Villous adenoma of the urinary tract: a report of 23 cases, including 8 with coexistent adenocarcinoma.

Villous adenoma originating in the urinary tract is uncommon. We present the first study of a large number of cases of villous adenoma of the urinary tract with clinical follow-up. Our series consisted of 15 patients with isolated villous adenoma and 8 patients with coexistent adenocarcinoma. The tumors occurred in elderly patients and had a predilection for the urachus, dome, and trigone of the urinary bladder. The typical clinical presentation was hematuria and irritative symptoms, and endoscopic examination usually identified a tumor growth. There was no gender predominance. Light microscopic examination showed morphologic similarity to colonic villous adenoma in all cases. Each tumor was composed of pointed or blunt finger-like processes lined by pseudostratified columnar epithelium. The epithelial cells displayed nuclear stratification, nuclear crowding, nuclear hyperchromasia, and occasional prominent nucleoli and mitotic figures. There was intense carcinoembryonic antigen immunoreactivity on the luminal surfaces (89%). Most cases (78%) contained cytoplasmic acid mucin, demonstrated by Alcian blue periodic acid-Schiff stain. Cytokeratin 20 was positive in all cases, cytokeratin 7 was positive in 56% of cases, and epithelial membrane antigen was positive in 22% of cases. Recurrence or invasive adenocarcinoma did not develop in any patient with isolated villous adenoma during a mean follow-up of 9.9 years. Lung metastasis developed in one patient with coexistent adenocarcinoma and multiple recurrences in another (mean follow-up, 3 years). We conclude that the prognosis is excellent in patients with isolated villous adenoma, and complete surgical resection is curative. Patients with coexistent adenocarcinoma may experience recurrence or distant metastasis, and more aggressive treatment may be indicated.

Epidermal growth factor system regulates mucin production in airways.

Goblet-cell hyperplasia is a critical pathological feature in hypersecretory diseases of airways. However, the underlying mechanisms are unknown, and no effective therapy exists. Here we show that stimulation of epidermal growth factor receptors (EGF-R) by its ligands, EGF and transforming growth factor alpha (TGFalpha), causes MUC5AC expression in airway epithelial cells both in in vitro and in vivo. We found that a MUC5AC-inducing epithelial cell line, NCI-H292, expresses EGF-R constitutively; EGF-R gene expression was stimulated further by tumor necrosis factor alpha (TNFalpha). EGF-R ligands increased the expression of MUC5AC at both gene and protein levels, and this effect was potentiated by TNFalpha. Selective EGF-R tyrosine kinase inhibitors blocked MUC5AC expression induced by EGF-R ligands. Pathogen-free rats expressed little EGF-R protein in airway epithelial cells; intratracheal instillation of TNFalpha induced EGF-R in airway epithelial cells, and subsequent instillation of EGF-R ligands increased the number of goblet cells, Alcian blue-periodic acid-Schiff staining (reflecting mucous glycoconjugates), and MUC5AC gene expression, whereas TNFalpha, EGF, or TGFalpha alone was without effect. In sensitized rats, three intratracheal instillations of ovalbumin resulted in EGF-R expression and goblet-cell production in airway epithelium. Pretreatment with EGF-R tyrosine kinase inhibitor, BIBX1522, prevented goblet-cell production both in rats stimulated by TNFalpha-EGF-R ligands and in an asthma model. These findings suggest potential roles for inhibitors of the EGF-R cascade in hypersecretory diseases of airways.

Bowman's Gland in Nasal Tissues: As A Possible

We have investigated the Bowman's gland of the nasal cavity after in vivo study in the Sprague-Dawley rats to asses its biomarker potential in vitamin A nutritional study. Fourty-eight male weanling SpragueDawley rats at 3-4 weeks of age were allotted to four groups of 12 each and were fed diets either vitamin A-deficient or supplemented with 30.000 IU vitamin A/kg for 150 days. Rats in their respective groups were also given fume of vehicle for 150 days. Fumigation was done 2x/day for 1 hour each. Rats given vitamin A-deficient diet and fume of vehicle had the most extensive inhibition of glycoprotein synthesis in cells of Bowman's gland in the olfactory region of the nasal cavities as determined by severe loss of Alcian blue-periodic acid-Schiff staining material. Histochemical lesions were not seen in nasal cavity of vitamin A-supplemented rats

Phenotype and differentiation potential of a novel rat tracheal epithelial cell line.

