Depression, Anxiety and Stress Symptoms among Afghan Healthcare Workers: A Cross-Sectional Analytical Study.

Decades of conflict and cycles of disasters in Afghanistan have caused enormous impacts on health, the economy, and even national security. We aimed to assess the levels of depression, anxiety, and stress symptoms and their determinants among Afghan healthcare workers. A cross-sectional study of 830 Afghan healthcare workers working in public and private hospitals was conducted between May and July 2021. We employed a non-probability sampling method to select our subjects. The questionnaire was composed of sections on sociodemographic information, working conditions, and Depression, Anxiety, and Stress Scale-21 (DASS-21). Multivariable linear regression models were fitted using SPSS 21 to identify determinants of mental health symptoms among Afghan healthcare workers at a 5% significance level. Of all participants, 52.3% (435) had symptoms of depression, 48.8% (405) anxiety, and 46.9% (389) stress. The likelihood of mental health symptoms was higher among those who worked in an urban setting (P = 0.001), were physically inactive (P = <0.001), had a decrease in income or an unpaid salary in the past six months (P = <0.001), thinking of leaving Afghanistan (P = <0.001), had medical comorbidity (P = <0.001), and being single (P = 0.048)]. This study highlights the important findings about the psychological health of healthcare workers in Afghanistan. These findings suggest rapid, actionable, and locally relevant interventions to assure potential improvements in working and living conditions for the health staff.

IJCM_397A: A Cross-Sectional Study on Dietary Intake In Relation To Sleep Quality Among Undergraduate Engineering Students

Background: Sleep is an important resting state and essential part of life that could have effect on many aspects of life. Life style factors such as physical activity and consumption of certain foods have effect on quality and quantity of sleep. In this context, there were very few studies done in comparing dietary intake &amp; sleep pattern. So, the present study is planned to be conducted among undergraduate engineering students. Objective: 1. To assess the dietary intake and sleep patterns among undergraduate students. 2. To determine the relationship between dietary intake and sleep quality. Methodology: A Cross-sectional analytical study was done among the undergraduate students of a government engineering college in November 2023. Students (n=200) were randomly selected and data was collected using pre-tested, semi structured questionnaire to assess the dietary intake and Pittsburgh sleep quality index questionnaire (PSQI) to assess sleep quality. Results: The mean age of the study population was 20.2 ± 1.6 years and 54.5% were female. Sleep quality was fairly good among 58%, good in 29%,fairly bad in 11.5%,and very bad in 1.5%. Participants skipping breakfast, lunch and dinner more than three times a week was reported to be 21%,10%, and 16.5% respectively. Conclusion: The quality and quantity of sleep can be influenced by the eating habits. Specific awareness regarding sleep and food intake should be created among the students, as in the long run NCD s can be prevented and to form a healthy nation.

A radiological study on anatomical variations of frontal sinus and its’ drainage pathway in a Nepalese adult population

Background: This study aims to assess anatomical variations in the frontal sinus and its drainage pathway within the Nepalese population. Methodology: This cross-sectional analytical study was conducted at the Department of ENT-HNS and Radiology, Maharajgunj Medical Campus in Nepal. Digital Imaging and Communications in Medicine (DICOM) computed tomography files of patients were analyzed over a year to assess pneumatization variations, frontal sinus size, presence of intersinus septa, olfactory fossa depth, frontal recess cells, and anterior ethmoidal artery position. Imaging files with obscuring disease processes or obtained in different positions, hindering frontal sinus or recess visibility, were excluded. Data analysis employed SPSS version 25, using descriptive statistics (median, range, interquartile range), Chi-squared tests, and Cramer’s V. Results: Among total sinuses analyzed, 7% exhibited unpneumatized frontal sinuses (3.3% bilaterally, 6.7% unilaterally). Median dimensions for right and left frontal sinuses were 10.2 x 11.15 x 11.7 mm and 10.2 x 11.1 x 11.6 mm, respectively. Predominant fin- dings included mixed upper uncinate process attachments, Type II olfactory fossa, agger nasi as the commonest anteriorly based cells, and supra bulla cells as the prevalent posteriorly based cells. Conclusion: Investigation of frontal sinus anatomy in the Nepalese population revealed distinct features. While generally well- developed, these sinuses exhibited relatively smaller dimensions compared to other ethnic groups. Consistent with prior studies, agger nasi and supra bulla cells were the most prevalent anteriorly and posteriorly based cells, respectively.

