Ectopic internal carotid artery presenting as an oropharyngeal mass

Ectopic internal carotid artery (ICA) is a very rare variation. The major congenital abnormalities of the ICA can be classified as agenesis, aplasia and hypoplasia, and they can be unilateral or bilateral. Anomalies of the neck artery may be vascular neoplasms or ectopic position. Carotid angiograms provide absolute confirmation of an aberrant carotid artery, while EcoColorDoppler (ECD) gives also important information about the evaluation of carotid vassels. Nevertheless Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) of the neck provide spatial information about the adjacent pharyngeal anatomy and are less invasive than angiogram. Injuries to the ICA during simple pharyngeal surgical procedures can be catastrophic due to the risk of massive bleeding. We report a case of a 56 year-old male patient suffering from dysphagia associated with aberrant ICA manifesting itself as a pulsative protruding of the left lateral wall of the oropharynx.

Aberrant carotid artery in the middle ear: Multislice CT imaging aids in diagnosis

An aberrant internal carotid artery in the middle ear has a high likelihood of being misdiagnosed as a glomus tumor, a hemangioma, or effusive otitis media, and a large loss of blood may result from performance of tympanotomy or unnecessary biopsy. To prevent this, it is important for the physician to be knowledgeable regarding this disease entity. The authors recently encountered a patient in whom this condition was suspected on the basis of the tympanic findings. Image diagnosis using multislice computed tomography (MSCT) resulted in accurate diagnosis of a case of aberrant internal carotid artery in the middle ear. The details of this case are presented herein.

Straightening of aberrant carotid arteries with age in velocardiofacial syndrome.

Two cases of velocardiofacial syndrome demonstrated spontaneous straightening over a 4-year period of previously deviated carotid arteries on contrast enhanced computed tomography. Based on this finding, uncomplicated traditional pharyngeal flaps were performed on both patients. It is unknown whether straightening of aberrant vessels with age is the rule or the exception in this patient population.

Aberrant carotid vessels as a cause of pseudotumour of the pharynx: CT, MRI and angiographic findings

Aberrant internal carotid arteries presenting as pharyngeal masses are described in the literature. Eight patients were evaluated by CT, angiography or MRI. Our studies indicated that the phyaryngeal pseudo masses were due to aberrant carotid vessels. Unlike the previous reports, in tour of our eight cases both common and internal carotid arteries were the cause of the suspected mass. Contrast-enhanced CT studies consistently demonstrated the abnormalities. MRI may be done if patients have a contraindication to intravenous iodinated contrast administration.

The otolaryngologist and the patient with velocardiofacial syndrome.

Velocardiofacial syndrome is the most common syndrome associated with clefting of the secondary palate. The endoscopically diagnosed occult submucous cleft palate is the most common palate anomaly associated with this syndrome. Patients with velocardiofacial syndrome present a special challenge to the otolaryngologist, who must be familiar with the diagnostic and therapeutic aspects of this syndrome. We report the findings in 21 patients with velocardiofacial syndrome. Only 11 (52%) had the typical manifestations, and others had only partial phenotype. Adenoidectomy must be avoided in these patients, since undiagnosed occult and overt submucous cleft palate are the most common cause of velopharyngeal insufficiency after adenoidectomy. Three patients had aberrant carotid arteries. This anomaly must be excluded by nasoscopy and computed tomographic scanning before pharyngeal flap surgery is considered in these patients. Criteria for exclusion of high-risk patients from adenoidectomy are presented and discussed.

Anterior Communicating Artery Aneurysm Presenting as Pulsatile Tinnitus

Objective tinnitus is caused by a vascular abnormality of the cervical region, skull base or intracranium. These lesions produce an abnormal flow lesion in the arterial or venus circulatory system of the brain, skull base or cervical region. This flow abnormality is perceived by the auditory system as pulsatile tinnitus. An aberrant carotid artery, a dehiscent jugular bulb or a glomus tumor are common causes. The cause of subjective tinnitus is often not found. Many serious vascular anomalies and tumors can present as pulsatile tinnitus. These include: arteriovenous malformations, atherosclerotic vascular disease, and intracranial tumors with elevated cerebrospinal fluid pressure. We recently diagnosed a patient with an anterior communicating artery aneurysm presenting with pulsatile tinnitus. An aneurysm presenting as pulsatile tinnitus is extremely uncommon. Only 8 have been identified in the literature. The clinician evaluating patients with tinnitus must be aware of this diagnosis. A complete evaluation should include an appropriate radiologic exam; either computed tomography or magnetic reasonance imaging.

