The present paper deals with the successful laparoscopy assisted right hemicolectomy performed for a patient of advanced ascending colon cancer. The patient was a 65-year-old woman, who visited the hospital complaining of anemia, and was diagnosed as having an ascending colon cancer by studies of barium enema. The operation was performed by abdominal wall lifting method, without peritoneal insufflation, through 4ports and 6cm of right trans rectal incision. Mobilization of the bowel was performed laparoscopically, lymph nodes at the root of the mesenterium were dissected through the incision, and the ileo-colic end to end anastomosis was performed extracorporeally. Total operating time was 330 minutes, blood loss amount was 120g. Postoperative course was uneventful, and the passage of flatus appeared at 2nd day postoperatively. In Japan, indication of laparoscopy assisted colon operation is confined only to the cases of both early cancer and benign diseases, for it is, technically, not feasible to dissect lymph nodes laparoscopically. We performed right hemicolectomy with dissection of lymph nodes at the root of the mesenterium, just as the same as performed routinely in open laparotomy, through the small incision so that the specimen could be excised extracorporeally. We conclude that the curative operation for the ascending colon cancer can be performed laparoscopically by our techniques.
