Heated tobacco product use and abnormal glucose metabolism: a working population-based study.

We aimed to assess the cross-sectional association of heated tobacco product (HTP) use with prediabetes and diabetes. The present analysis included 8950 workers from 5 companies (Study I) and 31,341 workers from another large company (Study II), who participated in the Japan Epidemiology Collaboration on Occupational Health Study. The participants were divided into five groups: never smokers, past smokers, exclusive HTP users, dual users of cigarettes and HTPs, and exclusive cigarette smokers. Diabetes and prediabetes were defined according to the fasting blood glucose and HbA1c levels and self-reported diabetes treatment, using the American Diabetes Association criteria. We analyzed the data of Study I and II separately, and then pooled these estimates using the fixed-effect models, with adjustment for a wide range of covariates. In this study that included 40,291 participants (mean age, 46.6years; men, 84.3%), about half of the current tobacco-related product users reported using HTPs. Exclusive HTP users had higher odds of prediabetes (pooled odds ratio 1.36; 95% CI 1.25-1.47) and diabetes (1.68; 95% CI 1.45-1.94) than never smokers. Similarly, dual users also had increased odds of prediabetes (pooled odds ratio, 1.26; 95% CI 1.13-1.39) and diabetes (1.93; 95% CI 1.63-2.29). The strength of these associations was comparable to that of cigarette smokers. We observed significantly higher HbA1c and fasting blood glucose levels among both exclusive HTP users and dual users compared to never smokers. HTP use was associated with an increased likelihood of prediabetes and diabetes. Prospective studies are warranted to confirm the cross-sectional association.

Evaluation of diagnostic parameters in detecting prediabetes in a cohort of Syrian healthcare providers and staff: correlations with risk factors

Diabetes is spreading more rapidly in lower-middle-income countries like Syria. Early prevention programs are crucial and achievable through identification, treatment, and revision of prediabetes.Evaluation of hyperglycemia diagnostic parameters (FPG, OGTT, HbA1c) in detecting prediabetes was executed through three phases of screening asymptomatic adults and applying the parameters sequentially. Relationships with risk factors from the American Diabetes Association (ADA) were assessed. Correlations amongst lipid profile (total cholesterol, LDL, HDL, and TG) and hyperglycemic parameters were additionally explored.Participants (212) were mainly males (60.4%), married (61.5%), healthcare providers (28.8%), and had first-degree relatives with diabetes (32%). 10.6% had hypertension, 6.8% had dyslipidemia, and 10.7% of the female participants had PCO. Following the ADA criteria, 18% and 1.9% of the participants were diagnosed with prediabetes and diabetes, respectively. The cohort of participants with prediabetes and diabetes had higher percentage of first-degree relative with diabetes (70%), obesity (55%), hypertension (25%), dyslipidemia (15%), and PCO in females (20%). They were mainly professors (40%) and healthcare providers (25%). Interrelations amongst the hyperglycemic parameters were revealed, also with lipids and risk factors mainly age, BMI, familial diabetes, dyslipidemia, and hypertension.Reports on prediabetes are scarce in Syria. This study, one of a few on the topic, investigated the hyperglycemic parameters in detecting prediabetes and revealed the prevalence and the correlation with risk factors and lipids. It adds substantial information to our understanding of the intertwined associations of the studied variables.

Incidence of type 1 diabetes in the children’s hospital dr. Robert reid cabral, 2010-2019. Dominican republic

Background The Children HospitalDr.Robert Reid Cabral, the principal children hospital in the Dominican Republic has no recent data on type 1 diabetes (T1D) incidence in children, therefore a study was undertaken to determine this in people aged <15 years (y). Methods Data were collected on all new T1D diagnoses between 2010-2019 from the The Children Hospital Dr. Robert Reid Cabral that care for children with T1D. Diagnosis was made according to standard American Diabetes Association criteria. No secondary ascertainment source was available. Significances around sex, age and incidence were assessed using the Chi-square (X2) and Pairwise Pearson Correlations tests. Results There were 513 new cases of T1D diagnosed in children aged < 15 y; a mean of 51.3 per year. Mean ± standard deviation age of T1D diagnosis was 8.5 ± 3.8 y, and there was also a not significant female preponderance ( p< 0.35). New cases were consistently highest in the 10-14 y (48.5%), and lowest in the 0-4 y age group (18.6%). Overall, mean crude annual incidence was 1.73 per 100,000 population, with no significant trend of increase or decrease.

