Comparision of Active Cycle of Breathing Technique and Autogenic Drainage Technique in Patient Who Had Under Gone Laparotomy

Objective: To Find Out Effectiveness of AD and ADT in the Patients Who Had Under Gone LaprotomyMethod: Study design: Comparative StudyStudy Setting: It Was Concluded In General Surgical Wards In Civil Hospital, Ahmadabad.Study Duration: 3 months. The duration of treatment programmed for each subjects was 3-to 4 days.Sample selection: Patients (between 25 years to50 years) who had under gone laprotomy were selected forthis study, total 40 patients were selected for this study on the basis of inclusion and exclusion criteria.Results: There was significant improvement in parameters of the patients of two groups after treatment byautogenic drainage and active cycle of breathing technique within the surgical wards.Conclusion: This study suggests that there is no significant deference in effects of obtained with autogenicdrainage and active cycle of breathing techniques. It was also observed that individually autogenic drainageand active cycle of breathing techniques are significantly effective.

Active Cycle Breathing Technique Terhadap Fungsional Paru Pasien Post CABG (Laporan Kasus Berbasis Bukti)

Coronary Artery Bypass Graft (CABG) is an operative action in cases of coronary heart disease (CHD) by cutting and replacing clogged coronary arteries from healthy vessels taken from the legs, arms, or chest. The main clinical problem that arises in post-CABG patients is decreased lung functional capacity, this is due to shortness of breath, pain, decreased thoracic expansion, sputum retention, and decreased pulmonary ventilation. Active Cycle of Breathing Technique (ACBT) is a physiotherapy modality that can be applied in cardiovascular cases. ACBT is a series of exercises that include breathing exercises, coughing exercises, and exercises to expand the chest. To determine the effectiveness of ACBT in increasing lung functional capacity in Post CABG patients, an electronic database search was performed, namely Pubmed, Cochrane, and Google Schoolar. From the search results, 6 articles were selected that met the criteria, namely 1 article on Pubmed and 5 articles on Google scholar. Based on the results of the research that has been presented, it can be concluded that ACBT can increase the functional capacity of the lungs in post CABG patients as evidenced by the evaluation of the results of the 6 minutes walking test, borg scale, and breath holding time.

Active Cycle Of Breathing Technique Terhadap Fungsi Paru pada Pasien Paska Operasi Bedah Jantung

The pulmonary complications after cardiac surgery is estimated to be 2-4%. These complication increase the length of hospital stay and increase cost. Postoperative cardiac surgery care focuses on pulmonary function with active cycle of breathing technique (ACBT). This study aimed to identify the effectiveness of ACBT in recovery pulmonary function in postoperative cardiac surgery. This study used pra experiment method involving 30 respondents with purposive sampling technique in adult surgery intensive care unit and Surgical ward. This group applies ACBT for 4 days: 2x/day, duration 15 minutes/session on 1 st and 2 nd day after surgery; 1x/day, duration 15 minutes/session on 3 rd and 4 th day after surgery. The Peak Expiratory Flow Rate is evaluated with Peak flow meter tools. The pre test data were obtainedd on the pre operation and the post test data were obtained on the 3 rd day after surgery. This data analyzies with distribution mean and wilcoxon. The result distribution mean of Peak Expiratory Flow (PEF) pre test is 420,33 l/minute and the post test is 432,33 l/minute. The result of wilcoxon test is p=0,001< α=0,05 , it is show that significant effect in PEF (Peak Expiratory Flow) pre, 3 rd day PEF. ACBT consists of deep breathing and hold breathing, so the air flow easier and the discharge mucous, on the last step is the huffing technique. It is not require stretgh to discharge mucous and reduce pain. ACBT can be used as a nursing intervention for airway management to recovery pulmonary function in postoperative cardiac surgery. Role of the active cycle of breathing technique combined with phonophoresis for the treatment of patients with chronic obstructive pulmonary disease (COPD): study protocol for a preliminary randomized controlled trial

