Abstract

Transitional care strategies, particularly for patients with chronic health conditions undergoing complex interventions, have demonstrated effectiveness in lowering re-admissions by identifying patients who require early medical interventions, reinforcing of discharge teaching and ensuring timely follow up with primary care providers and specialists (Wong et al, 2018). For most patients within this tertiary care institution, standardized transitional care interventions consist of using interactive voice response technology (IVR) to contact patients immediately post discharge and then at gradually increasing intervals as a routine part of the discharge process.
