A Comparative Study of Lung Compliance and Pulmonary Surfactant Activity in Human Subjects

Twenty-seven patients were studied by measuring dynamic lung compliance and lung extract surface activity to investigate a theoretical correlation. Fifteen patients were without respiratory symptoms or diffuse pulmonary disease. They were classified Group I (9 patients) with normal ventilation, normal lung compliance, and normal pulmonary surfactant activity, or Group II (6 patients) with either obstructed or restricted ventilation, low lung compliance (2 of 6 patients), and normal pulmonary surfactant activity. The other 12 patients, who made up Group III, were studied because they had radiographically apparent diffuse pulmonary disease. Their ventilatory impairment was consistent with the histopathology; however, lung compliance was normal in all but 1 patient, whereas pulmonary surfactant activity was abnormal in 4 including the patient with low compliance. It is concluded that (1) dynamic lung compliance may be independently abnormal in patients with obstructive ventilation; (2) pulmonary surfactant activity may be independently abnormal in lungs with parenchymal pathology; and (3) the I case of dynamic lung compliance and pulmonary surfactant found in this investigation does not constitute sufficient evidence to establish a correlation between these entities.

Effect of partial and total cardiopulmonary bypass with whole blood or hemodilution priming on pulmonary surfactant activity

1. 1. Minimal surface tension of the lungs of dogs was estimated in terms of surfactant activity before, during, and after cardiopulmonary bypass. 2. 2. Surfactant values of normal lungs sampled before the operation ranged from 1.8–11.8 dynes per cm. 2, averaging 5.1 dynes per cm. 2 3. 3. Animals perfused one or two hours with 50% hemodilution showed little change in surfactant values immediately after the perfusion was completed. However, in those that survived more than fifteen hours the values rose an average of 11.1 dynes per cm. 2 4. 4. 60% of animals perfused with whole blood for one hour had rises in surface tension (average 8.5 dynes/cm. 2) both at the completion of bypass and at the time of death. None of these dogs survived more than eight hours. 5. 5. No rise in minimal surface tension was observed in animals placed on controlled ventilation without cardiopulmonary by pass; only one of 6 animals with thoracotomy and heparin showed a persistent rise. 6. 6. Possible explanations for the temporary protection to lung surface afforded by hemodilution as compared to whole blood perfusion are discussed.

Effect of chronic exposure to ultrasonic aerosols on the lung.

Exposure for six hours to aerosols produced by ultrasonic nebulizers has been shown not to affect adversely the lungs of experimental animals. In order to evaluate the effects of long term exposure, 16 puppies were exposed to ultrasonic aerosols of normal saline solution or distilled water for 72 hours. Pathological changes compatible with severe bronchopneumonia were seen in all 8 animals exposed to saline; only 2 puppies exposed to distilled water had obvious pulmonary lesions. No significant alteration in pulmonary surfactant activity or deflation pressure volume curves were demonstrated. We conclude that continuous wetting of the lung with a high output of ultrasonic aerosol may be deleterious when used for prolonged periods. The exact time necessary to produce these lesions with various fluids remains to be determined. It seems reasonable to speculate, however, that the more hypertonic the fluid the more rapidly these lesions will develop.

Effects of atelectasis on pulmonary surfactant and quasi-static lung mechanics.

The effects of unilateral atelectasis on pulmonary surfactant activity and lung mechanics have been studied in rabbits. Total left lung collapse was produced by pneumothorax and maintained from 90 min to 8 days. Quasi-static pressure-volume relationships were determined with air and saline after varying intervals. Surface activity of minced lung extracts and of dried foam obtained by tracheal washings was measured on a surface balance. A marked decrease in lung inflatability progressing with duration of collapse was noted during air and saline pressure-volume studies. However, deflation characteristics of all atelectatic lungs were normal. Extracts from minced, in vitro reinflated atelectatic lung showed a variable but significant loss of surface activity compared to the right (normal) lung of the same animal. However, highly surface-active material could be demonstrated in the collapsed lungs by the more efficient extraction procedure of tracheal foaming.
