Validation of a 3D CT imaging method for quantifying implant migration following anatomic total shoulder arthroplasty.

Glenoid component loosening remains a common complication following anatomic total shoulder arthroplasty (TSA); however, plain radiographs are unable to accurately detect early implant migration. The purpose of this study was to validate the accuracy of a method of postoperative, three-dimensional (3D) computed tomography (CT) imaging with metal artifact reduction (MAR) to detect glenoid component migration following anatomic TSA. Tantalum bead markers were inserted into polyethylene glenoid components for implant detection on 3D CT. In-vitro validation was performed using a glenoid component placed into a scapula sawbone and incrementally translated and rotated, with MAR 3D CT acquired at each test position. Accuracy was evaluated by root mean square error (RMSE). In-vivo validation was performed on six patients who underwent anatomic TSA, with two postoperative CT scans acquired in each patient and marker-based radiostereometric analysis (RSA) performed on the same days. Glenoid component migration was calculated relative to a scapular coordinate system for both MAR 3D CT and RSA. Accuracy was evaluated by RMSE and paired Student's t-tests. The largest RMSE on in-vitro testing was 0.24 mm in translation and 0.11° in rotation, and on in-vivo testing was 0.47 mm in translation and 1.04° in rotation. There were no significant differences between MAR 3D CT and RSA measurement methods. MAR 3D CT imaging is capable of quantifying glenoid component migration with a high level of accuracy. MAR 3D CT imaging is advantageous over RSA because it is readily available clinically and can also be used to evaluate the implant-bone interface.

Reliability of a novel 3-dimensional computed tomography method for reverse shoulder arthroplasty postoperative evaluation

BackgroundLong-term function and survival of reverse shoulder arthroplasties (RSAs) are reliant on component positioning and fixation. Conventional postoperative analysis is performed using plain radiographs or 2-dimensional (2D) computed tomography (CT) images. Although 3-dimensional (3D) CT would be preferred, its use is limited by metal artifacts. This study proposes a new 3D CT method for postoperative RSA evaluation and compares its interobserver reliability with conventional methods.Materials and methodsPreoperative and postoperative CT scans, as well as postoperative radiographs, were obtained from 18 patients who underwent RSA implantation; the scapula, implant, and screws were reconstructed as 3D CT models. The postoperative 3D scapula and implant were imported into preoperative coordinates and matched to the preoperative scapula. Standardized scapula coordinates were defined, in which the glenoid baseplate version and inclination angle were measured. The percentage of screw volume in bone was measured from a Boolean intersection operation between the preoperative scapula and screw models. Four independent reviewers performed the measurements using 3D CT and conventional 2D methods. Intraclass correlation coefficients (ICCs) were used to compare the reliability of the methods.ResultsThe 3D CT method showed excellent reliability (ICC > 0.75) in baseplate inclination (ICC = 0.92), version (ICC = 0.97), and screw volume in bone (ICC = 0.99). Conventional 2D methods demonstrated poor reliability (ICC < 0.4). For radiographs, inclination showed poor reliability (ICC = 0.09) and the screw percentage in bone showed fair reliability (ICC = 0.54). Version was not measured with plain radiographs. For 2D CT slice measurements, inclination showed poor reliability (ICC = 0.02), version showed excellent reliability (ICC = 0.81), and the screw percentage in bone showed poor reliability (ICC = 0.28).ConclusionThe new 3D CT–based method for evaluating RSA glenoid implant positioning and screw volume in bone showed excellent reliability and overcame the metal-artifact limitation of postoperative CT and 3D CT reconstruction.

Standardization of torsional CT measurements of the lower limbs with threshold values for corrective osteotomy.

Re-establishing anatomic rotational alignment of shaft fractures of the lower extremities remains challenging. Clinical evaluation in combination with radiological measurements is important in pre- and post-surgical assessment. Based on computed tomography (CT), a range of reference values for femoral torsion (FT) and tibial torsion (TT) have historically been reported, which require standardization to optimize the significant intra- and inter-observer variability. The aims of this study were (re-)evaluation of the reference FT and TT angles, determination of the normal intra-individual side-to-side torsional differences to aid the surgical decision-making process for reoperation, and development of a novel 3D measurement method for FT. In this retrospective study, we included 55 patients, without any known torsional deformities of the lower extremities. Two radiologists, independently, measured the rotational profile of the femora using the Hernandez and Weiner CT methods for FT, and the tibiae using the bimalleolar method for TT. The intra-individual side-to-side difference in paired femora and paired tibiae was determined. A 3D technique for FT assessment using InSpace® was designed. FT and TT demographic values were lower than previously reported, with mean FT values of 5.1°-8.8° and mean TT values of 25.5°-27.7°. Maximal side-to-side differences were 12°-13° for FT and 12° for TT. The Weiner method for FT was less variable than the Hernandez method. The new 3D method was equivocal to the conventional CT measurements. The results from this study showed that the maximal side-to-side tolerance in asymptomatic normal adult lower extremities is 12°-13° for FT and 12° for TT, which could be a useful threshold for surgeons as indication for revision surgery (e.g., derotational osteotomy). We developed a new 3D CT method for FT measurement which is similar to 2D and could be used in the future for virtual 3D planning.

