Abstract
Acute left-sided visual loss, chemosis and proptosis with ophthalmoplegia developed in a 63-year-old man following head trauma. Neuroradiologic studies demonstrated an intraorbital arteriovenous fistula. Ligation of the intracranial ophthalmic artery reduced the degree of proptosis, chemosis and ophthalmoplegia, but the eye remained blind. Intraorbital arteriovenous fistulas are rare and cause symptoms similar to those that occur in cases of carotid-cavernous fistulas. Treatment may be necessary to retard ocular damage, because spontaneous remission has not been reported.