In this report we described the establishment and characterization of a continuous rat tracheal epithelial (RTE) cell line spontaneously derived from secondary RTE cell cultures. Designated SPOC1, this cell line is nontumorigenic and maintains a diploid karyotype with specific, nonrandom chromosomal alterations involving chromosomes 1, 3, and 6. SPOC1 cells demonstrate decreased requirements for peptide growth factors, compared with primary RTE cells. Upon inoculation into denuded rat tracheas, which are then implanted into syngeneic hosts, SPOC1 cells initially form a stratified squamous epithelium, which becomes less stratified with time and forms glandlike invaginations into the surrounding lamina propria. No evidence of ciliated cell differentiation is detected. The epithelium formed by SPOC1 cells in tracheal grafts reacts with antibodies specific for keratin 14, 13, and 19 (but not keratin 18) at both early and late time points, although the localization of antibody staining changes as the epithelium becomes less stratified with time. The suprabasal epithelial cells become positive for alcian blue-periodic acid-Schiff staining at later time points. The near-normal karyotype and differentiation potential of SPOC1 cells make this cell line a unique window into early changes occurring during immortalization of airway epithelial cells and will allow studies of relationships between differentiation state and neoplastic transformation.

Evaluation of three techniques for differential diagnosis of prostatic needle biopsy specimens.

To determine whether acidic mucin staining, lectin histochemistry using Wisteria floribunda agglutinin, and immunohistochemistry using the monoclonal antibody EAB 903 are of benefit in distinguishing between hyperplastic and carcinomatous prostatic glandular tissue in needle biopsy specimens. Formalin fixed, paraffin wax embedded prostatic needle biopsy specimens of benign and malignant tissue were examined. Alcian blue-periodic acid Schiff staining was performed on 33 benign and 34 malignant cases. Wisteria floribunda agglutinin (WFA) binding sites were demonstrated by the avidin-biotin peroxidase (ABC) technique with and without neuraminidase pretreatment on 34 benign cases and 32 malignant cases. EAB903 anticytokeratin antibody binding sites were demonstrated using both an indirect immunoperoxidase (IIP) technique and an avidin-biotin peroxidase complex method on seven benign and 31 malignant cases. Acidic mucin staining was found in 17 of 34 malignant glands and was weakly positive in five of 33 benign glands. WFA positivity before neuraminidase pretreatment was present in 29 of 32 malignant glands and in 19 of 34 benign glands. After neuraminidase all benign and malignant cases showed positivity. EAB 903 positivity was seen in 11 of 31 malignant glands using the IIP technique and in two of 31 malignant glands using the ABC technique. In seven benign cases there was positivity in all glands using the IIP method with predominant basal cell staining in three and superficial cell staining in four. In benign cases using the ABC method two cases were negative. None of the three methods studied showed sufficient sensitivity and specificity to allow their recommendation for routine diagnostic use.

Cystic Changes in Gastric Glands After Gastric Surgery and in the Intact Stomach

Cystic changes of gastric mucosal glands have been described mainly after gastric operations, and like intestinal metaplasia and dysplasia, they may represent a premalignant condition. Their association with gastritis raises the possibility of their being secondary to the inflammatory process. Enterogastric reflux of duodenal contents, local chronic ischemia, and inflammatory reaction as a result of gastric surgery and suture at gastroenterostomy have been considered responsible for this lesion. In 18 of 157 consecutive patients (11.5%) who underwent endoscopic gastric biopsy within a year we found cystic changes of gastric mucosal glands. Cystic changes were present in 43% of 30 patients after gastric operation for duodenal ulcer disease, within an average of 8.4 years in contrast to only 4% of patients with an intact stomach. This change is statistically significant (Z = 1.97, (p less than 0.05) and suggests that there is a cause-and-effect link between the operation and the development of cystic lesions. In three patients we traced the original operative specimen, and in none did we find cystic changes. All the cases were associated with chronic gastritis; mild dysplasia was found in four (22%). The cystic glands were shown (by alcian blue-periodic acid-schiff staining) to secrete neutral mucin like normal gastric glands, and unlike dysplastic glands or intestinal metaplasia where acid mucin is characteristic. Thus, our findings suggest an inflammatory cause for the cystic glandular change (reactive, hyperplastic change of glands), and suggest that it is probably not a preneoplastic state.