Comparison of 3-Dimensional Computed Tomography–Based Measurement of Glenoid Bone Loss With Arthroscopic Defect Size Estimation in Patients With Anterior Shoulder Instability

The purpose of this study was to compare four 3-dimensional (3D) computed tomography (CT) methods of measuring glenoid bone loss with the arthroscopic estimation of glenoid bone loss. Twenty patients with recurrent anterior shoulder instability underwent bilateral shoulder CT scans and were found to have glenoid bone loss. Arthroscopic estimation of glenoid bone loss was performed in all patients. Three-dimensional CT reconstruction was performed on the CT scans of each patient. The glenoid bone loss of each patient was measured using the surface area, Pico, ratio, and anteroposterior distance-from-bare area methods. The mean percent loss calculated with each method was compared with arthroscopy to determine the reliability of arthroscopy in the measurement of glenoid bone loss. The mean percent bone loss calculated with arthroscopic estimation, surface area, Pico, ratio, and anteroposterior distance-from-bare area methods was 18.13% ± 11.81%, 12.15% ± 8.50% (P= .005), 12.77% ± 8.17% (P= .002), 9.50% ± 8.74% (P < .001), and 12.44% ± 10.68% (P= .001), respectively. Repeated-measures analysis of variance showed that the 3D CT methods and arthroscopy were significantly different (F4,76= 13.168, P= .02). The estimate using arthroscopy is 55% greater than the average of the 3D CT methods. Our findings suggest that arthroscopy significantly overestimates glenoid bone loss compared with CT and call into question its validity as a method of measurement. A more internally consistent and accurate method for the measurement of glenoid bone loss is necessary to appropriately diagnose and treat shoulder instability. Level IV, case series.

Comparison of Computed Tomography Scout Based Reference Point Localization to Conventional Film and Axial Computed Tomography

Identification of source positions after implantation is an important step in brachytherapy planning. Reconstruction is traditionally performed from films taken by conventional simulators, but these are gradually being replaced in the clinic by computed tomography (CT) simulators. The present study explored the use of a scout image–based reconstruction algorithm that replaces the use of traditional film, while exhibiting low sensitivity to metal-induced artifacts that can appear in 3D CT methods. In addition, the accuracy of an in-house graphical software implementation of scout-based reconstruction was compared with seed location reconstructions for 2 phantoms by conventional simulator and CT measurements. One phantom was constructed using a planar fixed grid of 1.5-mm diameter ball bearings (BBs) with 40-mm spacing. The second was a Fletcher-Suit applicator embedded in Styrofoam (Dow Chemical Co., Midland, MI) with one 3.2-mm-diameter BB inserted into each of 6 surrounding holes. Conventional simulator, kilovoltage CT (kVCT), megavoltage CT, and scout-based methods were evaluated by their ability to calculate the distance between seeds (40 mm for the fixed grid, 30–120 mm in Fletcher-Suit). All methods were able to reconstruct the fixed grid distances with an average deviation of <1%. The worst single deviations (approximately 6%) were exhibited in the 2 volumetric CT methods. In the Fletcher-Suit phantom, the intermodality agreement was within approximately 3%, with the conventional sim measuring marginally larger distances, with kVCT the smallest. All of the established reconstruction methods exhibited similar abilities to detect the distances between BBs. The 3D CT-based methods, with lower axial resolution, showed more variation, particularly with the smaller BBs. With a software implementation, scout-based reconstruction is an appealing approach because it simplifies data acquisition over film-based reconstruction without requiring any specialized equipment and does not carry risk of misreads caused by artifacts.

Validation of a 3D CT method for measurement of linear wear of acetabular cups

Background We evaluated the accuracy and repeatability of a 3D method for polyethylene acetabular cup wear measurements using computed tomography (CT). We propose that the method be used for clinical in vivo assessment of wear in acetabular cups.Material and methods Ultra-high molecular weight polyethylene cups with a titanium mesh molded on the outside were subjected to wear using a hip simulator. Before and after wear, they were (1) imaged with a CT scanner using a phantom model device, (2) measured using a coordinate measurement machine (CMM), and (3) weighed. CMM was used as the reference method for measurement of femoral head penetration into the cup and for comparison with CT, and gravimetric measurements were used as a reference for both CT and CMM. Femoral head penetration and wear vector angle were studied. The head diameters were also measured with both CMM and CT. The repeatability of the method proposed was evaluated with two repeated measurements using different positions of the phantom in the CT scanner.Results The accuracy of the 3D CT method for evaluation of linear wear was 0.51 mm and the repeatability was 0.39 mm. Repeatability for wear vector angle was 17°.Interpretation This study of metal-meshed hip-simulated acetabular cups shows that CT has the capacity for reliable measurement of linear wear of acetabular cups at a clinically relevant level of accuracy.