BackgroundChronic obstructive pulmonary disease (COPD) is a chronic inflammatory lung disease characterized by coughing, the production of excess sputum, and dyspnea. Patients with excessively thick sputum may have frequent attacks or develop more serious disease. The guidelines recommend airway clearance for patients with excessive sputum who are hospitalized with COPD. The active cycle of breathing technique is the most common non-pharmacological airway clearance technique used by physiotherapists. However, the effectiveness of the technique is not always guaranteed. Active cycle of breathing techniques require the initial dilution of the sputum, usually by inhalation drugs, which may have limited effects. Recent studies have found that phonophoresis decreases inflammation, suggesting the potential of the combined usage of active cycle of breathing techniques and phonophoresis. Therefore, the aim of this study is to explore the effectiveness and safety of combining active cycle of breathing technique and phonophoresis in treating COPD patients.Methods and analysisWe propose a single-blind randomized controlled trial using 75 hospitalized patients diagnosed with COPD with excessive sputum production. The patients will be divided into three groups. The intervention group will receive active cycle of breathing techniques combined with phonophoresis. The two comparison groups will be treated with active cycle of breathing techniques and phonophoresis, respectively. The program will be implemented daily for 1 week. The primary outcomes will be changes in sputum viscosity and production, lung function, and pulse oximetry. Secondary outcomes include the assessment of COPD and anxiety, measured by the COPD Assessment Test scale and the Anxiety Inventory for Respiratory Disease, respectively; self-satisfaction; the degree of cooperation; and the length of hospital stay. All outcome measures, with the exception of sputum production and additional secondary outcomes, will be assessed at the commencement of the study and after 1 week’s intervention. Analysis of variance will be used to investigate differences between the groups, and a p-value of less than 0.05 (two-tailed) will be considered statistically significant.DiscussionThis study introduces a combination of active cycle of breathing techniques and phonophoresis to explore the impact of these interventions on patients hospitalized with COPD. Physiotherapists\u2019 use of airway clearance techniques during an acute exacerbation of bronchiectasis: a survey study

BackgroundAirway clearance techniques are recommended for individuals with bronchiectasis both in stable state and during an acute exacerbation, however the current use of airway clearance techniques in the management of individuals during an acute exacerbation is unclear. The aims of this study were to establish what current physiotherapy clinical practice comprises for adults and paediatrics during an acute exacerbation of bronchiectasis; identify physiotherapist’s perceptions of the effectiveness of airway clearance techniques and identify what factors influence their treatment decisions in this population.MethodsAn anonymous online survey was distributed to the members of the Australian Physiotherapy Association and Physiotherapy New Zealand between August 2016 and April 2017.ResultsThe survey was accessed by 130 physiotherapists and 121 of those deemed themselves eligible and consented to participate. Most participants (89%) reported prescribing airway clearance techniques for 81–100% of individuals during an acute exacerbation of bronchiectasis. The most commonly used airway clearance techniques with adults were huffing (92%), exercise (89%) and the active cycle of breathing technique (89%). The techniques perceived most effective for adults were physical exercise (100%), oscillating positive expiratory pressure devices (97%), directed huffing (95%) the active cycle of breathing technique (90%) and positive expiratory pressure (90%). The most commonly used airway clearance techniques for paediatric patients were: newborn-3 years - percussion (85%) and modified postural drainage (85%); 4–10 years - huffing (100%) and exercise (85%); 11–18 years - huffing (92%) and exercise (77%), active cycle of breathing technique (77%) and positive expiratory pressure therapy (77%). The techniques perceived most effective for paediatric patients were directed huffing (100%), percussion (100%) and positive expiratory pressure via a mask or mouthpiece (93%). The most commonly reported factors influencing choice of technique were patient clinical presentation (72%) and the presence/absence of contra-indications (72%).ConclusionThis survey demonstrates that airway clearance techniques are routinely used as part of physiotherapy management of individuals experiencing an acute exacerbation of bronchiectasis, and that choice of technique and perceived effectiveness varies depending on the age of the patient.

