Abstract
Background: Heart failure (HF) is a common, costly cause of hospitalization. Several studies have examined the use of electronic disease management (EDM)designed to curtail HF readmissions. Few studies have evaluated the long-term impact. We reported a mortality benefit among HF patients (pts) undergoing electronic daily weight assessment in the Weight Monitoring in Heart Failure (WHARF) trial. This trial reported mortality rates through 6 months but long-term data on transplant-free survival was available for up to 56 months.