Toxic effects of N-nitrosodiethylamine on nasal tissues of Sprague-Dawley rats and golden Syrian hamsters

The acute necrogenic effects of N-nitrosodiethylamine (NDEA) on nasal tissues of female Sprague-Dawley rats and golden Syrian hamsters were determined. Rats and hamsters were given a single ip dose of 0, 10, 20, 40, or 80 mg NDEA/kg body wt. After 24 hr, the rats and hamsters were killed and tissues were collected. Sections of nasal cavity and liver were evaluated histologically. All doses of NDEA caused inhibition of glycoprotein synthesis in cells of Bowman's glands in the olfactory region of rats and hamsters as determined by the loss of Alcian blue-periodic acid-Schiff staining material. Glycoprotein synthesis in other glands including the lateral nasal glands, maxillary glands, medial nasal glands, and the acinous glands near the vomeronasal organ was not affected by NDEA. Necrosis of Bowman's glands in the olfactory region of the nasal cavity occurred in rats given 20, 40, or 80 mg NDEA/kg body wt whereas the same cells were not necrotic in hamsters given NDEA. The results demonstrate the unique susceptibility of cells of the Bowman's glands to the toxic effects of NDEA given ip and indicate that nasal tissues of the rat are more susceptible to the necrogenic effects of NDEA than those of the hamster.

Histochemical studies of mucosubstances and keratins of thymic corpuscles.

Keratin and prekeratin have been visualized histochemically by testing the reactivity of disulfide groups in the thymic corpuscles of the sheep. Rhesus monkey, rabbit and dog, and the reactivity of sulfhydryl groups in the sheep, Rhesus monkey, rabbit and rat. Ten to 50% of the thymic corpuscles showed keratinization by this method. Acidic mucosubstances were visualized histochemically in the Rhesus monkey, sheep and rat but not in the rabbit. Staining for vic-glycol groups was prominent in all species tested. Neuraminidase treatment with subsequent Alcian blue-periodic acid-Schiff (PAS) staining indicated the presence of sialomucins in the thymic corpuscles of the monkey and rat, but sialomucins were not demonstrated in the rabbit.

Light and electron microscope localization of binding sites of antibodies against ovine luteinizing hormone and its two subunits in rat adenohypophysis using peroxidase-labeled antibody technique.

The binding sites of antisera generated in the guinea pig against ovine luteinizing hormone (oLH) and its two subunits (oLHalpha and oLHbeta) have been localized in rat anterior pituitaries taken from normal or castrated males and from ovariectomized females with the peroxidase-labeled antibody method, using light and electron microscopy. With the light microscope, the cells positive with antiserum to ovine luteinizing hormone (A-oLH) were violet after the Alcian blue-periodic acid-Schiff (AB-PAS) staining; they were also positive for A-oLHalpha and for A-oLHbeta and, from castrated males, they displayed an increased affinity for A-oLHbeta. Another cell type which was blue after the AB-PAS method reacted with the A-oLHalpha only; these cells, presumably thyrotropic cells, were retracted after castration and, besides their affinity for A-oLHalpha, acquired an affinity for A-oLHbeta. As seen through the electron microscope, two cell types were positive for A-oLH, A-oLHbeta, and A-oLHalpha and may be identified as luteinizing hormone-secreting cells. Type A cells were characterized by two classes of rounded, secretory granules. Type B cells were smaller and contained only small secretory granules. 1 mo after the rats were castrated the type A cells were hypertrophied and vacuolized. In both cases the secretory granules were the main sites of the antigenicity with the three antisera. A positive reaction was also found in the cytoplasm, particularly in hypertrophied cells from ovariectomized females and with A-oLHbeta. The cisternae of the rough endoplasmic reticulum were usually negative, except in highly degranulated cells from ovariectomized females and with A-oLHbeta.

An Examination of Skin from Patients with Collagen Disease Utilizing the Combined Alcian Blue-Periodic Acid Schiff Stain

Alcian blue, a relatively new phthalocyanine dyestuff (1), is used by industry for dyeing cotton. In 1950 Steedman described the use of this compound for staining mucin (2). Current evidence indicates that Alcian blue (AB) selectively stains structures containing acid mucopolysaccharides (3,4). The periodic acid Schiff (PAS) stain for 1,2 glycols, including carbohydrates, has been widely used since the fundamental work of McManus (5) and Hotchkiss (6) in 1946 and 1948. In the present study Alcian blue was combined with the periodic acid Schiff stain (AB-PAS) for the examination of involved skin from patients with known collagen disease.