Role of active cycle of breathing technique for patients with chronic obstructive pulmonary disease: A pragmatic, randomized clinical trial

BackgroundWhile active cycle of breathing technique for chronic obstructive pulmonary disease patients with more sputum can improve clinic outcomes, less is known about sputum viscosity and sputum production of the intervention. ObjectiveThe purpose of our study was to explore the effect of active cycle of breathing technique on sputum viscosity and production among patients with chronic obstructive pulmonary disease. DesignThis was a two-arms, parallel, randomized clinical trial. SettingStudy enrollment, randomization and implementation were conducted in the department of respiratory medicine inpatient at the Medical Center in Changchun, China. ParticipantsHospitalized patients due to chronic obstructive pulmonary disease who met additional eligibility criteria were randomized to active cycle of breathing technique (n = 50) or usual care group (n = 50). MethodsPatients in the intervention group received a week-long intervention from an experienced physical therapist. Patients in the usual care group received usual care as well as information and advice in the light of their health plan from respiratory medicine. The primary outcome was the changes on sputum viscosity and production. ResultsAmong one hundred patients who were randomized (mean [SD] age, 54.89 [12.06] years; females, 58%), ninety-six participants completed the study. No significant differences were found between two groups on the changes of sputum viscosity (t = 0.277, P = 0.782). And there were insignificant differences between groups in the average amount of sputum among 1 h (Z=-1.848, P = 0.065) and significant differences in the average amount of sputum among 24 h (Z=-2.236, P = 0.025). From admission to one week recovery, the changes in ratio of forced expiratory volume in 1 s to forced vital capacity (Z=-4.511, P<0.0001) and arterial oxygen saturation (Z=-2.997, P = 0.003) were better in active cycle breathing technique group. Total Chronic Obstructive Pulmonary Disease Assessment Test scale were similar among two groups (Z=-1.818, P = 0.069). No adverse events occurred during the study. ConclusionFor patients with chronic obstructive pulmonary disease, active cycle of breathing technique can significantly result in sputum production and respiratory function, especially those of Global Initiative for Chronic Obstructive Lung Disease classification level 3, but did not result in the short-term improvement of sputum viscosity, quality of life and cost effectiveness.Registration number: ChiCTR2000033068.

Active Cycle Breathing Technique on Pulmonary Complications After Coronary Artery Bypass Graft

The most widely recognized postoperative complexities are watched and overseen after abdominal or cardiothoracic surgery are pulmonary complications and it is continue to contribute to patient morbidity and mortality. Hence, this study was conducted with the aim to determine the effectiveness of active cycle of breathing technique on pulmonary complications after coronary artery bypass surgery. Pre-experimental research design was adopted to conduct the study with 20 samples who met the inclusion criteria. Demographic variables were collected by using structured questionnaire and assessed the physiological parameters such as oxygen saturation, respiratory rate, and heart rate, systolic and diastolic blood pressure. Followed by active cycle of breathing technique was administered to the participants twice a day for 3 days from the first post-operative day onwards. Pre and post assessment was done each day before and after the intervention and post-test value was taken for analysis. Post-operative complications were assessed at the end of 3rd day of intervention by using the checklist contains the pulmonary complications due to surgery. Data were analyzed using SPSS statistical package. There was a significant improvement was observed in oxygen saturation, respiratory and heart rate at the level of p&lt;0.05 and the percentage of complications also very minimal. The findings of the study concluded that active cycle breathing technique is effective in pulmonary function thereby prevent the pulmonary complications after coronary artery bypass graft.

CORRELATION OF DYSPNEA WITH AGE AND SPO2 LEVELS IN COVID-19 AND EFFECTIVENESS OF NEUROPHYSIOLOGICAL FACILITATION IN THE MANAGEMENT OF DYSPNEA-A RANDOMIZED CLINICAL CONTROL TRAIL

