finding strategies, such as antenatal testing, may be needed to achieve these targets. We conducted a rapid review to understand hepatitis C-specific antibody (anti-HCV) and HCV RNA test positivity in antenatal settings in the United Kingdom to inform guidance.MethodsArticles and conference abstracts published between January 2000 and June 2022 reporting anti-HCV testing in antenatal settings were identified through PubMed and Web of Science searches. Results were synthesised using a narrative approach.ResultsThe search identified 2,011 publications; 10 studies were included in the final synthesis. Seven studies used anonymous testing methods and three studies used universal opt-out testing. Anti-HCV test positivity ranged from 0.1 to 0.99%, with a median value of 0.38%. Five studies reported HCV RNA positivity, which ranged from 0.1 to 0.57% of the testing population, with a median value of 0.22%. One study reported cost effectiveness of HCV and found it to be cost effective at £9,139 per quality adjusted life years.ConclusionThe relative contribution of universal opt-out antenatal testing for HCV should be reconsidered, as antenatal testing could play an important role in new case-finding and aid achieving elimination targets.

Sleep disorders and their relation to injuries among young elite soccer players

Aim: to study the quality of sleep and the relationship between sleep disorders and injury among young elite soccer players.Methods: the study summarizes data from relevant questionnaires obtained through anonymous cohort testing of 236 male football players from leading football academies, two farm teams of leading Russian Premier League clubs and three national youth teams.Results: the average sleep duration in the study participants was 9 hours. The prevalence of sleep disorders was 7 % (PSQI, Pittsburgh Sleep Quality Index) and 10 % (ASBQ, Athlete Sleep Behavior Questionnaire) and among the youngest soccer players, this prevalence of violations was the lowest. Significant or moderate daytime sleepiness was found in 59 % of participants. The risk of serious disorders increased with age. Sleep disorders significantly affected injuries according to the results of the PSQI questionnaire and retrospective collection of information about injuries.Conclusion: the confirmed relationship between injuries and sleep disorders allows us to consider sleep quality improvement as an important component of injury and morbidity prevention among athletes of team sports.

Development of oral health resources and a mobile app for caregivers and autistic children through consensus building.

Caregivers of autistic children often lack knowledge regarding oral homecare and when and where to see the dentist. To address this need, we developed a series of information on oral health. An autistic child assisted in developing two social stories to showcase a dental visit. A mobile app was developed to deliver the above mentioned. Other features include creation of customised social stories and visual schedule and an inbox to allow dentists to send messages to parents. The developed information and social stories were reviewed by experts and parents. The app also underwent anonymous and independent testing by parents. Overall the information and app were well received by the experts and parents.

Insights into the Binding Profile of Anti-chlorpyrifos Recombinant Antibodies: From Computational Simulation to Immunoassay Validation.

A novel virtual screening strategy was proposed for the profiling and discovery of active variable regions (VRs) that encode hapten-specific recombinant antibodies (rAbs). Chlorpyrifos, a hazardous organophosphorus pesticide, was selected as the target. First, a VR model-14G4 from anti-chlorpyrifos hybridoma was built via homology modeling. Its binding pattern toward seven organophosphorus analogues was assessed through virtual screening by performing molecular docking. Based on energy scoring, visual examination, and molecular interaction analysis, chlorpyrifos-methyl was also inferred as the high-affinity target for model-14G4 and was then confirmed via a non-competitive surface plasmon resonance (SPR) assay. Subsequently, we attempted to discover hapten-specific VRs by creating a collection of VR models for anonymous testing. Chlorpyrifos and model-14G4 were employed as the known hit and active VRs, respectively. After molecular docking, a novel anti-chlorpyrifos VR (model-1) was identified due to its satisfactory energy scoring and a similar binding pattern to the reference model-14G4. Expressed by HEK293(F) mammalian cells, the newly prepared full-length rAb-model-1 and rAb-14G4 exhibited high sensitivities for detecting chlorpyrifos by the indirect competitive enzyme-linked immunosorbent assay (ic-ELISA), with IC50 of 3.01 ng/mL and 42.82 ng/mL, respectively. They recognized chlorpyrifos-methyl with a cross-reactivity (CR) of 2.5-17.3%. Moreover, the binding properties of rAb-model-1 for recognizing chlorpyrifos and chlorpyrifos-methyl were confirmed via a non-competitive microscale thermophoresis (MST) method. Thus, the experimental results showed good agreement with computational outputs on antibody profiling. Furthermore, the recognition diversity of rAb-model-1 for chlorpyrifos and chlorpyrifos-methyl was studied via molecular dynamics simulation. Overall, the proposed study provides a versatile and economical strategy for antibody characterization and promotes the in vitro production of rAbs for pesticide monitoring.

