Presence of aberrant thyroid tissue in the guinea pig.

Small highly active aberrant thyroid tissues were detected at various sites in thyroidectomized but not in normal guinea pigs of the Hartley and Zeimet strains by their ability to very actively accumulate I131. Histological study showed follicles lined with flat cuboidal cells and the lumens filled with colloid. Aberrant thyroids have been frequently detected as pathological growths at many different sites in man (1, 2). Studies utilizing serial histological examination of the thyroid area have indicated the presence of aberrant thyroids also in the guinea pig (3, 4, 5, 6), rabbit (3), dog (7, 8) baboon (9) and rat (10, 11). It seemed that a more detailed study of the distribution of aberrant thyroids could be made by the use of the sensitive I131 uptake technic.

Aberrant thyroid tissue is most commonly found in the thorax.1Many case reports have been published about aberrant thyroid tissue, whereas reports of intrathoracic goiters without visible cervical connections are rare. Many of the so-called intrathoracic goiters will be found to have some slight but definite connection within the neck. DeCourcy and Price2reported finding three complete intrathoracic goiters in a series of 1,000 thyroid operations, all of which had some cervical connection. In 1947 Rives3reviewed the English and European literature for the years 1936-1946. He found only 14 cases of intrathoracic goiter. Of these, only two had no cervical connection. Ellis, Good, and Seybold4in 1957 reviewed reports of intrathoracic goiters removed at the Mayo Clinic and found 24 cases on file in which the tumor was removed by routes other than cervical. According to Schrager, the first references to aberrant thyroids were by von Haller in 1779, Albers in 1839 and Gruber in 1845; the first accurate description was by Porta in 1849, and the first extirpation was by Stanley in 1850. Hinterstoisser in 1888 described 3 cases of lateral aberrant thyroid tissue, and von Eiselberg in 1901 collected 11 cases. In 1906 Schrager collected 15 cases from the literature of the preceding fifty years and added 2 of his own, and in 1908 McGlannan reported 3 cases. Wohl in 1917 added 4 cases. In 1923 Parcelier, Venot and Bonnin collected 23 cases from the literature and added 2 of their own. These authors noted that the disease becomes manifest most frequently between the ages of 20 and 40. Billings and Paul in 1925 made a resume of 34 previously reported cases and added 1 of their own, bringing the total to

RELATION OF TUMORS OF LATERAL ABERRANT THYROID TISSUE TO MALIGNANT DISEASE OF THE THYROID GLAND

Tumors of lateral aberrant thyroid tissue offer a particularly fruitful field for the investigator interested in the origins of cancer. In some of the earliest reports of this condition, Hinterstoisser 1 pointed out that the tendency to malignant degeneration in these primarily benign tumors is great. When one considers the fact that the dividing line between innocent and malignant tumors is nowhere less sharply drawn than in tumors of thyroid tissue showing the papillary design, it will be seen that intriguing opportunities are offered in the study of this group of neoplasms. Most investigators agree as to the possibility of malignant change, but there is considerable difference of opinion as to the point at which malignancy begins, the criteria of malignancy and the degree of malignancy, once the diagnosis is established. The occurrence of carcinoma of thyroid tissue at the base of the tongue is so rare as to justify reporting this case. If one accepts the statement of Gruber (1) that the incidence of aberrant thyroid tissue in routine post-mortem examinations is over 10 per cent, the number of reported neoplasms occurring in this tissue seems disproportionately small when compared with the frequency of tumor formation in the normally placed thyroid. Cattell (2) states that the term “aberrant thyroid” includes all portions of thyroid tissue in other than the normal position of the thyroid gland and unconnected with it. He classifies these as follows: (1) lingual thyroid and thyroid tissue found along the thyroglossal tract; (2) lateral aberrant thyroids or those in a cervical position lateral to the jugular vein; (3) the extremely rare intrathoracic goiter having no connection with the main thyroid body; (4) those portions of thyroid tissue found in teratoid tumors and sites far removed from the thyroid.

LATERAL CERVICAL TUMORS OF ABERRANT THYROID TISSUE

Tumors arising in lateral aberrant thyroid tissue are not uncommon, and the literature pertaining to them was reviewed in 1901 by von Eiselsberg, in 1906 by Schrager and by Payr and Martina, in 1925 by Billings and Paul and by Fedeli, and in 1926 by Wegelin. The greatest number of case reports included in any single review is the fifty-three collected by Fedeli. Since there are at least one hundred and three published cases, to which we are adding six, we believe that a critical study of the entire group will be profitable.1 It is probable that such tumors occur more commonly than the number of published cases would indicate. The anatomic origin of large invasive thyroid tumors is frequently difficult to determine, and it is likely that such tumors arising in aberrant thyroid tissue have in some instances been attributed to the thyroid gland. Aberrant thyroid tissue in the tongue is unusual. Carcinoma occurring in such an aberrant thyroid is almost unknown. Berard and Dunet1state that there is not yet on record a proved case of cancer of a lingual thyroid. The following is the history and pathologic report of such a case: REPORT OF CASE A. C., a man, aged 56, admitted to the Episcopal Hospital, Nov. 22, 1916, complained of a lump on the back of his tongue near the right lower molar teeth. This lump had been noticed for more than twenty years; first as a small hard mass that very gradually grew larger. After about five years, it began to give him pain.