Analysis of the Association of IL-1A, IL-1B and IL-1RN Polymorphisms with Peri-implantitis in a Portuguese Population.

The aim of this study was to investigate whether genetic variations in cytokine genes involved in the pathogenesis of peri-implantitis, could be associated with its occurrence, an issue that remains controversial and may vary according to the population evaluated. A cross-sectional analytical study was carried out on 102 Portuguese Caucasian individuals divided into two groups: 43 individuals with peri-implantitis and 59 individuals with peri-implant health. Samples from the buccal mucosa were obtained and genetic analysis was performed using the real-time polymerase chain reaction (PCR) technique for IL-1A and IL-1B and using PCR and restriction fragment length polymorphism analysis for IL-1RN. The IL-1A -889 C/T polymorphism showed a higher prevalence of the less common allele (T allele) in cases of peri-implantitis than in healthy cases (27.9% vs 16.9%, respectively), but without statistical significance (p = 0.060). For the IL-1B +3954 C/T and IL-1RN (variable number of tandem repeats) polymorphisms, the analysis revealed that the allele and genotype frequencies did not differ significantly between groups. There was a significant association between a history of periodontitis and peri-implantitis (p = 0.020). The genetic polymorphisms evaluated had no influence on the occurrence of periimplantitis in the population studied. Further research into genetic variations in different populations is needed to elucidate the role of genetic factors in the onset and progression of periimplant disease.

Histomorphologic analysis of ovarian tumors according to the New 2020 WHO classification of female genital tumors.

In terms of female genital tract-related cancers, ovarian tumors account for 3% of all tumors. On the basis of gross, radiological, and clinical features alone, ovarian neoplasms cannot be diagnosed. Therefore, a clear histological diagnosis is necessary before beginning a permanent course of therapy. Cross-sectional analytical study. In this ambispective study, a total of 480 patients with ovarian tumors were included from January 2015 to July 2022 at a tertiary care center in western Maharashtra. Descriptive statistics-percentages, mean, and Chi-square test (to calculate P value) were used to analyze the collected data. To determine clinical presentation, age distribution and incidence of various morphological and histological variants of ovarian tumors. Out of 480, 250 cases (52%) in 41-50 years of age, followed by 154 cases (32%) in 21-30 years of age. Our study showed 301 cases of benign, 42 cases of borderline, and 137 cases of malignant neoplasms of ovary out of 480 cases studied. Out of 480 cases, 244 cases (50.83%) were cystic, 138 cases (28.75%) were solid, and 98 cases (20.42%) were mixed (cystic/solid). Out of 480 cases, 326 cases (67.91%) were surface epithelial ovarian neoplasms. In this research, most frequent ovarian neoplasms were serous tumors in 216 cases (45%) followed by mucinous tumors in 78 cases (16.25%). In 96% cases, clinical diagnosis matched with histopathological diagnosis. In our research, benign ovarian neoplasms were most frequent. Serous tumors were the most frequent type of surface epithelial neoplasms followed by mucinous tumors. Peak incidence was seen in fifth decade. Higher risk of malignancy was seen in nulliparity or low parity and early menarche not associated with risk of malignancy. Cystic morphology more common in benign neoplasms and complex or solid morphology showed greatest increase in incidence of malignancy. Latest WHO classification has important impact on prognosis and therapy of the patient.

