Timing of Open Reduction and Internal Fixation of Ankle Fractures.

Background. Unstable ankle fractures are treated with open reduction internal fixation (ORIF) to prevent posttraumatic arthritis. Typically, ORIF is performed as an ambulatory surgery several days to a few weeks after injury. It is unclear what effect this delay may have on functional outcome. This study aimed to assess the effect of timing of ankle ORIF on wound complications and functional outcome. Methods. A retrospective review of 121 patients who underwent ankle ORIF was performed. A total of 58 patients had a follow-up of at least 24 months. Time between injury and surgery greater than 14 days was defined as "delayed." Demographic variables, injury characteristics, length of surgery, and postoperative stay were documented. Comparison of demographic variables, wound complications, and functional outcome determined by Foot and Ankle Outcome Score (FAOS) was performed. Results. 118 patients were included. The duration between injury and surgery was 6 days in the "early" group and 19 days in the "delayed" group. There were no significant differences in demographic variables, injury characteristics, and length of surgery between the groups. Wound complications in the early and delayed groups were 5% and 11.8%, although this difference was not statistically significant. Among 58 patients who had a follow-up of at least 24 months, the median follow-up time was 38 (range, 24-76) months. Each subscale of FAOS demonstrated no significant difference. Conclusion. Ankle ORIF more than 14 days after injury did not significantly increase the rate of wound complication, nor did it impair ultimate functional outcome in this group. Levels of Evidence: Level III.

Primary Ankle Arthrodesis vs ORIF for Severely Comminuted Pilon Fractures

Background:The treatment for highly comminuted pilon fractures remains controversial. The goal of this retrospective cohort study was to compare functional outcomes of primary arthrodesis of the tibiotalar joint (fusion) and open reduction internal fixation (ORIF).Methods:Patients who underwent primary ORIF or fusion for pilon fractures at our institution since 2000 were identified by Current Procedural Terminology ( CPT) code. Inclusion criteria for the ORIF cohort were patients with an AO/Orthopaedic Trauma Association type C3 pilon fracture. Additional inclusion criteria for the fusion cohort were patients whose fractures were deemed non-reconstructable by the treating surgeon. Outcome assessment was determined by the Foot and Ankle Outcome Score (FAOS) and Short Form 36-item health survey (SF-36), time to radiographic union or fusion, and wound-healing complications at a minimum of 2 years after their surgery.Results:Nineteen ORIF and 16 fusion patients completed the study’s outcome assessments. A higher rate of nonunion was observed in patients treated by primary ORIF than primary fusion (5/19 vs 1/16). Posttraumatic arthritis was observed in 11 of 19 primary ORIF patients. Primary fusion patients exhibited increased symptoms, pain, and physical role limits but were equivalent to primary ORIF patients on all other functional metrics examined.Conclusions:Primary ankle arthrodesis achieves a lower rate of nonunion and comparable functional outcomes to ORIF in patients with severely comminuted pilon fractures. The higher rate of nonunion observed in the primary ORIF group suggests that primary fusion should be considered an effective procedure for severe injuries to decrease the need for further operative intervention.Level of Evidence:Therapeutic Level III, retrospective cohort.

Ankle Arthroscopy for Diagnosis of Full-thickness Talar Cartilage Lesions in the Setting of Acute Ankle Fractures

To delineate the prevalence of chondral lesions, in particular full-thickness talar dome lesions, with concurrent arthroscopy in acute ankle fracture open reduction-internal fixation (ORIF) and evaluate the impact on clinical outcomes. We conducted a retrospective chart review of prospectively collected registry data at our institution from 2012 to 2016. Consecutive patients who underwent acute ankle fracture ORIF with concurrent arthroscopy were identified. Charts were reviewed to determine the prevalence and grade of chondral lesions, fracture type, and associated factors. Clinical outcomes with a minimum of 1year of follow-up were assessed using the Foot and Ankle Outcome Score. The study included 116 consecutive patients undergoing acute ankle fracture ORIF with concurrent arthroscopy. A chondral lesion was identified in 78% (90 of 116). A full-thickness talar dome chondral lesion was identified in 43% of these patients (39 of 90). Patient age was a significant predictor, with patients younger than 30years being less likely to have a chondral injury than those aged 30years or older (59% vs 85%, P= .0077). Of the patients who sustained a dislocation at the time of injury, 100% had a chondral lesion (P= .039). Patients with complete syndesmosis disruption and instability were also more likely to have a chondral lesion (96% vs 73%, P= .013). Patients with chondral lesions had statistically significantly worse clinical outcomes than those without them (Foot and Ankle Outcome Score, 81.2 vs 92.1; P= .009). Ankle arthroscopy performed concomitantly with ankle ORIF is a useful tool in diagnosing chondral injuries. Chondral lesions are common with ankle fractures. An ankle with a dislocation at presentation or a syndesmotic injury may be more likely to present with a chondral lesion and should thus prompt evaluation. The presence of a talar chondral injury may be associated with a negative impact on clinical outcomes. Level IV, therapeutic case series.

Rate of and Risk Factors for Intermediate-Term Reoperation After Ankle Fracture Fixation: A Population-Based Cohort Study.