During middle ear surgery in a 50-year-old female with chronic otitis media of the left ear, when a fine needle was inserted into the reddish mass of the promontrium, profuse aural hemorrhage occurred. This massive bleeding was stopped by packing with a large, thick fascia graft. CT scan and carotid artery angiography revealed an aberrant artery in the left middle ear.This paper reviews the literatures on the subject of aberrant carotid artery of the middle ear and temporal bone, and discusses the diagnosis and management of these lesions.

Aberrant Carotid Artery: Presentation in the Near Midline Pharynx

Three cases of an aberrant internal carotid artery presenting at or near the midline in the posterior part of the pharynx occurred. In all three cases, the anomalous finding was not correlated with the presenting symptoms of the patient. In two of the three cases intraoral pulsations were detected during initial examination. In the third case, pulsations were appreciated on reexamination after roentgenographic evaluation. Intraoral photographs, computed tomographic scan, magnetic resonance imaging, and arteriography of these findings are shown. A review of the literature and the embryology of the lateral pharyngeal carotid artery are presented along with the rare finding of the near midline carotid artery and the clinical implications of this anomaly.

Bruit caused by aberrant carotid artery in the middle ear

An aberrant course taken by the internal carotid artery during its development may lead it through, rather than anterior to, the middle ear space. The resulting symptoms and signs, including a pulsatile bruit in and around the ear and the presence of a mass behind the tympanic membrane, require differentiation from those of glomus jugulare tumor.

In Reply.— We appreciate Dr Ehni and Dr Sugar's comments regarding our report. During our research, we found references describing aberrant passage of the carotid artery through the middle ear dating back some 15 years. This anomaly is now readily detected via tomography or angiography or both. Biopsy of such a mass beneath the tympanic membrane should certainly be deferred until such studies have excluded this anomaly. We cannot state categorically that simple carotid artery ligation would have failed to control hemorrhage in our patient. The issue, of course, is retrograde flow. We disagree with Dr Sugar regarding such flow in the internal carotid artery. We have observed retrograde flow below the ophthalmic artery in several patients. His suggestion that, after internal carotid artery ligation, the carotid artery below the ophthalmic artery would become a blind segment may be true, but not in this instance. To the Editor.— The report of carotid artery injury during the course of biopsy of a mass in the ear by Reilly and co-workers (1983;249:1473) was of much interest to me because I had not known before reading their article that the carotid artery could actually appear in the middle ear as a developmental variation. At the time that John Barrett and I reported our case in the New England Journal of Medicine , 1 balloon catheters were unavailable and our patient, who was thought to have suffered carotid artery injury during the course of mastoidectomy and who survived a harrowing motor trip across Texas, during which he controlled his hemorrhage by digital compression of his own artery, finally yielded to carotid occlusion and hemostatic packing. To the Editor.— Dr Reilly and associates have written about fortuitous injury of an aberrant carotid artery at myringotomy. The statements are made that carotid artery ligation alone would have allowed retrograde flow into the pseudoaneurysm through the intact anterior communicating artery. An intracranial approach to the artery above the aneurysm might have damaged several cranial nerves permanently. The first statement indicates that a worldwide experience with internal carotid artery occlusion for aneurysm (in the years before clips and microscopes made direct approach more palatable) was misguided. In point of fact, internal carotid ligation in the neck allows contralateral flow only as far as the ophthalmic artery (if that far!) and certainly would not permit flow to the petrous portion of the carotid, since there is no place for the blood to make an exit (small branches of the internal carotid in its canal do not permit such retrograde

An ectopic carotid artery is rare. Its first symptoms may be neurological and could bring the patient to the neurologist during the initial evaluation. However, more often, the neurologist examines a patient in whom serious neurological deficit has occurred after transtympanic exploration of an undiagnosed vascular mass. Because diagnosis can be definitively established by angiography and the consequences of injury to an aberrant carotid artery are serious, operative exploration of vascular middle ear masses probably should be deferred until an ectopic carotid artery has been excluded by angiography.

THE NORMAL internal carotid artery enters the base of the skull through the carotid canal. The artery then bends medially and anteriorly, traversing the petrous bone and cavernous sinus, before emerging medial to the anterior clinoid process. As it passes through the bony petrous ridge, the artery is separated from the middle ear cavity by the tympanic plate. This 0.5-mm-thick plate may be disrupted in basilar skull fractures, displacing the carotid artery into the middle ear. The artery may also pass through the middle ear because of congenital anomalies in its course. Such a vessel may be seen through the tympanic membrane, where it may be mistaken for hemorrhage behind the drum or a tumor. The vessel may be injured during myringotomy; attempts to perform a biopsy of the mass may be disastrous.