Prediabetes is more than a pre-disease: additional evidences supporting the importance of its early diagnosis and appropriate treatment.

To identify Prediabetes (PreD) as early and serious diabetes step using clinical-biochemical characteristics in the population of the Primary Prevention Diabetes Buenos Aires (PPDBA) study. PPDBA Study evaluated benefits of adopting healthy lifestyles to prevent T2D. It recruited people 45-75 years of age with PreD (impaired fasting glycaemia [IFG], impaired glucose tolerance [IGT] or both, American Diabetes Association criteria), using an opportunistic approach. They completed a FINDRISC questionnaire, and those with a score ≥13 points were invited to participate. When they accepted, we performed an oral glucose tolerance test (OGTT) with a complete lipid profile and HbA1c while physicians completed a clinical history. We recruited 367 persons, and depending on OGTT results, the sample was divided into normals (NGT), PreD, or with diabetes (last one was excluded in our analysis). Data were statistically analyzed using parametric and nonparametric tests and logistic regression to identify parameters associated with PreD. From the recruited (n = 367) 47.7% have NGT, 48.5% PreD and 3.8% unknown T2D (excluded). People with PreD were significantly older, with a higher percentage of overweight/obesity, BMI, and larger waist circumference than NGT. They also showed significantly higher fasting and 2 h post glucose load, HbA1c, and triglyceride levels. No significant differences were recorded in the blood pressure, lipid profile though both groups had abnormally high LDL-c values. They also had a larger percentage of TG/HDL-c ratios (insulin resistance indicator) (55% vs. 37.5%). Logistic regression analysis showed that PreD was significant associated with age, waist circumference, and triglyceride above target values. Our findings showed that clinical and biochemical parameters were significantly different between people with PreD and those with NGT. This evidence supports the concept that PreD is a serious dysfunction, which should be early diagnosed and treated properly to prevent its transition to T2D and its complications.

ODP383 Clinical Presentation and Management of Diabetes Mellitus in Youth with Prader-Willi Syndrome

Abstract Background Diabetes mellitus (DM) is reported in 7–24% of adults with PWS. It is thought that abnormal glucose metabolism develops as a consequence of severe obesity and/or insulin resistance. Given obesity and insulin resistance often develop in childhood in children with PWS, DM occurs in youth as well. There are limited data detailing the clinical presentation of DM, diabetes complications, and therapies used in pediatric patients with PWS. Methods A retrospective chart review of patients less than 21 years of age with genetically confirmed PWS seen July 2014 through July 2021 was performed. Patients were included if they also had a diagnosis of DM based on a single value meeting American Diabetes Association criteria: fasting blood glucose of ≥ 126 mg/dL, HbA1c ≥ 6.5%, 2-hour blood glucose of ≥ 200 mg/dL on oral glucose tolerance test, or random blood glucose of ≥ 200 mg/dL with symptoms of hyperglycemia. Results Of the 88 patients identified with PWS, nine (10%) were found to have DM. Means and range sat diabetes diagnosis were age 11.4 years (7–16), HbA1c8.2% (6.6– 12.4), BMI 38.7 kg/m2 (23.1–73.6),and BMI z-score2.4 (1.3–3.1). Five patients were female. Six patients were non-Hispanic white, two non-Hispanic black, and one Hispanic. Of these nine patients with DM, six had at least three diabetes autoantibodies assessed. All antibodies were negative in three patients, two patients had a weakly positive islet cell antibody only, and one patient had all four autoantibodies positive. Most patients were identified incidentally on screening laboratory evaluation in clinic, two patients had polyuria, and one patient was admitted with vomiting and mixed diabetic ketoacidosis/hyperosmolar hyperglycemic syndrome (DKA/HHS). Diabetes treatment methods included one patient on metformin only; four on metformin and insulin; three on metformin, insulin, and GLP1 receptor agonist; and one patient on insulin only. Of the six patients on insulin, only two were using short acting insulin with meals. No patients were treated using insulin pumps or continuous glucose monitors. No patients underwent bariatric surgery. Only one patient had hypertension requiring treatment with medication. No patients had nephropathy, dyslipidemia, neuropathy, or retinopathy. Conclusions DM is seen in children with PWS and often diagnosed incidentally. Regular screening is important as development of DKA/HHS is possible if diabetes goes unrecognized. While type 2 diabetes is more common than type 1 diabetes, diabetes antibodies should be measured. Standards of care for treatment are lacking resulting in variability in therapeutic regimens. Screening for diabetes in the adult population above the age of 20years among inhabitants of rural agricultural and rural desert areas of Egypt.