Assessment of Vitamin D status of patients with human immunodeficiency virus

ABSTRACT Background: Vitamin D regulates calcium and phosphorus homeostasis. Vitamin D deficiency (VDD) commonly occurs in people living with human immunodeficiency virus (HIV) (PLWHIV) and is linked to bone disorders and metabolic and infectious diseases. HIV seropositivity and VDD may worsen the consequences on the individual. Methods: A cross-sectional analytical study was conducted using 90 serum samples from 42 HIV-positive and 48 HIV-negative patients attending the Nnamdi Azikiwe University Teaching Hospital (NAUTH). This study was approved by the NAUTH Ethics Committee. Serum Vitamin D levels were analyzed using enzyme-linked immunosorbent assay. Sociodemographic data were obtained using a questionnaire. Data analysis was performed using IBM SPSS Windows version 27, and P &lt; 0.05 was considered statistically significant. Results: Overall, 13.3% of the population had suboptimal Vitamin D levels, with 2.2% having VDD and 11.1% having Vitamin D insufficiency. VDD was found in 2.2% and 2.1% of the HIV-positive group and HIV-negative groups, respectively. No significant difference was observed in mean serum Vitamin D levels between the two groups (P &gt; 0.05). Within the HIV-positive group, education, place of living, and skin coloration were associated with suboptimal Vitamin D levels, and only skin coloration was associated with HIV-negative subjects. Conclusion: VDD is relatively low in the region; however, it may contribute to poor bone health in PLWHIV. Interventions should target individuals with poor educational attainment, living in rural domains, or dark skin coloration. Studies with large sample size are needed to determine the effects of Vitamin D supplementation or exposure to sunlight on HIV infection.

IJCM_409A: Prevalence And Pattern of Self-Medication With Antibiotics: A Community- Based Cross-Sectional Study From South-East Delhi

Background: Antibiotics are commonly self-medicated drugs worldwide, with over 50% purchased and used without a prescription. Antibiotic resistance has emerged as a global public health concern. Although many studies have been conducted worldwide to study the prevalence and pattern of self-medication with antibiotics and associated socio-demographic factors (SMA), very few have been conducted in India, and no such studies have been conducted in Delhi in community setup, especially in post-pandemic setup. Objective: 1. To estimate the prevalence and pattern of self-medication with antibiotics among the adults residing in the field practice areas of a medical college in South-East Delhi. 2. To determine the socio-demographic factors and their association with self-medication with antibiotics among the study subjects. Methodology: A cross-sectional analytical study was conducted in South-East Delhi. The sample size was calculated as 200 considering an absolute error of 5% at a 95% level of significance. Simple random sampling was done to select households by using a Random number generator. The total sample size of 200 was distributed proportionately between the UHTC and RHTC, according to Probability Proportional to Size (PPS). One individual from each selected household of age = 18 years who has used antibiotics in the past 3 months and met the eligibility criteria was included in the study. A self-designed, pre-tested, validated, structured questionnaire was used to collect the data. Results: 71(35%) out of 200 of the participants practiced SMA. Nearly 42% of them lived in urban areas. 30% had an education level of high school and above. The Chi-square test revealed that the participant’s level of education was significantly associated with SMA. The frequency of SMA ranges from one to a maximum of five times in the past 3 months. 69% of the respondents administered antibiotics twice in the past 3 months. The indications for SMA were fever (34%), followed by cold and flu (23%) and UTI (14%). Most of the participants who practiced SMA obtained antibiotics from the pharmacy (75%) followed by leftovers from previous treatment (17%). Conclusion: The study revealed that more than one-third of the respondents practiced SMA. The findings from the present study indicate the necessity of activating antimicrobial stewardship programs throughout the country, as well as implementing effective legislation to prohibit dispensing antibiotics without prescription.

Comparison of Conventional and Digital Impression Methods for Measuring Mesiodistal Teeth Dimensions: An Analytical Cross-sectional Study

Introduction: We aim to determine and compare the correlation between conventional and digital impression methods for measuring mesiodistal teeth dimensions. Methods: This cross-sectional analytical study was conducted on a total of 120 dental arch samples with less than 5 mm of tooth crowding, complete teeth on the dental arch, no missing teeth, and no fillings on the mesial or distal sides at Can Tho University of Medicine and Pharmacy. Conventional (extra-fast alginate) and digital impressions using a 3D intraoral scanner (CEREC Primescan) were taken from all participants, and the dimensions of the mesiodistal teeth were measured. Using R software, Pearson’s correlation coefficients were used to analyze the correlation between conventional and digital impression methods. Result: The pooled correlation for the maxilla was 0.8062 [95% confidence interval (CI): 0.7751–0.8334] (very strong positive correlation); for the mandibular, it was 0.7645 (95% CI: 0.7165–0.8054) (strong positive correlation), and for both jaws was 0.7863 (95% CI: 0.7581–0.8115) (strong positive correlation). Conclusion: In the Vietnamese population measurement of mesiodistal tooth width using a digital dental scanner can be used instead of conventional plaster models.