Background & Purpose: Hypoxemia and dyspnea are the frequent causes of high mortality in COVID-19 patients. The aim of this study is to find the correlation between dyspnea with age, SPO2 levels in COVID-19 and find out the effectiveness of Neurophysiological facilitation of respiration techniques in reliving dyspnea and improving SPO2 levels in COVID-19. Methods: A total of 117 patients admitted into COVID-19 hospital are recruited in a single blinded randomized controlled trail. The participants are randomly assigned into experimental group (n=59) and control group (n=58). The experimental group was given physiotherapy in the form of Neurophysiological facilitation of Respiration (NPF), Active cycle of Breathing technique (ACBT) and accupressure. The control group was given physiotherapy in the form of light aerobic exercises, ACBT and accupressure. The levels of dyspnea and SPO2 were assessed pre intervention and after 10 days of treatment. Results: There was a weak positive correlation between Levels of Dyspnea and age (r=0.0385) p>0.05, which is not statistically significant . There was a strong negative correlation between dyspnea and SPO2 levels (r=0.7579) p < 0.05, which is statistically significant. The experimental group showed better and statistically significant improvement in levels of dyspnea and SPO2 in comparison with control group. Conclusion: There is a weak positive correlation between levels of dyspnea with age, strong negative correlation with SPO2 levels in COVID-19 patients. We also conclude that Neurophysiological facilitation improves levels of dyspnea and SPO2 in COVID-19 patients. KEYWORDS: Dyspnea, COVID-19, NPF, Physiotherapy.

Effectiveness Active Cycle of Breathing Technique (ACBT) with Pursed Lips Breathing Technique (PLBT) to tripod position in increase oxygen saturation in patients with COPD, West Sumatera

Chronic Obstructive Pulmonary Disease (COPD) is a chronic lung disease characterized by obstruction or obstruction of airflow in a reversible or partial nonreversible airway. Dyspnea is a common symptom in COPD sufferers; this can cause problems with oxygen saturation, or the oxygen saturation value is below normal. Non-pharmacological actions given in the form of breathing exercises can be done by a nurse to help reduce shortness of breath in COPD patients. The exercise is given by adjusting the resting position that is comfortable and comfortable so that the extra breath muscles can work well. The position that can do the position of the tripod with Active Cycle of Breathing and Pursed lips are breathing so that shortness of breath is reduced and SaO2 can have increased so that more oxygen has obtained in Lung Hospital, West Sumatra. This research is quantitative research, with quasi-experimental research methods. They used two pre-test-posttest design groups. The sample in this study amounted to 30 people with quota sampling technique. In this study showed that there was a difference in the increase in oxygen saturation of COPD patients which was effective in the tripod position group with Active Cycle Breathing Technique (ACBT) with a p-value of 0.00 while in the tripod position group with the Active Respiratory and Lip Cycle there was a difference of p-value 0.023. It has concluded that the tripod position with ACT and PBLT could increase oxygen saturation and nasal breathlessness reduced, but the tripod position with ACT is more effective in COP patients because with chronic respiratory patients who are short of breath due to sputum buildup, then with ACT will clean the airway and the flow of the road effective breathing so that shortness of breath have reduced and oxygen saturation increases. Furthermore, it has recommended that in hospitals, especially nurses, in dealing with COPD patients in addition to the correct position of breathing exercises with ACT to help patients.

Effect of active cycle of breathing techniques in patients with chronic obstructive pulmonary disease: a systematic review of intervention.

Patients with chronic obstructive pulmonary disease (COPD) often suffer from expectoration. To address this problem, active cycle of breathing techniques (ACBT) can be applied. However, the effects of ACBT on COPD patients have shown mixed results. Therefore, we investigated the effectiveness of ACBT in patients with COPD by conducting a systematic review of the literature. Seven electronic databases (PubMed, Web of Science, Embase, CINAHL, China National Knowledge Infrastructure [CNKI], Chinese Biomedical Literature [CBM], and Wanfang Database) were carefully searched from August 17th to 19th, 2019. The search yielded 2224 records, and ultimately only eight articles were selected for analysis. A total of 390 patients were included in the eight studies. The intervention program is a comparison of ACBT with non-ACBT. For COPD patients, ACBT was more effective in sputum production and cough efficiency. Compared with usual care, ACBT may improve lung function, blood gas analysis, and other parameters. Our systematic review found that ACBT can effectively improve the sputum production and cough efficiency in patients with COPD. However, there was no definite conclusion on the effectiveness of ACBT on lung function, blood gas analysis and other aspects. More research and quantitative analyses are needed to confirm the effectiveness of ACBT on other aspects of COPD patient.