The present study focused on identifying the current prevalence of diabetes mellitus (DM) in rural desert and rural agricultural areas of Egypt and comparing these findings to those of previous studies that reported lower prevalence rates of DM in rural desert versus rural agricultural areas. The study included a total of 15,000 participants aged 20years or older; 10,000 were from rural agricultural areas and 5000 were from rural desert areas in Egypt. The prevalence of DM and impaired fasting glucose for each group was recorded, participants were interviewed, medical history was obtained, physical examinations were performed, and fasting plasma glucose was used for diagnosis of DM and IFG using American Diabetes Association criteria. The prevalence of DM was 12.7% in agricultural areas and 4.7% in desert areas. The prevalence of newly diagnosed cases was 15.8% and 9.9% in agricultural and desert areas, respectively. The prevalence of IFG was 11.14% in agricultural and 8.04% in desert areas. These results suggest that living in a rural area makes patients at a higher risk of developing DM (OR = 2.968 CI (2.570-3.428) p < 0.001) and IFG (OR = 1.43, CI (1.272-1.616), p < 0.001). Logistic regression analysis revealed that increased age, living in agricultural areas, higher body mass index and positive family history of diabetes were the significant predictors affecting the prevalence of DM. The prevalence of DM, IFG, and overall dysglycemia (DM + IFG) in Egypt has generally increased in rural areas, with a lower prevalence in rural desert compared to rural agricultural areas.

Type 2 diabetes remission after Roux-en-Y gastric bypass (RYGB), sleeve gastrectomy (SG), and one anastomosis gastric bypass (OAGB): results of the longitudinal assessment of bariatric surgery study

BackgroundSeveral studies on various bariatric surgeries involving patients with type 2 diabetes mellitus (T2DM) showed an overall rate of remission of hyperglycemia. However, there is little known about predictive factors on remission after different types of surgeries. The aim of this study was to identify the T2DM remission rate and to determine the effects of preoperative factors characteristics of remission of type 2 diabetes in Iran.MethodsWe conducted a retrospective analysis of 1351 patients with T2DM operated by three different types of surgeries (Roux-en-Y gastric bypass (RYGB), sleeve gastrectomy (SG), and One Anastomosis Gastric Bypass (OAGB)). Diabetes remission was defined according to the American Diabetes Association (ADA) criteria. Binary logistic regression analyses were employed.ResultsA total of 1351 patients, 675 patients (50.0%) undergoing OAGB, 475 (35.2%) RYGB, and 201 (14.9%) SG. 80.6%, 84.2% of OAGB, 81.7%, 82.6% of RYGB, and 77.1%, 81.5% of SG participants were in T2DM remission after 1 and 3 years, respectively. 1- and 3-year remission were associated with preoperative age, duration of T2DM, FBS and HbA1c, BMI, insulin therapy, and a family history of obesity (p < 0.05).ConclusionThe remission of T2DM after RYGB, SG, and OAGB surgery is dependent on various preoperative factors. Patients with younger age, shorter duration of T2DM, lower preoperative HbA1c and FBS, higher BMI, who were not on insulin therapy, and not having a family history of obesity were the best candidates to achieve a prolonged diabetes remission.

Association of serum leptin and insulin levels among type 2 diabetes mellitus patients: A case-control study.