Compression microvibration: Experience of using the method for the physiotherapeutic treatment of cellulite

BACKGROUND: Cellulite treatment and figure correction are among the most demanded requests in aesthetic medicine centers, which determines the high social and medical and economic significance of the problem, stimulates the active use of modern medical technologies.
 AIM: evaluation of the effectiveness of the non-invasive method of compression microvibration in the treatment of cellulite in women of peri- and postmenopausal age.
 MATERIAL AND METHODS: A single center prospective cohort study was conducted in which 27 female volunteers with mild to moderate gynoid lipodystrophy took part. All participants completed the Endospheres Therapy course according to the standard protocol: 12 procedures for 60 minutes twice a week using the Endospheres Therapy Body compression microvibration apparatus. Before and after the completion of the course of procedures, anthropometric parameters (linear and weight dimensions of the body) were measured, fat, lean, active cellular and skeletal muscle mass, specific basal metabolism, total and extracellular fluid, bioimpedance phase angle were evaluated using bioimpedancemetry. To obtain a subjective assessment of the effectiveness and quality of the procedures received, all participants underwent anonymous testing.
 RESULTS: Under observation were 27 women, who made up 3 clinical groups of 9 women each in accordance with age criteria: group 1 ― before menopause (age 4049 years); group 2 ― menopause (5059 years) and group 3 ― postmenopause (6069 years). After the Endospheres Therapy course, the mean group body mass index values did not change, however, a weight loss of 3.47.4 kg, a decrease in waist and hips by 2.23.02.5 cm, 4.15.36.3 cm in the respective age groups. In the first group of participants, the fat mass index decreased by 16.4% (p 0.005), active cell mass increased by almost 15% (p 0.005), skeletal muscle mass ― by 13% (p 0.005), the specific basic metabolism ― by 10% (p 0.005), the value of phase angle after the course Endospheres Therapy increased statistically significantly by 16% (p 0.05). In other age groups, bioimpedance measurements were similar, but less pronounced. After the procedures, in some cases, study participants reported a slight tingling/squeezing sensation, slight muscle soreness, reminiscent of the state after exercise, and temporary slight redness of the skin.
 CONCLUSION: The results obtained allow us to state the effectiveness and safety of the use of Endospheres Therapy in the treatment of cellulite, as a pathogenetically substantiated method of physiotherapy that affects the improvement of the skin condition, the transformation of the fat component, the increase in cell metabolism, muscle tone, stimulation of lymphatic drainage and tissue trophism.

Behavioral predictors associated with HIV screening needs in gay Korean men during the COVID-19 pandemic.

During the pandemic, the capacity of medical resources focused on testing, diagnosing and treating COVID-19 has severely limited public access to health care. In particular, HIV screening, for which homosexual males in Korea received free and anonymous testing at public health centers, was completely halted. This study investigated behavioral predictors related to the HIV screening needs of Korean male homosexuals during the pandemic. Data were collected by conducting a web survey targeting members of the largest homosexual portal site in Korea with support from the National Research Foundation of Korea (n = 1,005). The key independent variables are COVID-19-related characteristics and sexual risk behavior. The moderating variable is health information search behavior, and the dependent variable is the need for HIV screening. For a statistical analysis, a hierarchical multiple logistic regression analysis was conducted while controlling for potential confounding variables. According to the results of this study, the need for HIV screening was 0.928 times lower for older people (p<0.05, 95% CI = 0.966-0.998). However, if the respondent had a primary partner, the need for HIV screening was 1.459 times higher (p<0.01, 95% CI = 1.071-1.989). In addition, the need for screening was 1.773 times higher for those who preferred anal intercourse (p<0.01, 95% CI = 1.261-2.494) and 2.034 times higher (p<0.01, 95% CI = 1.337-3.095) if there was a history of being diagnosed with an STD. Finally, health information-seeking behavior was marginally significant. This study revealed that male Korean homosexuals who were young, preferred anal sex with a primary partner, and who had a history of a sexually transmitted disease had a high need for HIV screening at public health centers. They are more likely to be susceptible to HIV infection because they are usually consistent with gay men, characterized by risky behavior. Therefore, an intervention strategy that provides health information using a communication campaign is needed.

