The expanding spectrum of Galli–Galli disease

Galli-Galli disease (GGD), a rare genodermatosis in the spectrum of reticulate hyperpigmentation, is regarded as an acantholytic variant of Dowling-Degos disease. We herein report two patients with GGD exhibiting erythematous scaly plaques and lentigo-like macules on the trunk and lower extremities, rather than the characteristic reticulate hyperpigmented macules of large body folds. Digitate elongations of rete ridges coupled with foci of acantholysis were the clues to the diagnosis. A high index of suspicion is needed to diagnose GGD that lacks the characteristic reticulate hyperpigmentation of large body folds.

Galli-Galli disease: An unrecognized entity or an acantholytic variant of Dowling-Degos disease?

Galli-Galli disease is an inherited disease characterized by slowly progressive and disfiguring reticulate hyperpigmentation of the flexures, clinically and histopathologically diagnostic for Dowling-Degos disease, but also associated with suprabasal, nondyskeratotic acantholysis. A few patients exhibiting these features have been described, mainly in the non-English-language literature, which suggests that Galli-Galli disease is not an entity of its own, as originally thought, but is an acantholytic variant of Dowling-Degos disease. We report a typical case of Galli-Galli disease, which supports this concept. (J Am Acad Dermatol 2001;45:760-3.)

Acantholytic Variant of Squamous Cell Carcinoma of the Breast: A Case Report

Primary squamous cell carcinoma of the breast is a rare clinical entity. Two large review series found only five cases out of a total of 8351 breast malignancies. This case report presents a patient with metaplastic, pseudoangiosarcomatous carcinoma or acantholytic variant of a squamous cell carcinoma of the breast. This diagnosis was based on the histological finding of highly atypical, acantholytic squamous cells. Because the tumor stained positive for keratin and negative for factor VIII, the diagnosis of angiosarcoma was ruled out. Although only scattered case reports have been published on this histological variant, these tumors tend to follow an aggressive course.

Acantholytic Pilomatricoma

An 11-year-old girl had a slowly enlarging, painful, pink-red tumor on her scalp of 1 month's duration. The lesion was composed of several, round tumor cell nests in the dermis. Each nest was made up of peripheral basophilic cells and central shadow cells or eosinophilic materials. Occasionally the nests, having central eosinophilic degeneration, became epithelial cysts, some of which showed acantholytic changes in their cyst wall. This seems to be the first case of an acantholytic variant of pilomatricoma.

Acantholytic Variant of Squamous-Cell Carcinoma of the Breast

Three cases of acantholytic squamous-cell carcinoma of the breast are reported. They all had histological features resembling those of angiosarcoma or adenocarcinoma. They were not angiosarcoma, since in all three cases areas of squamous differentiation were present; in addition, the neoplastic cells were negative when stained for factor VIII, but were positive with anti-epidermal keratin. The glandular pattern exhibited, especially in Case 2, was difficult to differentiate from that of an ordinary carcinoma. However, the presence of dyskeratotic cells within the lumina, and the absence of alcian blue/periodic acid-Schiff positive material, and epithelial membrane antigen staining, were evidence against the diagnosis of adenocarcinoma. The patients died 5, 6, and 16 months after the diagnosis. Tumors with these histological features may have a very aggressive clinical course.