IJCM_79A: A cross sectional study to compare the emotional maturity between middle school children of working and non-working mothers.

Background: A child’s overall development is based on physical and emotional well-being. Parenting skills plays a vital role in child’s emotional well-being and can be influenced by many factors and one of the factors is mother’s working status. Objectives: 1. To assess the emotional maturity level of middle school children. 2. To compare the emotional maturity levels between middle school children having working mothers and middle school children having non- working mothers. Methodology: An analytical cross-sectional study among middle school children between the age of 13 and 16 years was conducted in a school. A questionnaire-based interview was conducted using a standard semi structured Emotional maturity scale questionnaire. Results: The total students interviewed were 150 and were equally divided based on working status of mothers. Among them, 54.7% were boys and 45.3% were girls. 1.3% of the children of working mothers exhibited very low emotional maturity, but none of the children of non-working mothers did. Of the children of working mothers, 12% demonstrated low emotional maturity, compared to 8% of the children of non-working mothers. 78.7% of children whose mothers were working had an average level of emotional maturity, compared to 85.3% of children whose non-working mothers. 8% of the children of working mothers exhibited a high level of emotional maturity, compared to 6.7% of the children of non-working mothers. There was no significant difference between emotional maturity of children of working and non-working mothers (P=0.595). Conclusion: The study showed nil significant difference in emotional maturity score between children of working and non-working mothers. However, very low emotional maturity level was seen in Children of working mother which needs further importance. So, similar Future Studies with higher sample size will be needed to address this statistically and the concept of emotional maturity needs to be looked into with more preference to emotional maturity factors.

IJCM_313A: Assessment of the health-related quality of life among ex-mine workers of Kolar gold fields, Kolar, Karnataka

Background: Karnataka ranks third with regards the number of mines in India and Kolar, at one time, had the world’s deepest and most productive gold mines. Miners are exposed to hazardous agents like silica, coal and asbestos which adversely impact health. Objectives: To assess the health-related quality of life (HRQL) of ex-mine workers at Kolar Gold Fields (KGF), Kolar, Karnataka. Methodology: An analytical cross-sectional study was conducted among 240 (estimated 196 with 50% prevalence) male ex-mine workers of KGF. An interview scheduled, consisting of socio-demographic factors, COPD Assessment Test (CAT) and Kings Brief Interstitial Lung Disease (KBILD) was administered. Lower CAT score and higher KBILD score indicate better HRQOL. Results: The mean age of the participants was 69.71±8.56 years. Participants were previously involved in drilling (113-47%) and blasting (127-53%) activities in the mines, with a mean of 14.68±6.46 years of work exposure. The mean scores for CAT and KBILD were 15.80±7.42 and 52.83±8.77 respectively. Based on CAT scores, half (51%) of participants reported medium impact of COPD on their health. Participants with longer duration of work experience had significantly higher CAT scores [OR-36.64 (12.14-110.6)]. KBILD score was significantly higher among those who worked in blasting (M=113. SD=52.14) than those involved in drilling (M=127, SD=53.43). Lower duration of work was significantly associated with higher KBILD scores (p&lt;0.01). Adjusted logistic regression showed higher education and income had a positive impact on HRQL and an increase in age and longer work experience adversely affected HRQL. Conclusion: This study reveals the hazardous impact of drilling and blasting work on the health-related quality of life of ex-mine workers. Understanding this, for the future, it is important to educate workers, improve financial stability and minimize hazard exposure to enhance the quality of life of miners amidst the challenges of mining operations.

