A 46-year-old female consulted our hospital with mild anemia and an elevated immunoglobulin G (IgG) level. On detailed hematological examination, anemia of chronic disorders was diagnosed. Liver dysfunction persisted, although markers for hepatitis viruses did not demonstrate any active infection. There were increases in γ-globulin and IgG. However, the patient was negative for antinuclear antibody (ANA), but positive for antismooth muscle antibody. A liver biopsy suggested chronic active hepatitis. The aggregate score was 20 according to the scoring system described by the International Autoimmune Hepatitis Group. This score indicated definite autoimmune hepatitis (AIH). The clinical features of the present case were similar to those of patients with AIH positive for ANA (type 1 AIH). It was observed that transient positivity for ANA was noted for a specific period in this case. Therefore, type 4 AIH appears to be the prodromal stage or an incomplete manifestation of type 1 AIH.