A multimodal scanner coupling XRF, UV–Vis–NIR photoluminescence and Vis–NIR–SWIR reflectance imaging spectroscopy for cultural heritage studies

AbstractScanning x‐ray fluorescence (MA‐XRF), Reflectance Imaging Spectroscopy (RIS) and Luminescence Imaging Spectroscopy (LIS) are scientific methods extensively used in the investigation of artworks in Cultural Heritage Science. Nevertheless, these three highly complementary imaging methods have never been implemented in a single instrument. In this paper, we present a scanning system that is the first to simultaneously combine MA‐XRF, RIS, and LIS techniques, generating spatially aligned hyperspectral datasets. The specifications of the instrument and of the collected datacubes are described, together with the processing pipeline developed for the coupled exploitation and interpretation. The operation of this instrument, combining XRF with RIS and LIS mapping in an extended range (400–2500 nm and 200–1000 nm, respectively), is exemplified on an anonymous test painting, studied previously. A focus on a restored area of this painting allows to illustrates the complementarity of the three aforementioned methods, for the visualization and characterization of pigments, varnish, and binders.

Characteristics of rectal chlamydia among men who have sex with men in southern Taiwan, 2020-2022: An emerging threat of rectal lymphogranuloma venereum L2b.

The prevalence of rectal chlamydia among men who have sex with men (MSM) without human deficiency virus infection (non-HIV) remains uncertain in Taiwan, and rectal lymphogranuloma venereum (LGV) among MSM has never been reported in the Far East. From January 2020 to April 2022, MSM coming for anonymous voluntary counseling and testing, for pre-exposure prophylaxis, and for antiretroviral therapy were enrolled. All participants submitted his fecal samples and completed a QR-code questionnaire. Medical records of those who took regular medical visits for HIV were recorded. Multiplex polymerase chain reaction (PCR) was performed for all fecal samples, and ompA gene sequencing was therefore performed for each Chlamydia-positive fecal sample. Among 341 MSM during 2020-2022 in southern Taiwan, 21 (6.2%) had rectal chlamydia infection. Risk factors of rectal chlamydia included co-infection with rectal gonorrhea (adjusted odds ratio [AOR] 6.78, 95% confidence interval [CI] 1.44-31.91, P=0.015) and multiple sexual partners (AOR 1.373, 95% CI 1.002-1.882, P=0.048). Further ompA gene sequencing from 19 Chlamydia-positive fecal samples revealed that the prevalent genotypes or genovariants were Da (26.3%) and L2b (26.3%), followed by B (21.1%), J (14.3%), and G (9.5%). All cases of rectal LGV genovariant L2b presented as acute proctitis with diarrhea, anal pain, or discharge and were treated successfully with prolonged treatment of doxycycline. Rectal gonorrhea and multiplex sexual partners are risk factors for rectal chlamydia. Clinicians in Taiwan should be aware of the emerging threat of rectal LGV among MSM with acute proctitis.

Prosodic perceptive thresholds in Spanish: intensity and duration

The following study aims at examining prosodic thresholds of perception for Spanish, parting from the algorithm of analysis offered by the Prosodic Analysis of Speech (PAS) model by Cantero (2019). The approach is based on defining the prosodic values for each syllable after a process of standardization, by relativizing the prosodic value associated with each syllable to that of the preceding one. As the perception threshold in the case of melody was defined as 10% for Spanish (FONT-ROTCHÉS; MATEO RUIZ, 2011), this research investigates, by the help of an anonymous perceptive test submitted to 30 native Spanish speakers, what prosodic relative values of intensity and duration change with respect to the previous syllable make a syllable perceived as prosodically prominent. The informants of the test had to decide whether in an invented Spanish word syllables with different range of increase in intensity or duration result in perceptibility as far as prosodic prominence is concerned, and whether these prosodic cues contribute to stress identification. As for the results, a 30% of increase in duration with respect to the previous syllable is already perceivable, but intensity growth of 5% is not, and neither of these thresholds trigger automatically stress identification. The results of the research can be used as a point of reference in speech synthesis programs.

Factors Influencing the Acceptability of Nominal, Non-nominal, and Anonymous HIV Testing Options Among Young Men Living in Vancouver, Canada: A Qualitative Study.