The Status of Dermoscopy in Chile: First National Study in Dermatologists.

Scientific evidence supports dermoscopy as an essential tool in dermatological diagnosis. The objective is to know the factors that influence its use in Chilean dermatologists. Analytical cross-sectional study. An adapted version of the survey was submitted from the pan-European study by Forsea et al to members of the Chilean Society of Dermatology, between September and December 2020. Analysis using descriptive statistics and multivariate analysis with ordinal logistic regression looking for factors associated with greater use of. One hundred and ninety-eight responses, mean age 46.3 years and 14.6 years on average practicing as dermatologists. 61.6% trained in dermoscopy during their residency. 98% use a dermatoscope. More than 80% consider dermoscopy useful for the diagnosis of melanomas, follow-up of melanocytic lesions, and diagnosis of pigmented and non-pigmented tumors. Between 50% and 70% consider it useful for monitoring non-melanocytic lesions, nail and hair pathologies. Greater confidence when evaluating pigmented and non-pigmented tumors and capillary pathology. Adjusting for age, sex, confidence, and education, participation in teaching was associated with greater use of dermoscopy in non-pigmented and pigmented tumors, and capillary pathology. Percentage of participation in the survey and training in dermoscopy higher than in the reference study, recognizing the usefulness of dermoscopy for the diagnosis and follow-up of tumor pathologies. Participating in teaching is a strong independent factor that is associated with a greater use of dermoscopy in Chile. Dermoscopy is positioned as a tool widely used by Chilean dermatologists in their daily practice.

Comparison of traditional and mobile photogrammetric method for the Tanaka Johnston mixed dentition analysis: an analytical cross-sectional study

Aim Comparison of Tanaka Johnston mixed dentition analysis using the mobile photogrammetric method and traditional method. Patients and methods Thirty-five healthy children between the ages of 6 and 10 were signed up for the study. Casts were considered using the conventional Tanaka Johnston analysis method. A digital caliper was utilized to measure the mesiodistal dimension of every tooth from the incisor to the first permanent molar on both sides. Occlusal photos were taken intraorally using a smartphone and an intraoral occlusal mirror. The principal investigator took the intraoral pictures. An assistant retracted the lips and placed the intraoral mirror inside the mouth. The acquired images were exported to the digimizer software, where they were processed and analyzed. To enable digital software to measure dental arch images, an occlusal ruler was kept at the occlusal level. A total of 20 tooth measurements in the maxillary and mandibular arches were taken for the Tanaka Johnston mixed dentition analysis. Results Thirty-five children were assessed. Intergroup comparison of different parameters between two groups was performed. This comparison revealed no statistically significant differences (P value &gt; 0.05) between the traditional and photogrammetric methods for all parameters, indicating that both methods produce similar outcomes. Conclusion The photogrammetric method is a reliable and valid method to measure individual tooth width. The photogrammetric method is alternative to traditional method for Tanaka Johnston mixed dentition analysis.

Assessment of Cytological Changes of Oral Mucosa in the Patients with Gastroesophageal Reflux Disease: An Analytical Cross-Sectional Study

Introduction: This study aims to investigate cytological changes of oral mucosa in patients with gastroesophageal reflux disease (GERD). Methods: In this cross-sectional study, 25 patients with GERD and 25 healthy persons underwent buccal mucosa brush biopsy for cytological assessment. The observed cellular changes were categorized(14) as Class I: normal; Class II: atypical; Class III: intermediate; Class IV:suggestive of cancer; Class V: positive for cancer. Results: All 50 participants (mean age 39.9± 11.8, 24 male) completed the study and were included in statistical analysis. Ordinal logistic regression (overall model test p&lt;0.001) showed GERD was a significant predicator for cytological changes of the oral mucosa (p&lt;0.001). Age and gender were not significant predicators (p˃0.05). Conclusion: GERD is associated with cellular alterations in the buccal mucosa, which can be detected by cytology. The most common outcomes were nuclear hyperchromatism and grade II cellular changes.