Establish baseline rates of and risk factors for reoperation within 1 or 2 years of ankle open reduction internal fixation (ORIF). Retrospective, population-based cohort study. Two hundred two hospitals in Ontario, Canada (approximate population 13.6 million in 2014). Forty five thousand four hundred forty-four patients who underwent ankle ORIF performed by 710 different surgeons between January 1, 1994, and December 31, 2011. Intermediate-term reoperation because of isolated implant removal, repeat ORIF, irrigation and debridement (I&D) for infection, or amputation. Multivariable logistic regression related potential prognostic factors (patient, provider, and injury) to reoperation. There were 8906 patients who underwent at least one subsequent operation (19.6%). The most common procedure was isolated implant removal (18.1%); odds of removal being higher for females [odds ratio (OR), 1.53; 95% confidence interval (CI), 1.45-1.62; P < 0.001]. N = 674 patients (1.5%) underwent reoperation for another reason. The odds of repeat ORIF and I&D infection were greater for open fractures (OR 2.17; 95% CI, 1.22-3.86; P = 0.008 and OR 3.12; 95% CI, 1.94-5.03; P < 0.001). Odds of amputation was highest for diabetics (OR 7.42; 95% CI, 3.73-14.86; P < 0.001). Isolated implant removal accounts for the vast majority of intermediate-term reoperations after ankle ORIF. Reoperation for other reasons (repeat ORIF, I&D, or amputation) was extremely rare, even among the highest risk patients. Concerns regarding reoperation for these reasons should not preclude operative treatment in any patient, provider, or injury group we considered. Prognostic Level II. Most Readmissions Following Ankle Open Reduction Internal Fixation are Unrelated to Surgical Site Issues

Category: Trauma Introduction/Purpose: Ankle fractures are commonly-sustained injuries, and frequently require open reduction internal fixation (ORIF). It is generally a safe and effective surgical procedure, however, as quality-based reimbursement models become increasingly affected by readmissions within thirty days, it is important to determine causes and risk factors for patients to be readmitted after discharge. Methods: Patients that underwent ORIF for ankle fractures were identified from the prospectively-collected American College of Surgeons National Surgical Quality Improvement Program (NSQIP) database from 2013 to 2014. Baseline demographics, comorbidities, and fracture characteristics (open vs. closed, location of fracture such as lateral malleolus, medial malleolus, bimalleolar, or trimalleolar) were determined. Modified Charlson Comorbidity Index (CCI) was used as a measure of overall comorbidity burden. Rates of thirty-day adverse events and readmissions were determined, as well as the causes for readmission. Multivariable logistic regression analyses were performed to identify risk factors significantly associated with having any adverse events and being readmitted within thirty days of surgery. Results: 5,056 ankle ORIF patients were included. 167 (3.3%) were open fractures. The rate of any postoperative adverse event was 5.2%. There were 127 readmissions, with 116 (91.3%) being unplanned readmissions. Of the 116 unplanned readmissions, 49 (42.2%) were for reasons related to the surgery or surgical site, with the most common causes being deep surgical site/hardware infection (12.9%), superficial site infection (11.2%), and wound disruption (6.9%). Most readmissions were for reasons unrelated to the surgical site (51.7%), including cardiac disorders (8.6%), pulmonary disorders (7.8%), and neurologic/psychiatric disorders (6.9%). With multivariable logistic regression, the strongest risk factors for readmission were history of pulmonary disease (Odds Ratio [OR] 2.29), ASA ≥ 3 (OR 2.28), and open fracture (OR 2.04, all p &lt; 0.05). (Figure 1) Conclusion: Postoperative readmissions following ankle fracture ORIF are important to consider in this era of quality-based hospital reimbursement models. In this cohort of 5,056 ankle ORIF cases, 2.5% of patients were readmitted within thirty days, with 51.7% of all unplanned readmissions due to causes unrelated to the surgery or surgical site. This suggests that close medical follow-up with non-orthopaedic providers may be necessary after discharge. To assist clinicians in preoperative risk stratification, predictors of readmission were history of pulmonary disease, increased ASA class, and open fracture. Higher bundled reimbursements may be justified for cases with these risk factors.

Impact of Diabetes Mellitus on Surgical Quality Measures After Ankle Fracture Surgery

To evaluate the impact of diabetes mellitus (DM) and associated complications on cost, length of stay, and inpatient mortality after open reduction internal fixation (ORIF) of an ankle fracture, and the implications of these variables during a time of health care payment reform. Retrospective study. The Statewide Planning and Research Cooperative System database, which includes all admissions to New York State hospitals from 2000 to 2011. A total of 58,748 patients were identified as having undergone the primary procedure of ORIF of the ankle (ICD-9-CM procedure code 79.36). ORIF of the ankle. Cost, length of stay, and inpatient mortality. Of the 58,748 patients evaluated, 7501 (12.8%) had DM. Mean length of stay and total hospital charges were significantly greater for the DM cohort compared to the without DM cohort (P < 0.01). Patients with DM had greater Charlson Comorbidity Index scores and greater in-hospital mortality than patients without DM (both P < 0.01). Of the patients with diabetes, 1098/7501 had complicated diabetes mellitus (C-DM). Patients with C-DM stayed 2.4 days longer and were $6895 more costly than those with diabetes alone (both P < 0.01). Patients with C-DM also had a significantly higher in-hospital mortality rate than those with diabetes alone. Patients with diabetes admitted to the hospital for ankle ORIF have more expensive hospital stays and higher in-hospital mortality rates than patients without diabetes. The presence of diabetic complications further increases these risks. These data will help provide risk-adjustment for future health care payment reform initiatives. Prognostic Level II.