Individuals unaware of their HIV seropositive status continue to represent a key "driver" of the HIV epidemic. Strategies to improve initiation into the HIV treatment cascade, including among young men, are required. This study assesses the acceptability of three HIV testing options among young cisgender men living in Vancouver, British Columbia. Using a modified grounded theory approach, we conducted semi-structured interviews with 45 young cisgender men ages 18-30 years to identify the factors influencing the acceptability of diverse HIV testing approaches. Participants ascribed value to non-nominal testing as a means of providing a secure pathway to HIV-related care for those who test positive while also safeguarding privacy. Anonymous HIV testing was problematized by participants as potentially creating difficulty in accessing ongoing HIV-related care. Most participants preferred non-nominal testing. Nevertheless, based on principles of equity, we argue that anonymous access to one's serostatus should be available, especially within a criminalized context.

HIV infections and AIDS in Poland in 2020.

The aim of the study was to assess the epidemiological situation of newly diagnosed HIV infections and AIDS cases and death among AIDS cases in Poland in 2020 in comparison to the changes in preceding years. Analysis of the epidemiological situation was based on reports of newly detected HIV cases and AIDS cases received from doctors and laboratories and the results of the annual survey of HIV testing conducted among the laboratories throughout the country. A dataset on clients from anonymous Voluntary Testing and Counselling (VCT) network was also used. The activities of the VCT are coordinated by the National AIDS Center, which anonymously collects epidemiological and behavior data on people who come for testing. In 2020 there were 934 HIV cases newly diagnosed in Poland (diagnosis rate 2.44 per 100,000), including 100 among non-Polish citizens. The number of newly detected HIV infections decreased by 39.9% compared to the previous year and was lower by 26.9% compared to the median in 2014-2018 years. Similarly, among VCT clients, number of HIV diagnoses decreased by 42%, from 536 in 2019 to 308 in 2020 year, what was connected with 44% decreased in number of HIV tests. The total number of AIDS cases was 50 (incidence 0.13 per 100,000). The HIV infection was most often detected in the age group between 30 and 39 (35.7%) and among men (83.5%). Among cases with known transmission route, 68.5% concerned among men who had sexual contact with men (MSM). The percentage of AIDS cases diagnosed at the same time with HIV diagnosis decreased by almost 14 percent points in comparison to 2019 (from 75.8% to 62% of all incident AIDS cases). In 2020, the number of newly detected HIV infections decreased compared to the previousyear, however, this is not only due to a decrease in the number of infections, but rather to a lack of continuity inthe diagnostic and reporting system during the COVID-19 pandemic, as indicated by a smaller number of testsand greater delays in data reporting. Due to the chronic course of HIV infection, a full assessment of the impactof the COVID-19 pandemic will be possible in the future.

Sexually Transmitted Diseases in Northern Israel: Insights From a Large Referral Laboratory.

The aims of the study are to investigate the distribution and frequency of different sexually transmitted diseases (STDs) among a large study population of individuals undergoing STD investigation both in inpatient and STD clinic settings and to evaluate influence of test anonymity on the positivity rate of pathogens. A retrospective study retrieved epidemiologic data from the following 3 sources: a secondary referral hospital and 2 STD clinics in Northern Israel. Positivity rate of Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG), Mycoplasma genitalium , and Trichomonas vaginalis (TV) was assessed and stratified based on age, sex, site of sampling, and anonymity of test. Adjusted odds ratios (ORs) were calculated by multivariable logistic regression. Overall, 3,753 assays were performed on 2,407 patients who were screened for STD. Chlamydia trachomatis (4.8%) was the most frequently detected STD, followed by NG (2.1%), MG (1.9%), and TV (0.6%). Mycoplasma genitalium (OR, 4.32; 95% CI, 1.70-10.97; p = .001) and NG (OR, 6.08; 95% CI, 2.18-16.96; p < .001) were significantly associated with male sex, while TV was more frequently encountered among female individuals (OR, 4.34; 95% CI, 1.49-12.50; p = .003). Mycoplasma genitalium infection was detected most commonly by urine samples, while rectal swabs were the leading source of positive tests for CT. Compared with fully identified patients, those tested anonymously were 6-fold more likely to be tested positive for TV (adjusted OR, 6.49; 95% CI, 2.06-20.42; p = .001). Chlamydia trachomatis and NG are the leading non-HIV STDs in Northern Israel. Anonymous tests predict higher positivity of TV. Rectal sampling should be increasingly used because of its efficacy in detecting CT infections.

A Key Comprehensive System for Biobehavioral Surveillance of Populations Disproportionately Affected by HIV (National HIV Behavioral Surveillance): Cross-sectional Survey Study.