Secretion of insulin is compromised in type 2 diabetes (T2DM) individuals and inadequate to accommodate for insulin resistance (IR) in peripheral tissue. Hyperleptinemia reflects leptin resistance, which is a key factor in the production of IR in T2DM patients, making leptin a potential biomarker for evaluating IR levels. The objective of the study was to assess the association of serum leptin and insulin levels among T2DM patients. This case-control research was carried out on T2DM patients. A total of 73 patients diagnosed with T2DM (the case group) and 40 healthy participants (control; group 3) were enrolled according to the American Diabetes Association (ADA) criteria. In the case group, T2DM patients were enrolled with metabolic syndrome (group 1, n = 38) or without metabolic syndrome (group 2, n = 35) according to the WHO criteria. Metabolic profiles of T2DM patients with or without metabolic syndrome were evaluated, and compare these two groups with healthy controls. The subjects of all groups were age- and gender-matched. Body mass index (BMI, P < .01), fasting (P = .0133) and postprandial (P < .01) blood sugar levels, % glycated hemoglobin (HbA1c, P < .01), and lipid profile (P < .01) were found significantly different and higher in group 1 as compared to groups 2 and 3. Serum leptin and insulin levels were found higher and significant in patients with metabolic syndrome (P < .01 for both). The values of serum leptin levels were 10.01 ± 2.7 ng/mL, 6.9 ± 2.4 ng/mL, and 4.11 ± 1.8 ng/mL, and those of serum insulin 120 ± 40.7 µIU/mL, 20.43 ± 5.2 µIU/mL, and 11.4 ± 2.5 µIU/mL in groups 1, 2, and 3, respectively. There was a positive linear correlation between BMI, blood sugar, HbA1c, serum cholesterol (TC), and triglycerides (TG) with serum insulin and leptin levels in the case group. An extremely significant correlation (R = 0.74, P < .001) was found in BMI and serum leptin level in the case group. Serum leptin and insulin levels have a positive association, with serum leptin being a significant predictor of IR syndrome (Evidence Level: 5; Technical Efficacy: Stage 3).

A Posteriori dietary patterns, insulin resistance, and diabetes risk by Hispanic/Latino heritage in the HCHS/SOL cohort

ObjectiveWe examined links among dietary patterns (DPs), insulin resistance (IR), and diabetes risk by heritage in the Hispanic Community Health Study/Study of Latinos.MethodsHispanics/Latinos of Cuban, Dominican, Mexican, Puerto Rican, Central American, and South American heritage aged 18–74 years and diabetes-free completed two 24 h dietary recalls at baseline (2008–2011) and provided 6-year follow-up data (2014–2017; n = 7774). We classified 6-year IR status [improved, unchanged (referent), worsened] using a 1-SD change in fasting insulin between visits and defined incident diabetes based on American Diabetes Association criteria. We derived heritage-specific DPs via principal factor analysis and estimated their associations with 6-year IR status (multinomial) and incident diabetes (binary) using complex survey-based logistic regression.ResultsFive overarching DPs based on high-loading foods were shared by two or more heritage groups: “Burger, Fries, & Soft Drinks”; “White Rice, Beans, & Red Meats”; “Fish & Whole Grains”; “Cheese & Sweets”; and “Stew & Corn”. Comparing highest-to-lowest DP quintiles, the Dominican “Burger, Fries, & Soft Drinks” and Cuban “White Rice, Beans, & Red Meats” DPs were associated with worsened 6-year IR status (log-odds: 2.35, 95% CI: 1.02, 3.68, Ptrend = 0.037 and log-odds: 1.27, 95% CI: 0.49, 2.06, Ptrend = 0.009, respectively). The Puerto Rican “Burger, Fries, & Soft Drinks” and the Central American “White Rice, Beans, & Red Meats” DPs were associated with greater diabetes incidence (OR: 3.00, 95% CI:1.50, 5.99 and OR: 2.41, 95% CI: 1.05, 5.50, respectively).ConclusionsA diet characterized by higher intakes of burgers, fries, and soft drinks and another characterized by higher intakes of white rice, beans, and red meats may be adversely associated with IR and diabetes risk in some Hispanic/Latino heritage groups. Future work is needed to offer more heritage-specific dietary guidance for diabetes prevention in this population.

Predictive and Prognostic Value of Plasma Zonulin for Gestational Diabetes Mellitus in Women at 24-28 Weeks of Gestation.