Background: Plastics, owing to their ease of use and affordability, have taken over every aspect of our modern world. It has become an essential aspect of our everyday lives, permeating every sector from food packaging to toys and electronics, to the degree that it is unimaginable to picture a world without it. Plastic takes around 15 to 1000 years to biodegrade. The consumption of plastic by individuals and the ineffective reuse or recycling of it has detrimental effects on the ecosysystem. Overusing plastic and neglecting disposal solutions for plastic over a long time led not only India but many other nations to adopt laws to limit plastic because of its increasing environmental damage. Objectives: 1. To determine the utilization and disposal of plastic waste among general population in urban slums. 2. To study association between sociodemographic factors and plastic usage and disposal. Methodology: Study Design: A cross sectional analytical study Study Population: General population residing in urban slum Sampling Technique: Simple random sampling Sample Size:216 (p= 50%, l= 7, NR=10%) Study Tools: Self-designed, semi structured, pilot-tested questionnaire will be used for the data collection which will be entered in Microsoft Excel and analysed using SPSS statistical software version 20. Results: Usage of plastic was seen in all the study participants and more than 96% of them were reusing the plastics more than once. Most common method was disposing plastic in municipal dust bins (60.2%) followed by open dumping (20.9%), whereas 16.4% were reusing them for other purposes. Level of education and religion has no association with proper disposal of plastic waste. Present study observed that there was a significantly high percentage (96.1%) of three generation families were following proper plastic waste disposal methods followed by nuclear and Joint families with 56.9% and 48.8% respectively. with P≤0.05. Conclusion: Most common method was disposing plastic in municipal dust bins followed by open dumping. Level of education and religion has no association with proper disposal of plastic waste.

Social support in recently diagnosed diabetic patients: Risk factor for depression?

Background: social support is important for adaptation in chronic diseases, such as diabetes and depression, because it favors recovery and adherence to treatment. Introducing its evaluation in the follow-up of diabetic patients can reduce complications derived from secondary non-adherence. Aims: to establish social support in diabetic patients and its correlation with depressive symptoms. Methods: a cross-sectional analytical study nested in a cohort of 173 recently diagnosed diabetic patients (<6 months) in Colombia over 18 years of age, treated in a cardiovascular risk program in 2022. The Chronic Illness Social Support Inventory was used. Results: Most of the participants were women (77.5%); single(83.8%), age (mean = 62.6 years (SD 12.3)); glycemia (mean = 146.4 (SD 65.5)), glycosylated hemoglobin (mean = 7.6 (SD 1.7)). Cronbach's α coefficient for the general scale of the social support instrument was 0.9859. The mean social support was 168.5 (SD 37.4), range 38-228. The total social support score was normally distributed (Shapiro Wilk p > 0.05). The correlation between domains was statistically significant. The PHQ9 total score was significantly associated with the domains of Personal Interaction and Guide but did not significantly correlate with the overall social support score. The respondents who were at risk of developing depression were referred for treatment. Conclusions: findings suggest that perceived social support may play a significant role in the prevention and treatment of depression in diabetic patients. It is desirable that health professionals consider evaluating and enhancing social support to improve their mental health. More research is needed to gain a comprehensive understanding of this relationship.

Prevalence of Paternal Prenatal Depression and Its Associated Factors in Saudi Arabia.

Paternal prenatal depression affects not only the fathers but also their spouses and children's future lives. Many socioeconomic and cultural factors affect the probability of paternal depression. Little is known about the prevalence of and factors associated with paternal prenatal depression in the Middle East. To estimate the prevalence of paternal prenatal depression among fathers visiting a tertiary university hospital, as well as to determine the factors associated with paternal prenatal depression within study participants. This analytical cross-sectional study included 442 fathers whose wives were pregnant and were undergoing regular assessments at antenatal clinics in a tertiary university hospital in Riyadh, Saudi Arabia. The Edinburgh Postnatal Depression Scale (EPDS) was used to assess paternal depression. Bivariate and multivariate analyses were performed. The prevalence of paternal prenatal depression was 26.9%. It was correlated with smoking (odds ratio (OR)=1.8, p=0.006), maternal depression (OR=4.59, p<0.001), and experiencing isolation (OR=5.34, p<0.001). The odds of paternal prenatal depression decreased with social support from friends and family (OR=0.227 and 0.133, respectively) and p<0.001. Paternal prenatal depression was prevalent within the study participants. Notably, experiences of isolation and maternal depression emerged as prominent factors that were significantly associated with the manifestation of paternal depression. Consequently, it becomes imperative to implement systematic depression screenings for expectant fathers and to meticulously consider the array of the factors associated with paternal depression.