Comparative Study of Flutter Device and Active Cycle of Breathing Technique in Airway Clearance in Subjects with Chronic Obstructive Pulmonary Disease

Background and Objectives: Chronic obstructive pulmonary disease is a disorder characterized by presence of air flow obstruction that is generally progressive, may be accompanied by airway hyperactivity and maybe partially reversible. COPD comprises major pathological changes. Conventional airway clearance techniques form the main stay of the routine Pulmonary Rehabilitation of subjects with chronic obstructive pulmonary disease. The procedure of Active Cycle of Breathing Technique causes an increase in lung volume which allows air to flow via collateral channels and may assist in mobilizing the secretions which but incorporating newer techniques that are effective, comfortable and can be independently performed by the subject without assistance help motivate the subjects to participate actively in their own health care. A device is proposed to have good efficacy, reliability and provides relief of the symptoms in a better way than the manual therapy in a way by excluding the bias and thus can be used as an alternative to it, so proves efficient to it. This study incorporates a device called ‘THE FLUTTER DEVICE’ which aids in mucus clearance in subjects with COPD effectively. Materials and Methodology: The Study participants were thirty subjects aged 35 to 45yrs with chronic obstructive pulmonary disease. After two weeks of ACBT and Flutter therapy with routine Physiotherapy each patient was told to record their VAS score for breathlessness on Visual Analogue Scale for breathlessness, the patient’s peak expiratory flow rate was then checked with the peak flow meter and FEV1 /FVC percentage was also obtained from the patient’s Pulmonary Function Test reports. Results: Data was analyzed by statistical means, standard deviation and students t- test and the results showed significant changes in terms of the VAS score for breathlessness, PEFR, FEV1 /FVC percentage of PFT. Conclusion: It can be concluded that application of flutter therapy for airway clearance to patients with Chronic obstructive pulmonary disease decreased the breathlessness drastically and increased the peak expiratory flow rate and FEV1 /FVC percentage than the application of ACBT.

Effectiveness of The Active Cycle Breathing Technique on The Peak Expiratory Flow Rate (PEFR) in Asthma Patient

Introduction: The Peak Expiratory Flow Rate (PEFR) is a person maximum expiratory airflow measurement. The active cycle breathing technique is a breathing exercise focus on release the abnormal lung secretion by increase the expiration power. The aim of this study was to analyze the effectiveness of the active cycle breathing technique on the PEFR in asthma patient.&#x0D; Methods: Design of this study was quasi-experimental with single pre-post test group. Study was conducted in the Kedungkandang Primary Health Care, Malang City, East Java Province, Indonesia. In total 25 asthma patients recruited into a single group. The participant asked to practice the breathing exercise for three times a week and being evaluated after one month. The PEFR was measured by Peak Flow Meter.&#x0D; Results: The Wilcoxon test showed p grade &lt; 0.05, which means there is a significant effect of the active cycle breathing technique on the PEFR grade. The increase of PEFR grade in asthma patients start to appear at the week third to the end of week fourth. Continuous treatment may increase the PEFR grade, however it was influenced by several factors such as age (years) and educational background.&#x0D; Conclusion: The active cycle breathing technique increase PEFR in asthma patients. The PEFR expected to be evaluated simultaneously, to assess the stages of improvement for asthma patient.&#x0D;

Effects of Air Pollution on the Vital Capacity of Commercial Motorcyclists in Nigeria Population