We aimed to examine the predictive and prognostic value of plasma zonulin for gestational diabetes mellitus (GDM) in women at 24-28 weeks of gestation. This retrospective study was carried out with pregnant women with GDM (n=98) and normal glucose tolerance (control group) (n=132). GDM was diagnosed according to American Diabetes Association (ADA) criteria with a one-step 75-g OGTT at 24-28 gestational weeks. Their serum zonulin levels measured during one-step 75-g OGTT and perinatal outcomes were compared, and the cut-off value of plasma zonulin for the prediction of GDM was calculated with receiver operating characteristic curve analysis. Plasma zonulin level was significantly higher in women with GDM compared to controls (28.8±24.9 and 7.3±11.3 ng/mL, respectively). According to logistic regression analysis, plasma zonulin levels and GDM were statistically significant. The plasma zonulin cut-off value was>45.2 ng/mL. The rate of cesarean section, the rate of meconium in the amniotic fluid, and the need for admission to the neonatal intensive care unit significantly differed between women with GDM and controls. In pregnant women with GDM, plasma zonulin increases, and with the cut-off level of>45.2 ng/mL, it can predict GDM with values of sensitivity and specificity levels significantly higher in pregnant women with GDM, suggesting that it can be used as a tool for its screening and early diagnosis.

Gestational diabetes is associated with alteration on pelvic floor muscle activation pattern during pregnancy and postpartum: Prospective cohort using electromyography assessment.

Background and objectiveGestational diabetes mellitus (GDM) is a comorbidity which may cause acute and lifelong disorders to mother and child. Alterations in muscular and connective tissues have been associated with GDM in translation studies, characterizing gestational diabetic myopathy. Pregnancy-specific urinary incontinence and sexual disabilities, disorders that depend on the pelvic floor muscle (PFM) integrity, are also associated with GDM both during and after pregnancy. The aim was to compare PFM activation patterns between GDM and non-GDM women from 24–30 gestational weeks to 18–24 months postpartum during a standard clinical test during gestation and postpartum.MethodsWe conducted a prospective three-time-point cohort study from gestation (24–30 weeks—T1, and 36–38 weeks—T2) to 18–24 months postpartum (T3). PFM electromyography was recorded in primigravida or primiparous women with one previous elective c-section with or without the diagnosis of GDM according to the American Diabetes Association criteria. A careful explanation of the muscle anatomy and functionality of the PFM was given to participants before EMG assessment. The outcome measures were PFM activation patterns assessed during pregnancy and postpartum, comparing intra and between groups. PFM activation patterns were assessed by normalized electromyography signal at rest and during 1-second (sec) phasic, 10-sec hold, and 60-sec sustained contractions.ResultsDemographic and obstetric data showed homogeneity between groups. The GDM group achieved peak PFM EMG amplitudes similarly to the non-GDM group, but they took longer to return to baseline levels during the ~1-sec contraction (flicks). During 10-sec hold contractions, the GDM group sustained lower levels of PFM activation than the non-GDM group at both 36–38 weeks of gestation and 18–24 months postpartum when compared to the non-GDM group.ConclusionThe results suggest that GDM impaired PFM control mainly on 1-sec flicks and 10-sec hold contraction, which appears to develop during late pregnancy and extends long-term postpartum. This motor behavior may play a role on pelvic floor dysfunctions.

Glycemic control and its influencing factors in type 2 diabetes patients in Anhui, China.

ObjectiveTo investigate the status of glycemic control and analyze its influencing factors in patients with type 2 diabetes (T2D) in Anhui, China.Methods1,715 T2D patients aged 18–75 years old were selected from 4 counties or districts in Anhui Province in 2018, using a convenience sampling method. All patients have undergone a questionnaire survey, physical examination, and a glycosylated hemoglobin (HbA1c) test. According to the 2022 American Diabetes Association criteria, HbA1c was used to evaluate the glycemic control status of patients, and HbA1c < 7.0% was defined as good glycemic control. The influencing factors of glycemic control were analyzed by multivariate unconditional logistic regression.ResultsThe prevalence of good glycemic control among people with T2D in the Anhui Province was low (22.97%). On univariate analysis, gender, education level, occupation, region, smoking, drinking, waist circumference and disease duration (all P < 0.05) were significantly associated with glycemic control. The factors associated with pool glycemic control were female gender [OR = 0.67, 95%CI (0.52, 0.86), P = 0.001], higher level of education [OR = 0.47, 95%CI (0.27, 0.83), P = 0.001], living in rural areas [OR = 1.77, 95%CI (1.39, 2.26), P < 0.001], central obesity [OR = 1.58, 95%CI (1.19, 2.09), P = 0.001] and longer duration of disease [OR = 2.66, 95%CI (1.91, 3.69), P < 0.001].ConclusionsThe prevalence of good glycemic control in people with T2D in Anhui Province was relatively low, and gender, region, education level, central obesity and course of the disease were influencing factors. The publicity and education on the importance of glycemic control should be further strengthened in T2D patients, and targeted intervention measures should be carried out for risk groups.