The National HIV Behavioral Surveillance (NHBS) is a comprehensive system for biobehavioral surveillance conducted since 2003 in 3 populations disproportionately affected by HIV: gay, bisexual, and other men who have sex with men (MSM); people who inject drugs; and heterosexually active persons at increased risk for HIV infection (HET). This ongoing and systematic collection and analysis of data is needed to identify baseline prevalence of behavioral risk factors and prevention service use, as well as to measure progress toward meeting HIV prevention goals among key populations disproportionately affected by HIV. This manuscript provides an overview of NHBS from 2003 to 2019. NHBS is conducted in rotating, annual cycles; these 3 annual cycles are considered a round. Venue-based, time-space sampling is used for the MSM population. Respondent-driven sampling is used for people who inject drugs and HET populations. A standardized, anonymous questionnaire collects information on HIV-related behavioral risk factors, HIV testing, and use of prevention services. In each cycle, approximately 500 eligible persons from each participating area are interviewed and offered anonymous HIV testing. From 2003 to 2019, 168,600 persons were interviewed and 143,570 agreed to HIV testing across 17 to 25 cities in the United States. In the fifth round (2017 to 2019), over 10,000 (10,760-12,284) persons were interviewed each of the 3 population cycles in 23 cities. Of those, most (92%-99%) agreed to HIV testing. Several cities also conducted sexually transmitted infection or hepatitis C testing. NHBS is critical for monitoring the impact of the Ending the HIV Epidemic in the United States initiative. Data collected from NHBS are key to describe trends in key populations and tailor new prevention activities to ensure high prevention impact. NHBS data provide valuable information for monitoring and evaluating national HIV prevention goals and guiding national and local HIV prevention efforts. Furthermore, NHBS data can be used by public health officials and researchers to identify HIV prevention needs, allocate prevention resources, and develop and improve prevention programs directed to the populations of interest and their communities.

Religious Governance as Collaboration for the Resolution of Disgust: The Case of Protestantism in South Korea

The pandemic enhanced disgust for the socially disadvantaged. In South Korea, hatred of the Chinese, the Shincheonji Church, sexual minorities, and migrant workers intensified during the pandemic. In this social atmosphere of fear and anxiety, Korean Protestantism turned into a representative group that promotes and spreads disgust. In particular, homophobia can be said to be led by the conservative Protestants in Korea. A secularization strategy proposed by David Martin has significant implications in resolving this disgust demonstrated by Korean Protestants. Martin asserts that Christianity should respond appropriately to the demands for the enhancement of the public good with the resources of religion that he calls secularization. This paper argues that religion-government governance can be a process and system that makes possible secularization in which religion realizes the virtues of reconciliation and hospitality beyond conflict and disgust. Among various types of governance, collaborative governance is the most appropriate for religion-government governance to resolve disgust for the socially disadvantaged. Here, collaboration means the process of pursuing the realization of the public good by creating new values beyond the actors’ interests. A case for this can be found in the collaboration between some Protestants and LGBTQIA+ rights activists and the government to support anonymous COVID-19 tests on homosexuals during the pandemic.

Impact of coronavirus disease 2019 on the HIV testing and health care delivery at a university hospital in Taiwan, 2020–2021

BackgroundTo contain the coronavirus disease 2019 (Covid-19) pandemic, non-pharmacologic interventions, including lockdown and social distancing, may have adverse impact on access to HIV testing and care. This study investigated the impact of Covid-19 on HIV testing and care at a major hospital in Taiwan in 2020–2021.MethodsThe numbers of clients seeking anonymous HIV voluntary counseling and testing were compared 2 years before (2018–2019) and 2 years after Covid-19 outbreak (2020–2021). People living with HIV (PLWH) who sought care at the hospital during 2018–2021 were included to examine the status of HIV care delivery and disposition.ResultsThe annual number of HIV screening tests performed had significantly decreased from 2507 to 2794 in 2018 and 2019, respectively, to 2161 and 1737 in 2020 and 2021, respectively. The rate of discontinuation of HIV care among PLWH was 3.7% in 2019, which remained unchanged in 2020 (3.7%) and 2021 (3.8%). The respective percentage of annual plasma HIV RNA testing <2 times increased from 8.4% to 7.8% in 2018 and 2019 to 7.0% and 10.7% in 2020 and 2021, so was that of annual syphilis testing <2 times (10.1% and 8.8%–7.9% and 12.0%). The rates of plasma HIV RNA <200 copies/ml ranged from 97.0% to 98.1% in 2018–2021.ConclusionsDuring the Covid-19 pandemic, access to HIV counseling and testing was significantly limited. While the number of HIV-related testing decreased, the impact of Covid-19 on the continuity of antiretroviral therapy and viral suppression among PLWH appeared to be minimal in Taiwan.