Three-dimensional Computerized Tomographic Analysis of the Deformity of Lateral Growth Disturbance of Proximal Femoral Physis

Lateral growth disturbance of the proximal femoral physis after treatment of developmental dysplasia of the hip, also known as type 2 avascular necrosis, has been described in a 2-dimensional (2D) plane with standard radiographs. It is not well described in the 3-dimensional plane using 3D computed tomography (CT) scan. The purpose of this study was to define the anatomy of the proximal femur after the lateral growth disturbance of the proximal femoral physis with 3D CT scans. Ten patients (11 hips) with lateral growth disturbance of the proximal femoral physis after treatment for developmental dysplasia of the hip (9 hips) and extracorporeal membrane oxygenation (2 hips), were studied with 2D and 3D CT methods. CT was done at an average age of 12.5 years (range, 9.8 to 16.69). In addition to the typical valgus configuration in the coronal plane, increased antetorsion of the femur (10 of 11 hips) and increased anteversion of the femoral head (8 of 11 hips) were a common finding seen in the transverse plane. In 6 of 7 hips for which the CT scan was performed before the closure of the capital femoral epiphysis, the physeal bridge was seen to be located in the anterolateral (5 hips) or posterolateral (1 hip) area of the proximal femoral physis. The version of the femoral head was correlated to the location of the physeal bridge. Acetabular dysplasia was seen in 4 hips. In the 3D reconstruction view, flexion deformity of the femoral head was seen in 6 hips and lack of coverage at the anterolateral portion of the femoral head was observed in 6 hips. The treating surgeon should be aware of the often marked antetorsion and also anteversion of the femoral head and neck associated with lateral growth disturbance of the proximal femoral physis. This knowledge allows surgical planning to correct rotational and sagittal plane correction of the proximal femur, which will allow normalizing hip mechanics. Acetabular side correction also may be needed if acetabular dysplasia is present. Level III, diagnostic study.

Bronchial morphometry in smokers: comparison with healthy subjects by using 3D CT

The assessment of airway dimensions in patients with airway disease by using computed tomography (CT) has been limited by the obliquity of bronchi, the ability to identify the bronchial generation, and the limited number of bronchial measurements. The aims of the present study were (i) to analyze cross-sectional bronchial dimensions after automatic orthogonal reconstruction of all visible bronchi on CT images, and (ii) to compare bronchial morphometry between smokers and nonsmokers. CT and pulmonary function tests were performed in 18 males separated into two groups: 9 nonsmokers and 9 smokers. Bronchial wall area (WA) and lumen area (LA) were assessed using dedicated 3D software able to provide accurate cross-sectional measurements of all visible bronchi on CT. WA/LA and WA/(WA+LA) ratios were computed and all parameters were compared between both groups. Smokers demonstrated greater WA, smaller LA, and consequently greater LA/WA and LA/(WA+LA) ratios than nonsmokers. These differences occurred downward starting at the fourth bronchial generation. 3D quantitative CT method is able to demonstrate significant changes in bronchial morphometry related to tobacco consumption.

Three-dimensional computed tomography cephalometric craniofacial analysis: experimental validation in vitro

The development of three-dimensional (3D) cephalometric analysis is essential for the computer-assisted planning of orthognathic surgery. The aim of this study was to transform and adapt D elaire's two-dimensional cephalometric analysis into the third dimension; this transposition was then validated. The comparative advantage of using 3D computed tomography (CT) surface renderings over profile X-rays was analysed. Comparison was made of inter- and intra-observer reproducibility of the cephalometric measurements done on profile X-rays and on 3D CT surface renderings on the same 26 dry skulls. The accuracy was also tested of the measurements done on 3D CT surface renderings (ACRO 3D ©) in relation to those directly taken on dry skulls with the help of a 3D measuring instrument. Inter- and intra-observer reproducibility proved significantly superior ( p < 0.0001) following the 3D CT method. There were no significant differences in the accuracy of measurements between the ACRO 3D software and the 3D measuring instrument. The ACRO 3D software was confirmed as being a reliable tool for developing 3D CT cephalometric analyses. Further research may entail clinical validation of the 3D CT craniofacial cephalometric method of analysis.

Three-dimensional microscopic computed tomography based on generalized Radon transform for optical imaging systems

We propose a 3D computed tomography method based on the generalized Radon transform defined for incoherent optical systems. We apply this method to reconstruct microscopic 3D images efficiently from 2D images acquired with multi-axis fluorescence microscopes which eliminate missing frequency bands. Simulation results based on digital algorithms are shown.