Background: Ambient air pollution is a serious challenge to human health in most West African countries including Nigeria.&#x0D; Objective: The objective of the study was to determine the effects of dust inhalation on the vital capacity of commercial motorcyclists in Abakaliki metropolis.&#x0D; Methods: 200 subjects participated in this study, 100 commercial motorcyclists (test group) and 100 non-motorcyclists (control group). The mean ± S.D of their age, height and weight were calculated. Vital capacity was measured using a spirometer. A questionnaire and consent form was filled by every participant before the commencement of the study. A handheld laser dust measuring device called air sampler PCE-PCO 1 was used to analyze the quantity, quality and sizes of particulate matters present in the research area.&#x0D; Data were analyzed using descriptive and inferential statistics; Mean and standard deviations were calculated and the Z-test was used to test if the mean were significantly different. Level of significance was set at 95%.&#x0D; Results: The age of participants were within the range of 25 – 30 years. Commercial motorcyclists were exposed to particulate matter 0.5, 1.0 and 2.5 µm. The vital capacity of commercial motorcyclists was significantly lower than that of non-motorcyclists (p =0.01). Furthermore, a significant decrease in the vital capacity of commercial motorcyclists was observed when correlated with the number of years spent in the job (p = 0.03). There was also a significant correlation between vital capacity and hours spent per week among commercial motorcyclists (r = 0.245).&#x0D; Conclusion: Commercial motorcyclists need to be encouraged to adopt respiratory health safety strategies like use of full-face helmets, practice of active cycle of breathing techniques and also, be educated on air pollution hazards by public health workers.

Impact of Simulated Patients on Physiotherapy Students' Skill Performance in Cardiorespiratory Practice Classes: A Pilot Study.

Purpose: To date, no evidence exists that high-fidelity simulation improves skill development among physiotherapy students in the university setting. With pressures to reduce costs and maintain or improve the quality of the learning experience, and with pressures on clinical placement, it is essential to investigate methods that might improve students' skill performance before they undertake clinical practice. Our study set out to investigate (1) the impact of using simulated patients (SPs) in a practical class on physiotherapy students' skill acquisition and (2) the students' reflections on the intervention. Method: We devised a pilot study using a single-centre randomized controlled trial. A total of 28 undergraduate physiotherapy students, matched using previous practical examination grades, undertook a 2-hour practical class in which they practised their core cardiorespiratory skills. Pre-session resources were identical. The control group practised on peers; the intervention group practised on SPs. The students' skill performance was assessed 2 weeks later using the Mini-Clinical Evaluation Exercise (MiniCEX), including gathering qualitative data from the students' reflections. Twenty-eight students undertook the practical class and subsequent MiniCEX assessment. Results: A statistically significant difference was found for all aspects of the MiniCEX except medical interview (p=0.07) and physical interview (p=0.69), and a large effect size was found for all areas except physical interview (0.154) and medical interview (0.378). The students' reflections focused on three key themes: behaviours and attitudes, teaching the active cycle of breathing technique, and feedback. Conclusions: Our findings suggest that interacting with SPs improves student skill performance, but further research using a larger sample size and an outcome measure validated for this population is required to confirm this.

Non-cystic fibrosis bronchiectasis (NCFB) is a neglected and orphan disease with poor advances through the 20th century. However, its prevalence is rising and with this come new challenges for physicians. Few guidelines are available to guide clinicians on how to diagnose and manage patients with NCFB. Many areas of debate persist, and there is lack of consensus about research priorities most needed to advance patient care and improve clinical outcomes. In this review, we highlight the current hot topics in NCFB and present updated evidence to inform the critical areas of controversy.Key pointsPostural drainage, active cycle of breathing techniques and pulmonary rehabilitation are non-pharmacological treatment options that should be offered to all patients with non-cystic fibrosis bronchiectasis (NCFB).Eradication of Pseudomonas aeruginosa (PA) colonisation in patients without an acute exacerbation remains debatable.Sputum cultures are the leading and most readily available tool to detect patients with chronic colonisation by PA and should be performed in all patients with NCFB.Antibacterial monoclonal antibodies and vaccine studies have shown promising results in the prevention of chronic colonisation with PA and should stimulate new studies in NCFB.NCFB patients colonised with PA are at more risk of a rapid decline in lung function, worsening quality of life and more hospital admissions.Dual therapy is a promising option for the management of patients with PA-related exacerbations.Patients with PA-related exacerbations benefit from prolonged courses of antibiotics (i.e. 14 days) but emerging and future studies, including dual therapy, may show promising results with shorter courses.Endophenotyping bronchiectasis to address its inherent heterogeneity is a promising avenue for future investment and research.