The NDDG criteria versus the IADPSG or the ADA criteria for diagnosing early-onset gestational diabetes mellitus or abnormal glucose tolerance.

To analyze the effects of substituting the National Diabetes Data Group (NDDG) criteria with the International Association of Diabetes and Pregnancy Study Groups (IADPSG) or American Diabetes Association (ADA) criteria for the diagnosis of early-onset gestational diabetes mellitus (Early-GDM) or first trimester abnormal glucose tolerance (1t-AGT). A retrospective cohort study was conducted of 3200 women: 400 with Early-GDM, 800 with GDM, and 2000 with Non-GDM, according to the NDDG criteria. Rates of women with missed and new Early-GDM according to the IADPSG or ADA criteria were calculated. Multivariate logistic regression analysis was used to compare perinatal outcomes between groups. Using the IADPSG criteria, 61.6% of women with Early-GDM according to the NDDG were undiagnosed (Missed-Early-GDM group), and 25.9% of women with GDM and 15.7% of women with Non-GDM were diagnosed with Early-GDM (New-Early-GDM groups). Perinatal outcomes were worse in Missed-Early-GDM than in Non-GDM and better in New-Early-GDM groups than in the Early-GDM group. According to the ADA recommendations, only 11.8% of women with Early-GDM according to the NDDG criteria were diagnosed. Replacing the NDDG recommendations for the diagnosis of Early-GDM with the IADPSG or ADA criteria would mean depriving a large number of women with AGT and higher risk of adverse perinatal outcomes from early treatment and treating others with lower risk.

The role of age on the risk relationship between prediabetes and major morbidities and mortality: analysis of the Hong Kong diabetes surveillance database of 2 million Chinese adults

Intensive lifestyle modification showed variable success in the prevention of major clinical events and mortality among people with prediabetes. We propose that age may partly explain the heterogeneity and that health hazards related to prediabetes are age-specific. We conducted a retrospective analysis of a territory-wide diabetes surveillance dataset from the Hong Kong Hospital Authority between 2000 and 2019. Prediabetes was defined according to the American Diabetes Association criteria. Proportional Cox regression was performed, stratified by baseline age categories (20-39, 40-59, 60-79 and ≥80 years). 1,630,942 individuals were included in the analysis. Compared with normoglycaemia, prediabetes was associated with greater hazards for cardiovascular disease (CVD) and all-cause mortality in most age groups but the effect size attenuated with ascending age (p value for trend <0·05). In the youngest and in the oldest age categories, the respective hazard ratios (95% confidence interval) of prediabetes vs normoglycaemia were 1·79 (1·59, 2·01) and 1·00 (0·95, 1·05) for CVD, and 1·36 (1·20, 1·55) and 0·99 (0·97, 1·02) for all-cause mortality. Similar associations were found for chronic kidney disease, end-stage kidney disease, all-site cancer, all-site infection, subtypes of CVD, and cause-specific mortality. The associations became attenuated but remained after excluding people who later developed diabetes and adjusting for metabolic factors. Similar associations were observed in prediabetes defined by impaired fasting glucose, but not HbA1c. Prediabetes is associated with higher risk of major clinical events, even excluding subsequent development of diabetes and adjusting for metabolic factors. The risk relationships are stronger in young than older people. This study did not receive any specific funding.

Assessment of Dyslipidemia, Lipid Ratios, and Atherogenic Indices as Cardiovascular Risk Factors in Prediabetic and Diabetic Subjects.