IJCM_35A: Prevalence and risk factors for domestic violence against infertile women in central India

Background: Infertility is a public health issue affecting a large part of the world’s population. Violence against women, Spousal or intimate partner violence is the most common form of domestic violence; a preventable cause of morbidity and mortality in women. Especially sexual violence, is one of the most pressing public health concerns affecting women today. Objective: To estimate the prevalence of spousal violence, various types of spousal violence and factors associated with it among reproductive age group women. Methodology: This was an analytical cross-sectional study conducted in Urban Health Training Centre area of a tertiary care teaching hospital in Nagpur, India among married women between 15-49 years of age. The reproductive age group women were 3985 according to health survey done by UHTC. Out of which the data was collected in a sample of 82 infertile women by face-to-face interview and analysed using SPSS v22. Predesign pretested questioner use for data collection. The ethical consideration was taken from the institute for the study. Results: The prevalence of spousal violence among reproductive age group (15-49 years) women was found to be 79.7%; emotional violence (64.1%) being the most common, followed by physical (71.2%) and sexual (53.6%) violence in that order Conclusion: Infertility and associated domestic violence is a significant public health issue, it would be necessary to pay attention to infertile women to ensure the quality of their of reproductive health.

Determinants and predictive model of failure of surgical repair of obstetric vesico-vaginal fistula in the Democratic Republic of the Congo

IntroductionSurgical repair of obstetric fistula aims to restore the anatomical and functional integrity of the urinary tract, enabling affected women to regain their dignity and quality of life. However, such repairs can end in a failure. The aim of this study is to develop a predictive score to identify factors influencing failure of surgical repair of obstetric vesico-vaginal fistula (FSROVVF) in the Democratic Republic of the Congo.MethodsThis was an analytical cross-sectional study of 318 women with obstetric vesico-vaginal fistula (OVVF) who had undergone surgical management. A bivariate and then a multivariate analysis were performed. Score discrimination was assessed using the ROC curve and C-index, and score calibration using the Hosmer–Lemeshow test.ResultsSurgical repair of OVVF was unsuccessful in 16.98% of cases (54/318). After logistic modeling, six criteria emerged as predictive factors for FSROVVF: the presence of fibrosis (AOR = 5.01; 95% CI:1.73–14.49), the presence of 2 or more fistulas (AOR = 9.04; 95% CI:3.01–27.13), the association of OVVF with another anatomoclinical entity of fistula (AOR = 3.16; 95% CI:1.09–9.13), the fistula size > 3 cm (AOR = 3.65; 95% CI:1.36–9.76), the peri-operative hemorrhage (AOR = 7.01; 95% CI:2.33–21.03), and the post-operative infection (AOR = 178.89; 95% CI:26.09–1226.64). A score ranging from 0 to 13 points was obtained, of which a value ≤ 5 points defines a low risk of FSROVVF, a value between 6 and 8 points defines a moderate risk and value ≥ 9 points corresponds to a high risk of FSROVVF. The area under the ROC curve of the score is 0.925 with a sensitivity of 61.11%, a specificity of 96.59%, a positive predictive value of 78.57% and a negative predictive value of 92.39%.ConclusionThis study demonstrated that the number of fistulas ≥ 2, fistula size > 3 cm, fibrosis, association of OVVF with other types of fistulas, peri-operative hemorrhage, and post-operative infection are factors predictive of FSROVVF. These six factors are key contributors to the score used to predict FSROVVF. Once validated, this score will inform and enable preoperative counseling regarding the prognosis and the chances of a successful outcome of surgical repair of OVVF.