Pulmonary Rehabilitation werebreathing retraining exercise (BE), mobilization technique (MT), chest mobility exercise (CM), activecycle breathing technique (ACBT), postural correction exercise (PC) for three weeks.Results: There were dyspnea, peak flow rate (PFR:60-70-60), peak cough flow (PCF: 70-90-60 L/m),and abnormal chest expansion (CE: 2.5 – 3 – 2) cm. Pulmonary Rehabilitation After Lobectomy On Pulmonary Aspergilloma: Case Report

Introduction: Complete resection of aspergilloma in chronic pulmonary aspergillosis (CPA), may has several health problem after surgery berupa sesak napas, batuk yang tidak efektif. Methods: A case presentation of 45 years old woman, after lobectomy on right upper of lung due to aspergilloma, with history of cough and haemoptysis for 3 months. Pulmonary Rehabilitation were breathing retraining exercise (BE), mobilization technique (MT), chest mobility exercise (CM), active cycle breathing technique (ACBT), postural correction exercise (PC) for three weeks. Results: There were dyspnea, peak flow rate (PFR:60-70-60), peak cough flow (PCF: 70-90-60 L/m), and abnormal chest expansion (CE: 2.5 – 3 – 2) cm. After three weeks of Pulmonary Rehabilitation, there were no dyspnea, increased the PCF: 193L/m, and CE: 2.5-4 -3. Chest Physiotherapy on Pulmonary Functions 4 Children with Cystic Fibrosis: Systematic Review of Randomized Controlled Trials

AbstractBackground: Pulmonary disease is the main cause of morbidity and mortality in cystic fibrosis (CF). Children with CF usually received chest physiotherapy as a part of their management.Aim of Study: To systematically review the effect of chest physiotherapy on pulmonary functions in children with CF.Patients and Methods: Articles were identified through literature search using PubMed (MEDLINE), physiotherapy evidence database (PEDro) and Cochrane database from 2000 up to February 2019. Studies were included if they were randomized trials focused on chest physiotherapy in children with CF. Data from included studies was extracted and its methodological quality was assessed using PEDro scale. The modified Sackett scale was used to assess level of evidence of each intervention.Results: Seven trials were identified with fair to good methodological quality. Studies were heterogeneous in regards to intervention techniques; findings were qualitatively ana-lyzed. This review found moderate evidence about the effec-tiveness of postural drainage (PD), autogenic drainage(AD), non-invasive ventilation and independent active cycle of breathing technique(ACBT); limited evidence for the effec-tiveness of modified PD, Positive Expiratory Pressure (PEP) and therapist-assisted ACBT. Moderate evidence was found that Hayek oscillator or chest physiotherapy during anaesthesia preoperatively is ineffective in improving pulmonary functions in children with CF.Conclusion: The present evidence supports the use of different chest physiotherapy interventions for improving pulmonary functions in children with CF.

Non Cystic Fibrosis Bronchiectasis-new clinical approach, management of treatment and pulmonary rehabilitation

Abstract Non-Cystic Fibrosis Bronchiectasis (NCFB) are characterised by abnormal, permanently damaged and dilated bronchi due to the innapropiate clearence of various microorganisms and recurrent chronic infections.The diagnosis is suggested by the clinical presentation and is confirmed by multiple investigations. There are some comorbidities associated with bronhciectasis, such as chronic obstructive pulmonary disease (COPD), cardiovascular disorders, gastro-esophageal reflux disease (GERD), psychological illnesses, pulmonary hypertension, obstructive apnea syndrome(OSA). The condition has a substantial socioeconomic impact because it requests a multidisciplinary management and periods of exacerbations are common. The aims of the management of bronchiectasis are to reduce symptoms (such as sputum volume and purulence, cough and dyspnea), reduce the frequency and severity of exacerbations, preserve lung function and improve health-related quality of life. The multidisciplinary approach of bronchiectasis patients require along with the medical treatment, a specific plan of nonphamarcological strategies, including balneological intervention. There are a lot of techniques improving the airway clearence, such as: active cycle of breathing techniques (which include breathing control, thoracic expansion exercises, forced expiratory technique), oscilatting possitive expiratory pressure, autogenic drainage, gravity-assisted-positioning, modified postural drainage.