Background Uttarakhand is a small state in northern India that comprises mixed population with people dwelling in both hilly and plain areas. Globally, diabetes mellitus (DM) has already been considered to be a pandemic. Furthermore, cardiovascular diseases (CVD) significantly increase mortality and morbidity in patients suffering from DM. Additionally, dyslipidemia has been identified as an important marker in the development of atherosclerosis and ultimately CVD in patients of prediabetes and diabetes. Thus, the identification of subjects with dyslipidemia in prediabetes might be fruitful in lowering their progression to diabetes and ultimately in decreasing incidences of CVD. Hence, this study was undertaken to assess dyslipidemia via the calculation of atherogenic indices (AI) and lipid ratios in prediabetic and diabetic groups attending tertiary care hospital in Uttarakhand. Materials and Methods This study reviewed retrospective biochemical data of 500 study subjects from e-hospital software of the All India Institute of Medical Sciences, Rishikesh. All study subjects were divided into three groups: 122 controls, 137 prediabetics, and 241 diabetics based on the American Diabetes Association criteria. Study subjects were evaluated for glycated hemoglobin (HbA1c), lipid profile, and AI (total cholesterol [TC]/high-density lipoprotein cholesterol [HDLc], low-density lipoprotein cholesterol [LDLc]/HDLc, TC-HDLc/HDLc, triglycerides [TG]/HDLc). Results Results showed that TC, TG, LDLc, and AI were significantly higher, and HDLc was significantly decreased in prediabetic and diabetic groups compared with controls. Furthermore, HbA1c showed significant positive correlation with lipid profile and AI except atherogenic coefficient (TC-HDL/HDL). Conclusion In conclusion, the current study showed the presence of dyslipidemia in both prediabetic and diabetic groups underlining their importance for screening at the prediabetic stage. Hence, we also recommend screening of the prediabetic group for dyslipidemia to arrest the development of early cardiovascular complications.

Abnormal glucose homeostasis in patients of HbEβ-thalassemia: Prevalence and possible pathogenesis using the Oxford HOMA model

Objectives: Eβ-thalassemia, the most serious form of HbE syndromes, may develop pre-diabetes (PD) and diabetes mellitus (DM), together constituting abnormal glucose homeostasis (AGH) as an endocrinopathy. This study aims to assess AGH prevalence and pathogenesis in this thalassemia subtype. Material and Methods: A cross-sectional study was conducted at a tertiary care hospital from February 2017 to December 2018 (1.9 years). One hundred and four HbEβ-thalassemia patients were randomly selected aged ≥5 years, irrespective of transfusion requirement. AGH was diagnosed by the American Diabetes Association criteria. The patient’s history, relevant examination details, and parameters related to glucose homeostasis were studied. The homeostasis assessment (HOMA) model of Oxford University was used, and formulae were applied to calculate HOMA-insulin resistance (IR) or HOMA-β (β-cell function). Results: The status of glucose homeostasis was as follows: Normal glucose homeostasis tolerance 83/104(79.8%), PD 20/104(19.2%), and DM one(1%). The patient’s age, age of starting transfusions, and HOMA-IR were significantly related to AGH. AGH was inversely associated with the age of starting chelation, though not significant (P = 0.07). There was no statistical significance of AGH development, with transfusion dependence (P = 0.63), family history of DM (P = 0.42), hepatitis C (P = 0.36), and higher ferritin levels (800/1000/1500/1700 ng/ml) (P &gt; 0.5)/HOMA-β (P &gt; 0.5). Conclusion: HbEβ-thalassemia patients are prone to develop AGH including overt diabetes. It is related to the patient’s age, age of initiation, and duration of transfusion therapy. The likely mechanism of pathogenesis is IR, though pancreatic β-cell destruction may also be contributory.

Factors correlated with targeted prevention for prediabetes classified by impaired fasting glucose, impaired glucose tolerance, and elevated HbA1c: A population-based longitudinal study.

BackgroundThere is still controversy surrounding the precise characterization of prediabetic population. We aim to identify and examine factors of demographic, behavioral, clinical, and biochemical characteristics, and obesity indicators (anthropometric characteristics and anthropometric prediction equation) for prediabetes according to different definition criteria of the American Diabetes Association (ADA) in the Chinese population.MethodsA longitudinal study consisted of baseline survey and two follow-ups was conducted, and a pooled data were analyzed. Prediabetes was defined as either impaired fasting glucose (IFG), impaired glucose tolerance (IGT), or elevated glycosylated hemoglobin (HbA1c) according to the ADA criteria. Robust generalized estimating equation models were used.ResultsA total of 5,713 (58.42%) observations were prediabetes (IGT, 38.07%; IGT, 26.51%; elevated HbA1c, 23.45%); 9.66% prediabetes fulfilled all the three ADA criteria. Among demographic characteristics, higher age was more evident in elevated HbA1c [adjusted OR (aOR)=2.85]. Female individuals were less likely to have IFG (aOR=0.70) and more likely to suffer from IGT than male individuals (aOR=1.41). Several inconsistency correlations of biochemical characteristics and obesity indicators were detected by prediabetes criteria. Body adiposity estimator exhibited strong association with prediabetes (D10: aOR=4.05). For IFG and elevated HbA1c, the odds of predicted lean body mass exceed other indicators (D10: aOR=3.34; aOR=3.64). For IGT, predicted percent fat presented the highest odds (D10: aOR=6.58).ConclusionSome correlated factors of prediabetes under different criteria differed, and obesity indicators were easily measured for target identification. Our findings could be used for targeted intervention to optimize preventions to mitigate the obviously increased prevalence of diabetes.

Autoimmune Hemolytic Anemia (AHA) with hyperglycemia in a 22-year-old woman: a case report

Background: Autoimmune hemolytic anemia (AIHA) is defined as increased erythrocyte destruction due to anti-erythrocyte autoantibodies with or without complement activation. Diabetes Mellitus (DM) is a chronic metabolic disease characterized by elevated blood glucose levels, which cause serious damage to various organs, including the heart, blood vessels, eyes, kidneys, and nerves. The impact of DM on a patient's survival with AIHA is unknown. Case Presentation: A 22-year-old woman complained of dizziness and fainting before admission. The patient looked pale; on physical examination, we found jaundice on the sclera. This patient was diagnosed with AIHA and DM with increased HbA1c (10.8%) since 2017 but did not take the anti-diabetic drugs regularly. Hematological examination showed anemia, increased reticulocytes, and a positive direct Coombs test. The result of increased fasting glucose, 2 hours OGTT, and HbA1c levels (4.6%) tests were not relevant to hyperglycemia. In this patient, we also found normal C-peptide levels. The decrease in HbA1c levels was thought to be due to hemolytic anemia; therefore, HbA1c was falsely low due to rapid erythrocyte turnover. Conclusion: This patient was diagnosed with DM based on American Diabetes Association criteria with normal C-peptide levels, suggesting Maturity Onset Diabetes of the Young with HbA1c incompatibility with glucose levels may be due to rapid erythrocyte turnover.

Assessment of Fasting Blood Glucose in Chronic Periodontitis Patients Visiting a Tertiary Hospital

Introduction: It is well-established that diabetes unfavorably influences periodontal health and results in periodontitis. However, upcoming evidence also shows that periodontitis initiates a chronic state of insulin resistance leading to hyperglycemia. Objectives: We designed this study to assess fasting blood glucose levels in chronic periodontitis patients and also explore the relationship between severity of chronic periodontitis and fasting blood glucose levels with an analysis of their risk factors in adult the population of eastern Nepal. Methods: A cross-sectional hospital-based study was conducted on a total of 141 chronic periodontitis patients aged 30-55 years who were divided into Group A (Mild periodontitis) and Group B (Moderate/ Severe periodontitis) according to the American Academy of Periodontology / Centres for Disease Control (AAP/ CDC) criteria. Fasting blood glucose level was analyzed as normal (&lt;100 mg/dl), impaired fasting (100-125 mg/dl), and diabetic (≥126 mg/dl) based on the American Diabetes Association (ADA) criteria. Results: Among the 141 subjects examined, Group A accounted to be 43.26% and Group B 56.74%. The mean fasting blood glucose level was 121.51 ± 53.67 mg/dl. The Chi-square test showed that severity of periodontitis and body mass index were significantly associated with the categories of fasting blood glucose. Multivariate logistic regression analysis showed that age (OR=1.08), gender (OR=3.48) and body mass index (OR=0.38) were significantly associated with impaired fasting glucose. The risk of acquiring diabetes in mild periodontitis was 0.255 times less likely than in severe periodontitis (OR=0.255, CI 0.065-0.997). Conclusions: The severity of chronic periodontitis showed a significant association with the categories of fasting blood glucose.
